Clinical Outcomes of Arteriovenous Graft in End-Stage Renal Disease Patients with an Unsuitable Cephalic Vein for Hemodialysis Access

  • Background: As the population of patients with end-stage renal disease has grown older, the proportion of patients with poorly preserved vasculature has concomitantly increased. Thus, arteriovenous grafts (AVG) have been used more frequently to access blood vessels for hemodialysis. Despite this increasing demand, studies of AVG are limited. In this study, we examined the surgical outcomes of upper-limb AVG creation. Methods: Among the arteriovenous fistula formation procedures performed between January 2014 and March 2019 at Dankook University Hospital, 42 cases involved AVG creation. We compared patients in whom the axillary vein was used (group A; brachioaxillary AVG [B-Ax AVG]; n=20) with those in whom upper limb veins were used (group B; brachiobasilic AVG or brachioantecubital AVG; n=22). Results: The 1-year primary patency rate was higher in group A than in group B (57.9% vs. 41.7%; p=0.262). The incidence of postoperative complications was not significantly different between groups. Conclusion: AVG using the axillary vein showed no major differences in safety or functionality compared to AVG using other veins. Therefore, accounting for age, underlying disease, and expected patient lifespan, B-Ax AVG can be considered an acceptable surgical method.

Surgical Treatment of Metastatic Lung Cancer (전이성 폐암의 외과적 치료)

  • There are follow-up data according to thirteen patients recieved the surgical resection for metastatic lung cancer arising from different primary tumor. The patients were received the surgical resection at Korean Cancer Center Hospital from July 1987 to Setember 1991 and followed-up to August 1992. There were 9 men and 4 women, ranging in age from 16 to 70 years[mean age, 42.8 years]. The primary tumors were 2 synovial sarcoma, 2 sarcoma, 2 osteosarcoma, 3 laryngeal ca, 1 melanoma, 1 ovarian ca and 1 bladder ca. The operative procedures were 5 wedge resections, 1 segmental resection, 5 lobectomies, 1 bil-obectomy and 1 pneumonectomy. There was no operative and hospital death. There were 3 deaths[each survival period: 2, 9 and 20 months, average 10.3 months]and 5 tumor recurrence during follow-up. At now, the average survival period of aliving patients is 29.1 months.

Early Surgical Intervention of Active Infective Endocarditis (심내막염 환자의 수술적 치료)

  • During one year period from Sep. 1986 to Sep. 1987, we have experienced 6 cases of infective endocarditis requiring surgical interventions. All 6 patients had class IV or V cardiac disability at the time of surgery. The indication for surgery was rapidly progressive congestive heart failure in all cases. Four patients underwent aortic valve replacement including one double valve replacement. Two other patients required other surgical procedures, removal of large left atrial vegetation mass in one patient and excision of destroyed pulmonary valve and aortic vegetation in the other patient. Two patients died; one of mitral annulus rupture after release of aortic clamp and the other of mediastinal bleeding 3 months after replacement of aortic valve. Three out of 4 survivors are in NYHA Class I and the remaining patient is in Class II. We emphasize that early operative intervention is life-saving in patients with persistent or progressive congestive heart failure, irrespective of the activity of the infective process or the duration of antibiotic therapy.

  • Congenital long-segment tracheal stenosis which involves nearly entire trachea and carina is very rare disease, but leads to life threatening obstruction in infancy and childhood. Symptoms are ranged from stridor and wheezing to severe cyanosis and respiratory failure. Routine chest X-ray is somewhat helpful to diagnose it, but definitive diagnosis can be made by bronchoscopy or tracheogram for severely narrowed tracheal lumen.Recently, we experienced a case of congenital tracheal stenois, type 1 by Cantrell classification with carinal involvement. After costal cartilage was designed as oval shaped flap and covered with pericardium, anterior and posterior augmentation was done with prepared costal cartilage.This patient died of respiratory failure at 13 days postoperatively, probably due to sustaining obstruction in association in with failure to make a sufficient widening at carinal level.Important issues in the management of congenital tracheal stenosis are rapid diagnosis, selection of appropriate surgical procedure, and detailed anesthetic schedule.In the future, more biocompatible material and more effective surgical procedures should be studied to reduce the surgical mortality and morbidity of the complicated tracheal stenosis.

