Myxoma is the most common type of primary cardiac tumor and about 75% of myxomas occur in the left atrium of the heart. Most myxomas are diagnosed by echocardiography. The growth rate of myxoma is clearly unknown and newly developed myxoma is difficult to understand clearly the onset of growth. We describe the case of a newly developed huge left atrial myxoma($7{\times}7cm$)which showed no echocardiographic evidence of the tumor four years ago. Four years later the patient underwent transthoracic echocardiography due to preoperative evaluation that was normal. Nine months later, she presented with dyspnea. Transthoracic echocardiography showed a large mass located in left atrium. Pathology showed a myxoma that measured $7{\times}7cm$ implying a growth rate of $0.15{\times}0.15cm/month$.

Background: We report our initial experiences of robot-assisted cardiac surgery using the da Vinci Surgical System. Methods: Between February 2010 and March 2014, 50 consecutive patients underwent minimally invasive robot-assisted cardiac surgery. Results: Robot-assisted cardiac surgery was employed in two cases of minimally invasive direct coronary artery bypass, 17 cases of mitral valve repair, 10 cases of cardiac myxoma removal, 20 cases of atrial septal defect repair, and one isolated CryoMaze procedure. Average cardiopulmonary bypass time and average aorta cross-clamping time were $194.8{\pm}48.6$ minutes and $126.1{\pm}22.6$ minutes in mitral valve repair operations and $132.0{\pm}32.0$ minutes and $76.1{\pm}23.1$ minutes in myxoma removal operations, respectively. During atrial septal defect closure operations, the average cardiopulmonary bypass time was $128.3{\pm}43.1$ minutes. The median length of stay was between five and seven days. The only complication was that one patient needed reoperation to address bleeding. There were no hospital mortalities. Conclusion: Robot-assisted cardiac surgery is safe and effective for mitral valve repair, atrial septal defect closure, and cardiac myxoma removal surgery. Reducing operative time depends heavily on the experience of the entire robotic surgical team.
Between 1977 and 1987, 8 patients underwent excision of cardiac myxomas at the Hanyang University Hospital. All had a left atrial myxoma. There were 4 female and 4 male patients ranging from 15 to 62 years of age. Preoperative findings consist of symptoms and signs of congestive heart failure except one. Diagnosis was confirmed by echocardiography[8 cases] and angiography[2 cases], preoperatively. A biatrial operative approach was utilized in all but 2, who were small sized. Complete excision of the tumor with a cuff of normal tissue[1 was atrial wall and 7 were atrial septum] was performed. all heart chambers were carefully explored for presence of multicentric myxomas or tumor debris. There were no operative deaths or intraoperative embolization. Follow-up has been 1/3 to 10 years. There has been 1 late death, due to recurrence and 1 patient had reoperation for mitral regurgitation due to dilatation of the annulus by a huge tumor mass. Surgical excision of the myxoma can be performed with low morbidity, and it provides excellent and sustained symptomatic relief. The recurrence rate is low, but long-term follow-up and serial echocardiography are advisable.
Cerebral embolic infarction is the most common neurologic complication of cardiac myxoma (CM). Development of cerebral aneurysms in CM is very rare. We present a 64-year-old woman with acute cerebral infarction and multiple cerebral aneurysms complicated by CM. The aneurysms were multiple, fusiform-shaped, and located in distal branch of major cerebral arteries. The serum interleukin (IL)-6 was highly elevated, which was normalized after surgical resection of CM. There was no regression of aneurysms on follow-up neuroimaging. Multiple cerebral aneurysms in CM are rare condition. Highly elevated serum IL-6 may be associated with increased risk of cerebral aneurysmal formation.
Abdominal aortic occlusion (AAO) caused by detachment of cardiac myxoma (CM) is a very rare complication in patients with CM. Although the nature of CMs has been well established, detachment of CM may cause unexpected serious complications such as vicious embolic events. Actually, in several cases of AAO caused by detachment of CM, it has been reported that CM fragments easily migrated to the brain, heart, and lungs, and caused lifelong neurological complications despite appropriate surgical therapy. Herein, we report a case of a patient with AAO caused by detachment of CM who underwent CM excision and abdominal aortic thromboembolectomy. Additionally, we have presented the preoperative and postoperative images using 64-multidetector computed tomography.
