Metabolic syndrome and homocysteine are associated with increased independent risk factors of cardiovascular disease. We investigated the difference between the degree of obesity, metabolic syndrome risk factors, plasma homocysteine and anthropometric obesity factors. Totally 398 participated in a medical check-up program were selected for this study. Subjects were categorized into three groups according to the number of metabolic syndrome components present as defined by the NCEP-ATP III criteria; Absent (0 criteria, n=124), Pre-MetS (1-2 criteria, n=220) and MetS (${\geq}3$ criteria, n=54). Body mass index (BMI) is a measure used to distinguish between normal weight, overweight and obesity. MetS presented higher homocysteine than Absent (p<.05) and obesity higher than normal weight (p<.01). When Absent+Pre-MetS was used to classify obese or not, obesty presented higher homocysteine than non-obese (p<.05). Further homocysteine levels positively correlated with weight, BMI, waist circumference, hip circumference and waist-hip ratio (WHR). Especially WHR is not only MetS (r=0.378, p<.001) but also Absent+Pre-MetS (r=0.305, p=0.029) significantly positively correlated with homocysteine. The results of our study indicate that homocysteine is related closely to obesity. Although obesity has not been diagnosed with metabolic syndrome, obesity related with increased homocysteine.
BACKGROUND/OBJECTIVES: The prevalence of obesity, a worldwide pandemic, has been increasing steadily in Korea. Reports have shown that increased intermuscular adipose tissue (IMAT) is associated with an increased risk of cardiovascular disease, independent of body mass index. However, the relationship between dietary intake and IMAT accumulation in the Korean population remains undetermined. The objective of this study was to evaluate regional fat compartments using advanced magnetic resonance imaging (MRI) techniques. We also aimed to investigate the association between IMAT amounts and dietary intake, including carbohydrate intake, among Korean individuals with obesity. SUBJECTS/METHODS: This cross-sectional study, performed at a medical center in South Korea, recruited 35 individuals with obesity (15 men and 20 women) and classified them into 2 groups according to sex. Anthropometry was performed, and body fat distribution was measured using MRI. Blood parameters, including glucose and lipid profiles, were analyzed using commercial kits. Linear regression analysis was used to test whether the IMAT was associated with daily carbohydrate intake. RESULTS: Carbohydrate intake was positively associated with IMAT in all individuals, with adjustments for age, sex, height, and weight. No significant differences in blood indicators were found between the sexes. CONCLUSIONS: Regardless of sex and age, higher carbohydrate intake was strongly correlated with greater IMAT accumulation. This suggests the need to better understand sex differences and high carbohydrate diet patterns in relation to the association between obesity and metabolic risk, which may help reduce obesity prevalence.
Purpose: To investigate factors related to cardio-cerebrovascular disease and groups disadvantaged by it in Korean middle-aged women, using the Korea National Health and Nutrition Examination Survey (KNHANES). Methods: The present study was conducted with 1,627 middle-aged women, aged 40 to 64 years, who participated in the 7th (2016) Korea National Health and Nutrition Examination Survey conducted by the Korea Centers for Disease Control and Prevention. Results: Cardio-cerebrovascular disease among middle-aged women was associated with quality of life, menopause, diabetes mellitus, body mass index, and family history of hypertension. The incidence of cardio-cerebrovascular disease in middle-aged women was found to be the most prevalent in women who have entered menopause, have a family history of hypertension, and have a body mass index greater than $30.0kg/m^2$. Conclusion: This study classifies the subjects according to the risk level of each disadvantaged group for cardio-cerebrovascular disease prevention and management in middle-aged women. The results provide evidence to support a tailored cardio-cerebrovascular disease prevention and management program based on the related factors of disadvantaged groups and to establish strategies in educational and practical aspects.
In hypertensives, electrocardiographic left ventricular hypertrophy (LVH) is an independent risk factor for cardiovascular disease. Animal and human studies suggest an association between left ventricular mass and dietary sodium (Na) intake. This study determined if dietary Na intake in a homogenous ethnic population is associated with electrocardiographic LVH (S in Vl + R in $V5{\geq}5.5 mV$). Blood pressure (BP), body mass index, EKG, and 24 hour urine Na and potassium (K) excretion were determined in 40 otherwise healthy Korean patients with untreated essential hypertension on the standard Korean diet. Among these variables, only Na excretion (mmol/day) was significantly different between those with and without LVH $[LVH+:357{\pm}50,\;LVH-;\;268{\pm}25(p=0.04]$. Thus, dietary sodium intake may be predictive of electrocardiographic LVH.

