치수절제술 시 고려해야 할 유구치 치근과 근관 (Consideration of primary molar's root and root canal for pulpectomy treatment)

  • The purpose of the pulp treatment is to make the function of the primary molar in the oral cavity possible for as long as possible until the exfoliation of the tooth or the development of the occlusion is as long as possible. The pulpectomy is a relatively common procedure for the pediatric dentist in the clinic with dental care of the children nowadays. Primary molar is morphologically more susceptible to dental caries than permanent tooth, and the dental pulp treatment is frequently performed. Furthermore, unlike permanent teeth, the root canal has a large degree of curvature and morphological diversity and complexity, careful consideration is needed. Therefore, it is very important to comprehensively understand the morphological characteristics and diversity of the root and root canals for the successful pulp treatment of the primary molars.

Characterization of mandibular molar root and canal morphology using cone beam computed tomography and its variability in Belgian and Chilean population samples

  • Purpose: This study used cone-beam computed tomography (CBCT) to characterize mandibular molar root and canal morphology and its variability in Belgian and Chilean population samples. Materials and Methods: We analyzed the CBCT images of 515 mandibular molars (257 from Belgium and 258 from Chile). Molars meeting the inclusion criteria were analyzed to determine (1) the number of roots; (2) the root canal configuration; (3) the presence of a curved canal in the cross-sectional image of the distal root in the mandibular first molar and (4) the presence of a C-shaped canal in the second mandibular molar. A descriptive analysis was performed. The association between national origin and the presence of a curved or C-shaped canal was evaluated using the chi-squared test. Results: The most common configurations in the mesial root of both molars were type V and type III. In the distal root, type I canal configuration was the most common. Curvature in the cross-sectional image was found in 25% of the distal canals of the mandibular first molars in the Belgian population, compared to 11% in the Chilean population. The prevalence of C-shaped canals was 10% or less in both populations. Conclusion: In cases of unclear or complex root and canal morphology in the mandibular molars, CBCT imaging might assist endodontic specialists in making an accurate diagnosis and in treatment planning.

Surgical Result of the Combined Anterior and Posterior Approach in Treatment of Cervical Spondylotic Myelopathy

  • Objective : The purpose of this study is to evaluate the efficacy and necessity of combined anterior approach [discectomy and fusion] and posterior approach[open-door laminoplasty] in the treatment of cervical spondylotic myelopathy. Methods : The authors reviewed 14 cases in whom combined anterior and posterior approach performed for cervical myelopathy due to simultaneous anterior and posterior pathology such as huge central disc herniation with narrow spinal canal between January 2000 and December 2002. Clinical symptoms were evaluated by Japanese Orthopaedic Association [JOA] score and then the cervical curvature, change of spinal canal to vertebral body[SC/VB] ratio and canal widening were measured and compared to the clinical symptoms. Results : The mean JOA score increased from $10.4{\pm}3.1$ preoperatively to $14.8{\pm}1.2$ at the final follow up with a mean recovery rate 66.4%. In all cases, there were not neurologic deterioration. Mild postoperative complications developed in two cases. One patient had a limitation of range of neck motion and the other one showed kyphotic change. Postoperative radiography showed an improvement of body to canal ratios [average $0.70{\pm}0.08$ before surgery to $1.05{\pm}0.12$ after surgery and mainte nance or recovery of cervical lordosis. Canal widening of antero-posterior diameter and dimension after operation is 6.8mm. $116.61mm^2$. Conclusion : Combined anterior and posterior procedure could be helpful in decompression of the spinal cord and good functional recovery in spondylotic myelopathy patients with combined anterior and posterior pathology such as huge disc herniation accompanying narrow spinal canal.

Orthopantomogram을 이용한 하악공 및 하악관에 관한 연구 (A Study of Mandibular Foramen and Mandibular Canal using Orthopantomograms.)

