An Effect of Carbon Arc Lamp and Low Level Laser Therapy on the Changes of Burned Mice's Cytokine (탄소방전등과 저강도 레이저 조사가 화상 쥐의 Cytokine 변화에 미치는 영향)

  • Purpose: To provide the basic method for physical therapy to contribute to early stage treatment of a burned patients. We investigated whether or not carbon arc lamp treatment and low level laser one among various laser treatments have the effect of reducing Cytokine, and to elucidate the effect of carbon arc lamp treatment and laser one. Methods: The 6-week old BALB/c types of 92 mice were used for the experimental test, and they were burned with $100^{\circ}C$ water, they were divided into 4 groups in accordance with the method of treatment respectively. And the blood and the tissue from the subject of each group were extracted and analyzed each time after they were cured for 3, 6, 9 days. For the analysis of the results, SPSS statistical program was used in this study. Results: The quantity of $TNF-{\alpha}$ and $IL-1{\beta}$ within the blood of a burned mouse increased more than that of a normal mouse(p<0.01). In only a burned mouse, carbon arc lamp treatment gave the effect on the decrease of $TNF-{\alpha}$ thickness the 6th day to the 9th day(p<0.01). $IL-1{\beta}$ quantity was more decreased than that of control group around the 6th day. In comparison with only a control group, low level laser treatment has more significant effect in decreasing the quantity of $TNF-{\alpha}$ a and $IL-1{\beta}$ than the two different methods(p<0.01). In case that the two treatment methods, carbon arc lamp treatment and low level laser, were executed together, there was the effect of decreasing $TNF-{\alpha}$ until the 6th day(p<0.01). Conclusion: Both low level laser treatment and carbon arc lamp one would be able to have an effect on the inflammation inhibition of burned patients and tissues reproduction. However, it must be also considered for the two treatments to be done at the same time.

  • Backgrounds : Pompholyx is a distinctive reaction pattern presenting as symmetric vesicular hand and foot dermatitis. Other than acute symptoms like itching and vesicles, chronic changes with scaling, fissure and pain may follow. As patients are treated by steroid for a long period, it tends to present side effects after quitting steroid treatment. Objectives : This study was carried out to investigate the effect of carbon arc lamp therapy and oriental medical therapy that venesection therapy, ointments and herbal medicine on chronic plantar pompholyx including steroids side effects. Methods : We treated one case of chronic plantar pompholyx which had continued about 10 years, including steroids side effects with venesection therapy, carbon arc lamp therapy, ointments and herbal medicine. To evaluate the results of this treatment, we categorized symptoms into six items which are vesicles, itching, fissures, pain, erosions and scales. Results : Except mild itching, vesicles and scales in a small area, all the symptoms were subsided. Conclusions : It is considered that various kinds of oriental medical therapy including carbon arc lamp therapy could replace steroid therapy which has many side effects.

  • Epidermolysis Bullosa(EB) is rare, chronic mechanobullous disease, which easily produce bulla by minor trauma or spontaneously. There are three major forms of Epidermolysis bullosa - Epidermolysis bullosa simplex, Junctional epidermolysis bullosa, Dystrophic epidermolysis bullosa. This report is on the 21-year-old female patient who has Dystrophic epidermolysis bullosa(Recessive Dystrophic epidermolysis bullosa-mitis). The Korean medical treatments such as acupuncture and herbal medication were used, with light therapy : Carbon Are, which is known for effect of dermal resuscitation. As a result, Carbon Arc and Korean medical treatment's effect of dermal resuscitation is proved in this study. But more cases are necessary for establishing more effective treatment methods.

  • Objectives The purpose of this report is to represent decubitus ulcer(pressure sore) improved by carbon arc and oriental medicine. Methods Patients with decubitus ulcer were treated with carbon arc, acupuncture, herbal medicine and simple dressing. Laser was irradiated at ulcer site once a day for 5 hour, using cored carbon number 3000,3002. Simple dressing with wound cleansing was performed to prevent reinfection. Herbal medicine and acupuncture treatment were applied to improve patients' general condition. Ulcer sizes and aspects were carefully checked by one week. Results Sizes of the decubitus ulcer decreased. Stage of ulcer was improved from stage 3 to stage 2. These results show that carbon arc may play a role in treating the decubitus ulcer with oriental medicine.

