A Case of Engorged Female Hard Tick in the External Auditory Canal of an Infant

  • An oval-shaped mass with a smooth surface was found in the ear canal of a 22-month-old male infant. Although the mass appeared to be almost entirely blocking the ear canal, it was successfully removed under a surgical microscope without general anesthesia at the outpatient department. Under an optical microscope with hematoxylin and eosin staining, the specimen was observed to have a cuticle with a serrated surface and a pore canal, as well as parts of the capitulum, salivary glands, muscles, midgut, and the legs. The specimen was identified as a hard tick of the family Ixodidae, based on gross and histological findings. This paper is the first report in Korea on the diagnosis and treatment of a tick bite in the ear canal.

근관치료의 근관장 측정에 관한 연구: 근관치료학 전공 교수 설문 (A survey on working length determination of endodontic treatment)

  • The purpose of this study is to evaluate the preferred method of root canal length determination and the apical limit for canal instrumentation among endodontic teachers of dental school. A questionnaire on the preferred method of root canal length determination and the apical limit for canal instrumentation was designed and distributed to endodontic teachers of various dental schools. The response rate was 90%. The most preferred method of root canal length determination was Electronic apex locator (EAL)(89%). The most favoured apical limit for canal instrumentation was 0.5 to 1.0 mm short of the radiographic apex(78%). The most preferred method of using EAL was that the working length is taken at 'APEX' mark and then distracted 0.5mm from that length.(41%). When there is no agreement between radiographic measurement and EAL measurement, 74% of respondents chose the length of EAL measurement. The majority of endodontic teachers from Korean dental schools preferred EAL to radiograph method in determining root canal length.

  • The purpose of this study was comparision of conventional tomography with reformatted computed tomography for dental implant in locating the mandibular canal. Five dogs were used and after conventional tomographs and reformatted computed tomographs were taken, four dentist traced all films. Mandibles were sectioned with 2mm slice thickness and the sections were then radiographed(contact radiography). Each radiographic image was traced and linear measurements were made from mandibular canal to alveolar crest, buccal cortex, lingual cortex, and inferior border. Following results were acquired : 1. Reformatted computed tomographs were exacter than conventional tomography by alveolar crest to canal length of -0.6mm difference between real values and radiographs. 2. The average measurements of buccal cortex to mandibular canal width and lingual cortex to mandibular canal width of conventional tomographs were exacter than reformatted computed tomographs, but standard deviations were higher than reformatted computed tomographs. 3. Standard deviations of reformatted computed tomographs were lower than conventional tomographs at all comparing sites 4. At reformatted computed tomography 62.5% of the measurements performed were within. : filmm of the true value, and at conventional tomography 24.1% were. 5. Mandibular canal invisibiity was 0.8% at reformatted computed tomography and 9.2% at conventional tomography. Reformatted computed tomography has been shown to be more useful radiographic technique for assessment of the mandibular canal than conventional tomography.

다양한 엔진 구동형 파일의 근관 성형 능력 비교 (SHAPING ABILITY OF NICKEL-TITANIUMROTARY FILES)

  • This study compared the shaping ability of nickel-titanium rotary files with different rake angle and radial land. The nickel-titanium files used in this study were Profile(Dentsply, Maillefer, Ballaigues, Switzerland), Hero 642(Micromega, Besancon, France) and K3 (SybronEndo, Glendora, Ca, USA) file. Resin blocks substituted for root canals. 36 resin blocks were divided into 3 groups with 12 canals each The time for canal preparation was recorded. The images of pre- and postoperative resin canal were scanned and those were superimposed Amounts of canal deviation, total canal widths, inner canal widths, and outer canal widths were measured at apical 1, 2 ,3, 4, 5, 6, and 7mm levels. The amount of canal deviation was the smallest in Profile group , and the time far canal preparation was the shortest in Hero 642 group. K3 group resulted in competent characteristics in both measurements. Positive rake angle seemed to result in fast shaping of root canal and radial land guide the instrument in center of the canals and around curvatures. Radial land also tended to reduce the sense of screwing into the root canal. The proper selection of the nickel-titanium fie based on the knowledge about file design is needed for the safer, simpler and faster root canal therapy.

