본 연구는 중·고령 초기 유방암 생존자를 위한 가족들의 돌봄 경험에 대해 이해하고자 한 질적 연구이다. 총 4명의 유방암 생존자 가족들을 대상으로 초점집단면접(FGI)을 실시하였고 추가로 1명의 가족에 대해 개별심층면접을 실시하였다. 초점집단면접과 개별심층면접 내용을 주제 분석한 결과, 다음 4가지 주제가 나타났다: 첫째, 살얼음 위를 걷는 듯한 일상의 감정들 (치료시기마다 서로 뒤얽히는 감정들, 생존자의 고통을 지켜볼 수밖에 없는 안타까움, 암 재발과 죽음에 대한 두려움, 관계 변화에 대한 걱정, 딸로서 겪는 다양한 감정), 둘째, 생활의 중심이 된 돌봄 (낯설고 서툴렀던 돌봄, 내 생활을 잃어감), 셋째, 회복의 시작, 생존자와 가족이 느끼는 온도 차이 (생존자가 느끼는 심연의 고통을 헤아리지 못함, 생존자가 아직 환자라는 사실을 잊고 소홀히 대함), 넷째, 앞으로 나아가기 (나아질 것이라는 희망, 지치지 않기 위한 나를 위한 돌봄). 본 연구 결과를 토대로 유방암 생존자 가족의 심리사회적 욕구를 반영하고 돌봄 과정에서 가족 지지를 지원하기 위한 사회복지서비스 및 정책 방안에 대해 제언하였다.
Purpose: This study investigated weight status in survivors of childhood acute lymphocytic leukemia (ALL) and identified related factors. Methods: A retrospective review of the electronic medical records of survivors of childhood ALL (n=230) was conducted. We analyzed the survivors' characteristics, including sex, age, weight status at diagnosis, central nervous system involvement, risk classification, length of treatment, radiation therapy, and hematopoietic stem cell transplantation. Analysis of variance and the chi-squared test were applied to investigate influencing factors. Results: The weight status distribution was as follows: 23 individuals (10.0%) were classified as underweight, 151 individuals (65.7%) were healthy weight, and 56 individuals (24.3%) were overweight/obese. Age at diagnosis (F=10.03, p<.001), weight status at diagnosis (x2=43.41, p<.001), and risk classification (F=10.98, p=0.027) showed significant differences among the weight status groups. Survivors who were older at diagnosis and those in the very high-risk category had a higher likelihood of experiencing underweight status during their survivorship, while survivors who were overweight/obese at diagnosis were more likely to remain overweight/obese at the time of survival. Conclusion: Considering the potential health implications related to an unhealthy weight status in survivors of ALL, it is imperative to undertake early identification and implement interventions for at-risk individuals.
Background and Objectives: As one of the most common treatments for breast cancer, mastectomy has adverse effects on the quality of sexual life and sexual functioning in the impacted women. Various strategies have therefore been proposed to resolve their sexual problems. The present study was conducted to determine the effect of sexual rehabilitation using the PLISSIT model in post-mastectomy breast cancer survivors. Materials and Methods: The present quasi-experimental study was conducted on a population of post-mastectomy breast cancer survivors and their husbands. Sample size was calculated as 50 each for intervention and non-intervention groups. The former received sexual counseling based on the PLISSIT model consisting of four levels of intervention: permission, limited information, specific suggestion and intensive therapy, presented in four 90-minute sessions. Data were collected using the Sexual Quality of Life-Female (SQOL-F) questionnaire and the Female Sexual Function Index (FSFI). Results: No significant differences were observed in the mean quality of sexual life scores between the intervention and control groups (P>0.05) before the intervention; however, a significant difference emerged between the groups after the intervention (P<0.01). Thus the mean score for sexual functioning in the intervention group was $26.3{\pm}3.76$ before and $30.0{\pm}4.38$ after the intervention (P<0.0001). In the control group, however, the difference between the pre- and post-intervention mean scores was not statistically significant (P=0.713). Conclusion: The present study showed that nurses can use the PLISSIT model in conjunction with chemotherapy and radiotherapy to teach coping and problem-solving skills to women with breast cancer and their husbands and to encourage their participation in group programs for expressing their feelings and attitudes about their current sex life and thus help enhance quality of sexual life and sexual functioning in this group.
