CANAL WALL DOWN TYMPANOPLASTY - PRELIMINARY REPORT- (Canal Wall Down Tympanoplasty(제1보))

  • 1984년 3월부터 1987년 2월까지 3년동안 canal wall dorm tympanoplasty를 시행한 환자 27 명을 관찰하여 다음 결과를 우선 얻을 수 있었다. 수술후 추적기간은 평균 4.6개월(1개월부터 1년 반까지)로 남자 18명, 여자 9명이었으며 평균 연령은 23.4세(7세부터 44세까지)이었다. 수술시 병명은 진주종성 중이염이 17례 (63%), intact canal tympanoplasty with recurrent or residual cholesteatoma가 4례(15%), 중이 근치 수술이 4례(15%)이었다. 수술 중 소견은 malleus (M), incus(I), stapes(S)의 전체 괴사가 11례 (41%), M·I 괴사와 수평반규관 노출이 각각 5례 (38%)씩 차지했다. 청력증진은 청력증진술을 1차에 시행한 18예 중, 추적 가능한 15례 중에서 5례 (30%)가 수술 후 8주이상의 청력검사결과 평균 31.4/10.4㏈를 나타냈다. 수술후 합병증으로 외이도 성형술을 동시에 시행한 20례중 경미한 육아조직 발생이 6예(30%) 외이도 성형술을 시행치 않은 7례 중 경미한 농성이루가 3례 (43%)로 나타났다.

A STUDY ON THE ANATOMY OF THE PULP CHAMBER FLOOR OF THE PERMANENT MANDIBULAR SECOND MOLAR (하악(下顎) 제2대구치(第二大臼齒) 치수저(齒髓底)의 해부학적(解剖學的) 고찰(考察))

  • A total of 114 extracted human mandibular 2nd molars were used to study the configuration of the floor of the chamber. The specimens were ground and the pulp chamber was examined with a magnifier and explored with sharp explorer. The study showed the shape of the pulp chamber, number of root canals, and the type of canal orifice. The results were as follows, 1. In so far as observing the shape of the pulp chamber of the teeth, 47.4% of the teeth were square, 42.9% were triangle and 9.7% were ovoid shape. 2. 35% of the samples had 2 root canal orifices, 62% had 3 root canal orifices, 3% had 4 root canal orifices. 3. 3% of the specimens showed 'H' shape, 5% showed 'Square' shape, 51% showed 'T' shape, 1% showed 'reverse-T' shape, 35% showed 'I' shape and 5% showed the specific 'C' shape.

Flow Characteristics of Lateral Irrigation Canals Diverted from Kimje Main Canal in Dongjin Irrigation Area (동진지구 김제간선 내 주요 용수지선의 흐름 특성)

  • The study was carried out to investigate the dimension of irrigation canal, and measure the water flow of those diverted from Kimje main canal in Dongjin irrigation area. The rating curves indicating the relationships between water level and discharge of the lateral canals were induced using the measured data with high regression coefficients of 0.957~0.999 at inlet points and 0.932~0.998 at end points, respectively. And Manning's roughness coefficients were estimated as the ranged from 0.015 to 0.074 at inlet points and 0.056 to 0.089 at end points with rectangular type of concrete open irrigation canals, the Manning's roughness coefficients of the end points were less than those values of the end points, which may be considered as the deposition of the bottom sediments.

A STUDY OF WORKING EFFICIENCY AND FILE DEFORMATION OF GT ROTARY FILE IN CURVED CANALS (GT rotary file을 이용한 만곡 근관형성시 작업 효율 및 file 변형 발생에 관한 연구)

  • Root canal preparation process is of utmost importance in successful treatment of root canal. Also, one of the most important purpose of the root canal preparation is to enlarge the root canal three dimensionally without changing the curvature of the root canal However as the curvature of the root canal increases, there are many difficulties involved in formation of optimum root canal. Therefore in order to solve the above mentioned problems, new developments in methods of root canal preparation and equipments for such purposes were made. Recently, vigorous studies about newly introduced engine-driven nickel-ti-tanium rotary file are conducted. As shown in research results to dates, it is well established that the use of nickel-titanium file is better suited for curved root canal than stainless steel file in maintaining the curvature or root canal and reducing the deformation of root canal. However it is also acknowledged that there are a few discrepancies in research results according to protocol, due to failure to remove variables in experiments. In addition, although it is recommended by the manufacturer that the GT rotary file should maintain a low rotational speed of 150~350rpm and 'light pressure' as light as not to break the lead of a pencil, academic studies about the vertical force which is not yet standardized are not sufficiently explored. Therefore, this research devised and utilized a special research equipment to standardize the appropriate range of vertical force for GT rotary file through experiments by breaking of the lead of a pencil as expressed by the manufacturer and to accurately measure factors involved through repeating and recreating the environment of root canal preparation. Forming nine experimental groups by varying the vertical forces (150g. 220g, 300g) and rpm (150rpm, 250rpm, 350rpm), the effects of changing vertical forces and rpm on working efficiency were measured in terms of time expended in root canal preparation by crown-down method using a transparent resin block with 35 degree curvature and GT rotary file (z-test). The following research using this special research equipment that involved nine experimental groups and varying the vertical force for root canal preparation from 300g which is within the normal vertical force range to 700g and 1000g which fall outside the normal rpm range. The results were as follows : 1. Analysis of the experiment results revealed that the time spent in root canal preparation decreased as the vertical forces and rpm increased (p<0.05). Also, the effects of rpm were greater than those of the vertical forces within the normal vertical force range ($\beta$-weight test). 2. Observation of the deformation of GT rotary file revealed that deformation increases in a direct correlation with the vertical force increase and in a reverse correlation with the rpm decrease. In the case of the vertical forces close to the normal range, the probability of GT rotary file deformation were quite different depending on the rpm changes. In the case of greater vertical forces, the occurrences of deformation of the file were more frequent regardless of the rpm changes. 3. Deformation and breakage of file were also commonly observed in the expended time measurement experiments and GT rotary file deformation experiments in which low speed rpm (150rpm) was used and at the curved portion of the resin block.