Primary cardiac tumors usually are benign and are left atrial myxoma. We experienced with two cases of left atrial myxoma recently, and underwent surgical resection successfully. Diagnoses were obtained by echocardiogram and the angiocardiography was not performed. The operation consisted of excision of the tumor with a segment of atrial septum or wall through a right atriotomy, transseptal approach. Postoperative functional results were good and the literature is reviewed.
Left atrial myxoma constitutes the most significant cardiac tumors, which occupies 30 to 50% of the total in most pathological series with the age range from 3 to 83 years, mostly between the age of 30 to 60 years. Over 90% of myxomas occur in the atria, with 3 to 4 times as many occurring in the left as in the right. They may cause severe and progressive disease mimicking mitral valvular disease with non-specific general symptoms. Recent attentions were focussed in the clinical features along with various diagnostic techniques including angiocardiography, echocardiography and cross-sectional sector scanning echocardiography, and definitive treatment with surgical removal under direct vision using cardiopulmonary bypass. There are several reports on the atrial myxomas removed from various cardiac chambers in Korea, and we report another typical case of left atrial myxoma of 6.8 x 3.8 x 1.7 cm3 attached to the upper margin of fossa ovalis with a short pedicle, also with calcification at it`s distal free end. 29 year old young mother complaining of mild fever, vaginal spotting, numbness of left upper and lower extremities, nocturnal attack of dyspnea and palpitation for 2 months was diagnosed as large left atrial myxoma with the aid of angiocardiography and echocardiography. Trans-right-atrial, transseptal surgical removal of the tumor was successfully performed under the cardiopulmonary bypass for 22 minutes on 24th of July 1980. Postoperative hospital course was uneventful and discharged from hospital on the 12th POD with complete recover from the previous symptoms and signs.

Background: Complete surgical excision is the only curative treatment for primary cardiac tumors. For wide excision, interatrial septal reconstruction (ISR) is commonly performed. We hypothesized that ISR may increase the risk of postoperative atrial tachyarrhythmia (AT) after surgical resection of cardiac myxoma. Methods: After excluding patients with a history of cardiac surgery and concomitant procedures unrelated to tumor resection and those with AT or permanent pacemakers, we finally enrolled 272 adult patients who underwent benign cardiac tumor surgery from 1995 to 2021 at our institution. They were divided into the ISR (n=184) and non-ISR (n=88) groups. The primary outcome was postoperative new-onset AT. Results: The study cohort predominantly consisted of women (66.2%), with a mean age of 57.2±13.6 years. The incidence of postoperative new-onset AT was 15.4%. No 30-day mortality or recurrence was observed. The cardiopulmonary bypass time and aortic cross-clamping time were significantly longer in the ISR group than in the non-ISR group (p<0.001). The median duration of hospital stay of all patients was 6.0 days (interquartile range, 5.0-7.0 days), and no significant difference was observed between the 2 groups (p=0.329). ISR was not an independent predictor of new-onset AT (p=0.248). Male sex and hypertension were found to be independent predictors of new-onset AT. Conclusion: ISR was not a significant predictor of postoperative new-onset AT. ISR might be a feasible and safe procedure for surgical resection of cardiac myxoma and should be considered if needed.
Seven patients [six cases of left atrial myxoma, one case of right atrial myxoma] from 24 to 66 years of age [4 male and 3 female, mean age 48 years] underwent excision of atrial myxoma between 1982 and 1989 at Keimyung University Dongsan Medical Center. All patients presented with congestive heart failure, six with cardiac murmur, three with syncope, two with sinus tachycardia, one each with sinus arrhythmia, atrial fibrillation, pleural effusion, peripheral embolization. Symptoms were present from 1 month to 8 years before operation [mean 28 months], All tumors originated from atrial septum and pedunculated. The myxomas were successfully removed in all patients, either shaving them from atrial septum [n=3] or by excising a portion at normal atrial septum with tumor [n=4]. One case was replaced mitral valve with carbomedics-31mm due to severe mitral regurgitation. Follow up is current. No recurrent myxoma has been identified clinically or by echocardiography. In this series, excellent results were obtained by simple excision of the tumor, with or without a margin of normal atrial septum.
