본 연구는 비보건계열 대학생의 자동제세동기와 심폐소생술에 대한 지식, 태도 및 수행능력을 조사하고 이들 간의 상관관계를 파악하기 위해 시도되었다. 2015년 2월~3월 사이 D시에 위치한 4년제 대학 중 무작위로 3개 학교 6개 비보건계열 학과를 선정하여 재학 중인 대학생 291명을 대상으로 하였다. 대상자의 77.0%가 자동제세동기를 '알고 있다'고 응답하였으나 61.9%는 자동제세동기 사용 방법을 전혀 모르고 24.4%는 잘못 사용할까봐 두려워하는 것으로 나타났다. 자동제세동기와 심폐소생술에 대한 지식 및 태도, 수행능력 점수는 심폐소생술 교육을 받은 경험(p<.002)과 자동제세동기 인식도(p<.001)에 따라 유의한 차이를 보였으며, 자동제세동기에 대한 수행능력은 지식(r=. 42, p<.001) 및 태도(r=. 55, p<.001)와 유의한 양의 상관관계가 있는 것으로 나타났다. 따라서 비보건계열 대학생의 자동제세동기와 심폐소생술의 수행능력을 높이기 위해 서는 반복적인 실습교육을 통한 자신감의 함양과 더불어 태도의 변화를 유도할 수 있는 교육프로그램의 개발이 필요할 것이다.
본 연구는 요양보호사들의 심폐소생술 교육경험과 태도가 심폐소생술 수행자신감에 미치는 영향을 파악하고자 실시한 서술적 조사연구로 2017년 2월에서 4월까지 C도 내에 근무 중인 요양보호사 140명을 대상으로 조사하였다. 수집된 자료는 SPSS/WIN 20.0 program을 이용하여 분석하였다. 심폐소생술 교육경험과 태도와 수행자신감의 비교는 t-test, ANOVA로 분석하였고, 사후검정은 Scheffe' test로 분석하였다. 대상자의 일반적인 특성과 심폐소생술에 대한 태도 및 수행자신감의 상관관계는 Pearson correlation coefficient로 분석하였고, 심폐소생술 수행자신감에 미치는 영향요인은 Multiple regression을 이용하여 분석하였다. 연구 결과 심폐소생술에 대한 태도와 수행자신감은 높은 상관관계가 나타났으며(r=0.41, p<.001), 심폐소생술 수행자신감에 영향을 미치는 요인으로는 긍정적인 심폐소생술과 관련된 태도(${\beta}=0.34$)와 4시간 이상의 교육시간(${\beta}=0.28$)으로 나타났다. 본 연구 결과를 바탕으로 요양보호사들이 노인 요양보호시설에서 발생하는 심정지 응급상황에 적절한 대처를 수행할 수 있도록 심폐소생술에 대한 지속적인 교육이 필요함을 알 수 있었으며 이를 위해서는 요양보호사를 위한 맞춤형 심폐소생술 교육 프로그램 개발이 진행되어야 할 것이며 요양보호사 직무교육 또는 보수교육에 심폐소생술에 대한 교육과정이 포함되는 방안이 마련되어야 할 것이다.
본 연구는 고등학교 남학생에서 심폐소생술에 대한 전통적교실학습과 영상자가학습의 교육효과를 통하여 심폐소생술 교육에 효과적인 교육방법 선택에 도움을 주며, 심폐소생술 교육 프로그램 마련에 기초 자료를 제공하고자 하였다. 연구대상자는 G도 소재 G고등학교 2학년 남학생 두 반의 학생으로 1개 반은 실험군, 1개 반은 대조군으로 임의 배정 하였다. 연구 결과는 다음과 같다. 첫째, 심폐소생술 교육 전 후 지식 차이는 대조군(2.38)이 실험군(2.32)에 비해 높게 증가하였지만, 태도 차이는 실험군(2.18)이 대조군(2.06)에 비해 높게 증가하였다. 지식과 태도에서는 통계적으로 유의한 차이는 나타나지 않았다. 둘째, 심폐소생술 교육 전 후 자신감 차이는 실험군(2.40)이 대조군(2.01)에 비해 유의하게 증가하였고(p<0.05), 술기수행 정확도 차이도 실험군(2.67)이 대조군(2.54)에 비해 유의하게 증가 하였다(p<0.05). 본 연구 결과 심폐소생술의 자신감, 술기수행 정확도에서 영상자가학습이 전통적교실학습보다 유의하게 증가하였다. 따라서 영상자가학습 교육 방법을 심폐소생술 교육에 활용할 필요가 있다.
