A Study on the Fee-Based Model Development of Day Care Centers for the Elderly (유료 노인 낮보호 시설 모형개발에 관한 연구)

  • The aim of this study is the development of a fee - based model day care center for the elderly by inquiring into the current condition of facilities in America and in Korea, and in surveying the opinion of domestic elderly about day care facilities. A field trip to U.S. day care services was held between July 5 and July 15 in 1997, and an on-the-spot study for domestic facilities took place during March in 1998. Our research reveals that the overall supply of day care facilities can not meet future demand in terms of quality and quantity. Therefore a model must be created for day care centers of a that consists of a director from a professional group. an adequate environment, and a standardized in order to offer a qualified public health service linked to the home and community in Korea. The director of a day care center is a critical variable in determining the quality of service. Professional skills related to the needs of the elderly and the person's quality of service should be considered in appointing director for the center. This study belleves that a professional nurse should be the director of a day care center. The operating environment of a day care facility should be made up of considerable space comparable to the number of residents, should be in a comfortable and safe location, and should have equipment that provides a qualified, safe service to the elderly. Our model is designed for 20 persons and allocates 4 Peng per person. This model is comprised of a reading room. a craft room, a health room, a room for physical therapy, a dining room, a staff office, and a multi -purpose room connected to other rooms. Day care service should be a comprehensive service program meeting the multidimensional needs of the elderly. A comprehensive service program needs a team of various professionals made up of the elderly family, participants, nurses, social workers, physical therapists, nutritionists, and medical doctors. The program will also include health care service, physical therapy, speech therapy. diet, occupational therapy, transportation service, health and an education program, etc. In conclusion, a model of a day care center is developed with the following components: a professional director and an environment and program, that considers the physical, mental, and social characteristics of the elderly. A model should also motivate self-reliance self-fulfillment in the elderly in order to fulfill their health needs and to prevent isolation from society and mental depression. Furthermore, This facility will be a beneficial factor in reducing a family's burden on caring for the elderly that includes unnecessary hospital expenses. The following is a suggestion based on results this study: A service program should be developed to fit the conditions of the elderly in Korea by specifically analyzing the needs of the elderly.

Effect of Long-term Care Worker's Person-Centered Care on Service Quality of long term care facility -Focusing on mediating effects of long term care worker's emotional labor and job satisfaction- (요양보호사의 인간중심케어와 서비스 질의 관계 -요양보호사의 감정노동과 직무만족의 매개효과 중심-)

  • In this study, we analyze the relationships between long-term care workers' person-centeredness and job satisfaction, emotional labor, and service quality. Self-administered questionnaires were used to collect data from 460 long-term care workers in long-term care facilities. In the research model, person-centered care was verified using independent variables, with emotional labor (surface behavior, internal behavior) and job satisfaction applied as parameters, and service quality applied as a dependent variable. The results of this study are as follows. First, the fit of the model is good. Second, the implicit behavior of emotional labor is found to be an important factor affecting service quality. Also, emotional labor is an important mediator, improving the value of person-centered care and service quality. There is a positive correlation between internal behavior and service quality, and a negative correlation between surface behavior and service quality. Third, emotional labor (surface behavior, internal behavior) is the most influential variable in terms of service quality. The results of this study demonstrate the necessity to pay clinical and academic attention to person-centered care in terms of long-term care workers' emotional labor and service quality.

Determinants of Amount of Service Use in Community-Based Long-term Care for Elders (노인장기요양보험 재가서비스 이용량 결정요인)

  • Purpose: This study was done to explore factors related to amount of service use for elders with long-term care needs. Methods: A descriptive-correlation design was used. The sample included 259 elders and their primary caregivers who had cared for the elders for at least 6 months. Data on long-term care need assessment, service use and interviews with primary caregivers were analyzed. Results: There was no significant relationship between the sociodemographic characteristics and the amount of services use. Amount of service use differed significantly by Long-term care classification. The mean scores for class 1, 2 and 3 were 22.68, 21.47 and 17.87 days respectively. Primary caregiver relationship with the elders and the number of family-friend helpers were also significant. Multivariate regression analysis showed that gender, marital status, activities of daily living, cognitive impairment, and secondary caregiver support explained 17% of the total variance of service use among these elders (F=3.50, p<.001). Conclusion: The results of this study indicate that critical factors including secondary caregiver support and individual background, and other functional dependencies except for physical function should be considered in accurately predicting the amount of service use for community dwelling elders with long-term care needs.

A Study on the Operating Status of Community-based Home Health Care Centers (일개 지역사회 중심 가정간호사업소의 운영실태 및 운영방안)

  • Purpose: This study was done to evaluate the operating status of community-based home health center for revitalization of the centers. Method: In this study performance data including organization plan and service utilization plan were evaluated according process theory. Target of evaluation was the community-based home health care center. Results: The vulnerable part of the organization was the information system and financial resources. The home health center introduced PDA (Personal Digital Assistants) in 2005, however home health care nurses did not make full use of it. This service received full support from Seoul city and local government and there were no other sources of income. The vulnerable part of service utilization was service expansion and standardization due to vulnerability of organizational aspects. Conclusions: The home health care center provides high quality services to underprivileged people. In the future, these services should be provided with equity for continuous health care for this population.

