Effect of Care Workers' Awareness of Professionalism on the Service Quality (요양보호사의 전문성 인식이 서비스 질에 미치는 영향)

  • This study aims to carry out empirical analysis on the effect of care workers' awareness of professionalism on the service quality, and to provide policy level and action oriented recommendations for the improvement of care workers' awareness of professionalism and service quality based on the key findings. The study targeted care workers who work at the senior citizen care take centers at the S and Y cities in the Jeollanam-do Province, carrying out survey research. A total of 190 questionnaires among the collected questionnaires were used for the analysis. Key research methods included factor analysis, reliability analysis, descriptive statistic analysis, correlation analysis, t/F-verification and regression analysis. The following are the results. First, conviction of the care workers' awareness of professionalism towards public service and the empathy of service quality were manifested at the highest level. Second, there was a difference in the service quality according to the care workers' experience. Third, care workers' awareness of professionalism affected service quality while service quality increased as the awareness of professionalism increased. The research results were utilized to recommend the need to develop and to disseminate structured education program for the care workers, to set the guideline for the care workers' employment form and salary, and to develop structure for the care workers' ethics.

The meaning and challenges of the introduction of the Customized Care Services for Older Adults (노인맞춤돌봄서비스의 도입 의미와 과제)

  • The South Korean government has introduced a new Customized Care Service for Older Adults by eliminating the six existing care services for them since 2020. It is a significant change of care service system for older adults and the service would play a key role in providing preventive services for them. The aim of the study is to examine the meaning and tasks of the introduction of the Customized Care Services for Older Adults in South Korea by using the framework of Gilbert and Terrell. The study found that the new service was designed under the government's policy contexts of the establishments of public-based social service providers and the emphasis of community care, and it has brought about a number of significant institutional changes of care system in terms of allocation, benefits, delivery, and finance. The institutional coverage of the service was increased to lower 70% of older adults and a large number of kinds of health and social care services would be provided for them. The sphere of living was set in each local area and the new service providers are entirely in charge of providing the services in each sphere. The financing system was changed to support service providers by providing government subsidies. Although a number of positive results would be expected to occur for the elderly, it is likely that some challenging tasks would happen in the fields as follows: the inadequate coverage, the vagueness of the application of self-care principle and the decrease in the amounts of direct care services, the inappropriate area of service provision, and the supplier-oriented provision of services rather than user-oriented.

Cost Analysis of Long Term Care Services (장기요양보호서비스의 비용분석)

  • The purpose of this study is to evaluate cost side by type of long term care services economically, and then to discuss the findings and implications for the results of analysis. For this research, primary caregivers that provide care the elderly requiring long term care services sanctioned by National Health Insurance Corporation were drawn and surveyed. Among collected data, data for 422 primary caregivers were used for this study. The subjects used in this study consisted of family caregivers from various settings that give care to the elderly. The results of this study can be summarized as follows. First, caregiver's household income level is low. Therefore, caregiving families with the elderly are likely to have financial difficulties. Second, under coverage of long term care insurance system, the direct cost caregiving households pay for the elderly is still very high. Third, indirect cost of caregiving households accounted for the larger proportion caregiving costs. Fourth, social cost burden for caregiving the elderly is very high. This cost amount is appropriately equal to household income of caregivers surveyed in the research. Fifth, service use cost of caregiving households is differentiated by type of long term care service. Sixth, direct cost of caregiving households is statistically significantly differentiated by type of long term care service, but is differentiated less than service use cost. Seventh, social indirect cost for caregiving the elderly is statistically significantly differentiated by type of long term care service. Eighth, social cost amounts for the elderly utilizing long term care service is very high, total social cost per capita by types of long term care service tend to converge on average social cost per capita of total service.

A Study on Satisfaction of Service by the Models of Informal-formal Resources Linkages for Long-term Care Elder (장기요양대상노인의 비공식적·공식적 자원연계 유형에 따른 서비스 만족도 연구)

  • This study is to examine the effective system for providing services for long-term care elders, despite the expansion of formal care, informal care still is needed. Thus, this study's purpose is to classify the informal-formal resources linkages types of long-term care elders and looks into service satisfaction by these types and discover effective informal-formal resources linkages models. For that, this study is to divide informal-formal resources linkages types by the degree of providing services of informal and formal caregivers and discover the informal-formal resources linkages types using cluster analysis and explores the effectiveness of service satisfaction using multiple regression. The study's results is to suggest four models, such as family care, complementary, separation, formal service and complementary type was founded to be the most effective and then based on the result, we discuss as follows. First, we must strive to combine informal resources into formal service systemto the system for providing of service is made up the complementary type. Second, the system for providing services of long-term care elders is required integrated care system to alining of medical and long-term care services. Third, we have to consider a measure to improve of formal service type, for the satisfaction of formal service appears low relatively. Based on research findings, this study propose that the informal-formal resources linkages models are subdivided into the dimension of quantity and quality of care for improving the effectiveness of long-term care services.