본 연구의 목적은 비외상성 심정지 환자에게 적용하는 심폐소생술에 대한 현장업무 프로토콜의 유용성을 검증하는 것이다. 연구는 2008년 5월 1일부터 동년 6월 27일까지, 전라남도와 전라북도에 소재한 J, K대학에 재학 중인 응급구조과 학생 150명을 대상으로 하였다. 비동등성 대조군 전.후 유사 실험 설계(nonequivalent control group pretest-posttest design)를 사용하여, 실무적합성 검증을 실시하였다. 현장업무 프로토콜을 적용한 실험군과 기존의 심폐소생술을 적용한 대조군으로 나누어, 각 프로토콜 항목들의 소요시간을 비교하여 통계 분석하였다. 연구 결과, 5개항목(의식상태 평가, 기도유지, 인공호,,흡 2회 실시, 순환 확인, 심폐소생술 5주기 실시)을 제외한 나머지 18개 항목에서 3초 이상 수행시간이 단축되었다. 60초 이상의 단축을 보인 항목은 6개로, 기관삽관 튜브 고정(78.70초), 말초정맥로 확보(64.45초), 의식상태 재평가, 동공반사 확인(110.05초), 목동맥 확인(112. 55초), 활력징후 평가(85.7초)였으며, 전체시간은 110.85초가 단축되었다. 실험군과 대조군의 프로토콜 적용에 따른 사전.사후 변화에서 전체시간(t=-6.580, p=.000)은 통계학적으로 유의하게 감소한 것으로 나타났다. 따라서 비외상성 심정지 환자의 심폐소생술에 대한 현장업무 프로토콜은 정확하고 신속한 의사결정을 지원할 수 있는 지침으로 활용될 수 있으며 응급의료 서비스를 향상시킬 것으로 사료된다.
Purpose : This study set out to compare the educational effects of a video self-instruction program for child CPR education on childcare teachers by applying the 2006 KACPR Guideline. By adopting the nonequivalent control group posttest quasi-experimental design, the study examined the educational effects on a group that did not receive instructions from the instructor, another group that received his instructions, and the other group that received an extra three-minute practice training session in addition to instructions. Methods : Data were gathered from August 6 to 18, 2008. As for research tools, the Knowledge Instrument of CPR by Connolly (2006) was used along with the National Practice Test Protocol for C1ass 1 Emergency Medical Technicians (2007) and Common Protocol for CPR (2006) to examine the performance of child CPR. By shooting the guide screen of $Resusci^{(R)}$ Junior CPR Manikin of Leardal with a video camera and using the Skill Guide Checklist of the Common Protocol for CPR (2006), the subjects' technical accuracy of chi1d CPR was evaluated. There were three subject groups: 29 childcare teachers randomly assigned to received the video self-instruction program training for chi1d CPR and no instructions from the instructor made up the control group; 22 childcare teachers randomly assigned to received the program training and instructions from the instructor made up experiment group I; 23 childcare teachers randomly assigned to received an extra three-minute practice training session in addition to the program training and the instructions made up experiment group II. The gathered data were analyzed with SPSS/PC+ (Version 14.0) in frequency, percentage, $X^2$-test, ANOVA, Scheffe test. Results : 1) There were no statistically significant differences (F=1.030, p=.362) among the groups in terms of knowledge scores after the child CPR education. 2) There were statistically significant differences (F=13.625, p=.000) among the groups in terms of performance abilities after the child CPR education. 3) There were no statistically significant differences (F=1.610, p=.207) among the groups in terms of technical accuracy of mouth-to-mouth resuscitation after the child CPR education 4) There were no statistically significant differences (F=1.484, p=.234) among the groups in terms of technical accuracy of chest compression after the child CPR education. Conclusion : The results indicate that childcare teachers can improve their performance abilities in child CPR when the instructors are active with their instructions and extra practice hours are secured through a VSI program. It's also needed to provide education with increasing concentration ratio about the items of lower knowledge points in order to help the teachers learn the accurate theory of child CPR. And there should be VSI programs of diverse conditions to increase the effects of child CPR training among childcare teachers.
