  • Aneurysmal rupture of sinus Valsalva is known as one of rare cardiac disease and not controlled with medical treatment. We experienced 9 cases which were composed with 7 case of male and 2 cases of female during last 15 years. 7 cases were ruptured into right ventricle from right. coronary sinus and 2 cases were ruptured into right atrium from non-coronary sinus. The diagnosis was made with cardiac catheteriza1icn and cineangiccardicgram but 2 cases were misdiagnosed with only cardiac catheterization. All cases were corrected under cardiopulmonary bypass with means of direct suture with tdlon pledget by transatrial or transventricular approach. All were not confirmed in it's origin because of no history, no evidences of syphilis, TB, or bacterial endocarditis and only fibrosis in pathologic report. In postoperative course, 1 case had postoperative bleeding and 1 case was sufferd from left hemiplegia due to may be air embolism. The worm was diagnosed as, likelihood, Dirofilaria immitis on microscopic examinations. The present case suggests that D. immitis can cause periocular infections.

  • Benign lymphoepithelial cysts of parotid gland were first described by Miculicz in 1888, have been presented for a long time, uncommonly. But, recently it has been reported with increasing frequency since the onset of human immunodeficiency virus (HIV) infection. Benign lymphoepithelial cysts can be diagnosed by the fine needle aspiration in conjunction with the history and physical examination. The therapeutic options of benign lymphoepithelial cysts have been limited to aspiration, surgical resection, medical therapy, radiotherapy and sclerotherapy. Recently, we experienced a case of benign lymphoepithelial cyst of right parotid gland, so we report this case with review of the literatures.

  • Pleomorphic adenoma is the most common benign mixed salivary gland neoplasm that accounts for 65% of all benign salivary glands tumors. While the majority arises from the parotid, only a 10% arises from the minor salivary glands. Pleomorphic adenoma of minor salivary gland in the palate is a common entity. However, they occur in the lip, cheek, tongue oropharynx and nasal cavity etc., rarely. We experienced a case of a 80 years old female presented a 10 years history of a right cheek mass which was proved to the pleomorphic adenoma on the histopathologic examination. We resected the mass and obtained good postoperative results. We report this case with a review of literature.

  • Intramural esophageal dissection is a rare disorder characterized by a separation of the mucosa and/or submucosa from deeper muscular layers of the esophagus, with or without perforation. Iatrogenic instrumentation such as endoscopy is one of the major causes of IED. We report a case of IED after endoscopy in a patient with hypopharyngeal cancer treated with concurrent chemoradiotherapy, and suggest that a history of chemoradiotherapy can be a risk factor of IED on endoscopy. In this case, chest computed tomography scans show not only typical esophageal double lumen but also eccentric esophageal wall thickening and abnormally thin the other side esophageal wall, and this CT finding may also be important to diagnose IED.

  • Warfarin sodium is a widely used oral anticoagulant and it sometimes causes hemorrhage in various sites such as traumatic or operative wounds, urologic organs, gastrointesitnal tract and subcutaneous tissues. Warfarin induced hemorrhage in upper respiratory tract causing upper airway obstruction is extremely rare. Despite the life-threatening nature of this complication, the symptoms are obscure like sore throat or hoarseness, suggesting infection and early diagnosis can be delayed. Careful medical history is emphasized When bleeding is observed, the patient should be admitted for close observation and prompt treatment. Recently we have exrperienced a case of laryngeal and upper tracheal submucosal hemorrhage induced by long term warfarin in 64 years old mm. We described the case with a review of the literature.