Background: Cross-culturally adapted questionnaires may not be comparable to their original version. Objects: To examine concurrent validity of two health-related quality of life (HRQOL) instruments for the Korean versions of EuroQOL-5 Dimension (EQ-5D) and the abbreviated version of the World Health Organization Quality of Life (WHOQOL-BREF) instrument. Methods: A total of 139 cancer survivors from two rehabilitation institutes was recruited. All participants were registered for palliative rehabilitation care. Both instruments were concurrently administered by health care providers following the second bout of the rehabilitation cares. Rasch partial credit model and Spearman's correlation analysis were used to investigate: 1) dimensionality, 2) hierarchical item difficulty, and 3) concurrent validity using correlations between two instruments. Results: For the WHOQOL-BREF, all items except negative feeling, pain, dependence of medical aid, were found to be acceptable, while all items of EQ-5D were acceptable. There was an evidence of negative correlations between EQ-5D and 4 domains of WHOQOL-BREF. Two correlations were strong (EQ-5D vs. physical health domain, ρ = -0.610, 95% CI = -0.716 to -0.475) and moderate (EQ-5D vs. psychosocial domain, ρ = -0.402, 95% CI = -0.546 to -0.236). Other two correlations were weak (EQ-5D vs. social relationship and environmental domains, ρ = -0.242, 95% CI = -0.401 to -0.075 and ρ = -0.364, 95% CI = -0.514 to -0.207, respectively). Item difficulty calibrations of the two measurements were ranged from -0.84 to 0.86 for the EQ-5D and -1.07 to 1.06 for the WHOQOL-BREF. Conclusion: The study provides some supports for the concurrent validity of the two Korean versions of HRQOL instrument, with evidences of weak to strong correlations between the EQ-5D and four domains of the WHOQOL-BREF applied to various cancer survivors. Additionally, the cancer survivors appeared to have more of a tendency to view the EQ-5D items as being slightly more challenging than the WHOQOL-BREF items.
본 연구는 우리나라에서 가장 유방암 발생률이 높은 40대 유방암 생존자들의 우울과 불안의 정도를 파악하고 그 위험요인들을 검토하기 위해 수행되었다. 자료 수집은 유방암으로 진단 받은 40-49세 유방암 생존자 중 연구에 동의하고 설문지를 완성한 609명을 대상으로 이루어졌다. CES-D와 GAD-7을 사용하여 측정된 우울과 불안의 평균점수는 각각 16.35(SD=9.24), 4.25(SD=4.17)였으며, 절단점을 기준으로 우울군은 전체의 47.7%, 불안군은 10.3%로 나타났다. BPI-K로 측정된 통증강도 평균은 1.91(SD=1.60)이었으며, 전체의 82.8%가 통증을 호소하였고 10.9%가 중증도 이상의 통증을 보고하였다. 위계적 회귀분석결과 최종모델에서 통증 일상생활방해정도, 직업 없음, 동거가족유형과 과거 정신과 질환이 우울의 유의미한 위험요인으로 나타났고, 불안의 위험요인은 통증 일상생활방해정도, 직업 없음 및 과거 정신과질환으로 나타났다. 이러한 결과는 우리나라 40대 유방암 생존자의 우울과 불안 유병률이 높으며, 우울과 불안의 위험요인에 초점을 두어 위험군에 대한 적절한 심리사회적 개입이 제공되어야 함을 지적해주고 있다.
본 연구에서는 암생존자의 스트레스가 우울과 삶의 질에 영향을 미치는 과정에 외상 후 성장이 조절효과를 나타내는지 확인하고자 하였다. 이를 위해 서울시, 경기도, 충청도, 전라도에 거주하는 암생존자 173명을 대상으로 설문조사를 실시하였고 응답한 자료로 위계적 회귀분석을 실시하였다. 분석 결과 첫째, 외상 후 성장은 스트레스와 상호작용하여 암생존자의 우울 및 삶의 질에 영향을 미쳤다. 즉 스트레스 수준이 높더라도 외상 후 성장이 높으면 외상 후 성장이 낮을 때보다도 우울 수준이 더 낮고, 삶의 질 수준이 더 높은 것으로 나타났다. 둘째, 외상 후 성장은 삶의 질의 하위요인 중 정서적 및 기능적 삶의 질에서만 조절효과를 나타냈다. 본 연구결과를 바탕으로 암생존자의 우울 감소와 삶의 질 증진을 위해서는 외상 후 성장을 높이는 교육 및 상담적 개입이 필요함을 논의하였다.
Purpose: The purpose of this study was to examine the effects of a smart-care services program for breast cancer survivors on cognitive function and physical health. Methods: A quasi-experimental control group pretest posttest design was used. Subjects were recruited in D city, and data were collected from July 2017 to February 2018. The experimental group (n=24) participated in the smart-care services program, whereas the control group (n=26) received conventional management. The smart-care services program consisted of addressing cognitive function problems arising from chemotherapy, diet, exercise, head/neck massage and self-monitoring using smartphone applications and smart bands. All participants underwent assessments at baseline, at 6 weeks, and at 12 weeks. Data were analyzed using descriptive statistics (frequency, percentage, mean and standard deviation), a chi-squared test, t-test, and repeated measures ANOVA. Results: After the smart-care services program, significant differences were found between the groups in cognitive function (F=18.91, p<.001) and sleep time (F=9.25, p<.001). No significant differences were found between the groups in caloric consumption after the program. Conclusion: The smart-care services program significantly improved the level of cognitive function and sleep time for breast cancer survivors. The use of this smart-care services program for breast cancer survivors might be an effective nursing intervention tool for improving cognitive function and health behaviors.
