Outer Canthus Indicator를 이용한 두부 자세 기록법의 재현성 (Reproducibility and reliability of head posture obtained by the outer canthus indicator)

  • 두부방사선 사진 채득 시 발생가능한 두부의 수직회전은 계측점 선정 및 계측치의 재현성에 영향을 줄 수 있으므로 진단자료의 객관화 및 중첩의 용이성을 위해 두부 회전을 최소화해야 한다. 본 연구에서는 연조직 계측점인 외안각(outer canthus)을 지표로 두부 자세를 기록하고 재현하는 방법의 신뢰도를 평가하고자 하였다. 정상 안모의 성인 남녀 21명에서 두부고정기에 부착한 Outer Canthus Indicator (OCI)를 이용하여 벽면응시를 통한 두부자세에서의 외안각의 위치를 계측하였다. 초진 시 두부자세에서 측모 광학 사진을 촬영하고 이틀 후 같은 계측치로 두부자세를 재현하여 얻은 두 장의 사진을 중첩하여 두부의 수직 회전과 외이도의 변위를 측정하였다. 두 명의 술자가 동일 과정을 반복했을 때 두부의 수직 회전은 각각 평균 $0.69\;{\pm}\;0.43^{\circ}$, $0.98\;{\pm}\;0.65^{\circ}$, 표준오차 0.09, 0.14로 나타났다. 외이도의 수평변위는 각각 평균 $0.90\;{\pm}\;0.72\;mm$, $0.72\;{\pm}\;0.55\;mm$, 표준오차 0.16, 0.12이고 외이도의 수직변위는 평균 $0.81\;{\pm}\;0.68\;mm$, $1.17\;{\pm}\;0.67\;mm$, 표준오차 0.15, 0.15이었다. 술자간 신뢰도를 평가하기 위한 paired t-test에서 변수들간 통계적으로 유의한 차이가 발견되지 않았다. Pearson 상관분석에서 외이도의 수평 변위와 수직 변위 사이에 통계적으로 유의성 있는 상관관계는 나타나지 않았다. 이상의 결과에 따라 OCI가 두부의 수직 회전에 의한 오차를 줄임으로써 규격화되고 상호 중첩 가능한 방사선 사진 및 광학 사진 자료를 채득하는 데 도움을 줄 것으로 사료된다.

Effective Lateral Canthal Lengthening with Triangular Rotation Flap

  • In Korea, lateral canthoplasty, along with medial epicanthoplasty, has become popular over the past years to widen the horizontal length of the palpebral fissure. However, the effect of the surgery differs greatly depending on the shape and structure of the eyes. If over-widened, complications such as eversion, scarring, and conjunctival exposure may occur. Thus, the author of this study suggests a more effective and safe method for lateral canthal lengthening that causes minimal complications. A total of 236 patients underwent lateral canthoplasty between July 2007 and December 2015. For each patient, a triangular flap 4-5 mm away from the lateral canthus was elevated and rotated 45 degrees laterally while the continuity of the lower eyelid gray line was maintained. A new lateral canthus was created by fixating the rotation flap to the lateral orbital rim with minimal skin trimming and tension-free sutures, preventing relapse and maintaining a triangular shape. In more than 95% of cases, effective and satisfactory extension was achieved. On average, a 3 mm extension of the lateral canthus was achieved. There were minor complications such as wound dehiscence, webbing, and scarring, which were easily corrected. The author not only extended the lateral canthus 3-4 mm laterally but also maintained the continuity of the gray line on the lower lid as a more natural-looking triangular shape, while minimizing complications such as webbing and conjunctival exposure.

  • Oblique facial cleft is a rare congenital deformity. Its incidence has been reported as 0.24% of all reported cases of facial cleft. We report on a patient who had a left-sided oblique facial cleft with anopthamia, including lip and palate, nose alar base, and medial canthus. The patient also had a right-sided oblique facial cleft, which included lip and palate, nose alar base, medial canthus, and upper eye brow. Primary closure of the facial cleft was performed using multiple Z-plasty after excision of scar tissue.

