백내장 수술환자의 진료결과에 미치는 요인 - 사전 연구를 중심으로 - (Factors Affecting Outcomes in Cataract Surgical Patient - Pilot study -)

  • Background : This study is to identify preoperative patient characteristics associated with a lack of improvement on one or more measures peri operatively, postoperative 3-4 months, and postoperative 12 months. Methods : For the assessment, prospective study was performed with 92 patients who had undergone either one eye or both eye cataract surgery by 3 ophthalmologists practicing at a university hospital. The criteria of improvement were (1) Snellen visual acuity, (2) a cataract-related symptom score(possible range: 0, 0 of 6 symptoms present or bothersome, to 18, all 6 symptoms very bothersome), and (3) VF-14 score - a measure of functional impairment in patient with cataract - (possible range: 0, inability to perform any of the applicable activities, to 100, no difficulty in performing any of the applicable activities). Results : Although 14 patients (15.2%) failed to improve on one or more of the outcome measures assessed, no one failed to improve on all three measures. Both eyes of surgery than one eye, preoperative cataract symptom score of 1-4, 5 or higher than 0 were associated independently with the increased likelihood of improvement (odds ratio 8.95, 7.16, 8.87 respectively). And the preoperative level of Snellen visual acuity was not associated with the likelihood of improvement Conclusion : We conclude that specific preoperative characteristics (both eyes, cataract symptom score) are independent predictors of patient outcome after cataract surgery.

Extraction of Canine Cataract Object for Developing Handy Pre-diagnostic Tool with Fuzzy Stretching and ART2 Learning

  • Canine cataract is developed with aging and can cause the blindness or surgical treatment if not treated timely. The first observation must be made by pet owners but they do not have proper equipment and knowledge to see the abnormalities. In this paper, we propose an intelligent image processing method to extract canine cataract suspicious object from non-professional equipment such as ordinary digital camera and cellular phone photographs so that even casual owners of pet dog can make a pre-diagnosis of such a surgery-needed disease as soon as possible. The experiment shows that the proposed method is successful in most cases except the dog has similar colored hair to the color of cataract.

  • The purpose of this study is to investigate the incidence rate, surgery tendency, and risk factors of cataracts among the elderly in Korea. The subjects were 40-95 years old men and women using the National Health Services' surgical statistics. Among the 33 major surgeries, which increased by 1.1% per year for the last 20years (1.6 million), the most operated surgery in 2020 was cataract surgery (454,000), the number of which was 858.8 per 100,000 people. Among the surgical patients, cataract patients (7.9% increased) was the third largest among the top (2016-2020) constantly with annually 2.2% increasing for 20 years (19.95 million). The medical expenses of major surgeries (excluding non-benefit) increased by an average rate of 9.3% per year over the 20 years (7,204 billion won) out of a total medical expenses of 7.2 trillion won, and the cataract surgery (813.2 billion won) was the second largest among the medical expenses in 2020 (top 3). This study showed that there was a high correlation between cataract and smoking, low income, and low-educated, whereas being no concentration of cataract patient in tertiary hospitals.

Random Forests 기법을 이용한 백내장 예측모형 - 일개 대학병원 건강검진 수검자료에서 - (A Prediction Model for the Development of Cataract Using Random Forests)

  • 백내장 질환은 노령인구가 증가하고 있는 시점에서 사회, 경제적으로 심각한 문제로 부각되고 있는 질병으로 조기 진단이 이루어진다면 발병률을 크게 줄일 수 있는 질병이다. 본 연구에서는 백내장을 조기 진단하기 위한 예측 모형을 구축하고자 1994년부터 2001년까지 연세대학병원에서 2회 이상 건강검진을 받고 의사진단을 통해 백내장 여부를 확인할 수 있는 30세 이상 남 녀 3,237명에 대한 건강검진 수검 자료를 활용하여 백내장 발생 위험 예측모형을 개발하였다. 모형개발에는 데이터마이닝 기법인 Random Forests를 사용하였고, 기존의 로지스틱 회귀분석, 판별분석, 의사결정나무 모형(Decision tree), 나이브베이즈(Naive Bayes), 앙상블 모형인 배깅(Bagging)과 아킹(Arcing)을 이용하여 그 성능을 비교 분석하였다. Random Forests를 통해 개발한 백내장 발생 예측모형은 정확도가 67.16%, 민감도가 72.28%였고, 주요 영향요인은 연령, 혈당, 백혈구수치(WBC), 혈소판수치(platelet), 중성지질(triglyceride), BMI였다. 이 결과는 의사의 안과검진 정보 없이 건강검진 수검 자료만으로 백내장 질환 유 무에 관한 정보를 70% 정도 예측할 수 있음을 보여주는 것으로, 백내장의 조기 진단에 많은 기여를 할 것으로 판단된다.

