Blockchain Data Management for Pharmaceuticals

  • The care and quality provided to service users depend heavily on the documentation of medication administration. The results of medication management will be greatly impacted by the Medication Data Management (MDM) sheets, which will be examined during audits. Along with this impact, registered hospitals, care facilities, and residential homes will all be inspected by the healthcare industry. In order to deploy MDM sheets, it is proposed to create a blockchain prototype, or more specifically, to develop a blockchain-based Electronic Health Records (EHR) application. Confidentiality and confidence with the auditors are provided by the usage of permissioned blockchain technology (e.g., Care Quality Commission - CQC). Results from testing the prototype in two scenarios are positive. According to the findings, the use of EHR with permissioned blockchain can result in reminders being sent to medical practitioners as well as other effects.

가정간호 서비스 질 평가기준 설정을 위한 조사연구 (Survey of Home Healthcare Nursing Services to Establish Quality Assessment Standards)

  • Purpose: The study aimedto provide basic data to improve the quality of home healthcare nursing services by evaluating quality of care in representative nationwide sites. Method: The current quality of home care service in 104 nationwide sites was evaluated in terms of structures, processes, and outcomes based on published standards of the Joint Commission on Accreditation of healthcare Organizations. Results: The mean score for three dimensions of quality of home care service was as follows in descending order: structures (77.6), outcomes (60.4), and processes (38.7). Additionally, by specific item compared level of quality of home care servicein each site, the highest score was 97.3 and the lowest score was 42.3 out of 100, with a mean score of 74.7. Conclusions: These findings provide a base for establishing the quality management system and to develop a tool for evaluating the quality of home healthcare nursing. The result should be continuous management and improvement of home healthcare nursing quality.

의료의 질 개선 전문가의 자격 시스템에 대한 현황 (The review of qualifying systems of quality improvement specialists in healthcare)

  • Objectives: The purpose of this study is to provide comprehensive information of qualification systems of developed countries needed to establish our national system for QI(Quality improvement) specialists. Methods: All articles related to any applicable domestic or foreign countries' laws, operational status, and detailed programs for professional qualification system of QI were reviewed. Result: In the United States, a non-profit organization, Healthcare Quality Certification Commission (HQCC) has set the policies, procedures and standards in the field of health care quality. And qualification system of CPHQ (certified professional in healthcare quality) has been operated in order to authenticate the qualifications in the field of quality management. IBQH(international Board for quality in healthcare), a qualification system of experts in the United Kingdom, was designed to assist the qualification of professionals to improve the quality of healthcare. In addition, Health Research Center of Feinberg School of Medicine in Northwestern University has been operating Master's and doctoral degree programs in the field of the quality of care and patient safety and IHI (institute for healthcare improvement) open school was operating a professional training course related to the quality of care and patient safety. Conclusion: Quantity and complexity of information of the quality of care and patient safety have been increased. For reform of the health care system, a special training course of the expertise and leadership are needed. So far, there is no national professional certification courses in our nation. Therefore essential job skill should be acquired individually. For systematic and effective quality improvement activities, the educational and certification system with professional development model are needed.

해외 사례 고찰을 통한 지자체 노인요양시설 인증 체계 및 조사원 고도화 방안 (A Study on the Advancement of Accreditation Systems and Surveyors' Expertise for Long-term Care Facilities: Focusing on Overseas Cases)

  • Purpose: This study aimed to suggest strategies for advancing local-government-based accreditation systems and surveyor training in long-term care facilities in Korea. Methods: A comprehensive review of the literature including research papers and official reports issued by governments from the United States, Australia, and Japan was conducted to explore domestic and international policies related to long-term care facility certification and accreditation systems. Results: The USA has two types of care quality assurance systems including mandatory certification (5-star rating system) by the Centers for Medicare and Medicaid Services and voluntary accreditation by the Joint Commission on Accreditation of Healthcare Organizations. Australia operates a government-based mandatory accreditation system for all long-term care facilities through the Australian Aged Care Quality Agency. Japan, particularly the Tokyo district, operates a third-party evaluation system that involves the voluntary participation of long-term care facilities. Conclusion: This study provides several strategies to enhance accreditation processes and surveyors'expertise. For instance, motivating facilities to voluntarily participate in accreditation is necessary by 1) providing sufficient and continuous consultations and feedback about how to improve care quality, 2) differentiating accreditation domains and indicators from the national health insurance certification system, and 3) actively utilizing accreditation results and providing incentives.

  • This study was done to investigate the independent organizations established for patient safety, related policies, and the duties of experts in other countries. Australia established an organization called the Commission in 2006, the United Kingdom established the National Patients Safety Agency in 2001, and the United States assigned its work to the Agency for Healthcare Research and Quality in 2005. This was done by law in all three countries. The experts for patient safety were mainly called the "patent safety and quality coordinator", and although there was no qualification system for carrying out patient safety work, all three countries had licenses in the health care field or required more than 4-5 years of practical experience. The main duties were planning on patient safety and quality of healthcare service, data collection and analysis, and education, etc. and for this, competencies such as communication, leadership, and teamwork were required.

