Clinical Experience with IABP - Report of 12 cases - (Intra-aortic balloon pump[IABP] 치험: 12례 보고)

  • Intra-aortic balloon pump [IABP] was applied to 12 patients between July, 1987, and September, 1990. The 12 patients included 8 who were assisted with IABP intraoperatively; 4 patients used IABP postoperatively. 8 patients could not be withdrawn from cardiopulmonary bypass [CPB], but 6 of them [75%] were able to separate from CPB with IABP. They all were withdrawn from the balloon. Four [50%] of them are hospital survivors, and alive at the time of this report. 4 additional patients were assisted with IABP, postoperatively. 2 of them [50Yo] were withdrawn from the balloon but died. The overall survival and balloon weaning rates are 33.3% [4/12] % 66.7% [8/12], respectively. IABP was most effective when applied early to patients who had transient and reversible injury to the myocardium.

  • An unusual but often lethal complication of mitral valve replacement is rupture of the left ventricle. From March 1977 through June 1990, 424 mitral valve replacements were performed as isolated or combined procedures. Rupture of the posterior wall of the left ventricle was observed in 3 patients. Their was one type I and two type II rupture. Once the diagnosis was made, all of the patient were connected to the heart-lung machine again and total cardiopulmonary bypass is re-established. Repair was attempted in all of them from the outside of the heart. One of them was successively repaired but two were failed due to myocardial ischemia by circumflex coronary artery injury and failure of adequate closure of the ruptured site. From this results, we concluded that prevention is the best solution. But if we encountered this condition, early diagnosis and rapid treatment may improve the patient`s chances for survival.

Clinical Analysis of Open Heart Surgery - Review of 477 cases - (개심술 477예에 대한 임상적 고찰)

  • Between April, 1986 and July, 1991, 477 patients underwent open heart surgery with hypothermic cardiopulmonary bypass. There were 242 patients [50.7%] of acyanotic congenital anomalies, 34 patients [7.1%] of cyanotic congenital anomalies, and 187 patients [39.2%] of acquired heart diseases, 8 patients [1.7%] of coronary artery diseases, and 6 patients [1.3%] of mixed anomalies. Among the 276 congenital cardiac anomalies, 147 patients [53.3%] were male and 129 patients [46.7%] were female, ranged in age from 2 years to 58 years. Among the 187 acquired heart diseases, 72 patients [38.5%] were male and 115 patients [61.5%] were female, ranged in age from 10 years to 68 years. The common congenital defect were VSD and ASD in acyanotic cardiac patients, and TOF in cyanotic cardiac patients. Among the 187 acquired heart diseases, 180 patients underwent operation for cardiac valvular diseases, 4 patients were resected left atrial myxoma, and 3 patients underwent operation for aortic regurgitation with ascending aortic aneurysm. The operative mortality rate was 1.2% in acyanotic cardiac patients, 11.8% in cyanotic cardiac patients, and 6.9% in acquired cardiac patients, with overall mortality rate 4.2%.

  • Three neonates with interrupted aortic arch with VSD underwent one stage repair using revised technique of cardiopulmonary bypass with short period of circulatory arrest. A left posterolateral thoracotomy was made to permit mobilization of the descending aorta and placement of polytetrafluoroethylene[PTFE graft for distal aortic perfusion. Then the patient was placed in the supine position and a median sternotomy was performed to permit the proximal dissection, VSD repair, and direct anastomosis between the ascending aorta and descending aorta. This technique has advantages to facilitate direct anastomosis between the ascending aorta and the descending aorta, to lessen circulatory arrest time, and to prevent dangerous laceration and post-operative narrowing of the thin small ascending aorta at cannulation site. There was no operative mortality but postoperative stenosis developed in one case which was relieved with balloon aortoplasty.

  • A cosmetic incision for minimal invasive cardiac surgery is described. Through a small submammary skin incision combined with partial low sternotomy some kinds of cardiac operations are possible without extra-difficulty but with excellent cosmetic effect, No special instruments nor techniques are required. By this method routine cannulations for cardiopulmonary bypass as well as aortic cross clamping bicaval snaring and venting of cardiac chambers are possible, We performed 5 cases of open heart surgeries using this approach which includes two cases of atrial septal defect closure one case of ventricular septal defect closure one case of pulmonic valvotomy and one case of mitral valve replacement.

  • Extracorporeal life support (ECLS) is a type of cardiopulmonary bypass. It is an artificial means of supplying oxygen and removing $CO_2$ on behalf of damaged lungs while patients are recovering from underlying diseases. Recently, the use of ECLS is rapidly increasing as this machine becomes smaller, less invasive and easier to use. In addition, the improvement of clinicians' technique and outcome is increasing their application to patients with acute respiratory distress. In this regard, the purpose of this review is to introduce the physiological principles, risk factors, and advantages of ECLS, clinical rationale for using ECLS, ventilatory strategy during ECLS, which are still causing different opinions, the weaning from ECLS, and the use of anticoagulant.