Purpose: The purpose of the study is to emphasize the importance of out-of-hospital cardiac arrest resuscitation. This resuscitation by paramedic is very effectively performed under the medical direction of the doctors. Methods: The cardiac arrest victim was 4 month old infant. Informed consent from the parents of the infant was received. CPR combined with 150J defibrillation was performed to the 4 month old infant. Results: We reported that the 4 month old infant survived the cardiac arrest. Out-of-hospital cardiac arrest infant survived after 150J automated external defibrillator and CPR performance. Conclusion: Specific operative protocol is important because the paramedic can apply the proper manual defibrillator effectively. It is important to extend the work scpoe of the EMT.
59세의 기왕력 및 가족력에 특이사항이 없는 남자가 가슴통증을 호소하다 의식을 잃고 쓰러지자 이를 목격한 가족이 119에 신고하였다. 응급의료상담원의 도움에 의해 목격자가 심폐소생술을 시행하였고 거주 중인 아파트에 설치된 자동심장충격기를 이용하여 제세동 1회를 시행하였다. 이후 도착한 119구급대에 의해 제세동 2회 실시 후 자발순환회복되어 인근 응급의료센터로 이송되었으며, 저체온 치료 15일 후 대퇴수행분류 1점으로 퇴원하였다. 현재 우리나라의 응급의료전환상담원 도움에 의한 목격자 심폐소생술 및 제세동의 시행은 시작 단계이지만 본 증례를 통해 충분한 효과를 볼 수 있다고 판단되며, 이에 따라 응급의료전화상담원의 적극적인 자동심장충격기 사용 안내와 그에 맞는 체계적인 교육이 필요하다. 특히 훈련받지 않는 목격자의 자동심장충격기의 사용을 도울 수 있고, 119구급대가 현장까지의 반응시간이 지연되는 원거리 지역에서 중요한 의미를 가질 것으로 사료된다. 또한 우리나라 PAD 프로그램의 양적인 보급뿐만 아니라 목격자의 접근성을 높일 수 있는 방안이 될 것이다.
Purpose: This study aimed to propose an effective one-rescuer infant cardiopulmonary resuscitation (CPR) chest compression technique by comparing the differences in efficacy, convenience, and pain levels between the two thumb-encircling and two finger techniques. Methods: Subjects were randomized to perform either two-thumb-encircling or two-finger technique for 8 minutes each on infant CPR manikins. After the chest compression, a survey was administered to the subjects to measure convenience and pain levels according to compression method. Results: Total compression depth over 8 minutes was significantly deeper for the two-thum-encircling technique ($43.5{\pm}4.8mm$) compared with the two-finger technique ($32.6{\pm}5.4mm$) (p<0.001). In terms of compression depth measured at 1-minute intervals, compression depth with the two-finger technique decreased from $38.3{\pm}4.23mm$ to $29.0{\pm}6.79mm$, whereas compression depth with the two-thumb-encircling technique did not show a significant change (from $43.7{\pm}4.12mm$ to $43.4{\pm}5mm$). The results of the survey indicated that, the majority of subjects found the two-thumb-encircling technique to be the most comfortable technique for compression depth (n=29, 64.4%). The majority of subjects (n=31, 68.9%) answered that the two-finger technique resulted in the most pain. Conclusion: Comparison of efficacy of the two-thumb-encircling and two-finger-techniques in the performance of one-rescuer infant CPR revealed that the two-thumb-encircling technique was more effective in maintaining chest compression depth.
