Bone mineral density deficits in childhood cancer survivors: Pathophysiology, prevalence, screening, and management

  • As chemotherapy and other sophisticated treatment strategies evolve and the number of survivors of long-term childhood cancer grows, the long-term complications of treatment and the cancer itself are becoming ever more important. One of the most important but often neglected complications is osteoporosis and increased risk of fracture during and after cancer treatment. Acquisition of optimal peak bone mass and strength during childhood and adolescence is critical to preventing osteoporosis later in life. However, most childhood cancer patients have multiple risk factors for bone mineral loss. Cancer itself, malnutrition, decreased physical activity during treatment, chemotherapeutic agents such as steroids, and radiotherapy cause bone mineral deficit. Furthermore, complications such as growth hormone deficiency and musculoskeletal deformity have negative effects on bone metabolism. Low bone mineral density is associated with fractures, skeletal deformity, pain, and substantial financial burden not only for childhood cancer survivors but also for public health care systems. Thus, it is important to monitor bone health in these patients and minimize their risk of developing osteoporosis and fragility fractures later in life.

Development and pilot study of a cancer rehabilitation smartphone application for cancer survivors

  • The purpose of this study is to develop a cancer rehabilitation program for cancer survivors in the form of a smartphone application and to confirm the effectiveness through Pilot study. The contents of the application consisted of health records, lab-test records, and health information, and the information recorded by the patient was graphically checked for changes over time on my page. 7 subjects who ended acute treatment and were undergoing follow-up were asked to use the application for 4 weeks, and then changes in variables (uncertainty, e-health literacy, self-efficacy, and cancer rehabilitation) were confirmed. It was confirmed that e-health literacy and self-efficiency increased significantly over time after using the application. In addition, the level of cancer rehabilitation was found to increase significantly over time. The application for cancer rehabilitation developed in this study needs to be expanded to improve the quality of life of cancer survivors.

Effects of 8 weeks of combined forest exercise on quality of life and physical self-concept of breast cancer survivors

  • The purpose of this study was to investigate the effect of 8 weeks of forestry exercise on the quality of life and physical self-concept of breast cancer survivors. The subjects of this study were eight breast cancer survivors 6 months after mastectomy. The forest combined exercise program consisted of aerobic exercise through forest walking and resistance exercise using elastic bands. The forest combined exercise was conducted twice for 8 weeks. Forest trekking consisted of a 2km walking speed and resistance exercise consisted of three levels of sets and intensity. The format was divided into gradual increases. The exercise time was 40 to 60 minutes for forest trekking, 20 to 30 minutes for descent, and 40 to 60 minutes for resistance exercise, for a total of 120 to 130 minutes per day. Breast cancer survivors' quality of life was measured using a questionnaire, and changes in quality of life were measured using a t-test (α=.05). Physical self-concept was assessed through in-depth interviews. There was no statistically significant difference in quality of life before and after 8 weeks of combined forestry exercise, but there was a slight tendency to increase in the area of physical well-being. Physical self-concept showed positive changes in motivation, physical strength improvement, health promotion, physical competence, and self-confidence through the forest composite exercise. Therefore, the forest composite exercise is believed to have a positive effect on the physical self-concept of breast cancer survivors.

Psychometric Characteristics of the Fear of Cancer Recurrence Inventory-Severity Subscale Among Korean Cancer Survivors

  • Objectives: Despite the importance of choosing and using a valid assessment tool for fear of cancer recurrence (FCR) for early detection and interventions, the validity of the FCR inventory has yet to be thoroughly investigated in Korea. This study explored the psychometric properties of the Fear of Cancer Recurrence Inventory-Severity (FCRI-S) subscale and assessed its applicability to cancer survivors in Korea. Methods: The survey involved 93 Korean individuals who had survived cancer. The reliability of the FCRI-S subscale was assessed using Cronbach's α and composite reliability (CR). Confirmatory factor analysis (CFA), along with tests for discriminant and convergent validity, was conducted to evaluate the construct validity of the FCRI-S subscale. Results: The FCRI-S subscale showed excellent internal consistency (Cronbach's α=0.88; CR=0.89). CFA showed a good factor structure for the FCRI-S subscale, and the correlations of the FCRI-S subscale with FCR-related measures (r=0.69 to 0.80) and other psychosocial measures (r=-0.23 to 0.37) confirmed both the convergent and discriminant validity of the FCRI-S subscale. Conclusions: This study confirmed the robust psychometric characteristics of the FCRI-S subscale among cancer survivors in Korea. The use of the FCRI-S subscale would be helpful for health professionals to rapidly screen FCR levels in clinical settings.

