An in vitro study was performed to evaluate adaptation of custom direct, custom indirect, and prefabricated post system on 15 extracted upper central incisors. 15 specimens were prepared and equally devided into 3 groups under random sampling. Each group of 5 cast posts was made with custom direct, custom indirect, and prefabricated post core method. Gap between inner wall of the dentin and outer wall of the cast post was measured on electron microphotographic prints at x500 magnification. The result were as follows ; 1. No significant difference of adaptation at cervical portion was found between each method. 2. Prefabricated post core method had poor adaptation compared with other methods. 3. Even distribution of adaptation was found in custom direct method between each portion. 4. Prefabricated post core method showed remarkable difference in adaptation between each portion.
The restorations of the severely damaged teeth by post core have been increased with the developement of endodontic procedures. But high failure rates of these procedures being reported, various restorative modifications were induced for successful treatments. Cast post-core and prefabricated post with core buildups are choice of treatment. The main causes of failure of the restorations are the fracture of post and core, root fracture, and recurrent caries due to microleakage. Recently, the acid etching technique and the use of dentin bonding agent at tooth surface to reduce the possible microleakage at the tooth-restoration interfaces were introduced. The object of this study was to measure and compare the microleakage by the types and cementation methods of post-core. For this study, forty extracted human anterior teeth were selected for specimen. After cleansing and routine endodontic procedures, anatomic crowns of each specimen were removed at the level of 2mm above the cementoenamel junction. Canals were preparated for post insertion and specimens were divided into four groups randomly. Post-cores were fabricated according to method for each group. Microleakage was measured by length of dye penetration at the tooth-restoration interfaces with measuring microscope at 50 magnification. Oneway ANOVA and t-test were performed for statistical analysis of resulting data. The following results were obtained from this study. 1. There wert significant statistical differences in degree of microleakage between each group (p<0.01). 2. Cast post-core cemented with ZPC (Group I) showed the most severe microleakage pattern$(1.5547{\pm}0.0872mm)$, and cast post-core cemented with adhesive resin cement after tooth surface treatment with dentin bonding agent (Group II) showed the least microleakage $(0.1497{\pm}0.0872mm)$. 3. Group IV revealed less dye penetrations than group III, but no statistical significance was seen between two groups.
PURPOSE. The aim of the present study was to evaluate the fracture resistances of zirconia, cast nickel-chromium alloy (Ni-Cr), and fiber-composite post systems under all-ceramic crowns in endodontically treated mandibular first premolars. MATERIALS AND METHODS. A total of 36 extracted human mandibular premolars were selected, subjected to standard endodontic treatment, and divided into three groups (n=12) as follows: cast Ni-Cr post-and-core, one-piece custom-milled zirconia post-and-core, and prefabricated fiber-glass post with composite resin core. Each specimen had an all-ceramic crown with zirconia coping and was then loaded to failure using a universal testing machine at a cross-head speed of 0.5 mm/min, at an angle of 45 degrees to the long axis of the roots. Fracture resistance and modes of failure were analyzed. The significance of the results was assessed using analysis of variance (ANOVA) and Tukey honest significance difference (HSD) tests (${\alpha}=.05$). RESULTS. Fiber-glass posts with composite cores showed the highest fracture resistance values ($915.70{\pm}323N$), and the zirconia post system showed the lowest resistance ($435.34{\pm}220N$). The corresponding mean value for the Ni-Cr casting post and cores was reported as $780.59{\pm}270N$. The differences among the groups were statistically significant (P<.05) for the zirconia group, as tested by ANOVA and Tukey HSD tests. CONCLUSION. The fracture resistance of zirconia post-and-core systems was found to be significantly lower than those of fiber-glass and cast Ni-Cr post systems. Moreover, catastrophic and non-restorable fractures were more prevalent in teeth restored by zirconia posts.
