Effect of Rosiglitazone on Myocardial Ischemia-Reperfusion Injury in Rat Heart

  • This study was undertaken to evaluate whether peroxisome proliferator-activated-receptor-gamma $(PPAR-{\gamma})$ agonist-rosiglitazone (ROSI) induces postischemic functional recovery in Langendorf heart model. Hearts isolated from normal rats were subjected to 20 min of normoxia or 25 min zero-flow ischemia followed by 50 min reperfusion. In this acute protocol, ROSI $(20\;{\mu}g/ml)$ administered 10 min before ischemia had no effect on hemodynamic cardiac function, but had protective effect on lipid peroxidation in in vitro experiments. In chronic protocol in which ROSI was given by daily gavage (4 mg/kg) for three consecutive days, ROSI could not prevent the hemodynamic alteration on cardiac performance, but has protective effect on the activity of superoxide dismutase (SOD). There was no significant difference in the contents of reduced glutathione (GSH) and catalase activity between ischemia-reperfusion (IR) and ROSI treated IR hearts. Although ROSI had no effect on hemodynamic factor, it had effect on antioxidant activity. Our results indicate that ROSI provides partial beneficial effects by inhibiting lipid peroxidation and/or recovering normal level of SOD activity in the ischemic reperfused heart.

선천성 심기형 환아에서 체외순환후 혈청 및 소변 Amylase치의 변화 (The Change of Derum and Urine Amylase Level Following Cardiopulmonary Bypass in the Patients with Congenital heart disease)

  • Pancreatitis is a known complication of cardiac surgery with cardiopulmonary bypass. Although ischemia is believed to be a factor, the exact cause of pancreatitis after cardiopulmonary bypass remains unknown.We prospectively studied 67 consecutive patients undergoing cardiac surgery with cardiopulmonary bypass for evaluation of the pancreatic injury after cardiopulmonary bypas. Serial measurement of amylase level in serum and urine was done postoperatively. Hyperamylasemia was detected in 15 patients[22.4% , of whom no patient had pancreatitis. There was no significant difference between serum amylase level and parameters such as cardiopulmonay bypass time, aortic cross clamp time, mean blood pressure, rectal temperature, flow rate, and use of circulatory arrest during cardiopulmonary bypass. Hyperamylasuria was detected in 8 patients[11.9% , and urine amylase level was elevated significantly in the groups with prolonged cardiopulmonary bypass, mean blood pressure more than 40mmHg, and rectal temperature more than 20 $^{\circ}$C. We recommend that serum amylase level and/or amylase-creatinine clearance ratio is measured for ealy detection and management of pancreatitis after cardiopulmonary bypass.

Complete Repair of Coarctation of the Aorta and a Ventricular Septal Defect in a 1,480 g Low Birth Weight Neonate

  • Although outcomes of neonatal cardiac surgery have dramatically improved in the last two decades, low body weight still constitutes an important risk factor for morbidity and mortality. In particular, cardiac surgery in neonateswith very low birth weight (${\leq}$1.5 kg) is carried out with greater risk because most organ systems are immature. We report here on a successful case of early one-stage total repair of coarctation of the aorta and a ventricular septal defect in a 1,480 gram neonate.

  • The author conducted a literature review to better characterize the current state of knowledge regardeding the relationship between psychological and behavioral factors and cardiovascular disease. This review focus on several Problems : hypertension, coronary artery disease such as myocardial infarction and angina pectoris, sudden death, arrhythmia, vasomotor(vasodepressor) syncope, and psychogenic cardiac nondisease. We describe model for understanding the relationship of psychological factors to the disease, review the results of relevant research studies and provide recommendation for further research.

Neuregulin-1 promotes cardiomyocyte differentiation of genetically engineered embryonic stem cell clones

  • Embryonic stem (ES) cell-derived cardiomyocytes (ESCMs) must be specifically purified in order to prevent teratoma formation, and this confusing issue has hampered their clinical application. We therefore investigated a technique to generate pure labeled ESCMs for possible use in cardiac repair. We generated transgenic ES cell lines expressing enhanced green fluorescent protein (EGFP) under the transcriptional control of the $\alpha$-cardiac myosin heavy chain ($\alpha$-MHC) promoter. Differentiated EGFP-positive ES cells displayed characteristics of CMs. Furthermore, neuregulin-1 (NRG-1) upregulated the expression of the cardiac-restricted transcription factors Nkx2.5 and GATA-4, as well as differentiated CM factors ($\alpha$-MHC, $\beta$-MHC). Immunohistochemistry demonstrated that NRG-1 increased expression of cardiac-specific troponin T in the beating foci of the embryoid bodies. This work revealed a potential method for specifically labeling and enriching ESCMs by combining genetically-engineered ES cell clones and exogenous growth factor treatment.