Background: Risk assessment for pulmonary resection in patients with early-stage non-small-cell lung cancer (NSCLC) is important for minimizing postoperative morbidity. Depletion of skeletal muscle mass is closely associated with impaired nutritional status and limited physical ability. We evaluated the relationship between skeletal muscle depletion and early postoperative complications in patients with early-stage NSCLC. Methods: Patients who underwent curative lung resection between 2016 and 2018 and who were diagnosed with pathological stage I/II NSCLC were included, and their records were retrospectively analyzed. The psoas volume index (PVI, ㎤/㎥) was calculated based on computed tomography images from routine preoperative positron emission tomography-computed tomography. Early postoperative complications, defined as those occurring within 90 days of surgery, were compared between the lowest sex-specific quartile for PVI and the remaining quartiles. Results: A strong correlation was found between the volume and the cross-sectional area of the psoas muscle (R2=0.816). The overall rate of complications was 57.6% among patients with a low PVI and 32.8% among those with a normal-to-high PVI. The most common complication was prolonged air leak (low PVI, 16.9%; normal-to-high PVI, 9.6%), followed by pneumonia (low PVI, 13.6%; normal-to-high PVI, 7.9%) and recurrent pleural effusion (low PVI, 11.9%; normal-to-high PVI, 6.8%). The predictors of overall complications were low PVI (odds ratio [OR], 2.18; 95% confidence interval [CI], 1.07-4.09; p=0.03), low hemoglobin level (OR, 0.686; 95% CI, 0.54-0.87; p=0.002), and smoking history (OR, 3.93; 95% CI, 2.03-7.58; p<0.001). Conclusion: Low PVI was associated with a higher rate of early postoperative complications in patients with early-stage NSCLC.

Background and Objectives: The prognostic value of left atrial (LA) function in terms of long-term clinical outcomes after mitral regurgitation (MR) surgery remains unclear. Therefore, we investigated the impact of preoperative LA global longitudinal strain (LAGLS) on the long-term postoperative clinical outcomes in chronic severe MR patients who underwent mitral valve (MV) repair surgery. Methods: From January 2012 to December 2017, we analyzed 338 patients (mean age, 51.9±12.5 years; 218 males [64.5%]) treated with MV repair surgery for severe MR. The primary outcome was cardiovascular events, defined as the composite of all-cause death, newly developed atrial fibrillation (AF), and re-hospitalization for cardiovascular causes. Results: During a median follow-up of 45 months (interquartile range, 26-65), 30 (8.9%) cardiovascular events, 5 (1.5%) all-cause death, 8 (2.4%) newly developed AF, and 26 (7.7%) re-hospitalizations occurred. On multivariable analysis, baseline LAGLS was an independent predictor of cardiovascular events (adjusted hazard ratio [HR], 0.91; 95% confidential interval [CI], 0.85-0.97; p=0.004) and re-hospitalization (adjusted HR, 0.93; 95% CI, 0.86-1.00; p=0.037). According to the optimal cutoff value of LAGLS, patients with low LAGLS (<23.6%) had a significantly higher risk of cardiovascular events (adjusted HR, 2.70; 95% CI, 1.04-7.00; p=0.041) than those with high LAGLS (≥23.6%). In a subgroup analysis, patients with high LAGLS had better clinical outcomes regardless of whether the patient had a LA volume index <60 mL/m2. Conclusions: In patients with chronic severe MR who received successful MV repair surgery, preoperative LAGLS is an independent predictor of long-term postoperative outcomes.