  • The mandibular canal must be considered carefully during surgical treatment, especially surgical extraction of the impacted tooth and intraosseous implant because it contains the important inferior alveolar nerve and vessels. The author investigated the curvatUre of the mandibular canal, the positional frequency of mandibular foramen to the occlusal plane and gonial angle and the positional frequency of the mental foramen to the tooth site using orthopantomograms. The materials consisted of 295 orthopantomograms divided into seven groups ranging from the first decade to 6th. decade. The results were as follows: 1. The position of mandibular foramen was most frequently below occlusal plane in Group Ⅰ (78.6%) and Group Ⅱ (71.2%), above occlusal plane in Group Ⅲ (63.0%), Group IV (71.1%), Group V (57.6%), Group (76.7%) and Group VII (70.0%). 2. The curvature of mandibular canal was 142.8° in Group Ⅰ, 142.09° in Group Ⅱ, 139.34° in Group Ⅲ, 141.48° in Group Ⅳ, 138.45° in Group Ⅴ, 140.77° in Group Ⅵ and 143.89° in Group Ⅶ. 3. The gonial angie was 125.82° in Group Ⅰ, 123.18° in Group Ⅱ, 124.06° in Group Ⅲ, 120.45° in Group Ⅳ, 121.12° in Group Ⅴ, 121.63° in Group Ⅵ and 121.24° in Group Ⅶ. 4. The position of the menta] foramen was most frequently below the apex of mandibular first premolar in Group Ⅰ (57.2%), between the apex of mandibular first and second premolar in Group Ⅱ (59.6%) and Group Ⅲ (48.9%), and below the apex of mandibular second premolar in Group Ⅳ (39.2%), Group Ⅴ (48.5%) Group Ⅵ(46.6%) and Group Ⅶ(56.4%)

Homeopathy - A Safe, Much Less Expensive, Non-Invasive, Viable Alternative for the Treatment of Patients Suffering from Loss of Lumbar Lordosis

  • Objectives: Loss of lumbar lordosis causing pain and curvature of the vertebral skeleton to one side is a relatively uncommon disease. To our knowledge, successful treatment of loss of lumbar lordosis with any potentized homeopathic drug diluted above Avogadro's limit (that is, above a potency of 12C) has not been documented so far. In this communication, we intend to document a relatively rare case of loss of lumbar lordosis with osteophytic lippings, disc desiccation, and protrusion, causing a narrowing of secondary spinal canal and a bilateral neural foramina, leading to vertebral column curvature with acute pain in an adolescent boy. Methods: The patient had undergone treatment with orthodox Western medicines, but did not get any relief from, or cure of, the ailment; finally, surgery was recommended. The patient's family brought the patient to the Khuda-Bukhsh Homeopathic Benevolent Foundation where a charitable clinic is run every Friday with the active participation of four qualified homeopathic doctors. A holistic method of homeopathic treatment was adopted by taking into consideration all symptoms and selecting the proper remedy by consulting the homeopathic repertory, mainly of Kent. Results: The symptoms were effectively treated with different potencies of a single homeopathic drug, Calcarea phos. X-ray and magnetic resonance imaging (MRI) supported recovery and a change in the skeletal curvature that was accompanied by removal of pain and other acute symptoms of the ailment. Conclusion: Homeopathy can be a safe, much less expensive, non-invasive, and viable alternative for the treatment of such cases.

Quantification of Pediatric Cervical Growth : Anatomical Changes in the Sub-Axial Spine

  • Objective : In order to provide normal values of the pediatric sub-axial cervical spinal canal and vertebral body growth pattern using computed tomographic scans, a total of 318 patients less than 10 years old were included. Methods : The growth of the vertebral body and canal space was investigated using four different age groups. The Torg ratio (TR) was calculated and all patients were classified into a low TR group and a high TR group according to a cutoff value of 1.0. To account for spinal curvature, the C3-7 angle was measured. Results : Very little axial expansion and growth in height were observed (2.9 mm and 3.4 mm, respectively), and the spinal canal increments (1.8 mm) were much smaller than the dimensions of the vertebral body. The mean TR values were $1.03{\pm}0.14$ at the C3 vertebral level, $1.02{\pm}0.13$ at C4, $1.05{\pm}0.13$ at C5, $1.04{\pm}0.13$ at C6, and $1.02{\pm}0.12$ at C7 in all patients. The mean sub-axial angle (C3-7) was $7.9{\pm}10.6^{\circ}$ (range: $-17-47^{\circ}$). Conclusion : The upper sub-axial spinal canal continuously increased in size compared to the lower sub-axial spine after 8 years of age. Considerable decrements in the TR was found after late childhood compared to younger ages. Generally, there were no significant differences between boys and girls in vertical length of the cervical vertebrae. However, the axial dimension of the vertebral body and the spinal canal space varied according to gender.