  • We assess service delivery system for outpatients of general hospital(A) using service blueprint. Service blueprint for outpatients' service process currently being implemented in general hospital(A) is analysed to improve hospital services and define hospital service delivery system more accurately. In addition, comparative analysis of service blueprint between before and after improvement is conducted to find that health care services Is now more customer-oriented and hospital employees can link their duties to service delivery system. Dealing with the efficiency of health care service delivery system based on service blueprint analysis is expected to pave the way for continual service quality improvement of general hospitals in the future. The analysis of service blueprint of outpatients' service process suggested in this study is useful for setting strategies for health care service. It also helps service process design and service digitalization of other general hospitals in the future.

Facts and Needs of Comprehensive Child Edu-care Services in Daejeon (대전지역 보육서비스의 실태와 요구 분석 - 어린이집 교사를 대상으로)

  • The purpose of this study was to provide the basic data needed for developing comprehensive child edu-care programs that fits the regional needs. Subjects of the survey were the teachers of child edu-care centers in Daejeon. 95 teachers were selected from 20 child edu-care centers, using stratified random sampling method, from all of five municipal districts of the city. Gathered data were analyzed using SPSS 10.0 for reliability, factor analysis, frequency and percentile, mean, standard deviation, paired samples t-test and F-test. The summary of the findings were as follows: First, the result of survey on the child edu-care facts showed that the most child edu-care centers had the focus on caring and education programs of children, while emotional supports for family services and community social services were rather secondary importance. Therefore, to achieve the comprehensive service programs, child edu-care services should be harmonized by the education and welfare approaches. Second, centered around teacher's request, comprehensive child edu-care program that fits the regional characteristics focuses on family service; thus the program requires family group consulting, parents education etc., as well as focusing on community service to utilize local community's human, physical resources and to strengthen the tie between local community and child care services.

  • The purpose of this study was analyse the necessity for the Scam work at home care service. The subjects of the study were 14 home m nurses who work in community and in physical ana 16 physical therapist. The data were collected from Sep. 1 to Dec. 31, 1999. The result were analysed statistically by $X^2$, t-test. The main results of this study were as follows : 1. There were statistical significances of the general characteristics between the physical therapists and the home care nurses, 2. There were statistical significances of currencies of the home us service which the subjects experienced. 3. There were no statistical significances of the necessity of team work and the participation in home care service. 4. Regarding the effects of expectation which consisted 9 items, there were statistical significances in 8 items.

  • Due to the shortage of elderly care services in urban areas, multi-functional welfare facilities are proven to be very effective for delivering various service needs of elderly in Japan. Introduction of new longterm care policy for elderly in Korea would change many aspects of elderly care service facilities. Especially elderly home care services like adult daycare centers will expand drastically after beginning of elderly longterm care insurance. The purpose of this research is to study and analyze multi-functional welfare facilities in Japan focusing on the types of day service in those facilities. Planning of daycare centers in multi-functional welfare facilities for the elderly can be classified to 8 types. Those types are daycare centers with senior housing, longterm care insurance facilities, senior centers, small multi-function facility, medical facilities, educational facilities, community facilities and general housing projects. Each type has different benefits for the networking of services for the elderly. Design of daycare centers in multi-functional welfare facilities have distinctive features in entrance and user approach, space allocation and circulation planning. The study of daycare centers and multi-function planning should be followed to make better home care environments for the elderly in Korea.

Effects of Awareness of Patient Safety Culture, Emotional Labor and Job Stress on Patient Safety Nursing Activities by Comprehensive Nursing Care Medical Service Ward Nurses (간호·간병통합서비스 병동 간호사의 환자안전문화인식, 감정노동, 직무스트레스가 환자안전간호활동에 미치는 영향)

  • Purpose : This study aimed to examine the effects of the awareness of patient safety culture, emotional labor, and job stress on patient safety nursing activities by comprehensive nursing care medical service ward nurses. Methods : The participants were 112 nurses from the comprehensive nursing care service wards from six hospitals. Data were collected using structured questionnaires and analyzed using a t-test, ANOVA, Scheffé test, Pearson correlation coefficients, and multiple regression analysis using SPSS version 20.0. Results : The factors that significantly influenced patient safety nursing activities in the comprehensive nursing care medical service ward nurses included job stress (𝛽=-0.18, p<.032) and awareness of patient safety culture (𝛽=0.54, p<.001). The explanatory power of these factors was 31.0% (F=13.43, p<.001). Conclusion : This study showed that the factors that influenced patient safety management activities of comprehensive nursing care medical service ward nurses included job stress and awareness of patient safety culture.

Self-care Education Experience, Self-care Performance, and Home Care Service Needs of Patients With Rare and Incurable Diseases Who Use a Home Ventilator (가정형 인공호흡기를 적용하고 있는 희귀, 난치성 질환자의 자가관리 교육 경험과 수행정도, 가정간호 요구도)

  • Purpose: This study aimed to investigate the self-care education experience, self-care performance, and home care service needs of patients with rare and incurable diseases who use a home ventilator. Methods: From2 ventilator rental companies and 5 hospitals, records of 162 subjects were collected to obtain data regarding self-care education experience (23 items) and home care service needs (25 items). Results: Of the subjects, 65.4% were male and 55.6% had a myotrophic lateral sclerosis. The items with the highest and lowest levels of self-care education experience were "using the ventilator"(94.0%) and "community-available resource information"(27.0%), respectively. Meanwhile, self-care was performed well in terms of "personal hygiene"(59.6%) but not performed well in "community-available resource information"(23.7%). "Health status assessment" had the highest need(88.2%) of home care services. Generally, the need of home care services was higher for the patients using invasive ventilators than that for those using non-invasive ventilators. Conclusion: Home care nurses need to not only provide physical care for the patients but also strengthen their roles of enhancing patient access to and utilization of various community resources to provide systematic and individualized training and care.
