This article reviewed theological background and historicak development in dental caries management by risk assessment. Dental caries prevention and treatment according to CAMBRA model is patient-centered, risk-based, evidence-based practice. Team approach is necessary and clinician need to integrate science, practice and product. Dental hygienist take a important role in implementing CAMBRA. CAMBRA model could be incorporated into dental and dental hygiene education. Dentist and dental hygienist able to provide scientific and ethical care managing dental caries by risk assessment.
Objectives: The water fluoridation program has been implemented since 2008, in a region of Geoje City. This study aimed to evaluate the effectiveness of the water fluoridation program on caries prevention in the permanent dentition. Methods: Evaluation surveys were conducted on 8-, 10- and 12-year-old children in the fluoridated and non-fluoridated regions of Geoje City, in 2015. The survey included 834 children from the fluoridated regions and 703 from the non-fluoridated regions. The data obtained from the survey were analyzed using the IBM SPSS statistical package version 23.0. The effectiveness of community water fluoridation in caries prevention was estimated by the differences in the decayed, missing, filled teeth (DMFT) and decayed, missing, filled surfaces (DMFS) scores, between the program and control groups. The mean numbers of fissure-sealed teeth and surfaces were adjusted to reduce their confounding effect. Results: The mean DMFT and DMFS scores (0.69 and 0.91, respectively) of 10-year-olds from the fluoridated regions, after adjusting for the numbers of fissure-sealed teeth and surfaces, were significantly lower than those of the 10 year olds (DMFT: 0.95, DMFS: 1.32) from the non-fluoridated regions. The effectiveness of water fluoridation on caries prevention in the permanent dentition, estimated based on the differences in the mean DMFT and DMFS scores, adjusted for the number of fissure-sealed teeth and surfaces, between the program and control groups of 10-year-old children, was 27.4% and 31.1%, respectively. Conclusions: These results suggest that the effectiveness of water fluoridation on caries prevention in the permanent dentition, in Geoje City was so high that this program should be implemented in other regions in Korea as well.
Background: The purpose of this systematic review was to investigate the effects of topical fluoride gel application on dental caries prevention in the permanent teeth of children and adolescents. Methods: We searched the EMBASE, PubMed, and Cochrane Library databases for randomized controlled trials (RCTs) assessing the effects of self-applied or professionally applied topical fluoride gels in patients <18 years of age; the search was completed on April 1, 2018. All included trials involved an experimental group (fluoride gel application) and a control group (placebo or no treatment). The outcome measures were the D(M)FS and D(M)FT indices, which were compared between the two groups. Review Manager software was used for quantitative synthesis of the final selected articles, and a forest plot was generated via a meta-analysis conducted using a random effects model. Results: The results showed that the D(M)FS and D(M)FT indices were lower in the fluoride application group than in the control group, thus indicating that fluoride gel application was effective in dental caries prevention. We also performed a subgroup analysis to determine whether the effects of fluoride application differed if patients received oral prophylaxis (self or professional) before fluoride gel application. Therefore, the two groups showed slightly larger differences when studies without oral prophylaxis before fluoride gel application were considered; however, the difference was not statistically significant. Conclusion: These findings should be utilized to raise awareness about the caries-prevention effects of topical fluoride application among patients and guardians. Further RCTs should evaluate the effects of fluoride application with or without preceding oral prophylaxis, and appropriate fluoride application guidelines should be developed to maximize the effects of fluoride application in clinical practice.