초음파 근관기구를 이용한 근관충전법의 근관폐쇄능에 관한 주사 전자현미경적 연구 (A SCANNING ELECTRON MICROSCOPIC EVALUATION OF THE ROOT CANAL FILLING WITH ULTRASONIC ENDODONTIC INSTRUMENT)

  • The purpose of this study was to evaluate the adaptation of root canal filling material to the dentinal wall of root canal and to compare the sealing ability of the root canal filling materials using ultrasonic endodontic instrument with injection-molded thermoplasticized gutta-percha filling method and lateral condensation method. Fifty fresh human single root exlracted for orthodontic treatment, were randomly selected, and instrumented by step-back technique. And then, the teeth were divided into 5 groups according to each root canal filling methods. In the experimental group 1 and group 2, the root canals were filled with gutta perdia cases using ultrasonic instrument with and without sealer. In the experimental group 3 and 4, using jection-moldeed thermoplasticized gutta-percha method by obtul$^{(R)}$ canals were filled with and without sealer. In the control group, the canals were filled with sealer by lateral candensation. And then, 5 teeth of each group were immersed in black Indian ink, decalcified and cleared. The depth of dye penetration into the root canal were evaluated with stereoscope (Reichert Ltd., USA). Among the 5 teeth remaining in each group, the single longituding grooves were made on the labial and lingual root surfaces and then immersed in the liquid nitrogen to fracture the teeth spontaneously without any distortions of gutta-percha. Each specimens were examined with X-650 Scanning Electron Microscope(Hitachi ltd, Japan) to show the adaptation to the canal wall, void, homogenicity of filling material and location of gutta-percha or sealer in the dentinal tubules of the root canal. The observations were as follows : 1. The experimental group 1 showed smaller mean dye penetration than control group, and showed the penetraton of sealer in the dentinal tubules of apical third of the root canal. 2. The experimental group 2 and group 4 showed the penetration of gutta-percha in the dentinal tubules of root canals. 3. The experimental group 1 and group 3 showed less mean dye penetration than the experimental group 2 and group 4. 4. The experimental group 1 and group 2 showed better adaptation of filling materials than control group.

전산화 단층촬영을 이용한 하악관의 해부학적 위치에 관한 연구 (THE ANATOMICAL LOCATION OF THE MANDIBULAR CANAL BY COMPUTED TOMOGRAM)

  • This study was performed to define the anatomical position of the mandibular canal and the findings during the sagittal split ramus osteotomy of the mandible. The mandibles of 20 adult Korean were used. The dimension of mandibular canal from the mandibular foramen to the 1st molar was measured at 4 specific coronal-sectional location by CT scan. The results were as follows ; 1. The distance from the mandibular canal to the medial aspect of the buccal cortical plate was greatest($4.5{\pm}1.1mm$) at 2nd molar area and was not significantly greater than at any other section. 2. Buccal cortex was thickest($3.8{\pm}0.9mm$) at 2nd molar and thinnest ($2.5{\pm}0.3mm$) mandibular foramen um 3. The distance from the mandibular canal to the medial aspect of the lingual cortical plate was not significant at any sections. 4. The distance from the mandible canal to the inferior border of mandible was greatest at the mandibular foramen($20.7{\pm}3.9mm$). The canal was located more closely to the inferior border at 1st, 2nd molar area 5. The diameter of the mandibular canal was between $2.5{\pm}0.3mm$ and $2.8{\pm}0.6mm$. 6. The total mandibular thickness was greatest($21.1{\pm}2.6mm$) at 2nd molar area and narrowest($17.2{\pm}3.2mm$) at mandibular foramen area.

근관치료된 상악 제1대구치의 치근단 병소: CBCT 연구 (Apical periodontitis of endodontically treated maxillary first molar: Cone-Beam Computed Tomography study)

  • Objective: The purpose of this study was to analyze the Cone-beam computed tomograghy(CBCT) scan of endodontically treated maxillary first molars and investigate how second mesiobuccal (MB2) canal is treated, how the prognosis of mesiobuccal (MB) root is different compared to other roots and the prognosis factor on apical periodontitis. Methods: Subjects were endodontically treated maxillary first molars whose were collected from CBCT scans taken from January 2018 until December 2019. A total of 525 maxillary first molars were analyzed by an endodontist to determine the presence of the MB2 canal, the quality of the root canal filling, and the presence of apical periodontitis. The chi square test and Fisher's exact test was used to examine the relationship between each variable. Results: MB2 canals were found in 46.3%, of which 76.5% were not treated. The more main canal of mesiobuccal root (MB1 canal) was well filled, the more significantly MB2 canal was well filled (p<0.001). The apical periodontitis of MB root was not related to the filling quality of MB1 canal (p=0.370) and was related to the filling quality of MB2 canal (p=0.004). The apical periodontitis of MB root was related to the quality of canal filling of MB2 canal and the apical periodontitis of DB and P root. Conclusions: It was found that the majority of MB2 canals were not treated. The apical periodontitis of MB root was analyzed to be related to the quality of canal filling of MB2 canal. The apical lesion of the MB root was not correlated with the treatment of the MB1 canal, but it was significantly related to the quality of filling of MB2 canal.