치과용 단층촬영을 이용한 Stainless-Steel File과 MFile-System® 전동식 기구의 근관 성형 능력에 대한 비교 연구 (A Comparison of Stainless-Steel File and MFile-System® Ni-Ti Rotary Instrument in Canal Preparation using Dental Computed Tomography)

  • Objectives The aim of this study is to compare the quality of root canal preparation completed using MFile-$System^{(R)}$ instrument ( Komet, Gebr.Brsaseler, Germany) and conventional stainless steel file in the canals of Maxillary molar teeth that had a canal curvature between $25^{\circ}$ or more Materials & Methods Buccal canals of 24 first and second maxillary molar teeth, extracted for periodontal and prosthetic reasons were used. Tissue fragments and calcified debris were removed from teeth by scaling and the teeth were stored in 10% formalin solution for 24 hour. Then, teeth were stored in saline until used. To be included the roots had to have completed formed apices and angle of curvature ranging between $25^{\circ}$ or more according to the criteria described by Schneider(1971). Palatal and Second mesiobuccal canals were not included. Teeth were embedded into transparent acrylic. The teeth were randomly divided into two experimental groups. All teeth were scanned by Dental CT (PSR9000N, Asahi, Japan) to determine the root canal shape before instrumentation. Image slices were prepared from the apical end point to the pulp chamber. The first two sections were 2 mm from the apical end of root and 2 mm below the orifice. Further section was recorded, dividing the distance between the sections of apical and coronal levels into two equal lengths. 12 teeth were instrumented using stainless steel fileand another 12 teeth were instrumented using MFile-$System^{(R)}$. Following the completion of the instrumentation, the teeth were again scanned and compared with the cross sectional images taken prior to canal preparation. Amount of transportation and centering ability was assessed. Student's t-test was used for statistical analysis. Result Less transportation occurred with MFile-$System^{(R)}$ rotary instrumentation than stainless steel instrument. MFile-$System^{(R)}$ had better centering ability than stainless steel instrument. Conclusion MFile-$System^{(R)}$ rotary instrumentation transported canals less and had good centering ability.

레진모형근관에서 엔진구동형 및 수동형 ProTaper 파일로 근관성형 후 근관형태 변화에 대한 비교연구 (A COMPARATIVE STUDY OF THE CANAL CONFIGURATION AFTER SHAPING BY PROTAPER ROTARY AND HAND FILES IN RESIN SIMULATED CANALS)

  • 본 연구는 형태 및 재질이 동일한 엔진구동형 ProTaper파일과 수동형 ProTaper파일을 이용하여 레진모형근관을 성형한 후 근관형태 변화를 비교 분석하고자 시행되었다. 본 연구에서는 레진모형근관으로 총 40개의 J자와 S자의 근관 형태가 재현된 ENDO-TRAINING BLOC을 사용하였다. 근관 성형 기구로는 엔진구동형 Ni-Ti 파일로 $ProTaper^{TM}$, 수동형 Ni-Ti 파일로 $ProTaper^{(R)}$ For Hand Use를 사용하였다. 사용된 레진모형근관과 파일의 종류에 따라 10개씩 4개의 그룹으로 나누어 근관성형을 시행하였다. 근관 성형 전$\cdot$후 이미지를 스캐너 (Color scanner, UMAX Technologies, Inc., USA)를 이용하여 얻은 후. Photoshop 7.0 프로그램 (Adobe System Inc., USA)을 이용하여 이미지를 중첩하였다. 이미지 분석 프로그램 (Image-$Pro^{(R)}$ Plus, Media Cybernetic, USA)을 이용하여 치근단 쪽에서부터 0, 1, 2, 3, 4, 5, 6 및 7 mm 수준에서 근관 성형에 따른 근관의 내측 및 외측 폭경의 변화량, 근관 총폭경의 변화량, 근관 중심축의 변위량을 측정하였다. 또한 근관 성형 시간을 기록하였다. 두 기구 사이의 유의성 검정을 위해 독립 표본 t-검정을 시행하여, 근관 성형 시 수동형 ProTaper 파일이 엔진구동형 ProTaper 파일에 비해 근관 중심축의 변위가 유의하게 덜 일어나며 근관의 원래 형태를 더욱 잘 유지할 수 있지만, 근관 성형시간은 길어질 수 있다는 결론을 얻었다.