간편한 선별검사법에 의한 농촌 노인인구의 백내장에 대한 역학조사 (The Epidemiological Study on the Cataract of the Rural Aged Population Using a Simplified Screening System)

  • 본 조사는 경북 농촌지역의 백내장 분포를 알아보기 위하여 실시하였다. 백내장은 전 세계적으로 설명과 시력장애의 주원인일 뿐 아니라 노인인구의 증가로 인해 백내장 환자의 수는 현저하게 증가하였다. 이러한 추세에도 불구하고 백내장으로 인해 심하게 시기능이 저하된 상태에 있으면서도 안과적 진단 기회를 얻지 못하고 있는 환자들이 아직도 많이 있다. 이런 환자들의 선별과 분포조사를 위해 경북 칠곡과 문경 지역에서 일반 건강검진을 희망한 50세 이상 주민 636명(1272안)을 대상으로 최근 일본 Kanazawa 의과대학의 Sasaki 등이 제안한 간편한 백내장 일차선별검사를 적용하여 다음과 같은 결과를 얻었다. 전체 조사대상자 중 시력장애를 호소한 사람은 모두 493명(77.5%)이었다. 제1단계 선별검사로 448명(90.9%), 제2단계 선별검사로 308명(48.4%)의 백내장이 의심되는 시력불량군을 분류하였고, 제1, 2단계일차 선별검사에 의해 최종적으로 백내장으로 분류된 사람의 수는 421명(66.2%)이었다. 연령에 따른 유병율은 50대 27.5%, 60대 62.5%, 70대 86.1%, 80세 이상 94.3%이었다. 따라서 백내장이 실명과 시력장애의 주원인 일 뿐 아니라 국내에서도 공중보건분야의 중요한 문제인 만큼 적절한 관리와 안보건 대책이 요구되며 특히 안보건을 위한 의료활동이 농촌지역에까지 확대되어야 할 것으로 사료되었다.

Treatment outcome and risk analysis for cataract after radiotherapy of localized ocular adnexal mucosa-associated lymphoid tissue (MALT) lymphoma

  • Purpose: We retrospectively reviewed the results of radiotherapy for localized ocular adnexal MALT lymphoma (OAML) to investigate the risk factors of cataract. Methods: Sixty-seven patients with stage IE OAML treated with radiotherapy at Seoul St. Mary's Hospital from 2001 to 2016 were included. Median treatment dose was 30 Gy. Lens protection was done in 52 (76%) patients. Radiation therapy (RT) extent was as follows: superficial (82.1%), tumor mass (4.5%), and entire orbital socket (13.4%). The risk factors for symptomatic cataract were analyzed using the Cox proportional hazard model. Results: Median follow-up time was 50.9 months (range, 1.9 to 149.4 months). All patients were alive at the time of analysis. There were 7 recurrences and there was no local recurrence. Median time to recurrence was 40.4 months. There were 14 cases of symptomatic cataract. Dose >30 Gy had hazard ratio of 3.47 for cataract (p = 0.026). Omitting lens protection showed hazard ratio of 4.10 (p = 0.008). Conclusions: RT achieves excellent local control of ocular MALT lymphoma. Consideration of RT-related factors such as lens protection and radiation dose at the stage of RT planning may reduce the risk of RT-induced cataract after radiotherapy.

Fine localization of a new cataract locus, Kec, on mouse chromosome 14 and exclusion of candidate genes as the gene that causes cataract in the Kec mouse

  • A mouse with cataract, Kec, was generated from N-ethyl-N-nitrosourea (ENU) mutagenesis. Cataract in the Kec mouse was observable at about 5 weeks after birth and this gradually progressed to become completely opaque by 12 weeks. Dissection microscopy revealed that vacuoles with a radial or irregular shape were located primarily in the cortex of the posterior and equatorial regions of the lens. At the late stage, the lens structure was distorted, but not ruptured. This cataract phenotype was inherited in an autosomal recessive manner. We performed a genetic linkage analysis using 133 mutant and 67 normal mice produced by mating Kec mutant (BALB/c) and F1 (C57BL/6 $\times$ Kec) mice. The Kec locus was mapped to the 3 cM region encompassed by D14Mit34 and D14Mit69. In addition we excluded coding sequences of 9 genes including Rcbtb2, P2ry5, Itm2b, Med4, Nudt15, Esd, Lcp1, Slc25a30, and 2810032E02Rik as the candidate gene that causes cataract in the Kec mouse.

