Breast, Cervix and Colorectal Cancer Knowledge among Nurses in Turkey

  • Background: Cancer is one of the most common causes of death in Turkey. Nurses are essential providers of preventive care for patients, especially breast, cervical and colorectal cancer screening as part of routine preventive practice. The aim of this study was to assess knowledge of these cancers among nurses in Karabuk State Hospital. Materials and Methods: This cross-sectional and descriptive study was performed from April 1 to July 30, 2013. The study sample consisted of 226 nurses working in Karabuk State Hospital. Results: Mean age of the nurses was $32.07{\pm}8.39$. 62.4% of nurses practiced breast self examination when they remembered it, while 39.8% of them did not take a Pap smear test since they did not think it was necessary. 64.2% of nurses would like to receive information about cancer and screening tests. Majority of them had given true answers to questions on breast, cervical and colorectal cancer. There were significant relationships between cancer knowledge scores and marital status, working experience, and level of education. Conclusions: Nurses possess adequate knowledge about breast cancer but they need more information on cancer risk estimation. Awareness may be raised in nurses by establishing continuing education programs regarding the risk factors, symptoms, protection methods, early diagnosis, and scanning of breast, cervix and colon cancers.

생명보험(生命保險) 유진사(有診査) 및 무진사가입자(無診査加入者)의 사망(死亡)에 관한 고찰(考察) (An Analysis on the Difference of Death Rates Between the Insured with Medical Examination and the Insured Without Medical Examination)

  • In Korea, life insurance policies are sold to the policy holders by insuring either the insureds undergo a medical examination at a clinic or the insureds' report their history of diseases ever experienced that replaces the medical examination. This study aimed to measure the level of death rates for the insureds between those who received medical examination and those who did not receive medical examination, and to examine differences of the rates in terms of the insureds' characteristics such as age, sex, cause of death and duration. A total number of 32,358 insureds were selected for the population of this study from the D. Life Insurance Company located in Seoul City. Out of the 32,358 insureds, 2,997 received medical examination and the rest of 29,381 did not received any medical examination. Results of analysis are summarized as follows: 1. Death rate per 100,000 insureds for the all was 19.3 in the first year, 96.3 in the second year, 143.8 in the third year 93.4 in the fourth year. For the group of medical examination received, the rate was zero in the first year, 41.3 in the second year, 55.4 in the third year and 268.8 in the fourth year, and for the group of non-medically examined the rate was 21.3 in the first year, 101.9 in the second year, 152.2 in the third year and 76.8 in the fourth year. The levels of death rates between the insureds with medical examsination and the inureds without medical examination were non-significant in the differences by duration except the levels of the third year, which indicated the death rate of non-medically examined group was higher than that of the medically examined group. 2. 73.0 per cent of the total deaths observed during the insured period were caused by various diseases and the rest of 27.0 per cent deaths were due to accidents. For the group of medical examination received, 55.6 per cent deaths were caused by diseases, and for the group of nonmedically examined, 74.7 per cent of deaths were due to diseases. 3. cancer was the most frequent cause which accounted for 22.0 per cent of the total deaths. Proportion of deaths due to cancer from the group of medical examination received was 22.2 per cent, and the corresponding rate for the group of non-medically examined also showed high rate of 22.0 per cent.

유방암 선별검사의 경계연령에 대한 고찰 (Screening Examination of Breast Cancer: Review of the Recommended Guidelines)

  • 영남대학교 의과대학 부속병원에서 1992년 3월부터 1999년 6월까지 유방암으로 확진된 820례 중 35세 미만인 예는 102례(12.4%)이며 60세 이상인 예는 122례(14.9%)로 젊은 여성에 있어서도 유방암환자가 잦은 빈도로 발견되었다. 유방촬영술 또는 유방초음파검사를 시행한 35세 이하군 49명의 유방촬영술의 병변검색율은 83.8%, 유방암에 대한 민감도는 67.6%이었고, 초음파검사의 병변검색율은 100%, 유방암에 대한 민감도는 87.2%이었다. 60세이상의 대조군 48명 중 유방촬영술을 시행한 경우는 34명이며, 유방초음파검사는 81명에서 시행되었다. 유방촬영술의 병변검색률은 100%, 유방암에 대한 민감도는 91.2%이였고, 초음파검사의 병변겸색율은 100%, 유방암에 대한 민감도는 96.7%이었다(표 2). 조직학적으로는 35세 이하군에서 침윤성 유도관상피암 30명(61.2%), 유도관내 암종 8명(16.3%)이며 핵분열정도는 I이 3.8%, II가 26.9%, III가 39.2%이었고, 60세이상의 대조군에서는 침윤성 유도관상피암 32명(66.6%), 유도관내 암종 4명(8.33%)이고 핵분열정도는 I이 9.4%, II가 31.3%, III가 59.4%이었다(표1, 표3). 결론적으로 35세 이하군의 조직학적 분류상 악성도가 높은 종양이 많고 발생빈도가 60세 이상군과 유의한 차이가 없으므로 조기유방암의 선별을 위해서는 American Cancer Society에서 권장하는 baseline mammogram의 시행시기인 35-40세보다 더 이른 시기에 선별검사가 필요하다고 생각한다.

