형제에서 발생한 활로4증 (A Report of Fallot's Tetralogy in Siblings)

  • Tetralogy of Fallot has shown its familial aggregation in several familial studies. This reported case is another example which occurred in a family in two brothers. They revealed no cytogenetic abnormalities, but the anatomical cardiac malformation of them showed much similarity, total conal defect in ventricular septum and .one had patent foramen ovale, the other atrial septal defect. The familial recurrence tendency of Tetralogy of Fallot as well as other congenital heart diseases could be explained on multifactorial inheritance as shown in many reports. In spite that we couldn`t find out any environmental trigger or teratogens, our case may be accepted on the base of multifactorial mechanism.

승모판막치환후 좌심방 혈전증부검 1 (One case of left atrial thrombus after mitral valve replacement)

  • Thromboembolism is a major cause of morbidity and death following implantation of cardiac prosthetic devices. Effective systemic anticoagulation is very important. The presence of thrombus can often be detected by pulmonary edema associated with the disappearance of valve clicks. 2-D echocardiography and phonocardiography are also valuable tools. The most common treatment is reoperation and replacement after discovery early. We have experienced one death of fatal thromboembolism after St. Jude valve replacement was done in 48 years old male diagnosed of severe mitral stenosis. He was treated with warfarin, aspirin, ticlopidine for 10 weeks but died suddenly. At autopsy, valve dysfunction was seen due to organic thrombus arising from atrial septum and confirmed with microscopic findings.

좌심방내 점액종 치험 2예 (Left Atrial Myxoma: Report of 2 Cases)

  • Left atrial myxoma, an unusual clinical entity, may cause severe and progressive cardiac disease mimicking mitral valvular disease. In recent years, increased clinical awareness and improved diagnostic techniques has led to a higher incidence of correct preoperative diagnosis. Recently we experienced 2 cases of left atrial myxoma, which were removed successfully under the cardiopulmonary by pass. The first case was 45 years old woman and the second was 23 years old female. Preoperative definite diagnosis was entertained by angiocardiography and echocardiography in both cases. In the first case, tumor was removed with left atriotomy and atrial septectomy was done with additional right atriotomy. In the second case, tumor and atrial septum were removed en bloc through the right atrium. Both patients were discharged with good results 2 weeks postoperatively.

  • A direct communication between the left ventricle and right atrium is a relatively uncommon defect. Familiarity with this anomaly has become increasingly important, however, since the preoperative findings may be identical with those of an atrial septal defect. The left ventricle is directly related to the right atrium over. an area of the membranous ventricular septum which extends superior to the septal attachment of the right atrioventricular valve in the LV-RA communication. The clinical triad of a ventricular septal defect murmur, cardiac enlargement, and an arteriovenous shunt at atrial level is characteristic of the malformation. A 2-year-old boy with left ventriculo-right atrial communication has operated at the Maryknoll Hospital. Under the cardiopulmonary bypass, the atrium was opened, there was a jet-blood stream just above the atria-ventricular portion adjacent to the septal leaflet of the tricuspid valve.

  • 대동맥궁단절과 우폐동맥이 대동맥에서 기시하는 형태의 대동맥폐동맥창의 조합인 Berry증후군은 매우 드문 복합심기형의 하나이다. 14일된 신생아에 있어서 국소순환하에 순환정지 없이 일차완전교정술을 시행하여 좋은 결과를 얻었기에 보고하는 바이다.

  • We have experienced a patient, 16 year-old male, with ventricular septal defect with tricuspid recur- gitation due to blunt chest trauma. He suffered from congestive heart failure after the trauma. Echocardiogram and cardiac catheterization revealed left to right shunt at the ventricular level (muscu- far portion of interventricular septum) and tricuspid regurgitation. At the time of the operation, marked systolic thrill was palpable over the rlght ventricle near the apex and a chorda tendina was seen sharply ruptured just near the medial papillary muscle. We repaired the ventricular septal defect with a Dacron patch and chordal reconstruction with autologous pericardium. The postoperative course was uneventful and the patient was discharged in good condition.

  • Cardiac myxomas are the most common primary benign tumors of uncertain etiology. They usually present as polypoid or oval-shaped masses projecting into a heart chamber from the interatrial septum and have a soft, gelatinous consistency without a cystic structure. We report a case of left atrial myxoma with a single cystic form.

  • From August 1990 to October 1991, we performed operation on 5 patients with cardiac myxoma, which was located in the left atrium, There was 1 male and 4 female patients ranging in age 45 years to 61 years. Chief complaints included exertional dyspnea, palpitation, cough, orthopnea, general weakness. M-mode and two dimensional echocardiography provided an accurate diagnosis in all cases which was utilized as safe, reliable and noninvasive imaging modalities. In all cases, myxoma was excised successfully. Follow up is current. No recurrent myxoma has been identified clinically or by echocardiography. In this series, excellent results were obtained by simple excision of the tumor, with or without a margin of normal atrial septum.

Double-chambered Right Ventricle in a Cat

  • A 12-month-old, 3.0-kg, male, Scottish fold cat was referred to Irion animal hospital with dyspnea and cardiac murmur. In radiograph, right-sided cardiomegaly and pleural effusion were identified. In echocardiographic examination, abnormal stenotic region was identified in right ventricle and dilation of right atrium and concentric hypertrophy of interventricular septum and the free wall of right ventricle were also identified. In the stenotic region, abnormal muscular band was identified. The turbulent flow was identified in infundibular region of right ventricular outflow tract with color-Doppler examination and high velocity flow (4.0 m/s) was also identified in stenotic region. With these results, double-chambered right ventricle (DCRV) was diagnosed.

  • In a 3-year-old Samoyed, aortic bulging was found on radiography during a general check-up. On echocardiography, turbulent flow was found in left ventricular outflow tract (LVOT) with high velocity (6.1 m/s). A linear structure was attached to the interventricular septum and connected to the chordae tendineae reaching the papillary muscle. A part of the structure moved during cardiac cycle, similar to mitral motion. This dog was diagnosed with LVOT obstruction caused by accessory mitral valve tissue (AMVT). This is the first report of AMVT in veterinary medicine. AMVT should be considered as a possible cause of LVOT obstruction in dogs.