Outcomes after Extracorporeal Membrane Oxygenation in Neonates with Congenital Diaphragmatic Hernia: A Single-Center Experience

  • Background: Congenital diaphragmatic hernia (CDH) is a rare disease often requiring mechanical ventilation after birth. In severe cases, extracorporeal membrane oxygenation (ECMO) may be needed. This study analyzed the outcomes of patients with CDH treated with ECMO and investigated factors related to in-hospital mortality. Methods: Among 254 newborns diagnosed with CDH between 2008 and 2020, 51 patients needed ECMO support. At Asan Medical Center, a multidisciplinary team approach has been applied for managing newborns with CDH since 2018. Outcomes were compared between hospital survivors and nonsurvivors. Results: ECMO was established at a median of 17 hours after birth. The mean birth weight was 3.1±0.5 kg. Twenty-three patients (23/51, 45.1%) were weaned from ECMO, and 16 patients (16/51, 31.4%) survived to discharge. The ECMO mode was veno-venous in 24 patients (47.1%) and veno-arterial in 27 patients (52.9%). Most cannulations (50/51, 98%) were accomplished through a transverse cervical incision. No significant between-group differences in baseline characteristics and prenatal indices were observed. The oxygenation index (1 hour before: 90.0 vs. 51.0, p=0.005) and blood lactate level (peak: 7.9 vs. 5.2 mmol/L, p=0.023) before ECMO were higher in nonsurvivors. Major bleeding during ECMO more frequently occurred in nonsurvivors (57.1% vs. 12.5%, p=0.007). In the multivariate analysis, the oxygenation index measured at 1 hour before ECMO initiation was identified as a significant risk factor for in-hospital mortality (odds ratio, 1.02; 95% confidence interval, 1.01-1.04; p=0.05). Conclusion: The survival of neonates after ECMO for CDH is suboptimal. Timely application of ECMO is crucial for better survival outcomes.

Hypokalemia as a risk factor for prolonged QT interval and arrhythmia in inherited salt-losing tubulopathy

  • Purpose: To analyze electrocardiograms (ECGs) of patients with a salt-losing tubulopathy (SLT) and to determine the frequency and risk factors for long QT and arrhythmia. Methods: A total of 203 patients aged <19 years with SLT, specifically Bartter syndrome and Gitelman syndrome, who had a 12-lead ECG were included in this retrospective study. We analyzed the presence of an arrhythmia or prolonged corrected QT (QTc) on ECGs obtained for these patients. Demographic and laboratory data were compared between patients with abnormal and normal ECG findings. Results: Out of the 203 SLT patients, 38 (18.7%) underwent electrocardiography and 10 (40.0%) of 25 patients with inherited SLT had abnormal ECG findings, including prolonged QTc and arrhythmias. The abnormal ECG group had significantly lower serum potassium levels than the normal group (median [interquartile range]: 2.50 mmol/L [2.20-2.83] vs. 2.90 mmol/L [2.70-3.30], P=0.036), whereas other serum chemistry values did not show significant differences. The cutoff level for a significant difference in QTc interval was serum potassium level <2.50 mmol/L. One cardiac event occurred in a 13-year-old boy, who developed paroxysmal supraventricular tachycardia and underwent cardiac ablation. No sudden cardiac deaths occurred in this cohort. Conclusions: The incidence of ECG abnormalities in patients with inherited SLT was 40.0%, whereas the ECG screening rate was relatively low (18.7%). Therefore, we recommend ECG screening in patients with inherited SLT, especially in those with serum potassium level <2.50 mmol/L.

게이트심장혈액풀 스캔에서 Half-Time 획득 인자 적용에 따른 임상적 유용성 평가 (The Evaluation of Clinical Usefulness on Application of Half-Time Acquisition Factor in Gated Cardiac Blood Pool Scan)

  • Purpose: The scan time reduction helps to yield more accurate results and induce the minimization of patient's motion. Also we can expect that satisfaction of examination will increase. Nowdays medical equipment companies have developed various programs to reduce scan time. We used Onco. Flash (Pixon method, SIEMENS) that is an image processing technique gated cardiac blood pool scan and going to evaluate its clinical usefullness. Materials and Method: We analyzed the 50 patients who were examined by gated blood pool scan in nuclear medicine department of Asan Mediacal Center from June $20^{th}$ 2008 to August $14^{th}$ 2008. We acquired the Full-time (6000 Kcounts) and Half-time (3000 Kcounts) LAO image in same position. And we acquired LVEF values ten times from Full-time, Half-time images acquired by the image processing technique and analyzed its mean and standard deviation values. To estimate LVEF in same conditions, we set automatic location of the LV ROI and background ROI based on same X and Y-axis. Also we performed blinding tests to physician. Results: After making a quantitative analysis of the 50 patients EF values, each mean${\pm}$standard deviation is shown at Full-time image $68.12{\pm}7.84%$, Half- time (acquired by imaging processing technique) $68.49{\pm}8.73%$. In the 95% confidence limit, there was no statistically significant difference (p>0.05). After blinding test with a physician for making a qualitative analysis, there was no difference between Full-time image and Half-time image acquired by the image processing technique for observing LV myocardial wall motion. Conclusion: Gated cardiac blood pool scan has been reported its relatively exact EF measured results than ultrasound or CT. But gated cardiac blood pool scan takes relatively longer time than other exams and now it needs to improve time competitive power. If we adapt Half-time technique to gated cardiac blood pool scintigraphy based on this study, we expect to reduce possible artifacts and improve accessibility as well as flexibility to exam. Also we expect patient's satisfaction.