전립선암 발견에 있어 경직장 초음파 검사의 유용성: 전립선특이항원 수치가 10 ng/ml 이하인 환자를 대상으로 (Effectiveness of the Transrectal Ultrasonography in the Detection of Prostate Cancer: in Patients with Prostate Specific Antigen of 10 ng/ml or Less)

  • Background: This study was performed to reconsider the efficacy of transrectal ultrasonography (TRUS) in diagnosing prostate cancer by analyzing the results of a digital rectal examination (DRE), serum prostate-specific antigen (PSA) and a transrectal ultrasonography in patients with prostate specific antigen levels of 10 ng/ml or less. Materials and Methods: One-hundred and eighty one men with PSA levels of 10 ng/ml or less, who had a TRUS-guided tissue biopsy performed, were included in this study. The detection rate of prostate cancer was compared according to the TRUS result and the presence or absence of nodularity and the consistency of the prostate on DRE. Results: In a total 181 patients, there were 73 patients with PSA levels of 4 ng/ml or less and 4 of them had prostate cancer. Thre were 108 patients with PSA levels of 4-10 ng/ml and 18 of them were prostate cancer. TRUS was performed in 152 patients and 16 out of 58 patients diagnosed with prostate cancer, 3 out of 39 diagnosed with suspicious prostate cancer, and 2 out of 55 patients diagnosed as having no prostate cancer were found to have prostate cancer. In 40 patients, a nodule was palpated on DRE and 8 of them were found to have prostate cancer. Five out of 19 patients with a stony hard consistency, 3 of 12 with a firm to hard consisency, 12 of 129 with a firm consistency, 0 of 13 with a soft to firm consistency, and 2 of 8 with a soft consistency were prostate cancer. In the prostate cancer patients, there were 4 patients with PSA levels of 4 ng/ml or less and all these patients were diagnosed with prostate cancer or suspicious prostate cancer on TRUS but the nodule was not palpated in all patients. Two were soft and 2 were firm consistency on DRE. Conclusion: In patients with serum PSA levels of 10 ng/ml or less, TRUS is a more useful supporting method than DRE and a more active application of TRUS may lead to an early diagnosis and pertinent treatment of prostate cancer.

Knowledge of Colorectal Cancer Screening among Young Malaysians

  • The objective of this study was to determine the knowledge and associated factors regarding colorectal cancer screening among university students in Malaysia. The questionnaire consisted of three parts: socio-demographic characteristics, lifestyle practice and knowledge of colorectal screening. A cross-sectional study was conducted among 300 students ($21.3{\pm}1.4$ years old). The majority of the participants were Malay with a monthly family income of less than 5,000 Ringgit Malaysia (equal to 1,700 USD) (67.0% and 76.0%, respectively). Regarding their lifestyle practices, the majority were non-smokers and had never consumed alcohol (83.7%, and 88.0%, respectively). The majority of the participants had no knowledge of digital rectal examination, colonoscopy, barium enema and fecal occult blood screening (63.3%, 60.7%, 74.0% and 62.3%, respectively). Univariate and multivariate analysis revealed that their age and the discipline which the students were studying significantly influenced their level of knowledge about colorectal screening. The present study results indicate that education campaigns about colorectal cancer should be promoted.

Early Gastric Cancer with Neurofibroma Mimicking a Metastatic Node: A Case Report

  • Neurofibromas are benign tumors that originate from the peripheral nerves, including neurites and fibroblasts. Generally, a solitary neurofibroma is located in the skin and rarely in other places. A 72-year-old female suffered from epigastric discomfort for 2 months. Endoscopic findings showed an early gastric cancer type IIc at the antrum. Abdominal computed tomography revealed early gastric cancer with a 1.6 cm-sized metastatic node posterior to the duodenum. Laparoscopic assisted distal gastrectomy and retro-pancreatic dissection were performed uneventfully. Histological examination revealed gastric adenocarcinoma, invading the mucosa without nodal metastasis, and a neurofibroma. Herein, we present a case of a gastric cancer patient with a solitary retroperitoneal neurofibroma which mimicked a distant metastatic node.

