수술중 경두개 초음파 집중감시 (Intraoperative Transcranial Doppler Monitoring)

  • Trancranial Doppler(TCD) monitoring is a new application of ultrasonography which allows the nonivasive detection of blood flow velocity in the horizontal (M1) segment of the middle cerebral artery (MCA) and detects microembolic phenomena in the cerebral circulation. Recent studies emphasized the potential of using this technique in vascular surgery (carotid endarterectomy, cardiopulmonary bypass), interventional and intensive care setting. Although the disparity between CBF and blood flow velocity and number of microemboli could be used to prevent cerebral ischemic and embolism based on clinical studies. A reduction of more than 60% of MCA can reflex hemodynamic ischemic state and acoustic feedback of high intensity transient signals(HITS) from the TCD monitoring unit has a direct influence on surgical technique. TCD monitoring can immediately provide information about thromboembolism and hemodynamic changes, which may be a useful tool in the study and prevention of stroke.

대동맥판협착 및 폐쇄부전증을 동반한 관상동정맥루 -1례 치험- (Coronary Arteriovenous Fistula Associated with Aortic Stenosis and Regurgitation -Report of a Case)

  • Congenital coronary arteriovenous fistula is relatively uncommon and often associated with additional congenital and acquired heart disease. If coronary arteriovenous fistula is suspected, the diagnosis can be made readily by cardiac catheterization and selective coronary arteriography. Surgical treatment is very satisfactory, with a low mortality and apparent good long term result. Recently, we experienced one case of congenital coronary arteriovenous fistula which was associated with aortic stenosis and regurgitation. The tortuous fistula tract was noted between the left anterior descending coronary artery and the main pulmonary artery. Under the cardiopulmonary bypass, aortic valve replacement[Carbomedics 23mm] and suture closure of the draining orifice of coronary arteriovenous fistula in the main pulmonary artery just above the pulmonary valve were performed, Postoperative hospital course was uneventful and the patient was discharged postoperative 9th day without any problems.

심방 점액종 -4례 보고- (Atrial Myxoma; Report of 4 Cases)

  • Hyuk An, M.D.Atrial myxoma constitutes the most significant of all cardiac neoplasm. About 75% occur in the left atrium and 25% in the right. They may cause severe and progressive disease mimicking valvular heart disease. But recently attention to differential point in the clinical features along with various diagnostic techniques have allowed pre-operative diagnosis in the most cases. From April, `77 to Aug., `78, three cases of left atrial myxoma and one case of right atrial myxoma were operated in Seoul National University Hospital. Pre-operative diagnosis was established with echocardiography and cardiac angiography. In all cases, tumors were resected with cardiopulmonary bypass successfully, and discharged with good results.

체외순환을 위한 혈액희석법에 대하여 (Hemodilution in Clinical Extracorporeal Circulation)

  • Open heart surgery has been performed on 20 patients, using hemodilution principle under the moderate hypothermia from Dec. 1975 through Aug. 1977 at the Department of Thoracic and Cardiovascular Surgery, Kyungpook National University School of Medicine. All these patients, body surface area ranged from 0.53 to 1.67$M^2$, were divided into two groups as a child group [below 15 yrs] and a adult group[above 15 yrs]. The oxygenator were primed with fresh ACD blood, 5 per cent dextrose, Hartmann`s solution, 15 per cent mannitol, sodium bicarbonate, dexamethasone and antibiotics. The average flow rate was 2.0 L/$M^2$/min. in child group and 2.3L/$M^2$/min. in adult group. The degree of hemodilution in child and adult group was 30.7% and 29.3% respectively. The minimal value of rectal temperature was $30.84{\pm}0.7^{\circ}C.$, in child group and $30.0{\pm}1.5^{\circ}C.$, in adult group. We studied the changes of hemodynamic status, blood components, electrolyte, acid-base status. blood lactic acid and urine output during and after cardiopulmonary bypass.

총폐정맥 환류이상증의 외과적 치험 4예 (Total anomalous pulmonary venous return -Report of 4 cases-)

  • Total anomalous pulmonary venous return is a cardiac malformation in which there is no direct connection between any pulmonary vein and the left atrium but, rather all the pulmonary veins connect to the right atrium or one of its tributaries. TAPVC is a relatively uncommon anomaly, accounting for only about 1.5-3% of cases of congenital heart disease. Recently improvement in intraoperative techniques did eventually bring substantial improvements in the results in infants. 4 cases of TAPVC was successfully treated with one-stage operation, in the Dept. of Thoracic and Cardiovascular Surgery, National Medical Center in which 2 cases are supracardiac types and the other 2 cases are cardiac types. Sex ratio was 1:1, and the range of age was 2 years-18 years. The common pulmonary venous sinus was connected to the left vertical vein and innominate vein: in 2 supracardiac types and coronary sinus in 2 cardiac types. All cases are operated with standard cardiopulmonary bypass, and the hospital mortality was 0%.