한국 암생존자의 행복감에 대한 진화론적 개념분석 (Evolutionary Concept Analysis of Korean Cancer Survivors' Happiness)

  • 본 연구는 한국 암 생존자의 삶에서 경험하는 행복감을 진화론적 개념분석 방법을 통해서 확인하고자 하였다. 분석 자료는 2000년 1월부터 2017년 9월까지 본 연구의 목적에 부합되는 11개의 논문을 분석하였다. 그 결과 한국 암 생존자의 행복감에서 나타나는 속성은 마음의 평정을 이루는 주관적 경험, 삶의 만족감과 안녕감을 위한 긍정적 마음, 작은 것의 소중함과 고마움을 깨닫게 되는 삶의 의미, 타인을 이해하고 도우며 더불어 살아가는 삶을 위한 관계형성이었다. 선행요인으로는 삶의 위기를 수용하고 현실에 대처하며 자기성찰과 환경적 지지가 이루어지는 것이며, 결과요인은 내면의 힘이 증가하고, 사랑을 느끼며, 새로운 삶을 추구하고, 자기실현을 이루게 된다. 본 연구는 한국 암 생존자의 행복감을 증진시킬 수 있는 단계적 사정을 위한 기본틀을 제시하였다는데 의의가 있으며, 다학제적 융합프로그램을 통한 암 생존자 관리가 이루어져야 할 것이다.

한국 여성 유방암 생존자의 건강신념과 건강행위에 대한 질적 연구 (A Qualitative Study of Health Beliefs and Health Behaviors among Korean Breast Cancer Survivors)

  • 본 연구는 질적 연구 방법 중의 하나인 초점 집단 접근을 통해 우리나라 유방암 생존자의 1) 질병경험, 2) 건강신념(원인적 사고), 및 3) 건강행위 (진단 후 건강 행동 변화) 등을 조사함으로써, 유방암 생존자의 질병경험 및 건강신념이 어떻게 건강행위와 관련이 있는지를 파악하는 것을 목적으로 한다. 서울 및 경기에 거주하는 16명의 유방암 생존자를 대상으로 3개의 초점 집단이 수행되었으며, 각 집단에는 약 5명에서 6명 정도의 구성원이 참석하였다. 본 연구는 유방암 생존자들의 질병경험, 건강신념, 그리고 건강행위에 대한 다양한 신체 및 행동적, 심리적, 그리고 사회적 문제 등과 관련된 주제를 도출하였다. 본 연구를 통한 우리나라 유방암 생존자들의 건강신념 및 건강행위에 대한 이해는 건강신념에 근거한 건강행위 증진을 위한 사회복지실천 개입 방안을 모색하는데 중요한 토대가 될 것으로 보인다.

부인암 생존자의 심리적 디스트레스와 성역할태도, 가사분담이 삶의 질에 미치는 영향 (Impacts of Psychological Distress, Gender Role Attitude, and Housekeeping Sharing on Quality of Life of Gynecologic Cancer Survivors)

  • Purpose: To determine impacts of psychological distress, gender role attitude, and housekeeping sharing on quality of life of gynecologic cancer survivors. Methods: Subjects completed questionnaires consisting of four measurements: FACT-G (ver. 4) for quality of life, anxiety and depression from SCL-90-R for psychological distress, gender role attitude, and housekeeping sharing. A total of 158 completed data sets were analyzed by descriptive statistics, independent t-test, ANOVA, Pearson's correlation coefficient, and multiple regression analysis using SPSS 21.0 program. Results: Quality of life was significantly and negatively correlated with psychological distress (r=-0.64, p<.001), but not with gender role attitude (r=-0.14, p=.820) or housekeeping sharing (r=0.08, p=.350). Psychological distress was significantly and positively correlated with gender role attitude (r=0.25, p<.010). Factors that significantly impacted quality of life were type of cancer (t=2.27, p=.025), number of treatment methods (t=2.58, p=.011), education level (t=3.33, p<.001), and psychological distress (t=-9.96, p<.001). Conclusion: Nursing interventions that can relieve psychological distress need to be developed for gynecologic cancer survivors. Nurses need to put priority on ovarian cancer survivors who have low education level with multiple treatment methods when performing nursing interventions to improve the quality of life of gynecologic cancer survivors.