This study was performed to evaluate the importance of parents, knowledge, cognition and behaviour about dental health care on the development of dental caries in preschoolers. Subjects were comprised of 170 preschoolers collected from three preschoolers placed in Pusan and Kimhae area and study period were 6 months from January, 2000. Examination on the preschoolers' dental health and questionnarie survey on parents' knowledge, cognition and behaviour about dental health care were conducted. Collected data were analyzed by chi-square test and t-test using SAS program. Dental caries was so prevalent as the rate of 73.5%. And, the rate was different by the area(p=0.001), and increased with preschooler's age(P=0.004). The parents gain the knowledge about dental health care mainly from dentist or dental hygienist(32.9%), newspaper, magazine booklets(26.5%), TV and VTR tape(20.0%), etc, but rarely from school health education(4.7%). Caries prevalence was significantly lower in case of regular brushing than that of insufficient brushing(p=0.002) and in case of consistently informing brushing time to their children(p=0.014). This results showed that the dental health care for preschoolers is insufficient and the most important factor for caries prevention is regular brushing. And it seemed that the parent's concern and knowledge play an important role for the prevention of preschooler's dental caries. Dental health education would be focused on the parents, especially for the practice of preventive behavior by preschoolers themselves.

Purpose : This study aimed to analyze the level of understanding of Korean children and adolescents of caries risk assessment items in order to develop caries risk assessment tools. Methods : A total of 52 parents and 108 middle and high school students were analyzed for their understanding of nine items of the CAMBRA CRA for Korean children and adolescents. The nine questions were analyzed through a Likert five-point scale. Centralized distribution analysis was conducted to compare significant differences in the three groups' understanding of the questions. Results : The evaluation of the middle school and high school students' understanding showed that four items scored less than 3.0 points. The items were "Hyposalivatory medications (antihistamines, antipsychotics, asthma, etc.)," "Brushing below once a day," "F varnish last six months," and ".12 % chlorhexidine gluconate mouthrinse daily seven days monthly." The understanding of elementary school parents of ".12 % chlorhexidine gluconate mouthrinse daily seven days monthly" was less than 3.0 points. In addition, as a result of comparing the averages of the three groups, a statistically significant difference (p<.05) was found for a total of five items: "Hyposalivatory medications (antihistamines, antipsychotics, asthma, etc.)," 'Not used oral products such as floss and interdental toothbrushes," "F toothpaste 2× daily or more," "F varnish last six months," ".12 % chlorhexidine gluconate mouthrinse daily seven days monthly." Conclusion : This study was conducted to provide basic data for the development of a caries prevention program by identifying whether the caries risk assessment reconstituted in Korean style can be used for children and adolescents. More than half of the questions scored 3.0 points or higher and were generally completed. Nonetheless, a systematic program can only be completed if a plan is secured to improve repondents' understanding of the questions before a caries prevention program is developed.
Objectives : The purpose of this study was to survey Korean solders' oral healthcare recognition and perceived oral health concern. Methods : The data was collected from a questionnaire given 157 soldiers in Chungcheongnamdo. The data was analyzed into t-test, one-way ANOVA and Pearson's correlation analysis. Results : 25.5% replied that they were concerned about oral health. Soldiers who have received oral healthcare education exhibited higher recognition on the prevention of dental caries and periodontal disease. Moreover, those who have used oral hygiene devices also showed higher recognition on the prevention of dental caries and periodontal disease. The recognition of preventive effects on dental caries, in particular, showed a statistically significant difference (p<0.05). In regard to the relationship between oral health concern and recognition of oral healthcare, those who were more concerned about oral health were higher recognition of periodontal disease prevention(r=0.254, p<0.01). Conclusions : To improve concern and recognition of oral health for the members, the soldiers needs to develop oral health education and policy.