  • Background: It is widely known that carbon dioxide (CO2) arc welding generates carbon monoxide (CO). However, to the best of our knowledge, no case reports have been published regarding CO poisoning in CO2 arc welders. Therefore, we aimed to report a case of CO poisoning-induced encephalopathy in a CO2 arc welder in the Republic of Korea to inform about the dangers of CO exposure among CO2arc welders. Case presentation: A 40-year-old man working as a CO2 arc welder for 15 years visited a local hospital with a tremor, involuntary urination, and speaking gibberish, on April 9, 2019. He stated that he had intermittent headache and forgetting symptoms for the last 5 years, and had been lost on the way to work several times. On April 9, 2019, he was diagnosed with CO poisoning-induced encephalopathy through brain magnetic resonance imaging. He received hyperbaric oxygen therapy, and some of his symptoms improved. According to the exposure assessment of his work environment, he was continuously exposed to high concentrations of CO for 15 years while operating CO2 arc welding machines. Conclusions: After evaluating the patient's work environment and evaluating his medical history, we concluded that his encephalopathy was caused by CO exposure during CO2 arc welding. Thus CO2 arc welders must be aware of the risk of CO poisoning and strive to avoid CO exposure.

A Case of Atopic Dermatitis with Nummular Eczema (화폐상습진을 동반한 성인형 아토피피부염의 한방 치험 1례)

  • Introduction : Atopic dermatitis(AD) is chronic eczematous dermatitis which is related strongly to a family history of atopic disease and hereditary disposition. Adolescence and adult atopic dermatitis (AAD) patients have been overlooked because they were relatively small population and AAD were considered merely consecutive of infant and childhood AD. Objective : The purpose of this study was to estimate the efficacy of oriental medical treatment and management on chromic AAD with nummular eczema and breast eczema. Methods : We tented this case with herb-medication, herb-irrigation, acupuncture, negative therapy, full spectrum visible lay therapy (by carbon arc) and external application (SA1, Siwon Ilhwang-Go). We observed for 7 months. Results : The grades of the severity of AD with nummular eczema, breast eczema were lower only by the oriental medical treatments and managements. Conclusion : Considering the above results, more study is needed on the case with AAD with diffuse nummular eczema, effectiveness of external application and correlation between immunological and results of oriental media treatments.

  • Atopic Dermatitis is chronic eczematous dermatosis which is related strongly to a family history of atopic disease and hereditary disposition. Its threshold of itching is low so that severe itching occurs and causes secondary eczema. The purpose of this study was to estimate the efficiency of oriental medical treatment and management on atopic Dermatitis. We studied 2 patients who visited Wonkwang University Kwangju Oriental Hospital Dept. of Dermatol. with atopic dermatitis from April 2004 to February 2005. We treated him with herb-medication, acupuncture, negative therapy, full spectrum visible ray therapy(by carbon arc). We used the ADSI(Atopic Dermatitis Severity Index) to assess the severity of atopic dermatitis. The grade of the severity of atopic dermatitis was lower only by the traditional chinese medical treatments and management. Considering the above result, more study is needed the factors of increasing severity of atopic dermatitis, and the clinical interchange or atopic Dermatitis between oriental medicine and western medicine.