인간 이도의 소리응답특성과 음향공간의 재현 (Characteristics of Sound Response in Ear Canal of Human and Reproduction of Acoustical Space)

  • The human ear canal amplifies the sound pressure level at specific frequency bands. The characteristics of the ear canal are very similar to those of curved cylindrical tube. In this study, the characteristics of sound transfer in human ear canal were measured and the acoustical space of ear canal was reproduced from the canal cavity geometry. For the measurement of sound transfer function in ear canal, a probe microphone and a reference microphone were used. The sound transfer functions were measured for 5 human subjects. To reproduce the acoustical space of the ear canal, two kinds of ear simulator were designed. The first one is a straight cylindrical tube type and the other is a real-shape ear of which geometry was taken from a micro-CT scanning of a human ear. The characteristics of the reproduced apparatus were compared with those of the human and a commercial ear simulator, RA0045 of G.R.A.S. Inc. The comparison results show that the developed apparatus well represent the ear canal characteristics in the low frequency, but have limited coincidence in level over high frequency range.

Apical periodontitis in mesiobuccal roots of maxillary molars: influence of anatomy and quality of root canal treatment, a CBCT study

  • Objectives: This study aimed to evaluate the prevalence of apical periodontitis (AP) in the mesiobuccal roots of root canal-treated maxillary molars. Materials and Methods: One thousand cone-beam computed tomography images of the teeth were examined by 2 dental specialists in oral radiology and endodontics. The internal anatomy of the roots, Vertucci's classification, quality of root canal treatment, and presence of missed canals were evaluated; additionally, the correlation between these variables and AP was ascertained. Results: A total of 1,000 roots (692 first molars and 308 second molars) encompassing 1,549 canals were assessed, and the quality of the root canal filling in the majority (56.9%) of the canals was satisfactory. AP was observed in 54.4% of the teeth. A mesiolingual canal in the mesiobuccal root (MB2 canal) was observed in 54.9% of the images, and the majority (83.5%) of these canals were not filled. Significant associations were observed between the presence of an MB2 canal and the quality of the root canal filling and the presence of AP. Conclusions: AP was detected in more than half of the images. The MB2 canals were frequently missed or poorly filled.

ProTaper와 ProFile을 사용한 Hybrid instrumentation method의 근관 형성 전, 후 근관 단면적과 근관벽에서 치근외면까지 최단거리의 변화에 미치는 영향에 대한 실험적 연구 (The experimental study of the effect of the hybrid instrumentation method with ProTaper and ProFile on the change of root canal area and distance from the canal to the root surface after canal shaping.)

  • The aim of this study was to investigate the effect of the hybrid instrumentation method with ProTaper and ProFile on the change of root canal area and distance from the canal to the root surface after canal shaping. The mesial canals of twenty extracted mandibular first molars having $10-20^{\circ}\Delta$ curvature were scanned using X-ray microcomputed tomography (XMCT)-scanner before root canals were instrumented. They were divided into four groups (n=10 canals ter group). In Group 1, root canals were instrumented by the step-back technique with stainless steel K-Flexofile after coronal flaring. The remainders were instrumented by the crown-down technique with, ProTaper system (Group 2), ProFile (Group 3) or ProTaper (Group 4). All canals were prepared up to size 25 at the end-point of preparation and scanned again. Pre- and post-operative cross-sectional images of 1, 3, 5, and 7 mm from the apical foramen were compared. For each level, change of cross-sectional canal are and distance to the nearest external root surface was calculated using Adobe Photoshop 6.0 and image software program. In the change of cross-sectional area, Group 4 was less than Group 2 at 3 mm and 5 mm level (p<0.05). In the difference of the distance from the canal to the root surface after canal shaping, Group 4 was least among the other groups at 7 mm level (p<0.05). According to the results, the methods using ProFile or K file only and the hybrid instrumentation technique using ProTaper and ProFile are more appropriate methods of canal preparation than ProTaper system for narrow of curved canals.