64 채널 Multi-Detector Computed Tomography를 이용한 관상동맥검사의 선량 : 검사 프로토콜 다변화에 따른 환자선량 감소 (Doses of Coronary Study in 64 Channel Multi-Detector Computed Tomography : Reduced Radiation Dose According to Varity of Examnination Protocols)

  • MDCT의 시간분해능 향상과 등방성 해상능(isotrophic resolution) 영상의 획득, 그리고 지능적인 심전도 동조를 바탕으로 하여 심혈관 질환의 효과적인 진단검사로 인정받고 있는 후향적 심전도 동조화(retrospective ECG gating) 하의 coronary CT angiography는 상대적으로 많은 환자선량을 제공함으로 인해 우수한 진단방법으로서의 장점을 반감시키고 있다. 이에 각 장치 제조사에서는 환자선량을 감소시키는 방법의 연구가 활성화되어 왔으며, 이의 일환으로 지능적인 cardiac dose modulation 기술과 전향적 심전도 동조화(prospective ECG gating)를 사용한 sequential scan이 도입되고 있다. 이에 본 연구에서는 64 채널 MDCT에서 54 kg, 163 cm인 여성 인체모형팬텀을 대상으로 하고 형광유리선량계를 사용하여 후향적 심전도 동조화 하의 coronary CT angiography 프로토콜에서 환자선량의 정량적 평가와 환자선량 감소를 위해 본원에서 선택적으로 적용하고 있는 5가지 검사 프로토콜을 적용하였을 경우의 effective dose와 중요 부위의 organ dose를 측정 비교하여 다음과 같은 결과를 얻었다. 1) Dose modulation없이 120 kVp와 210 mAs의 노출조건으로 retrospectively ECG gated helical scan으로 시행한 conventional coronary CT angiography 프로토콜의 effective dose는 17.8 mSv였으며, 심장의 organ dose는 103.8 mGy였다. 2) 관전압을 120 kVp에서 100 kVp로 낮추었을 경우 effective dose는 11.0 mSv로 conventional coronary CT에 비해 38.2%가 감소하였으며, 심장은 67.3 mGy로 45.2%가 감소하였다. 3) Cardiac dose modulation을 적용한 경우 effective dose는 13.3 mSv로 conventional coronary CT에 비해 25.3%가 감소하였으며, 심장은 80.0 mGy로 22.9%가 감소하였다. 4) 100 kVp의 저관전압과 cardiac dose modulation을 적용한 경우 effective dose는 8.1 mSv로 conventional coronary CT angiography에 비해 54.5%가 감소하였으며, 심장은 49.5 mGy로 52.3%가 감소하였다.

가미옥녀전(加味玉女煎)이 db/db 마우스 당뇨(糖尿)모델에서 지질대사(脂質代謝)와 항당뇨(抗糖尿) 효능(效能)에 미치는 영향(影響) (Effects of Gamiolnyeo-jeon on Lipid Metabolism and Blood Glucose Level in db/db Mice)

  • Objectives : Abnormal regulation of glucose and impaired lipid metabolism that result from a defective or deficient insulin are the key etiological factor in type 2 diabetes mellitus. The our study investigated the effects of Gamioknyeo-jeon (GO) on blood glucose and lipid metabolism improved by it in db/db mice (a murine model of type 2 diabetes mellitus).Methods : The animals were divided into 3 groups: Normal groups were not-treated C57BL/6 mice; Control groups were treated orally with DW in db/db mice; GO groups were treated orally with GO (200 ㎎/㎏/day) in db/db mice. After mice were treated with GO for 5 weeks, we measured AST, ALT, creatinine, BUN, body weight, food intake, blood glucose, insulin and lipid levels (total cholesterol, HDL cholesterol, and LDL cholesterol and atherogenic index(AI) and cardiac risk factor(CRF).Results : Serum AST, ALT, creatinine, BUN levels were not changed by GO do not show any toxic effects. GO groups were decreased in body weight, food intake and blood glucose level among compared to Control groups. Also, GO groups were found to have atherogenic Index and cardiac risk factor as well as lipid metabolism improvement (total cholesterol and LDL cholesterol decrease). Finally, GO groups were increased the insulin compared to Normal and control groups.Conclusions : We suggest that GO may have the control effects of diabetes mellitus by improving blood glucose control and lipid metabolism.