A COMPARATIVE STUDY ON THE CANAL CONFIGURATION AFTER SHAPING BY PROFILE, PROTAPERTM AND K-FLEXOFILE IN SIMULATED CANALS WITH DIFFERENT ANGLES OF CURVATURE (ProFile, ProTaperTM K-Flexofile 근관 성형시 근관의 만곡도에 따른 근관 형태 변화 비교연구)

  • The purpose of this study was to evaluate the canal configuration after shaping by ProFile. ProTaper and K-Flexofile in simulated resin canals with different angles of curvature. Three types of instruments were used: ProFile. ProTaper. K-Flexofile. Simulated root canals. which were made of epoxy resin. were prepared by ProFile. ProTaper with rotary instrument using a crown-down pressureless technique. and hand instrumentation was performed by K-Flexofile using a step-back technique. All simulated. canals were prepared up to size 25 file at end-point of preparation. Pre and post instrumentation images were recorded with Scanner. Assessment of canal shape was completed with Image Analysis program. Measurements were made at 1. 2. 3. 4. 5. 6. 7. 8. 9 and 10mm from the apex. At each level. outer canal width. inner canal width. total canal width. and amount of transportation from original axis were recorded. Instrument deformation and fracture were recorded. Data were analyzed by means of one-way ANOVA analysis of variance and the Sheffe's test. The result was that ProFile and ProTaper maintain original canal shape regardless of the increase of angle of curvature than K-Flexofile. ProFile show significantly less canal transportation and maintained original canal shape better than ProTaper.

Vertebral Metastasis from Squamous Cell Carcinoma of the Anal Canal

  • Distant metastasis of squamous cell carcinoma from the anal canal is an uncommon event. However, hematogenous spread to the vertebrae may occur in the course of this disease. The route of metastasis from the anal canal seems to be Batson's vertebral venous system. A 52-year-old female patient presented with lower back and right leg pain of one-week history. She has undergone radiotherapy and chemotherapy for squamous cell carcinoma of the anal canal and then was followed by surgical resection. Three months later, magnetic resonance images of the lumbar spine disclosed a well-enhanced mass of L5 vertebral body compressing the thecal sac. Surgical decompression and biopsy were performed. Histopathological study confirmed carcinoma of the squamous cell origin. We report a ra re case of vertebral metastasis from squamous cell carcinoma of the anal canal with a pertinent review of literature.

STUDY ON THE ANATOMICAL POSITION OF MANDIBULAR CANAL USING COMPUTED TOMOGRAPHY IN MANDIBULAR PROGNATHISM PATIENTS (하악 전돌증 환자에서 전산화 단층 촬영을 이용한 하악관의 해부학적 위치에 관한 연구)

  • Purpose: In this study, we analyzed and compared the anatomical position of the mandibular canal in normal occlusion and mandibular prognathism patients. Patients and Methods: Computed tomography image from 58 patients were divided into normal occlusion group and mandibular prognathism group, and each measurement were taken in the each measuring points(2nd premolar, 1st molar, 2nd molar, 3rd molar, ramus). Measurements were statistically analyzed by student's t-test. Results: BC (Thickness of the buccal cortex) value was 2.3~2.7 mm, CB (Distance from the canal to the lingual aspect of the buccal cortex) value was 1.3~4.3 mm, MC (Diameter of the canal) value was 3.2~3.8 mm, LI (Distance from the canal to the lingual aspect of the lingual cortex) value was 2.0~3.7 mm, TM (Thickness of the total mandible) value was 9.5~12.9 mm and CM (Distance from the canal to the inferior border of the mandible) value was 6.9~17.5 mm. Conclusion: In the comparison between two groups, there was statistically significant difference in CB value of 2nd, 3rd molar between normal occlusion and mandibular prognathism, and other value in the rest of the measuring points didn't show statistically significant difference.