병원표준화심사의 질 향상 관련 항목에 대한 평가와 개선방안 (Opinions of Hospital Staffs on the Korean Hospital Standardization Program and Its Criteria for Quality Activities)

  • Background: Korean Hospital Standardization Program(KHSP), as a hospital accreditation program initiated and promoted by Korean Hospital Association since 1981, has played a key role in the hospital quality improvement in structural aspect particularly. Recently, however, KHSP has been critisized by hospitals and health personnels in that it is unpractical to be utilized as an initiative to improve hospital quality. In particular, the section of quality assurance in KHSP, strongly influenced by old version of Hospital Accreditation of Joint Commission in U.S.A., has been required to be fundamentally revised. For design of new criteria in quality assurance section, a survey for evaluation of existing program and collection of opinions for ideal one was conducted. Methods: For a month in 1994, structured questionnaires were administered by mailing to 470 hospital staffs from 200 hospitals, participated in the survey for the 1994 KHSP in each hospital. The total number of respondents was 116. Results: Less than half of the respondents(34.5%) value positively on the impact of current KHSP in general on the quality improvement of their hospitals. Moreover, most responses indicated that KHSP should be reorganized towards more practical and applicable one. Current KHSP criteria for quality activities in hospitals were regarded as a unpractical one which should be basically renewed. For new criteria and standards, most respondents emphasized the importance of applicability of those in real situations. Conclusion: For the KHSP to be effective, new evaluation criteria for quality activities should be more practical and fully accommodated to hospital situations in reality.

의료기관인증의 효과에 대한 체계적 문헌고찰: Balanced Score Card 관점으로 (Impact of Healthcare Accreditation Using a Systematic Review: Balanced Score Card Perspective)

  • Purpose: The purpose of this study was to analyze the impact of healthcare accreditation and to provide empirical evidence to validate positive effectiveness. Methods:Six electronic databases (KERIS, KoreaMed, NDSL, Ovid-medline, Embase, Cochrane library) were accessed in May 2016. Keywords used were 'accreditation' and 'Joint Commission on Accreditation of Healthcare Organization (JCAHO)'. Of the initially identified 3,008 articles, 60 studies on healthcare accreditation were selected based on inclusion criteria that are hospital accreditation, accreditation by disease and clinical center accreditation. These were retrieved and analyzed. Result: The 60 study results were on the impact of healthcare accreditation. Results were classified into four perspectives of Balanced Score Card (Financial, Customer, Internal Process, Learning & Growth). In internal process perspective, results revealed that healthcare accreditation has made a positive impact on "care process and procedure". In learning & growth perspective, healthcare accreditation has made a positive influence on "leadership", "organizational cultures" and "change mechanisms". However, it revealed that healthcare accreditation does not directly affect financial performance. It is also difficult to reach a definitive conclusion that healthcare accreditation programs affect patient satisfaction of customer and clinical outcome of the internal process. Conclusion: Healthcare accreditation programs provide positive impact on change of care process and building communication-oriented hospital culture. However, more rigorous and diverse research is needed on financial effects and clinical outcomes of healthcare accreditation.

대한민국 수용자 의료처우의 실질적 개선 (Substantial Improvement of Medical Care for Detainees in Republic of Korea)

  • The responsibility to ensure the health rights of detainees, particularly their medical rights, fundamentally lies with the state in all nations. However, in the correctional facilities of the Republic of Korea, these rights are currently not adequately safeguarded. Numerous detainees express dissatisfaction with the medical services provided and show a preference for voluntary external treatment. However, barriers such as prolonged application processes for external treatment and the requirement for detainees to cover their medical expenses present significant challenges. Therefore, the National Human Rights Commission of Korea has advocated for an increased medical budget in correctional facilities and a bolstered professional medical workforce to improve the medical care of detainees. Recommendations for improvements include: (1) establishing dedicated correctional hospitals for detainees, (2) setting up specialized correctional wards, (3) collaborating with military hospitals, (4) launching mobile medical buses for diverse specialties, (5) enhancing collaboration with public and private medical institutions, (6) increasing compensation for partnering external medical institutions, (7) improving the working conditions of medical officers, (8) safeguarding the defense rights of medical staff, (9) improving the working conditions of public health doctors from the Ministry of Justice in correctional facilities, and (10) pre-assigning public health specialists and military doctors to correctional facilities. By implementing these measures, it is anticipated that the quality of medical services in the Republic of Korea's correctional facilities will improve, reducing the demand for external treatments among detainees and ensuring their health and medical rights are realistically upheld.

영국 성인돌봄서비스 시장에 대한 감독 개혁과 한국 장기요양의 시사점 (The reform of inspection of adult social care market in the UK and policy suggestions for long-term care in South Korea)

  • 영국은 사회복지 돌봄에서 시장화를 유럽에서 가장 먼저 추진한 나라로 유명하다. 영국의 시장화 정책은 사회민주주의 국가에게도 영향을 끼칠 정도로 여러 나라가 시장화 정책을 도입해서 추진하고 있고 우리나라도 장기요양보험제도를 통해서 시장화 정책을 적극 도입했다. 본 연구의 목적은 영국 정부가 최근에 단행한 성인 돌봄 시장의 감독 강화 개혁을 분석하고 이를 토대로 한국 장기요양보험의 정책적인 시사점을 모색하는데 있다. 연구결과에 따르면, 영국은 시장을 적극적으로 관리 감독하고 있는 것으로 나타났다. 특히, 대규모 요양시설의 갑작스러운 파산 이후에 '2014년 돌봄 법'을 제정해서 돌봄품질위원회(Care Quality Commission)가 6단계별로 대규모 서비스 제공기관의 재정 상태와 서비스 질 등 측면에서 파산의 위험성을 사전적으로 분석하고 있다. 이를 통해서 제공기관이 일정한 수준으로 위험성이 높아지면 그 결과를 지방정부에 통보해서 파산과 같은 상황에 체계적으로 대응하도록 했다. 영국의 개혁은 장기요양 시장에 문제가 있는 한국에 시장에 대한 감독 강화, 새로운 감독 시스템 구축, 공공 감독 조직의 확대, 국민건강보험공단의 정보 공개 등의 정책적인 시사점을 제시한다.