Surgical Treatment of Patent Ductus Aretriosus: Report of 40 cases (개방성 동맥관 수술 보고 (40 예))

  • Since 1973, 40 patients with Patent Ductus Arteriosus were operated in The Department of Thoracic and Cardiovascular Surgery, Korea University Medical College. Clinical analysis of these cases: 1. Age ranged from 8 months old to 28 years old Sex ratio was 22 females to 18 males. 2. Pulse pressure widening above 50 mmHg was appeared in 27 patients. In 31 patients, continuous machinery murmur was heard on left second and third intercostal space, but 9 patients has holosystolic murmur on left sternal border. 3. Retrograde Aortography was performed in 11 patients and right heart catheterization, 17 patients, In 8 patients, pulmonary hypertension [above 30 mmHg in` systole] was noted. 3 out of 8 patients was combined with Ventricular Septal Defect. Severe pulmonary hypertension [above 80mmHg in systole] was presented in 3 patients. 4. In 38 patients, operative method was performed with multiple suture ligation of PDA, and in 2 patients, suture closure through pulmonary arteriotomy under cardiopulmonary bypass. 5. One operative death occurred in a patient in this group. Cause of death was right heart failure after multiple suture ligation of PDA and pulmonary artery bandings.

Surgical treatment of the aortic aneurysm (대동맥류의 외과적 요법)

  • Ten patients underwent operation for aortic aneurysms from Jan. 1983 to April 1988 at the Department of Thoracic and Cardiovascular Surgery, Keimyung University Hospital. There were 7 males and 3 females in this series. The age ranged from 16 to 70 years with the mean age of 45 years. The cause of the aneurysm was atherosclerosis in 8 patients, mycosis in 1 patient and unknown in 1 patient. There were two patients with ascending aortic aneurysm treated by Dacron graft replacement, with no hospital death. One patient with aortic arch aneurysm was received Dacron graft replacement under cardiopulmonary bypass and died on the 21st postoperative day because of cerebral edema. There were three patients with descending aortic aneurysm. The aneurysm in two patients was successfully repaired by Dacron graft. One additional patient with ruptured aneurysm died at operation because of ventricular fibrillation. Four patients with abdominal aortic aneurysm were underwent Dacron graft replacement and the results were good.

Pulmonary Embolectomy for Acute massive Pulmonary Embolism - 1 case -

  • Pulmonary thromboembolism originated mostly from the venous thrombus, especially deep vein thrombosis in the lower extremities, which migrated upward and lodged the pulmonary vasculatures, Massive pulmonary thromboembolism usually leads to in \ulcornerhospital mortality if the patient was not treated properly. Under the cardiopulmonary bypass, a 49-year old man was treated successfully by emergent pulmonary embolectomy of pulmonary thromboembolism, which originated from the deep vein thrombosis in the right leg. Ligation or filtering device insertion of the inferior vena cava was not performed. The patient’s postoperative course was uneventful and discharged on postoperative 15th day He continued to receive oral anticoagulation with aspirin and persantin, which had been started on the third postoperative day. And he was well till recent days through the outpatient follow-up. The clinical courses of this patient are described, and massive pulmonary embolism and its management are discussed.

Effects of Autotransfusion using Cell Saver in Cardiac Surgery (개심술시 자가 수혈체계[Cell Saver]의 이용 효과)

  • Autologous blood transfusion is a common method of reducing the need for homologous blood transfusion during cardiac operations. Between June 1991 and May 1992, 12 cases [Group I] were experienced autologous blood transfusion using Cell Saver undergoing double valve replacement or redo-valve replacement. Control group [N=12, Group II] was selected to above similar operation during same period. The Cell Saver system [Haemonetics Corp.] was employed for autologous blood transfusion. The blood shed in the operative field before and after cardiopulmonary bypass and remained cardiotomy reservior was aspirated by means of a locally heparinized collecting system. After centrifused salvaged blood, the resulting red cell concentrate reinfused subsequently. The patient receiving autologous blood transfusion required significantly less homologous blood transfusion than their control group. [Group I; 3519 $\pm$ 869, Group II; 4622 $\pm$ 856, Respectively; P=0.005] There were no clinical infections in the autotransfusion group. And there was no apparent intergroup difference of the clinical findings, hematologio datas and coagulation parameters. We conclude the autotransfusion using Cell Saver is effective for reducing of the hom-ologlous blood transfusion in cardiac surgery.