The purpose of this study was to analyze the stress distribution in mandibular second premolars restored with different post and core techniques. Sixteen two-dimensional finite element model of mandibular second premolars restored with post and core and complete crown were developed according to the diameter, length, and material of post and core. Vertical force, 10N in magnitude, was applied first to the central fossa and then $45^{\circ}$ oblique force of same magnitude was applied to the buccal contact surface of buccal cusp. The obtained results were as follows : 1. Stress distribution within the dentin 1) Regardless of the material of the post and core and the diameter and length of the post, the pattern of stress distribution within the dentin was similar. 2) Maximum dentinal stress was observed on the lingual root surface of alveolar crest level with oblique loading and on lingual side of root dentin at the crown margin on vertical loading. 3) Cast post and cores produced the lowest dentinal stress concentrations and the highest stress concentration was observed in composite resin post and cores. 2. Stress distribution within the post and core 1) Within the amalgam and composite resin post and core, the patterns and maximum values of stress were similar. Maximum stress located at the central fossa of core portion on vertical loading and at the lingual junction of post and core with oblique loading. 2) Among the all post and cores, the cast post and core registered the highest stress concentration and maximum stress value within the post. Maximum stress located at the post apex on vertical loading and at lingual half of the post surface with oblique loading. 3) In case of Para-post and amalgam core, maximum stress located at the central fossa of core portion and lingual tip of the post head on vertical loading. With oblique loading, maximum stress located at the lingual half of the post surface.
In the restoration of endodontically treated teeth, carbon fiber post was recently introduced. The purpose of this in vitro study was to investigate the fracture strength of teeth restored with a pre-fabricated carbon fiber post in comparison with teeth restored with a prefabricated titanium post & custom cast gold post after cyclic loading in the different environment. A total of 30 recently extracted human central incisors of similar dimension with crowns removed were used. All teeth were placed into acrylic blocks and every steps for post and core fabrication were made accord-ing to manufacture's instruction. The post length and core dimensions were standardizd. All teeth were divided into 6 groups: 1) carbon fiber post / atmosphere, 2) titanium post / atmosphere, 3) gold post / atmosphere, 4) carbon fiber post / wet, 5) titanium post / wet, 6) gold post / wet. Carbon fiber post and titanium post were cemented in place using resin cement and cores were fabricated with Ti-Core. Custom cast gold post was made from Duralay pattern resin and cemented using resin cement, too. All specimens were thermocycled 10,000 times. After 50,000 cyclic loading, failure strength was measured using Instron testing machine. Kruskal-Wallis test followed by Mann-Whitney test was used to compare the mean fracture strength. Results were as follows : 1. All specimens showed lower fracture strength in wet environment after cyclic loading than in atmosphere condition, but did not reveal a significant difference. 2. There was no significant difference between carbon fiber post specimen and titanium post specimen in the same environment. 3. Gold cast post specimen showed significant different greater fracture strength than those of others in the same environment. 4. Carbon fiber post specimen showed no root fracture.
Objectives: To determine and compare the fracture resistance of endodontically treated maxillary central incisors restored with different posts and cores. Materials and Methods: Forty-eight upper central incisors were randomly divided into four groups: cast post and core (group 1), fiber-reinforced composite (FRC) post and composite core (group 2), composite post and core (group 3), and controls (group 4). Mesio-distal and bucco-lingual dimensions at 7 and 14 mm from the apex were compared to ensure standardization among the groups. Twelve teeth were prepared for crown restoration (group 4). Teeth in other groups were endodontically treated, decoronated at 14 mm from the apex, and prepared for posts and cores. Resin-based materials were used for cementation in groups 1 and 2. In group 3, composite was used directly to fill the post space and for core build-up. All samples were restored by standard metal crowns using glass ionomer cement, mounted at $135^{\circ}$ vertical angle, subjected to thermo-mechanical aging, and then fractured using a universal testing machine. Kruskal-Wallis and Mann-Whitney U tests were used to analyze the data. Results: Fracture resistance of the groups was as follows: Control (group 4) > cast post and core (group 1) > fiber post and composite core (group 2) > composite post and core (group 3). All samples in groups 2 and 3 fractured in restorable patterns, whereas most (58%) in group 1 were non-restorable. Conclusions: Within the limitations of this study, FRC posts showed acceptable fracture resistance with favorable fracture patterns for reconstruction of upper central incisors.
Post and core is used to restore endodontically treated teeth, and it is a very important part which supplies retention and support to the prosthesis. But occasionally, caries occured due to the microleakage at the post and core and tooth interface, the failure of prosthesis has happened. In this study, the microleakage of cast gold post and core, amalgam core, and composite resin core was investigated and compared. The coronal part of the extracted upper anterior teeth were removed and endodontically treated with conventional method. The teeth were divided into three groups. In group 1, cast gold post and core was cemented with resin cement and in group 2, ready-made post was cemented with resin cement and amalgam core was built. In group 3, ready-made post was cemented with resin cement and composite resin core was built. All specimens were thermocycled between $5^{\circ}C\;and\;55^{\circ}C$ with dwell time of 15 seconds, and immersed in 0.5% aqueous solution of basic fuchsin dye for 24 hours. After embedded in the epoxy resin, the specimens were sectioned longitudinally and observed with stereomicroscope with the magnification of 25. From the findings of this study, the following conclusions were obtained. 1. There was microleakage at the core/tooth interface of all specimens. 2. The microleakage of gold post and core was significantly less than those of the other two groups. 3. There was no significant difference between the microleakage of amalgam core and that of composite resin core.