Purpose: The aim of this study was to analyse and review deep learning convolutional neural networks for detecting and diagnosing early-stage dental caries on periapical radiographs. Materials and Methods: In order to conduct this review, the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA) guidelines were followed. Studies published from 2015 to 2021 under the keywords(deep convolutional neural network) AND (caries), (deep learning caries) AND (convolutional neural network) AND (caries) were systematically reviewed. Results: When dental caries is improperly diagnosed, the lesion may eventually invade the enamel, dentin, and pulp tissue, leading to loss of tooth function. Rapid and precise detection and diagnosis are vital for implementing appropriate prevention and treatment of dental caries. Radiography and intraoral images are considered to play a vital role in detecting dental caries; nevertheless, studies have shown that 20% of suspicious areas are mistakenly diagnosed as dental caries using this technique; hence, diagnosis via radiography alone without an objective assessment is inaccurate. Identifying caries with a deep convolutional neural network-based detector enables the operator to distinguish changes in the location and morphological features of dental caries lesions. Deep learning algorithms have broader and more profound layers and are continually being developed, remarkably enhancing their precision in detecting and segmenting objects. Conclusion: Clinical applications of deep learning convolutional neural networks in the dental field have shown significant accuracy in detecting and diagnosing dental caries, and these models hold promise in supporting dental practitioners to improve patient outcomes.

The effect of xylitol and sorbitol chewing gums on dental caries levels among 5-year old kindergarteners was investigated. Three kindergartens the operations of which were partly subsidized by municipal government of Daegu city, Korea, were chosen as the study sites. The child base of the kindergartens were considered demographically and ethnically similar, representing middle-income families. The subjects were examined at their own kindergarten by the two dentists. Participation in the program was voluntary. Over 12 months, 123 participants chewed xylitol chewing gums(X group; 42 subjects), sorbitol chewing gum(S group; 42 subjects), or did not receive chewing gum as a control group(C group; 39 subjects). Consumption of xylitol and sorbitol was 4.5 to 5.0 g/day/subject, consumed in five daily chewing episodes of 5 min. Oral examination, Dentocult-SM test and interproximal dental plaque collection were completed at baseline and 12 months later. The dmfs of group C increased 59.2%, but group S increased 33.4% and group X increased only 31.3% during 12 months study period. The caries prevention ratio was 47.1% at group X and 43.6% at group S. There also appeared the reduction of caries activity at group X(1.39), group S(1.50) than control group(1.79). Compared with groups S and C, there was a statistically significant reduction of S. mutans in interproximal plaque in group X. The results suggest that xylitol chewing gum can prevent dental caries of deciduous dentition, and may be a little more effective than a sorbitol-containing product in controlling some caries-associated parameters in kindergarten-age subjects.
A number of studies have been conducted to confirm the preventive effect of xylitol on dental caries as a whole or partial alternative to dietary sugars. This study reviewed the oral health effects of xylitol on the prevention mechanism of dental caries, the prevention of dental caries, the inhibition of mother-to-child transmission, and the oral health effects in the elderly based on existing studies on the oral health of xylitol. Carbohydrates and dietary sugars in food are fermented by acid-producing microorganisms in the mouth and produce dental plaque and acid, which cause dental caries. However, most dental decay-causing bacteria cannot produce acids by metabolizing xylitol. Xylitol, stored in cells as a non-metabolizable metabolite by Streptococcus mutans (S. mutans), affects bacterial glucose metabolism and inhibits bacterial growth. Xylitol consumption also reduces the amount of plaque and the population of S. mutans in both plaque and saliva. In addition, xylitol acts in the remineralization process. Xylitol has been confirmed to effectively prevent dental caries, inhibit mother-to-child transmission of MS, prevent dental caries, and increase salivary flow in the elderly. In conclusion, xylitol is an adequate sugar substitute for dental health, from infants to the elderly. For future studies, the researchers recommend reviewing the effects of xylitol on the oral and intestinal microbial environment and the side effects of excessive intake.

Current treatment concept of dental caries has been changed, because it has been proved that it is a preventable disease. The philosophy has been changed from purely restorative treatment to preventive caries control. Therefore the methods or criteria of oral examination has been changed. The clinician have to detect not only cavitation, but also the lesion of non-cavitation stage. International Caries Detection and Assessment System II (ICDAS II) was developed recently, which is a new criteria of classification of dental caries. This system was based on the current concept of prevention, early detection and patient-centered management of caries. Therefore this philosophy is in accord with the perspective of pediatric dentistry. The purpose of this article is to introduce this system for oral examination of children.