Patients with pediatric cancer often undergo multiple therapies, such as chemotherapy, radiation therapy, and stem cell transplantation. These treatments, while essential, can result in dental developmental issues, including hypodontia, microdontia, short roots, and delayed dental development. This report presents two cases of pediatric patients diagnosed with neuroblastoma who exhibited severe tooth mobility due to short roots as a complication of cancer treatment. Moreover, we investigated the conservative management of the patients' conditions using resin wire splints and orthodontic miniscrews for skeletal anchorage along with long-term follow-ups to evaluate their prognosis.
목적 본 연구는 함께 치료를 받았던 암 환우의 죽음이라는 간헐적이나 지속적으로 발생하는 사건에 노출된 청소년 소아암 생존자의 슬픔을 탐색하기 위해 시도되었다. 방법 자료는 만 13-18세의 소아암 청소년 생존자 12명과 반구조화된 면접(일대일면접, 전화면접, 인터넷 채팅), 자조 집단의 참여관찰, 그리고 인터넷 개인 홈페이지나 블로그에 올려 있는 참여자들의 글을 수집하였다. 근거이론방법론 분석의 일환인 Clarke (2005)의 상황분석을 이용하여 자료를 분석하였다. 결과 상황분석에 의거하여 세 가지 수준의 지도를 도식화하였다. 암 환우를 잃은 슬픔에 빠진 청소년 소아암 생존자와 관련된 모든 상황적 구성요소들이 첫 번째 상황적 지도의 두 가지 버전에 의해 마련되었고, 두 번째로 슬픔에 처한 청소년 소아암 생존자를 중심으로 주변 인물들의 관계 및 슬픔이 일어나는 주요 장소에 대한 사회적 세계 지도가 그려졌다. 마지막으로 타인과의 관계 수준에 따른 슬픔의 표출 정도를 위치 지도에 도식화하였다. 청소년 소아암 생존자의 슬픔은 어두운 잔존 기억에 대처하기 위해 내면의 자아와 의사소통하는 동시에 친구 및 가족과의 상호작용 속에서 끊임없이 자신의 생각, 감정, 행동을 협상하였다. 슬픔은 특히 어머니에게 표출하지 않고자 노력하였으며, 슬픔으로 초래되는 정신적 힘듦을 피하기 위해 아무렇지 않은 듯 혼자 시간 보내거나 의식적으로 암을 앓고 있지 않은 건강한 친구와의 관계맺음을 선호하였다. 결론 청소년 소아암 생존자의 슬픔은 다양한 상황적 맥락에 연루된 사회 환경적 요소들에 따라 숨겨지거나 표출되었다. 상황분석은 이러한 요소들 간의 관계를 시각적으로 정렬하며 비교, 분석하는 데 있어서 유용하였다.
본 연구는 유방암예방 자원봉사활동을 통한 유방암 생존자들의 자기효능감, 우울, 및 외상 후 성장의 장기적 효과를 탐색하기 위해 실시되었다. 본 예비연구는 종단적 방법을 이용하였으며 참여자는 유방암예방교육프로그램에 참여한 유방함 생존자 14명이었다. 자료는 2013년 2월부터 8월까지 수집되었으며 자가보고식 설문지를 이용하여 유방암 자가관리에 대한 자기효능감, 우울, 및 외상 후 성장을 교육 전(T0), 교육 후 1개월(T1), 교육 후 3개월(T2), 교육 후 6개월(T3)에 각각 측정하였다. 효과분석을 위해 일반화 추정 방정식을 사용하였다. 자기효능감 점수는 T0과 비교했을 때 T1(${\chi}^2$=5.56, p=.018),T3(${\chi}^2$=4.56,p=.033)시점에 유의하게 증가하였다. 6개월 동안 우울은 일반인에 비해 낮은 수준으로 큰 변화가 없었고 외상 후 성장은 비교적 높은 점수가 유지되었다. 이러한 결과는 유방암 생존자들의 유방암예방 자원봉사활동이 자기효능감을 향상시키고 심리적 안녕을 유지하는데 효과적일 수 있음을 시사한다. 유방암 생존자의 유방암예방 자원봉사활동은 일반인 뿐 아니라 참여한 유방암 경험자들에게도 유용한 교육프로그램임을 보여주었다.