Statement of problem: The endodontically treated tooth is generally restored with post and core, owing to the brittle and the loss of large amount of tooth structure. As periodontal treatment was developed, there are many cases that periodontally involved teeth used in prosthetic treatment. Purpose: The purpose of this study was to analyze the stress distribution in the dentin and post structures by the various post materials and the amount of remaining alveolar bone height. Material and method: The 3-dimensional finite element models of mandible 1st premolars were divided into six types according to the various amount of remaining alveolar bone and post type. All types were modeled using equal length, diameter and shape of the post. Three types of post and core materials were used: prefabricated titaniumpost and amalgam core, prefabricated stainless steel post and amalgam core, and cast gold post and core. 300 Newton force was applied to functional cusp of mandible 1st premolar. Results: The results were as follows: First, there was no apparent difference in the pattern of stress distribution according to the alveolar bone condition concentrate on the post middle area. Second, there was difference in pattern of stress distribution according to the core materials, gold post and core generated same than amalgam core. Third, there was no apparent difference in the pattern of stress distribution within the dentin according to the post and core materials. But a cast gold post and core generated the lowest maximum stress value, a stainless steel post generated the highest maximum stress value. Fourth, in the reduced alveolar bone model, maximum stress value is 1.5 times than that of the normal alveolar bone model. Conclusion: Within the limitations of this study, to provide minimal stress to the root with alveolar bone reduced, the post length may be as long as apical seal was not destroyed. To prevent fracture of tooth, it is rational to use gold alloy which material was good for stress distribution for post materials.

There was no apparent difference in the pattern of stress distribution according to the amount of remaining coronal tooth structure. 2. There was no apparent difference in the pattern of stress distribution within the dentin according to the post and core materials. A cast gold post and core generated lower dentin stress than a stainless steel post and resin core. 3. Max. dentinal stress resulting from vertical force was observed in the lingual side of dentin around the crown margin.This stress resulting from oblique force was observed in the lingual root surface of alveolar bone crest level.
Statement of Problem. Endodontically treated teeth frequently required posts and cores to provide retention and resistance form for crowns. In spite of excellent mechanical properties of metal post and core, its metallic color can be detected through all ceramic restorations occasionally. To solve esthetic problems of metal post and core zirconia post system has been introduced recently. Purpose. The purpose of this study was to examine the fracture strength and mode of resin root analogs restored with zirconia, gold and titanium posts with resin, ceramic and metal cores after cementation with metal crowns. Materials and methods. To avoid the morphological variations of natural teeth, 40 root analogs were fabricated with composite resin. Forty resin root analogs were randomly assigned to four groups according to post and core materials: Group A: cast gold post and core and complete cast crowns, as control. Group B: titanium posts (Parapost, Coltent/Whaledent Inc., NJ, USA) and composite resin cores. Group C: zirconia posts (Cosmopost, Ivoclar AG, Schaan/Liechtenstein) and composite resin cores Group D: zirconia posts and heat-pressed ceramic cores (IPS Empress Cosmo Ingots, Ivoclar AG) After thermocycling ($5^{\circ}C{\sim}55^{\circ}C$, 30 sec.), cyclic loading was applied at 3mm below the incisal edge on the palatal surfaces at an angle of 135 degree to the long axis (2Hz, 50N, 50000cycles). Fracture strength was measured by universal testing machine (Instron, High Wycombe, UK) and fracture pattern of restored resin root analogs was also evaluated. Results and conclusion. Within the limitations of this study following results were drawn. 1. Resin root analogs restored with zirconia posts and composite resins demonstrated lowest fracture strength among tested groups. 2. There was no significant difference in the fracture strength between zirconia posts and heat pressed glass ceramic cores and cast gold posts and cores 3. The fracture strength of resin root analogs restored with titanium posts and composite resin cores was lower than that of gold posts and cores. 4. The deep oblique fracture lines were dominantly observed in root analogs restored with cast gold post and core and zirconia post and heat-pressed ceramic core groups.
