Analysis of Prescriptions for Oral Solid Dosage Forms Split at Primary Health Care Using National Health Insurance Database (의원의 건강보험청구자료를 이용한 고형경구제 분할 처방 분석)

  • Tablet splitting is used in pharmacy practice to adjust the dose to be administered. However, it also causes several problems such as undesirable effect for sustained release or enteric-coated dosage form, inaccuracy of dose, and pharmacist's safety by splitting hazardous drugs. This study investigated the current status of oral dosage form splitting for patients older than 19 years by analyzing Korea National Health Insurance Claims Database. Out of oral solid drugs prescribed (N=1,486,584) 9.8% of them included tablets (or capsules) split. There were some splitting cases even in sustained release (4.9%), enteric-coated forms (1.3%) and hazardous drugs (2.7%) that were selected by NIOSH (The National Institute for Occupational Safety and Health). The most frequently split drugs were antihistamines, neuropsychotics and steroids. In case of digoxin and warfarin, unit doses in a domestic market were not diverse compared to foreign markets. Guidelines for splitting oral solid dosage forms, approval of diverse doses and conducting dose-response studies for the commonly splitting ingredients on Korean people are needed for the saff and effective use of oral solid drugs.

Inferior Capsular Shift Procedure using Splitting Subscapularis and Capsule for Instability of the Shoulder (견관절 불안정성의 견갑하근 및 관절 낭 수평 분할을 이용한 하방 관절 낭 이동술)

  • Purpose: The aim of this retrospective study were to report the short-term results of inferior capsular shift procedure using splitting subscapularis and capsule for the patients who had shoulder instability and were apt to recur after arthroscopic stabilization procedure. Materials & Methods : Fifteen cases of instability of the shoulder were included with an average follow-up of 2 years (range: 1$\~$3 years). There were 13 men and 2 women with an average age of 27years. Multidirectional instability was found in 4 cases, voluntary instability in 3 cases, bony Bankarte lesion in 2 cases and 6 cases were contact sportmen. Thirteen shoulders underwent the inferior capsular shifts and Bankart repairs and 2 shoulders without Bankart lesion underwent the inferior capsular shift only. Average 19 mm of shift (range: 10$\~$25 mm) was done. Results : Fourteen patients showed good and excellent results with one subluxation and one positive apprehension test. Postoperative ranges of motions did not change in forward elevation, external rotation at side and external rotation at 90$^{\circ}$ abduction (p>0.05). Conclusion : Inferior capsular shift procedure using splitting subscapularis and capsule can be helpful in shoulder instability patients who were high-risk group of recurrence with arthroscopic procedure.

Penetration and Splitting of Optic Nerve by Tuberculum Sellae Meningioma

  • Preservation of the optic nerves is an important issue in the resection of tuberculum sellae meningiomas. We report the case of a patient whose optic nerve was penetrated by a tuberculum sellae meningioma. During surgery, a bulging tumor was found to penetrate the right optic nerve. The tumor was gross totally removed, including tumors bulging through the optic nerve. Two trunks of the split optic nerve were preserved. The penetrated shape of the optic nerve may lead to misjudgment and its damage. Divided trunks of optic nerves are difficult to recognize and may be confused for the tumor capsule, because they may be thinned and seem to contain tumors. In addition, a single trunk may be confused for the whole nerve; thus, the other trunk may be easily damaged if not dissected cautiously. Treatment strategy according to the remnant visual acuity was suggested.

A Study on Crack Healing Properties of Cement Composites Mixed with Self-healing Microcapsules (자기치유 마이크로 캡슐을 혼합한 시멘트 복합재료의 품질 및 균열 치유 특성에 관한 실험적 연구)

  • In this study, self - healing microcapsules which can be mixed directly with cement composites were prepared, and the quality and crack healing performance of cement composites with self - healing microcapsules were evaluated. In the past, it has been focused on evaluating self-healing capsules and crack healing properties. Therefore, self - healing microcapsules have been studied for their effect on the quality of cement composites when mixed with cement composites. The table flow and the air flow rate of the cement composite material mixed with self-healing microcapsules were found to have no significant influence on table flow and air volume regardless of mixing ratio. Compressive strength and splitting tensile strength tended to decrease with increasing capsule mixing ratio. As a result of evaluation of crack healing properties according to water flow, initial water permeability decreased, and reaction products were generated over time and cracks were healed.
