A striking feature of mitochondrial disorders is the vast heterogeneity in their clinical symptoms that ranges from a single organ to severe multisystem involvement. Though a variety of ocular symptoms such as ptosis, pigmentary retinal degeneration, external ophthalmoplegia, and optic nerve atrophy can occur in association with mitochondrial cytopathies, progressive bilateral cataracts are rare among their ocular findings. A 5-year-old girl with no previous medical history came to our hospital presenting symptoms of seizure. She started showing progressive developmental regression, increased seizure frequency, hypotonia, general weakness, dysphagia and decreased vision. Lactic acidosis was noted in metabolic screening test and we confirmed mitochondrial respiratory chain complex I defect in spectrophotometric enzyme assay using the muscle tissue. Progressive bilateral cataracts then developed and were fully evident at the age of 7. She underwent cataract extraction with posterior chamber lens implantation. We are reporting a case of mitochondrial respiratory chain defect with multiorgan involvements including bilateral progressive cataract, an uncommon ocular manifestation. Ophthalmologic evaluation is highly recommended not to overlook the possible ocular manifestations in mitochondrial disorders.

This study was designed to evaluate the effects of arthritis and comorbid chronic conditions on the health-related quality of life (HRQoL) in the Korean older population. The study subjects were 2,708 Korean adults aged 65 and older from the $3^{rd}$ Korea National Health and Nutrition Examination Surveys data: 1,357 persons with a single chronic condition, such as arthritis, herniations of the intervertebral disc, osteoporosis, asthma, peptic ulcers, stroke, or cataract; 886 persons with arthritis and a second chronic condition described above; and 465 persons with no chronic condition. An analysis of covariance was performed to compare the EQ-5D index among the groups. The effects of arthritis, second chronic condition and their interactions were analyzed by multiple linear regression analysis. The results are as follows. Compared to men with arthritis only, men with stroke only, stroke and arthritis, or cataract and arthritis had a lower age adjusted EQ-5D index. Women with a stroke only, asthma only, cataract only, osteoporosis and arthritis, peptic ulcer and arthritis, stroke and arthritis, or cataract and arthritis had a lower age adjusted EQ-5D index than women with arthritis only. Arthritis and comorbid conditions had additive effects on the HRQol in both genders except for arthritis and stroke in women. In conclusion, comorbid chronic medical conditions in older people with arthritis may reduce the HRQoL in an additive manner.
We investigated refractive errors and corneal power with 3 factors such as M, $J_0$, and $J_{45}$ as power vector to find out the changes of refractive errors of the before and after cataract surgery in 119 adults aged 45~85 years with cataract. After the surgery, the 3 factors were changed as $-0.29{\pm}2.38D$ to $-0.18{\pm}0.69D$ in spherical equivalent power which is the M factor, $-0.34{\pm}0.68D$ to $-0.05{\pm}0.42D$ in the $J_0$ factor, and $0.11{\pm}0.45$ to $0.02{\pm}0.17$ in the $J_{45}$ factor. Before and after the surgery, corneal mean refractive power, $J_0$, and $J_{45}$ were changed from $44.11{\pm}1.61D$ to $44.20{\pm}1.58D$, $0.01{\pm}0.50D$ to $0.08{\pm}0.49D$, and $0.02{\pm}0.29$ to $0.08{\pm}0.49$, respectively. The results showed that $J_0$ was the highest relativeness in correlation of the pre- and post-surgery for refractive errors, mean corneal power was the highest correlation for corneal power factor, and corneal power factor was the higher correlation much more than refractive error factor.

Background : There is increased recognition that rigorous approach to functional assessment will complement the assessment of clinical status. This study is to develop the appropriate visual function index in Korean patients with cataracts by the assessment of the reliability, validity of visual function indexes including VF-14. Methods : An prospective study was performed with 92 patients who had undergone either one eye or both eye cataract surgery by 3 ophthalmologists practicing at a university hospital. Patients were interviewed. and clinical data were obtained. Doctors were questioned with self-entered questionnaire forms. Medical record was examined to understand surgery process. The survey was conducted at 1-2 days before cataract surgery. Results : We used following measures : 7 visual function indexes, global measures of patients' trouble and satisfaction with vision, and best-corrected visual acuity(VA) in each eye. 7 visual function indexes showed a high internal consistency reliability (Cronbach's alpha coefficients ${\geq}$ 0.86). The better visual acuity(operated eye, better eye), the higher patient satisfaction with vision, the lower symptom score, the lower patient trouble with vision was correlated with preoperative visual function index scores. Among 7 indexes, VF-12 with the exclusion of 2 items drivings from VF-14 is a high Cronbach's alpha coefficient and correlation coefficient and the highest $R^2$. Conclusion : We conclude that VF-12 is reliable and valid in Korea. So we suggest that in Korea, all items of VF-14, the visual function index, which is in use internationally, ought to be surveyed and VF-12 which has been excluded by 2 items of drivings from VF-14 ought to be used for analysis.
Small area variations in health care utilization have long been studied as an important issue related to boto cost containment and quality assurance. This study was conducted to investigate if variations in hospital services across small geographic areas in Korea existed. The claims data of the fiscal year 1992 obtained from the regional health insurance societies were used for the study. Main findings of the research can be summarized as follows : 1. Extremal Quotients(EQ) of hospital expenditure per capita and hospital days per capita varied among diagnosis types. The EQ ranged from 2.05(cataract) to 41.67(pneumonia) in hospital expenditure per capita and from 1.86(cataract) to 45.89(pneumonia) in hospital days per capita. The diagnosis groups which showed high variation were pneumonia, cephalo-pelvic disproportion, gastritis and duodenitis, fracture of rib, and acute bronchitis. Those which showed low variation were acute appendicitis and cataract. 2. The EQ level of admission rate was different in terms of diagnosis types, ranging from 2.57(catarct) to 44.45(pneumonia). The variations were high in medical disorders such as pneumonia, oephalo-pelvic disproportion, gastritis and duodenitis and acute bronchitis, while relatively low in surgical conditions such as acute appendicitis and cataract. 3. As an indicator of service intensity, the EQ of expenditure per admission ranged from 1.67(acute appendicitis) to 31.27(essential hypertension). The diagnoses which had high variation were essential hypertension, gastric ulcer, whereas those which had low variation were cephalopelvic disproportion and acute appendicitis. With regard to hospital days per admission, the EQ ranged from 1.55(acute appendicitis) to 28.13(gastric ulcer) by diagnosis types. The diagnosis groups with showed high variation were gastric ulcer, essential hypertension, and acute bronchitis, whereas those with low variation were cephalo-pelvic disproportion, intervertebral disc disorders, and acute appendicitis. Both the expenditure and hospital days per admission showed lewwer variations than the expenditure per capita, hospital days per capita and admission rate. 4. Comparing patterns of variation in utilization indices, diagnoses such as essential hypertension, gastric ulcer, fracture of rib showed higher variations in expenditure per admission than in admission rates, whereas diagnoses such as pneumonia, cephalo-pelvic disproportion and gastric ulcer showed higher variations in admission rate than expenditure per admission. These findings suggest that wide variations existed in several diagnoses groups across small areas in Korea. Further research should be performed to investigate factors related to small area variations including provider behavior.