Objective: The purpose of this study was to estimate the efficacy of oriental medical treatment and management on chronic adult atopic dermatitis. Subject: We studied 3 patients who visited Dongguk University Gangnam Oriental Hospital Dept. of Dermatol. with chronic adult atopic dermatitis from August 2001 to April 2002 and were observed for over 4 months. Methods: We treated them with herb-medication, acupuncture, negative therapy, aroma therapy, full spectrum visible ray therapy(by carbon arc) and external application(Jaun-go). We used the SCORAD index to assess the severity of atopic dermatitis. Results: There were 3 patients with chronic adult atopic dermatitis we could check for over 4 months by clinical scoring using the SCORAD system. 1. The grades of the severity of atopic dermatitis were lower only by the oriental medical treatments and management. 2. Intensities of relapse cases were lower then before the oriental medical treatments. 3. The secondary treatment term for relapse cases was shorter then the first treatment term after oriental remedies were used. Conclusion: Considering the above results, more study is needed on the chronic relapse cases with adult atopic dermatitis, the factors of increasing seventy of adult atopic dermatitis, the prognostic factors of adult atopic dermatitis, and the clinical interchange of chronic adult atopic dermatitis between oriental medicine and western medicine.

An Overview of Korean Medicine Research for Pompholyx - Literature Review based on recently published studies in Korea - (한포진에 대한 최근 한의학적 연구 동향 - 국내 학술지를 중심으로 -)

  • Objectives : This study was conducted to review the research trends in Korean Medicine of pompholyx based on publications in Korea. Methods : Literature published after 2000 were collected using OASIS, NDSL, KISS, and key words searched were "한포진" "발한이상" "pompholyx" "dyshidrotic eczema". Results : 10 case reports and 4 patient studies were selected for study. Conclusions : Korean medicine treatments included acupuncture, pharmacopuncture, venesection therapy, herbal medicine, ointments and carbon arc lamp therapy. The most commonly used diagnostic method for pompholyx was inspection along with pulse diagnosis, abdominal diagnosis, tongue diagnosis, culture test, and KOH smear screening. The most frequently used treatment evaluation method was Likert scale while VAS(Visual Analog Scale) and Visual&Subjective symptoms were each used in 4 studies. Among 29 patients in 14 papers who showed clinical manifestations of pompholyx, there were 15 male patients (52%), 14 female patients (48%), and the male to female ratio showed 1.07:1. Symptoms were improved using Korean Medicine treatments including acupuncture and herbal medicine. Concurrent treatment using special treatments such as pharmacopuncture, jiyang-go, bojungikgi-tang(ex), external applications showed higher treatment effects. Korean medical treatment of pompholyx was effective in all studies, but the lack of numbers of research on Korean medicine of pompholyx was witnessed compared to other diseases such as psoriasis. Also considering the number of patients suffering from pompholyx, further studies should be conducted.

Evaluation of the Usefulness of MapPHAN for the Verification of Volumetric Modulated Arc Therapy Planning (용적세기조절회전치료 치료계획 확인에 사용되는 MapPHAN의 유용성 평가)

  • Purpose: Latest linear accelerator and the introduction of new measurement equipment to the agency that the introduction of this equipment in the future, by analyzing the process of confirming the usefulness of the preparation process for applying it in the clinical causes some problems, should be helpful. Materials and Methods: All measurements TrueBEAM STX (Varian, USA) was used, and a file specific to each energy, irradiation conditions, the dose distribution was calculated using a computerized treatment planning equipment (Eclipse ver 10.0.39, Varian, USA). Measuring performance and cause errors in MapCHECK 2 were analyzed and measured against. In order to verify the performance of the MapCHECK 2, 6X, 6X-FFF, 10X, 10X-FFF, 15X field size $10{\times}10$ cm, gantry $0^{\circ}$, $180^{\circ}$ direction was measured by the energy. IGRT couch of the CT values affect the measurements in order to confirm, CT number values : -800 (Carbon) & -950 (COUCH in the air), -100 & 6X-950 in the state for FFF, 15X of the energy field sizes $10{\times}10$, gantry $180^{\circ}$, $135^{\circ}$, $275^{\circ}$ directionwas measured at, MapPHAN allocated to confirm the value of HU were compared, using the treatment planning computer for, Measurement error problem by the sharp edges MapPHAN Learn gantry direction MapPHAN of dependence was measured in three ways.