  • Congenital coronary arteriovenous fistula is uncommon disease, and was first described by Krause in 1865. About 20% of the cases, it associates additional congenital heart diseases. A 5-year-old female patient was diagnosed as coronary AV fistula with VSD, and was taken surgical correction under cardiopulmonary bypass. VSD was small and subarterial in type, and the fistula was dilated as adult thumb tip size at its distal portion. VSD was closed directly through the pulmonary arteriotomy and the aneurysmal dilation was opened vertically, then it was obliterated using 5-0, 6-0 prolene continuous suture fashioning into a long slender tube. Postoperative course was uneventful.

대동맥동 동맥류 파열 - 1례 보고 - (Ruptured Sinus of Valsalva Aneurysm - A Case Report -)

  • Aneurysm of the sinus of Valsalva is an uncommon cardiac anomaly, usually congenital in origin, which may occur as an isolated defect or in conjunction with other cardiac malformation. This report is a case of a ruptured sinus of Valsalva aneurysm with ventricular septal defect in a 18-year-old female patient who complained progressive exertional dyspnea. She underwent operative management using total cardiopulmonary bypass. The fistula originated from the right coronary sinus and ruptured into the right ventricle and coexistent lesion was supracristal ventricular septal defect. The repair was done through aortic and right ventricular approach. The ruptured sinus of Valsalva was closed with pledget suture and the ventricular septal defect was closed with patch. The postoperative result was good.

관상동정맥루의 수술치험 1례 (Right coronary artery-right ventricular fistula -Report of one case-)

  • Coronary arteriovenous fistula is a relatively rare congenital heart disease and it drains into right atrium, right ventricle, pulmonary artery, coronary sinus or superior vena cava. We experienced one case of fistulous communication between right coronary artery and right ventricle which was most common condition. The patient was a 12 year old female and the only sign was continuous cardiac murmur. The cardiac catheterization and coronary angiography showed the fistulous communication between right coronary artery and right ventricle and aneurysmal dilatation of right coronary artery. Under the cardiopulmonary bypass with the hypothermic cardioplegia, fistula opening[7mm] which was located at right ventricle below the tricuspid valve annulus between septal and posterior leaflet was closed with 4 - 0 prolene continuous suture through right ventriculotomy. Her postoperative course was uneventful.

  • The potential for enhancing myocardial protection by adding high-energy phosphate to cardioplegic solutions [St. Thomas Hospital solution] was investigated in a rat heart model of cardiopulmonary bypass and ischemic arrest. Creatine phosphate was evaluated as an additive to the St. Thomas Hospital cardioplegic solution. Creatine phosphate 10.0 mmol/L as the optimal concentration which improved recovery of aortic flow and cardiac output after a 30 minute period of normothermic [37oC] ischemic arrest. In comparing mechanical function in both groups the mean postischemic recoveries of aortic flow, cardiac output, stroke volume and stroke work [expressed as a percentage of its preischemic control] were significantly greater in STH-CP group than in CP- free control group. In addition to improving function and decreasing CK release, CP reduced reperfusion arrhythmias significantly decreasing the time between cross-clamp removal and return to regular rhythm from 81.8 * 13.9 [sec] in CP-free group to 35.9 * 6.8 [sec] in CP group [P< 0.05] so, exogenous CP exerts potent protective and antiarrhythmic effects when added to the St. Thomas Hospital cardioplegic solution. However, the mechanism of action remains to be elucidated.

  • A congenital coronary artery fistula is an uncommon anomaly which has a direct communication between a coronary artery and the lumen of any one of the four cardiac chambers, or the coronary sinus, or its tributary veins or the superior vena cava. The right coronary artery is involved most frequently, and the abnormal communication in most often is to the right ventricle followed in incidence by drainage into the right atrium and the pulmonary artery. Recently. we experienced a case of congenital coronary artery fistula associated with valvular heart disease. The fistulous communication was noted between the left circumflex artery and the left atrial appendage. Under the cardiopulmonary bypass, the internal obliteration of the left atrial appendage, mitral valve replacement, and aortic valve exploration were accomplished. Postoperative hospital course was uneventful and the patient was discharged without any problems.