Clinical Significance of Combined Detection of Serum Tumor Markers in Diagnosis of Patients with Ovarian Cancer

  • Objective: To explore the predictive value of tumor markers, including cancer antigen 72-4 (CA72-4), cancer antigen 15-3 (CA15-3) and cancer antigen 125 (CA125), in single or combined detection, for the diagnosis of ovarian cancer. Methods: 120 patients diagnosed with ovarian cancer from August 2011 to March 2013 and 80 patients diagnosed with benign ovarian tumors were enrolled in this test, along with 50 health examination women randomly selected from the database as controls. Serum levels of CA72-4, CA15-3 and CA125 in this study were determined by electrochemiluminescence (ECL). Results: Serum levels of CA72-4, CA15-3 and CA125 in ovarian cancer were higher than those in healthy group and benign group (P<0.01).The sensitivity of combined detection of those three tumor markers for diagnosis of ovarian cancer was obviously higher than with single detection with each marker (P<0.01). Conclusions: CA72-4, CA15-3 and CA125 could be a good combination in the diagnosis of ovarian cancer. Patients whose tumor markers continue to increase should be highly suspected of malignancy.

Determining the Awareness of and Compliance with Breast Cancer Screening among Turkish Residential Women

  • Background: Breast cancer is the leading cause of cancer-related deaths in women. Despite being associated with high morbidity and mortality, breast cancer is a disease that can be diagnosed and treated early. Materials and Methods: In this cross-sectional study of 321 women, data were collected by Questionnaire, Breast Cancer Risk Assessment Form and Champion's Health Belief Model Scale. Mann-Whitney U, Kruskal-Wallis, Chisquared tests and logistic regression were used in the statistical analysis. Results: It was found that only 2.2% of women have high and very high risk levels of breast cancer risk. There is a positive correlation between early diagnosis techniques and Health Belief Model Sub-Dimension scores which are sensibility, health motivation, BSE (Breast self-examination) self-efficient perception and negative correlation between mammography barrier score and BSE barrier score (p 0.05). When factors for not having BSE were examined, it was determined that the women who do not have information about breast cancer and the women who smoke have a higher risk of not having BSE. Conclusions: It is important to determine health beliefs and breast cancer risk levels of women to increase the frequency of early diagnosis. Women's health beliefs are thought to be a good guide for planning health education programs for nurses working in this area.

Determining the Factors that Affect Breast Cancer and Self Breast Examination Beliefs of Turkish Nurses in Academia

  • Purpose: To define factors that affect the performance status of BSE and confidence of student nurses. Materials and Methods: This descriptive study was conducted in the academic year 2010-2011 in a nursing faculty in $\dot{I}$zmir, Turkey. "Informative data form" and "Champion's Revised Health Belief Model Scale (CHBMS)" were used as data collection forms. Results: The mean age of the participant nurses was $21.0{\pm}1.49$. The mean CHMS scores of the student nurses were as follows: perceived susceptibility regarding breast cancer, $7.78{\pm}2.46$; perceived seriousness regarding breast cancer, $22.4{\pm}5.43$; perceived benefit regarding BSE application, $20.5{\pm}4.45$; perceived barriers regarding BSE application, $23.8{\pm}7.13$; perceived confidence regarding BSE application, $36.3{\pm}7.78$; the mean score of health motivation sub-scale, $25.7{\pm}4.59$; and mean of the total score of the scale, $36.5{\pm}15.01$. Conclusions: The outcomes obtained in this study indicated the importance of better education to student nurses, who have a key role in teaching preventive health behaviour including BSE to society and other university students as colleagues.

Socio-economic Factors Influencing Tumor Presentation and Treatment Options in Chinese Breast Cancer Patients

  • The impact of income and education level on the clinical and pathologic characteristics, implementation of clinical breast examination (CBE), and treatment patterns of a small population of Chinese female breast cancer patients was studeied in order to provide a theoretical basis and statistical reference for further nationwide research. We included 484 pathologically confirmed female primary breast cancer inpatients of the First Affiliated Hospital of Medical College of Xi'an Jiaotong University from February 2003 to January 2004. All cases were reviewed and relevant information was collected using a designed case report form (CRF). Chisquare tests, rank-sum tests, and Fisher's exact tests were used in the analysis. Our analysis showed that: (1) women in different occupation groups had significant differences in tumor size, pre-operative mammography, surgical options, post-operative estrogen receptor (ER), progestin receptor (PR) and human epidermal growth factor receptor 2 (Her2) status, and post-operative radiotherapy and chemotherapy (P < 0.05); and (2) women with different education levels had statistically significant differences in tumor size, post-operative ER, PR and Her2 status, and post-operative chemotherapy, radiotherapy, and endocrine therapy (P < 0.05). In Xi'an, China, women in low-income occupations or with low education levels are more likely to have advanced tumor stages at presentation, lower implementation rate of clinical breast examination, and less treatment.