Lifestyle and dietary behavior intervention as the primary prevention of lipid disorder seems safe and compatible with other treatments of cardiovascular diseases. Cross-sectional associations between lifestyle factors and dietary behavioral factors with plasma lipid and lipoprotein levels were analyzed in 189 middle-aged men in Suwon, Korea. Overnight fasting plasma levels of total cholesterol, high-density lipoprotein(HDL)-cholesterol, triacylglycerol and glucose were analyzed. Blood pressure and anthropometric data were also measured. Lifestyle factors such as smoking status, alcohol consumption and frequency of physical exercise were evaluated by a self-administered questionnaire. Questions regarding dietary behavior were also asked. The subjects were 43.8%${\pm}$7.9 years old, and 23.8%${\pm}$2.6kg/m$^2$. From stepwise regression analyses, significant correlates with total cholesterol level were body mass index(BMI), alcohol intake(negative), age and coffee drinking(model R$^2$=14.3%). BMI, breakfast-skipping, age, and sleeping hours were significant for triacylglycerol level(model R$^2$=15.8%). BMI, alcohol drinking(negative), age, and coffee drinking were significant for low-density lipoprotein(LDL)(model R$^2$=11.7%). Age(negative), BMI(negative), alcohol drinking, stress level(negative), physical exercise, and cigarette smoking(negative) were significant for high-density lipoprotein(HDL)(model R$^2$=12.1%). From stepwise regression analyses, excluding BMI and age as factors in the model, alcohol intake(negative) and coffee drinking were significantly correlated with total cholesterol level(model R$^2$=4.4%) : breakfast-skipping with triacylglycerol(model R$^2$=3.2%) : alcohol intake (negative) with LDL level(model R$^2$=3.4%) : alcohol intake, physical exercise and stress level(negative) with HDL level(model R$^2$=6.3%). The findings suggest that a healthy daily lifestyle and dietary behavior may have an anti-atherogenic effect by altering plasma lipid and lipoprotein levels in middle-aged Korean men. The purpose of this research was to examine the relationship between the plasma LDL particle size and blood lipid profile, dietary factors and anthropometric values (body mass index, waist circumference and waist/hip ratio). The subjects were 173 adults aged 23 to 81 years, selected from the Outpatient Clinic and Cardiovascular Department of the Seoul Municipal Hospital. Dietary data were obtained using a 3-day food record and analyzed using Korean and US nutrient databases. The subjects were divided into three groups by LDL particle size : type A (large buoyant LDL, > 25.5 nm, n=96), type I (Intermediate LDL,$25.2\leq-\leq25.5$ nm, n=18), and type B (small dense LDL, < 25.2 nm, n=59) groups. The type B group had higher age, waist circumference, and waist/hip ratio (WHR) than the type A and type I groups. Serum concentration of triglyceride, Apo B, LDL/HDL cholesterol ratio and atherogenic index were significantly higher in the type B group as compared to those in the other two groups. HDL cholesterol level and Apo A-I/Apo B ratio were significantly lower in the type B group than the other two groups. The plasma LDL particle size was highly correlated with triglyceride (r= -0.450), Apo B (r= -0.402) and HDL cholesterol (r= 0.418). However, there was no correlation between plasma LDL particle size and dietary intakes. This study showed that small dense LDL was an important biochemical risk factor that was associated with other risk factors.

Hypertension and obesity are important modifiable risk factors for cardiovascular disease, the leading cause of death in Korea. Therefore, we assessed the association between dietary pattern and obesity in hypertensive patients to formulate health promotion strategies for the older population. Dietary information was collected from hypertensive patients visiting community health education and information center by using 24 hour recall method. The 2005 DRIs for Koreans was used to evaluate the dietary adequacy. When subjects were categorized by body mass index (BMI) as normal, overweight and obese, no significant difference in energy intake was found among groups. Dietary intakes of folate, and vitamin C in obese hypertensive patients were significantly lower than in normal weight patients (p < 0.05). When we compare the nutritional status by waist circumferences, dietary intakes of zinc, vitamin A, thiamin, vitamin C and folate were significantly lower in the obese group. Vegetable intake was significantly lower in the obese group according to BMI as well as waist circumference. Energy intake from carbohydrate was significantly higher in obese hypertensive patients (p < 0.05). Obese hypertensive patients had a higher risk of nutritional inadequacy compared to normal weight patients. Our results indicated the need for developing interventions that encourage greater consumption of vegetables while cutting down salt intake with wise selection of staple foods, for obese hypertensive patients.
Purpose: Dyslipidemia was a known risk factor for cardiovascular disease and was a leading cause of mortality in worldwide. This study aimed to determine the factors associated with prevalence and medication treatment of dyslipidemia in Korean adult population. Methods: In this study, based on the criteria set by the Korean Society of Lipidology and Atherosclerosis, the factors associated with prevalence and medication treatment of dyslipidemia was evaluated in a population of 12,506 people ($age{\geq}20$), who participated in the Korean National Health and Nutrition Examination Survey (2013-2015). The findings were tested by using multivariate logistic regression. Results: Dyslipidemia prevalence rate was 36.5%. Among populations with dyslipidemia, 17.5% were treated with lipid-lowering drugs. In the multivariate logistic regression model, male, increase in age, lower education level, non-drinker, current smoking, less physical activity, increase of body mass index, hypertension, and diabetes were associated with an increased odd of dyslipidemia. Female, increase in age, higher income, excess fat intake, hypertension, diabetes, myocardial infarction, and angina were associated with an increased odd of medication treat. Conclusion: The results of this study could be used to screen patients at the high risk for dyslipidemia or to predict medication adherence.