신체활동 수준이 암 생존자의 건강 관련 삶의 질에 미치는 영향: 2014-2018년 국민건강영양조사 자료 활용 연구 (Influence of Physical Activity Level on the Health-related Quality of Life of Cancer Survivors: Based on the Korea National Health and Nutrition Examination Survey for 2014-2018)

  • Purpose: This study investigated the associations between physical activities (PAs) and the health-related quality of life (HRQOL) and perceived health status (PHS) of cancer survivors. We further examined the interaction effects of PAs and covariates on HRQOL and PHS. Methods: Data sets were obtained from the 2014-2018 Korea National Health and Nutrition Examination Survey. The subjects were 1,349 cancer survivors aged over 18 years old. Data were analyzed using R 4.0.3 and SPSS 18.0. Logistic regression analysis was conducted considering only the main terms, or including additional interaction terms between PAs and covariates. Results: Moderate and high PAs showed significantly improved HRQOL related to self-care domain, euro quality of life-5 dimension index, and PHS. Interaction analysis revealed that high PAs resulted in improved HRQOL associated with self-care and pain/discomfort in cancer survivors having depression. Moreover, for low- and middle-income levels, higher PAs served to improve HRQOL associated with depression/anxiety. In contrast, higher PAs rather reduced HRQOL for the high-income group. Conclusions: To improve HRQOL, we recommend PAs higher than the moderate level for cancer survivors. In case of cancer survivors having depression or belonging to the high-income group, it is necessary to manage individual PAs considering the interaction effects.

유방암 생존자의 질병 극복 경험 (A Qualitative Study on Breast Cancer Survivors' Experiences)

  • Purpose: This study was performed to understand the characteristics and the meaning of the illness experience of breast cancer survivors as basic data for the development of an intervention program. Methods: The participants were 25 breast cancer survivors who had completed treatment at a tertiary hospital in Seoul. Data were collected through in-depth and unstructured audio-recorded interviews by the investigator. The participants were asked to describe their illness experience. The data were analyzed according to Giorgi's method for phenomenological analysis. Results: The interview data were organized by theme into 6 categories that emerged from the analysis. The themes were acceptance of the illness, active coping with reality, gaining strength through the support of surrounding people, struggling to overcome a negative mindset, self-reflection, and the pursuit of a meaningful new life. Conclusion: We recommend the development of a survivorship program based on self-reflection, which can engender self-transcendence and spiritual well-being.

Prevalence of Depression in Breast Cancer Survivors: a Systematic Review of Observational Studies

  • Background: Depression is common in breast cancer patients. The aim of this paper was to make a systematic review of its prevalence and associated factors oin breast cancer survivors. Materials and Methods: An extensive systematic electronic review (PUBMED, CINAHL, PsyINFO and Ovid) and handsearch were carried out to retrieve published articles up to November 2012, using Depression OR Dysthymia AND (Cancer OR Tumor OR Neoplasms as the keywords. Information about the design of the studies, measuring scale, characteristics of the participants, prevalence of depression and its associated factors from the included studies were extracted and summarized. Results: We identified 32 eligible studies that recruited 10,826 breast cancer survivors. Most were cross-sectional or prospective designed. The most frequent instrument used to screen depression was the Center for Epidemiological Studies for Depression (CES-D, n=11 studies) followed by the Beck Depression Inventory (BDI, n=6 studies) and the Hospital Anxiety and Depression Scale (HADS, n=6 studies). CES-D returned about similar prevalence of depression (median=22%, range=13-56%) with BDI (median=22%, range=17-48%) but higher than HADS (median=10%, range=1-22%). Depression was associated with several socio-demographic variables, cancer-related factors, treatment-related factors, subject psychological factors, lifestyle factors, social support and quality of life. Conclusions: Breast cancer survivors are at risk for depression so that detection of associated factors is important in clinical practice.