Surgical Treatment of Lung Cancer with Bronchoplastic Procedures (기관지 성형술을 이용한 폐암의 수술적 치료)

  • Thirteen patients with lung cancer have undergone bronchoplastic procedures from January 1991 to July 1994 in Kyung Hee univ. Hospital.Among them,twelve patients were male and one patient was female from 33 to 76 years of age.[ mean 56.6 11.5 years of age The pathologic diagnoses were squamous cell carcinoma in twelve patients and adenoid cystic carcinoma in one patient.The bronchoplastic procedures consisted of seven right upper sleeve lobectomies, one right middle sleeve lobectomy, one left lower sleeve lobectomy and four sleeve pneumonectomies. In these sleeve pneumonectomies, right sleeve pneumonectomy was done in three patients and the other was left sleeve pneumonectomy. Including the four sleeve pneumonectomies, carinoplasty was performed in seven patients. Postoperatively, we carried out bronchoscopic suction of intrabronchial secretions in necessary for the prevention of postoperative complication. All except one were discharged without any complication and have been in good condition. Postoperative mortality was in one patient who expired 25days after right middle sleeve lobectomy with respiratory failure.

  • Pulmonary arteriovenous malformation (PAVM) is a rare anomalous direct communication between the pulmonary artery and vein with a considerable risk of serious complications such as cerebral thromboembolism or abscess and pulmonary hemorrhage. Although the past, surgical resection such as lobectomy was mostly used to treat PAVM, the recent development of endovascular treatment has made it a primary consideration to perform transcatheter embolization using coils or detachable balloons. We report a case of successful transcatheter embolization of giant PAVM with the second generation Amplatzer vascular plug II as a new self-expanding device.

A case of endobronchial hamartoma (기관지내에 발생한 과오종 치험 1례)

  • Hamartoma has been considered rare disease as a congenital malformation of a tumorous lesion since the original description by Albrecht in 1904. Pulmonary hamartoma is interesting to chest surgeon because of good result by surgical procedures and of slightly high incidence then another organs. It is characterized as lesion of very slow growing mass as peak incidence of age of 50 years age group and as sex ratio is 2:1. Especially endobronchial hamartoma is rarer than peripheral type. We had experienced a case of endobronchial hamartoma who has 53 years old male patient at right main bronchus and then treated by excision of tumor mass through right bronchotomy and preserving right lung except any lobectomy and pneumonectomy.

  • Here presented six cases of benign tumors of the lung treated surgically between Dec. 1957 and Dec. 1983 in the Department of Thoracic Surgery, Seoul National University Hospital. They include four cases of hamartoma, one case of benign mesothelioma, and one case of Castlemans disease. The operative procedures are three cases of lobectomy, one case of segmentectomy, and two cases of mass excision. The operative results are excellent without complication.

  • Forty patients with spontaneous pneumothorax underwent vertical axillarty thoracotomy for surgical bullectomy of spontaneous pneumothorax between June, 1991 and september, 1992. We evaluated the method in terms of postoperative pain, wound complication, days of hospital stay and cosmetic result. It`s concludid that verical axillary thoracotomy provides satisfactory exposure for limited procedures within the thoracic cavity and offers the specific advantages of minimal and cosmetically acceptable results when compared with the thoracotomy methods.

  • Reconstructive surgical procedures for hypopharyngeal and cervical esophageal defects have still a lot of technical defficulties and varieties to be performed as a optimal treatment according to the clinical situation patient faced. We have experienced a case of successful reconstruction of cervical esophageal defect, which was resulted from graft failure of free jejunal transfer in 43 year old male with eso-phagocutaneous fistula, using free fasciocutaneous dorsalis pedis flap. This article describes the review of our case and literature relevant the reconstructive maneuvers of cervical esophageal defects.