The effect of suture by absorbable material on corneal astigmatism after phacoemulsification

  • Objectives: To investigate the effect of absorbable suture on surgically-induced corneal astigmatism in 3.0-mm sclera tunnel cataract surgeries. Methods: Medical records of patients who underwent phacoemulsification cataract surgery using a 3.0-mm sclera tunnel incision made by a single surgeon were reviewed. Uncorrected distant visual acuity, corneal astigmatism and surgically-induced astigmatism were measured in 56 patients' eyes that underwent sclera tunnel cataract surgery with absorbable sutures (sutured group) and in 23 patients' eyes without sutures (unsutured group). Uncorrected visual acuity, intraocular pressure, slit lamp examination, and automated keratometry were evaluated preoperatively and at 3 days, 2 weeks, 4 weeks, and 8 weeks after cataract operation. Results: There were no significant differences in preoperative average uncorrected distant visual acuity of the two groups (sutured group: $0.79{\pm}0.64$, unsutured group: $0.68{\pm}0.72$, P = 0.145). Corneal astigmatism measured using keratometry in the sutured and unsutured group at postoperative day 3 were $2.27{\pm}2.12D$ versus $0.83{\pm}0.55D$ at (P < 0.001), a difference which had disappeared after 4 weeks. Surgically induced astigmatism using the Holladay and Vector methods showed similar outcomes, suggesting that the sutured group exhibited higher astigmatism compared with the unsutured group until 2 weeks post-surgery. Conclusions: TPatients undergoing scleral tunnel cataract surgery with absorbable sutures have greater surgically induced astigmatism, especially in the early postoperative period, compared with those without sutures. However, this surgically induced astigmatism due to absorbable sutures in scleral tunnel cataract surgery is temporary and disappears at 4 weeks post-surgery.

구조화된 수술 전 교육이 백내장 수술 환자의 불안과 자가간호 수행에 미치는 효과 (The Effects of Structured Preoperative Instruction on Anxiety and Self-care Compliance in Patients with Cataract Surgery)

  • Purpose: The purpose of this study was to investigate the effects of a structured preoperative instruction upon anxiety and postoperative self-care compliance. Methods: A randomized control group pre-post design was used. Sixty subjects undergoing cataract surgery were randomly assigned to one of two groups. The instruction consisted of cataract surgery procedure, sensory information, deep breathing, use of eye drop, and post op self-care regimen and was provided to the experimental group with control group receiving an usual treatment. State anxiety NRS, pulse, BP, and self-care compliance scale developed by Cho & Rho were used. Data were collected at two time periods: on the day of surgery in the clinic and prior to anesthesia in the operating room. The postoperative self-care compliance scores were measured at their second visits to the hospital after surgery. Data were analyzed using Kolmogorov Smirnov test, t-test, and Mann-Whitney U test. Results: There were significant differences between the two groups in the postoperative state anxiety (t=-3.57, p=.001) and the postoperative self-care compliance score (t=3.92, p<.001). There were no significant difference between the two groups in the postoperative blood pressure and pulse rate. Conclusion: The results of this study suggest that the structured preoperative instruction could be a nursing intervention for cataract surgery patients.

Effects of Furanocoumarins from Angelica Dahurica on Aldose Reductase and Galactosemic Cataract Formation in Rats

  • The ether extract from the roots of angelica clahurica was found to inhibit bovine lens aldose reductase (BLAR0 activity in vitro by 100% at 100.mu.g/ml. Systematic fractionation of the ether soluble fraction and subsequent active frctions monitorede by bioassy led to isolation of four furanocoumarins, isoimperatorin (I0, imperatorin (II), ter-O-methyl byakangelicin (III) and byakangelicin (IV), among which compound III and IV were identified as potential AR inhibitors, their $IC_{50}$ values being $2.8{\mu}M{\;}and{\;}6.2{\mu}M$, respectively. Galactosemic cataract formation tors, in rats treated with 40 g/kg/day of galactose was blocked almost completely throughout the exeprimental periods up to 44 days by i.p. administrations of byakangelicin (IV) at 50 mg/kg/day. In coincidence with the inhibitory action on cataract formation, the galactitol accumulation in rats treated with byakangelicin (IV) was found to be markedly prevented by approximately 80.5% compared to those of the contro. These results indicate that byakangelicin (IV), as a main principle of this plant, possesses high potential for a clinically useful drug of the future which prevents and/or improves sugar cataract as well as diabetic complications.