Distribution of Vascular Canal at Lateral Wall of Maxillary Sinus in Korean Population Using Computed Tomography (컴퓨터단층촬영을 이용한 한국인의 상악동 측벽의 혈행 분포)

  • Purpose: The aim of this study was to investigate the blood supply to the maxillary sinus in Koreans using computed tomography of the lateral wall of the sinus, and to analyze the data according to demographic data, and to compare our results with previously published research. Methods: One hundred and three patients (males 51, females 52) who visited the dental clinic were evaluated. We investigated the canals with cone-beam computed tomography, and measured the diameter and length from the bony notch of the vascular canal at the lateral wall of the sinus to each of the reference planes. Results: Most types of vascular canal were type I & II (total 72.2%) which were driving through inside the sinus wall. Type IV, V were frequently investigated in 1st premolar and 2nd molar. Mean height; from anterior nasal spine-posterior nasal spine plane to vascular canal was 5.56 mm in 1st premolar, 2.11 mm in 2nd premolar, 0.98 mm in 1st molar, 4.32 mm in 2nd molar; from the basal layer of the sinus was 4.93 mm in 1st premolar, 5.00 mm in 2nd premolar, 6.05 mm in 1st molar, 7.91 mm in 2nd molar; and from alveolar crest, 20.80 mm in 1st premolar, 16.57 mm in 2nd premolar, 14.01 mm in 1st molar, 16.17 mm in 2nd molar. The mean height of the vascular canal of each reference plane showed no significant difference between sex, site (left or right) and age. The mean diameters of the canals were 1.76 mm in male, and 1.50 mm in female. Diameter decreased with age, and tooth site (from 1st premolar to 2nd molar). Conclusion: These results show that the mean diameter of the vascular canal of the maxillary sinus varies according to age, sex, and tooth site, but that the mean height of canal had no significant difference based on these three factors.

A STUDY ON COMPARISON OF STAINLESS STEEL, NICKEL-TITANIUM HAND, NICKEL-TITANIUM ENGINE-DRIVEN FILE INSTRUMENTATION USING COMPUTED TOMOGRAPHY (수동형 Stainless Steel, Nickel-Titanium 및 엔진 구동형 Nickel-Titanium File의 근관형성 능력에 관한 비교 연구)

  • The aim of this study was to determine the shaping ability of stainless-steel K file (S-S K file), nickel-titanium K file (Ni-Ti K file) and engine driven nickel-titanium file (Quantec file) in resin simulated root canal. Computed tomography was used to evaluate the change of the root canal morphology. Thirty nine resin simulated root canal were divided into four groups (A:12, B:12, C:12, D:3). Resin simulated canals were scanned by computed tomography before instrumentation (1st C-T scan). Canals were instrumented using step back preparation technique with S-S K file in group A and Ni-Ti K file in group B. Group C was prepared with engine driven Ni-Ti file. Group D was uninstrumented to compare the 1st C-T scan images with 2nd C-T scan images of root canal. Instrumented canals were again scanned using computed tomography (2nd C-T scan), and reformated images of the uninstrumented canals were compared with images of the instrumented canals. In the sections of 2mm and 6mm from the apex, Quantec file caused significantly less canal transportation than S-S K file and Ni-Ti K file (p<0.05). Quantec file produced more centered than S-S K file and Ni-Ti K file in the sections of 2mm and 4mm from the apex (p<0.05). There was no significant difference in the removed volume of canals among the each groups (p>0.05). However the removed canal volume from the apex to 5mm were significantly higher than them from 5mm to 1mm (p<0.05) in each groups. Under the conditions of this study, preparation with Quantec file was more effective and produce more appropriate canal shapes than S-S K file and Ni-Ti K file.

A Study on the Acoustic Analysis Method of the External Ear Canal Using DICOM Images (DICOM 영상을 이용한 외이도 음향해석 방법에 관한 연구)

  • This study simulated external ear canal modeling with different external ear canal lengths, vertical flexion angles, and inner/outer diameter ratios using digital imaging and communications in medicine(DICOM) of the head temporal region and measured the acoustic sensitivity. The experiment was performed by increasing the audible frequency for humans by 200 Hz and expressing the frequency constantly transmitted at 1 Pa as the eardrum acoustic volume and presented the measurements by linear and quadratic curve regression analysis. The results showed that the longer the external ear canal length and the higher the ratio of the outer/inner diameter, the faster the acoustic response at lower frequencies. The acoustic sensitivity correlation of the meta-model using regression analysis showed a 77% influence by the external ear canal length and 5% by the external/internal diameter ratio, while the vertical flexion angle did not show a significant relationship. This showed that auditory acoustic sensitivity of humans is a factor that reacts faster at a low frequency when the external ear canal length is longer and when the difference between the outer and inner diameter is higher.