Correction of a post-traumatic scar fold in the lateral canthus using modified Z-plasty

  • Post-traumatic scar deformations are common in many skin injury cases. These are usually aesthetic problems, but if these deformations impair mobility and the patient feels uncomfortable, we must resolve them. In a patient with a lateral post-traumatic scar fold in the lateral canthus that shortened the horizontal orbital fissure and created traction on eye opening, we performed modified classic Z-plasty, resulting in aesthetic and functional improvements without recurrence.

Reconstruction of the Medial Canthus Using an Ipsilateral Paramedian Forehead Flap

  • Background The medial canthus is an important area in determining the impression of a person's facial appearance. It is composed of various structures, including canthal tendons, lacrimal canaliculi, conjunctiva, the tarsal plate, and skin tissues. Due to its complexity, medial canthal defect reconstruction has been a challenging procedure to perform. The contralateral paramedian forehead flap is usually used for large defects; however, the bulkiness of the glabella and splitting at the distal end of the flap are factors that can reduce the rate of flap survival. We reconstructed medial canthal defects using ipsilateral paramedian forehead flaps, minimizing glabellar bulkiness. Methods This study included 10 patients who underwent medial canthal reconstruction using ipsilateral paramedian forehead flaps between 2010 and 2012. To avoid an acute curve of the pedicle, which can cause venous congestion, we attempted to make the arc of the pedicle rounder. Additionally, the pedicle was skeletonized from the nasal root to the glabella to reduce the bulkiness. Results All patients had basal cell carcinoma, and 3 of them had recurrent basal cell carcinoma. All of the flaps were successful without total or partial flap loss. Two patients developed venous congestion of the flap, which was healed using medicinal leeches. Four patients developed epiphora, and 2 patients developed telecanthus. Conclusions Large defects of the medial canthus can be successfully reconstructed using ipsilateral paramedian forehead flaps. In addition, any accompanying venous congestion can be healed using medicinal leeches.

여러 통상적인 전뇌방사선치료 기법에서의 정상조직의 조사선량 비교 (Comparison of the Dose of the Normal Tissues among Various Conventional Techniques for Whole Brain Radiotherapy)

  • 목 적: 여러 통상적인 전뇌방사선치료 기법에서의 뇌와 수정체의 방사선 조사선량을 비교하고자 하였다. 대상 및 방법: 전뇌방사선치료를 위한 치료계획을 11명의 환자를 대상으로 컴퓨터단층촬영 영상을 이용하여 다음과 같이 수립하였다. 중심점이 조사영역의 중앙에 위치하고 가쪽눈구석(lateral bony canthus)에서 양측 빔이 평행한 통상적인 계획을 수립하고, 뇌에서 1 cm (렌즈에 대해서는 5 mm)의 여유로 차폐물을 생성하였다(P1). 이후 중심점을 가쪽눈구석으로 옮기고(P2), 조준기를 90도 회전시킨 후 5 mm 두께의 다엽조준기로 차폐를 하였다(P3). 각 환자의 모든 계획에서 P1의 중심점에 30 Gy를 처방하였다. 뇌와 수정체의 선량 지표들은 paired t-test를 이용하여 비교하였다. 결 과: 뇌의 $D_{max}$$V_{105}$는 P1에서 111.9%와 23.6%였다. 뇌의 $D_{max}$$V_{105}$는 P2와 P3에서 유의하게 감소하였는데, 그 값은 각각 107.2%와 4.5~4.6%였다(p<0.001). 수정체의 $D_{mean}$의 평균값은 P1에서 3.1 Gy, P2와 P3에서는 2.4~2.9 Gy였다(p<0.001). 결 론: 전뇌방사선치료 시 다른 복잡한 방법을 이용하지 않고, 중심점을 가쪽눈구석에 위치시키는 것만으로도 뇌와 수정체의 조사선량 측면에서 유리하였다.