Objective : This study is an experimental study which is designed to examine the differences between knowledge and self-confidence before and after theory education(CPR PPT material) based on guidelines of CPR and emergency cardiac treatment of American Heart Association(AHA, 2005) and video self-instruction program for the general public by Korean Association of Cardiopulmonary Resuscitation(KACPR), trace CPR performance ability after CPR and AED education and investigate the accuracy of artificial respiration and chest compression, and know the difference in CPR performance abilities including AED. Methods : Subjects of this study include ground crews and staffs at M airport in G province equipped with emergency equipments for CPR according to Art. 47, Sec. 2 of Emergency Medical Law, airport police, rent-a-cops, security guard, quarantine officer, custom officer, and communication, electricity, civil engineering, facility management staff, airport fire fighting staff, air mechanic, traffic controller, and airport management team among airport facility management staffs. They were given explanation of necessity of research and 147 of 220 subjects who gave consent to this research but 73 who were absent from survey were excluded were used as subjects of this study. of 147 subjects, there were 102 men and 45 women. Results : 1) Knowledge score of CPR was $6.18{\pm}0.87$ before instruction and it was increased to $15.12{\pm}1.78$ after instruction, and there was statistically significant difference. 2) Self-confidence score in CPR was $3.16{\pm}0.96$ before instruction and it was increased to $7.05{\pm}0.75$ after instruction, and there was statistically significant difference. 3) Total average score in CPR performance ability after instruction was 7.46 out of 9, performance ability was highest in confirmation of response as 144(97.95%), follwed by request of help as 140(95.25%) and confirmation of respiration as 135(91.83%), and lowest in performing artificial respiration twice(gross elevation of chest) as 97(65.98%). Accuracy of artificial respiration(%) was $28.60{\pm}16.88$ and that of chest compression(%) was $73.10{\pm}22.16$. 4) Performance ability of AED after instruction showed proper performance in power on by 141(95.91%) and attaching pad by 135(91.83%), hand-off for analyzing rhythm showed 'accuracy' in 115(78.23%) and 'non-performance' in 32(21.77%), delivery of shock and hand-off confirmation showed 'accuracy' in 109(74.14%) and 'inaccuracy' in 38(25.86%), and beginning chest compression immediately after AED was done by 105(71.42%).
Purpose: There has been an increase in the number of prehospital cardiac arrests due to increases in both cardiovascular diseases and the average age of the population, We performed this study to identify the proper resuscitation technique and AED to be used to increase the survival rate in prehospital cardiac arrest. Methods: We studied 57 victims with prehospilal cardiac arrest by EMT's Reports form January to December, 2004. Results: Fifty-seven of 92 victims were trasported with ECG in prehospital cardiac arrest. Ventricular fibrillation(VF)/ ventricular tachycardia was 56.2%, asystole was 33,3%, and pulseless electrical activity(PEA.) was 10.5% in initial rhythm analysis, There weren't attempt CPR in 14.0%. Twenty of the 32(62.5%) were shocked by AED for the adequacy method. There were no statistical differences the transportation time. Conclusion: With the increase in cardiovascular disease and old age, the number out-of-hospital cardiac arrests has risen accordingly. However, there were lack of CPR by bystander, defibrillation, advanced cardiac life support(ACLS) in prehospital stage. To improve the adequancy of basic life support and to increase the performance of ACLS, especially AED, we must create challenges to develop new protocols in prehospital care.