수종의 근관형성 방법이 근관 형태에 미치는 영향 (THE EFFECT OF SOME CANAL PREPARATION TECHNIQUES ON THE SHAPE OF ROOT CANALS)

  • The purpose of this study was to compare the shape of root canal after instrumentation with some engine driven NiTi files. Thirty narrow and curved canals(15-35 degree) of mesial canals of extracted human mandibular first molars were divided into three groups. Group 1: After radicular access with Gates Glidden drill, apical shaping using step back method with Flexo file Group 2: After radicular access with Gates Glidden drill, apical shaping with Profile .04 Group 3: Canal shaping with GT file and Profile .04. Using modified Bramante technique, the root was sectioned at 2 mm from apical foramen, height of curvature, 2 mm from canal orifice. Canal centering ratio, amount of transport, amount of dentin removed, shape of canal were measured and statistical analysis is done using SPSS Program V 7.5. The results were as follows: 1. Canal centering ratio of group 3 was the lowest at coronal part, but there was no statistical difference. Centering ratio of group 2 was the lowest at curve part, and there was statistical difference between group 1(P<0.05). Centering ratio of group 2 was the lowest at apical part, but there was no statistic difference. 2. Amount of transport of group 3 was the lowest at coronal part, but there was no statistical difference. Amount of transport of group 2 was the lowest at curve part, and there was statistical difference between group 1(P<0.05). Amount of transport of group 3 was the lowest at apical part, and there was statistical difference between group 1 and group 2, group 1 and group 3(P<0.05). 3. Amount of dentin removed of group 3 was the lowest at coronal part, bur there was no statistical difference. Amount of dentin removed of group 2 was the lowest at curve part, but there was no statistical difference. Amount of dentin removed or group 2 was the lowest at apical part, and there was statistical difference between group 1 and group 2, group 1 and group 3(P<0.05). 4. The shape of the canals after instrumentation varied among the groups. The majority of canals at coronal and curve part for group 1 were round in shape(7 in 10), those at apical part were oval(8 in 10). The majority of canals at coronal part for group 2 were round in shape(7 in 10) and there was no difference in the number of shape at other part. There was no difference in the number of shape at every part for group 3. As above results, NiTi rotary instrumentation showed a trend to remain more centered in the canal than SS file instrumentation. At using NiTi file, coronal shaping with Gates Glidden drill was not statistically different from shaping with GT file. But shaping with GT file showed tapered canals, so it may be said that shaping with GT file is a safe and valuable instrumentation method.

The cover of an ear thermometer probe as a split-thickness skin graft mold in external auditory canal reconstruction

  • Maintaining the patency of the external auditory canal (EAC) during reconstruction is important because of its physiological role in hearing and immunological protective functions. The curved shape of the EAC presents a challenge when performing a skin graft. One of the key points for a successful skin graft is to ensure compression on the wound bed, and many novel methods, including prefabricated ear molds, have been reported for this purpose. In this study, we present a case of a skin graft performed to reconstruct a skin defect following excision of actinic keratosis in the EAC, using the cover of an ear thermometer probe as a mold for the graft to match the curvature of the EAC. This is an economical and practical method for secure compression dressing of a skin graft in the EAC.