Naturally occurring coumarin compounds have received substantial attention due to their pharmaceutical effects. Esculetin is a coumarin derivative and a polyphenol compound that is used in a variety of therapeutic and pharmacological strategies. However, its effect on aldose reductase activity remains poorly understood. In this study, the potential beneficial effects of esculetin on lenticular aldose reductase were investigated in galactose-fed (GAL) rats, an animal model of sugar cataracts. Cataracts were induced in Sprague-Dawley (SD) rats via a 50% galactose diet for 2 weeks, and groups of GAL rats were orally treated with esculetin (10 or 50 mg/kg body weight). In vehicle-treated GAL rats, lens opacification was observed, and swelling and membrane rupture of the lens fiber cells were increased. Additionally, aldose reductase was highly expressed in the lens epithelium and superficial cortical fibers during cataract development in the GAL rats. Esculetin reduced rat lens aldose reductase (RLAR) activity in vitro, and esculetin treatment significantly inhibited lens opacity, as well as morphological alterations, such as swelling, vacuolation and liquefaction of lens fibers, via the inhibition of aldose reductase in the GAL rats. These results indicate that esculetin is a useful treatment for galactose-induced cataracts.
Aldose reductase (AR) has been reported to play an important role in sugar-induced cataract. In the present study, the AR inhibitory activity of Enicostemma hyssopifolium (EH), Gymnema sylvestre, Eclipta alba, and Tinospora cordifolia (TC) were studied along with their effect on sugar-induced cataractogenic changes in sheep lenses in vitro. AR inhibitory activity of the aqueous extracts of plants and their anticataract potentials were evaluated in vitro in sheep lenses, considering the activity of normal sheep lenses as 100%. The concentration of the plant extract that showed maximum activity was selected to further study its effect on galactose-induced polyol accumulation in vitro. The $IC_50$ values of EH and TC were calculated to be 102 and 85 ${\mu}g$/ml, respectively. EH showed a significant inhibition (61.3%) in polyol accumulation followed by TC (53.1%). EH and TC possesses a significant anticataract activity in vitro and its anticataract potential could be related with its AR inhibitory effect.
Purpose: We report a rare case of isolated traumatic aniridia in a pseudophakic eye. Case summary: A 69-year-old female came to our emergency department complaining of right eye pain and visual disturbance after trauma due to fall on the stairs. Five years earlier she had undergone an uncomplicated right sutureless phacoemulsification cataract extraction through a 2.2 mm temporal clear corneal incision, followed by insertion of a folding intracapsular intraocular lens. Total iris expulsion occurred through the cataract incision without extension of the wound or disruption of the posterior capsule or intraocular lens. Conclusions: We report a rare case of isolated traumatic aniridia in a pseudophakic eye, which has not been reported in the Republic of Korea.
In order to study the effect of some oriental prescriptions for polydipsia on experimental diabetes mellitus and cataract, Cheongsimyeonja-eum, Kamijowiseungki-tang and Kikukjihwang-tang were administered to streptozotocin(STZ)-induced diabetic and galactose-induced cataractous rats. The increased level of serum glucose and urea nitrogen owing to 572 were significantly decreased by administration of Cheongsimyeonja-eum, Kamijowiseungki-tang and Kikukjihwang-tang. The increased level of serum triglyceride and total cholesterol owing to STZ were significantly decreased by administration of Kamijowiseungki-tang and Kikukjihwang-tang. Reduction of body weight owing to STZ was significantly recovered in 7th day since administration of Cheongsimyeonja-eum and Kamijowiseungki-tang. Weight of liver was significantly recovered by administration of Cheongsimyeonja-eum and Kamijowiseungki-tang and weight of kidney was significantly recovered only by administration of Kikukjihwang-tang. The polydipsia of water and food were significantly decreased by administration of Cheongsimyeonja-eum, Kamijowiseungki-tang and Kikukjihwang-tang. Onset of cataract in experimental group owing to galactose was not significantly different from control group.