비근점 변화를 최소화하는 간단한 융비술 (Augmentation Rhinoplasty Minimizing Change of Nasion Level: A Simple Method)

  • Nasions are flat and located more caudally in Asian than in Caucasian. Implant insertions in rhinoplasties are apt to move nasions more cephallic, which can cause unsatisfactory results in Asian. The purpose of this study is introducing a simple technique in rhinoplasty avoiding unnatural nasion in augmentation rhinoplasty in Asian patients. Multiple radiating incisions were made at nasion level on inner side of silicone implant to fit it into the curvature around the nasion. Between October 2001 and October 2003, 27 patients underwent augmentation rhinoplasties using this technique and results were evaluated. Preoperative and postoperative photographs of patients were used to measure the levels of nasion from medial canthus. The mean nasion level from medial canthus of preopertive patients were $3.46{\pm}0.87mm$ and postoprative patient was $4.98{\pm}0.96mm$. The mean difference between preoperative nasion level and postoperative nasion level was 1.52 mm. There was no complication such as extrusion, displacement or infection. This technique of radiating incisions on inner side of the silicon implant can minimize cephalic migration of the nasion level in augmentation rhinoplasty in Asian.

Changes in Eyebrow Position and Movement with Aging

  • Background This study evaluated dynamic changes in eyebrow position related to aging. Methods Female participants were recruited and separated into two groups aged 20-30 years (the younger group, n=20; mean age, 24.8 years) and 50-70 years (the older group, n=20; mean age, 55.8 years). Photogrammetry was used to determine the eyebrow position at the medial canthus (MC), lateral limbus, lateral canthus, and lateral end point (EP) for 6 actions: smooth opening (the reference action) and closing of the eye, forward gaze, maximum opening and closing of the eye, and maximum frown. Videos were also recorded. Results No differences in eyebrow position were detected at the MC when opening or closing the eyes smoothly, gazing straight ahead, or closing the eyes maximally. For all 6 actions, the position of the lateral EP in the older group was significantly lower than in the younger group (P=0.003), and the smallest degree of vertical movement at this point was found in both age groups (P<0.001). Vertical movement at the 4 landmarks of the eyebrows decreased with aging. Conclusions Eyebrow position was unchanged at the MC with aging, except at maximal eye opening and maximal frown. No differences in eyebrow position were observed between the younger and older groups when eyes were maximally closed, except at the EP. It is important to focus on correction of the lateral EP for periorbital rejuvenation.

  • This study was performed In evaluate sleep efficiencies and conditions for comfortable sleep based on the analysis of Physiological signals under variations in thermal conditions. Five female subjects who have similar life cycle and sleep patterns were participated for the sleep experiment. It was checked whether they had a good sleep before the night of experiment. EEGs were obtained from C3-A2 and C4-A1 electrode sites and EOGs were acquired from LOC (left outer canthus) and ROC (right outer canthus) for REM sleep detection. Sleep stages were classified, then TST (total sleep time), SWS (slow wave sleep) latency and SWS/TST were calculated for the evaluation of sleep efficiencies on thermal conditions. TST was defined as an amount of time from sleep stage 1 to wakeup. SWS latency was from light off time to sleep stage 3 and percentage of SWS over TST was calculated for the evaluation of sleep quality and comfort sleep under thermal conditions. As result, the condition which raise a room temperature provided comfortable sleep.

The Versatility of Cheek Rotation Flaps

  • Background: The cheek rotation flap has sufficient blood flow and large flap size and it is also flexible and easy to manipulate. It has been used for reconstruction of defects on cheek, lower eyelid, or medial and lateral canthus. For the large defects on central nose, paramedian forehead flap has been used, but patients were reluctant despite the remaining same skin tone on damaged area because of remaining scars on forehead. However, the cheek flap is cosmetically superior as it uses the adjacent large flap. Thus, the study aims to demonstrate its versatility with clinical practices. Methods: This is retrospective case study on 38 patients who removed facial masses and reconstructed by the cheek rotation flap from 2008 to 2015. It consists of defects on cheek (16), lower eyelid (12), nose (3), medial canthus (3), lateral canthus (2), and preauricle (2). Buccal mucosa was used for the reconstruction of eyelid conjunctiva, and skin graft was processed for nasal mucosa reconstruction. Results: The average defect size was $6.4cm^2$, and the average flap size was $47.3cm^2$. Every flap recovered without complications such as abnormal slant, entropion or ectropion in lower eyelid, but revision surgery required in three cases of nasal side wall reconstruction due to the occurrence of dog ear on nasolabial sulcus. Conclusion: The cheek rotation flap can be applicable instead of paramedian forehead flap for the large nasal sidewall defect reconstruction as well as former medial and lateral canthal defect reconstruction.