The Effects of After-school Service Use on Self-care of Children: Focusing on the Types of Service, the Number of Types, and the Patterns of Service Use (방과후 서비스 이용이 아동의 자기보호에 미치는 영향: 이용하는 방과후 서비스의 유형, 유형의 개수, 혼합이용 패턴을 중심으로)

  • This study explored the effects of after-school services on whether they reduced the possibility of self-care of children. In terms of characteristics of the use of after-school services, it focused on the 1) types, 2) diversity, and 3) use patterns of the after-school services. For the analysis, it used data of the cohort of elementary-school children from the Korean Children and Youth Panel Survey. The main findings are as follows. First, private mentoring and after-school services provided by welfare centers reduced the possibility of self-care. Second, when the total amount of educational time was controlled, the diversity of participating after-school services did not explain the possibility of self-care. Third, when the total amount of educational time was controlled, the use patterns of after-school services did not explain the possibility of self-care. Based on these findings, it suggested policy implications and future research.

The impact of continuous positive airway pressure on radiation dose to heart and lung during left-sided postmastectomy radiotherapy when deep inspiration breath hold technique is not applicable: a case report

  • Deep inspiration breathing hold (DIBH) compared to free-breathing (FB) during radiotherapy (RT) has significantly decreased radiation dose to heart and has been one of the techniques adopted for patients with breast cancer. However, patients who are unable to make suitable deep inspiration breath may not be eligible for DIBH, yet still need to spare the heart and lung during breast cancer RT (left-sided RT in particular). Continuous positive airway pressure (CPAP) is a positive airway pressure ventilator, which keeps the airways continuously open and subsequently inflates the thorax resembling thoracic changes from DIBH. In this report, authors applied CPAP instead of FB during left-sided breast cancer RT including internal mammary node in a patient who was unable to tolerate DIBH, and substantially decreased radiation dose the heart and lung with CPAP compared to FB.

A Study of Nurses' Perception of the Visiting Nursing Services of Long-term Care Insurance (노인장기요양보험 재가간호서비스에 대한 방문간호사의 인식)

  • Purpose: The purpose of this study is to understand nurses' perception of visiting nursing services of long-term care insurance. Method: The descriptive survey study involved 188 nurses selected by the convenient sampling of visiting nurses who participated in professional education sessions. Results: Of the 188 participants, 149 (79.3%) were aware of long-term care insurance. Awareness of aspects of long-term care services was 78.7% for facility service, 77.7% for ordinary visiting care service, 85.1% for visiting nursing service and 77.7% for visiting bathing service. Concerning visiting nursing service provision, the majority of the study subjects considered nurse-aid not to be the appropriate route for delivery of services including nasogastric tube exchange, tracheostomy tube management and stitch removal. Conclusion: Continuous evaluation and research on the standards and requirements of the nursing workforce is needed to secure and maintain the high quality of visiting nursing services. Exhaustive studies concerning task division and workforce separation according to nursing services type and level of difficulty should be done to develop the appropriate job description for visiting nursing service staff.

Working Mothers' Perception and Demand on Child Care Policy and Institution Supporting the Working Mothers (취업모의 자녀양육지원제도에 대한 인식 및 요구)

  • This study surveyed the perception and demand for the child care policy and child care service of working mothers with children under age 6. The subjects were 266 working mothers in Seoul and Kyoung-Gi Province. Data were collected with the questionnaire method and analyzed by spss-win program, including median, mode, mean, standard deviation, and t-test. The result of this study can be summarized as follows. (1) The working mothers considered both parents have the responsibility on child care and strongly demanded the national support on child care. (2) The working mothers prefered own mother and relatives to child care center as a carer of infants and toddlers, but prefered child care center for children age 3-5. (3) The working mothers demanded the increase of finantial support for family and child care center. (4) The working mothers focused on 'the security and care Program' and focused next on 'diet and health care program' in child care service. (5) The working mothers did not nearly know about the accreditation system of child care centers. This results suggest that the national and social support for child care service is an essential part in promoting children's and the working mothers' welfare.

Predictors of Nursing Service Need for Nursing Homes Residents (장기요양시설 노인의 기능상태에 따른 간호서비스 필요도 예측)

  • Purpose: The purposes of this study were to explore the functional status of elderly residents and to analyze time use, and finally identify factors to predict nursing care needs in relation to functional status and health related variables. Methods: In this study a descriptive-correlational design was used. Functional status of participants was obtained through interviews, and nursing care time was examined using a 1 min time-motion study with a standardized instrument developed by Korea Long-Term Care Planning Committee (2005). Results: The mean total functional score was 65 (range 28-125) and mean total nursing care time was 144.15 min per day. There were significant positive relationships between total nursing care time, marital status, back pain, dementia, and vision impairment. Multiple regression analyses showed that a liner combination of number of illnesses, types of primary disease, ADL, IADL, cognitive function, nursing demand, and rehabilitation demand explained 42.8% of variance of total nursing time. ADL (${\beta}$=-.533) was the most significant predictor of nursing service need. Conclusion: Identifying factors that result in variations of service need has implications for adequate nursing service, estimation of optimum nurse to patient ratio, quality of care and patient safety.

Enhanced User Choice of Voucher Program and its Impact and Implications on Care Worker (바우처 제도의 이용자 선택권 강화가 돌봄노동자에게 미친 영향과 그 의미)

  • This study is to analyze experiences and perceptions of care workers for enhanced user choice since the introduction of voucher in Korea. The analysis shows that as it has increased the influence of service users on care workers'employment, dismissal, labor intensity, and so on, more and more care workers appear to experience 'labor alienation'such as weakened position in the labor domain, the sense of loss and frustration in the meaning of work, and identity confusion as a care work. Because the labor alienation felt by them possibly has a negative impact on quality of social service, it suggests to address improving service users'rights as well as protecting human rights and rights to work of care workers to provide a high quality social service.
