The Reliability of Preoperative Simulation Surgery Planning for Distraction Osteogensis in Craniosynostosis Patients

  • Background Craniosynostosis management using distraction osteogensis represent a challenge for surgeons due to the great variability of the skull deformity even within the same etiology. The ability to apply the simulation surgery for improving the preoperative planning for distraction osteogensis could improve the results.Planning and Simulation 14 patients presented with craniosynostosis had been subjected to simulation surgery prior to real surgery. 3D CT scans was obtained upon patient admission. Adjustment of all skull position to Frankfort horizontal plane was done. 3 different distraction osteogensis plans were done for each patient according to the skull morphology. For each plane, movement for each bone segment was done according to the pre-planned distraction vectors. Also the distances of distractions were pre-determined according to the cephalic index as well as brain volume. Intraoperatively, we choose the most appropriate plan for the patient by the surgeon. At the end of distraction, 3D CT scan was obtained, and was compared to the simulation plan. Also the distance and the direction of distraction was compared to that of the plan. Accordingly, the distance was almost matching that of the simulation surgery, however the vector of distraction was not matched.Conclusion Preoperative stimulation planning for craniosynostosis patient is very valuable tool in the surgical management of craniosynostosis patients.

Preoperative CT Navigation of Perigastric Vessel Anatomy for Gastrectomy

  • The aim of this report is showing the case that we could give exact navigation of perigastric vessels for gastrectomy with 3D CTA. A 74-year-old male patient visited hospital with gastric cancer. Early gastric cancer, type IIb was found at stomach antrum great curvature side. Before surgery, he underwent 3D CT angiography. 3D volume rendering images and MIP images were made by post processing. He had replaced Lt. hepatic artery arising from Lt. gastric artery. Surgeon could get patient's specific vascular anatomy before surgery including surgically relevant anatomical distance and direction and could finish gastrectomy within 4 hours and just 53ml blood loss.

  • Filtered-Back-Projection technique is used in X-ray CT image reconstruction. This requires X-ray transmission data from all directions. As the transverse cross-section of the body is approximately 50 cm, transmitted X-rays in this direction are strongly attenuated. If X-ray transmission data in this direction is avoided, exposure to the patients seems to be reduced one 20th of usual value. Some alternative method has to be found for clinically sufficient image quality. New methods are under development and tentative results are reported that utilizes the principle of superposition.

  • 모바일 환경에서 브로드캐스트는 그 확장성 때문에 매우 유용한 데이터 전송 방법이다. 'push-based' 데이터 전송 방식에서 서버는 넓은 대역폭을 통해 클라이언트에게 다양한 데이터를 반복적으로 브로드캐스트 한다. 〔1,2〕 브로드캐스트에 기반을 둔 정보 시스템의 데이터간의 연관성에 관한 연구는 미흡한 실정이다. 상호 연관 데이터의 브로드캐스트에서, 클라이언트는 자연스럽게 상호 연관된 데이터의 집합을 요청하게 되며, 데이터의 상호 연관성을 고려할 때 기존의 스케줄링 및 캐싱 기법 등은 달라져야 한다. CBS〔3〕에서는 모든 데이터간의 연관도를 계산하여 최소 비용 경로를 구해, 이 순서대로 브로드캐스트하는 기법을 제안하였다. CBS 기법은, 클라이언트가 연관된 데이터를 동시에 요청하지 않고, NP-문제인 최소 비용 경로를 많은 데이터에 대해서 실시간에 계산해야 되며, 데이터 아이템간의 상호 연관성이 클라이언트마다 다르게 정의되는 문제점이 있다. 따라서 본 논문에서는 응답 시간을 줄이기 위해, 브로드캐스트 되는 상호 연관 데이터의 prefetching기법을 제안한다, 제안된 CT 기법은 상호 연관도와 브로드캐스트 대기시간을 고려하여 캐시를 관리한다. CT를 현실적으로 적용한 ACT의 알고리즘을 소개하였으며, 시뮬레이션을 통해 CT의 성능과 특징을 실험하였다.

  • In this paper, a simulation program of relaying system including the CT and CPD ( capacitive potential device ) is developed to study the effect of its operation under various system conditions. To deal with the dynamic characteristics of relaying system, state space technique is applied, and then the state equations of CT, CPD and mho distance relay are constructed. Also the dynamic response characteristics of overall relaying system is verified by digital simulation. Since the proposed model is capable of taking arbitrary input waveforms from EMTP in analyzing its dynamic responses, the effects of CT-saturation and CPD-subsidence transient characteristics on the operating points of who distance relay can be accurately prodicted. It gives more effective results, compared with the model without considering those characteristics by checking the exprimental data.

  • We tested the feasibility of measuring fat thickness using a miniaturized chip LED sensor module, testing 12 healthy female subjects. The module consisted of a single detector and four sources at four different source-detector distances (SD). A segmental curve-fitting procedure was applied, using an empirical algorithm obtained by Monte-Carlo simulation, and fat thicknesses were estimated. These thicknesses were compared to computed-tomography (CT) results; the correlation coefficient between CT and optical measurements was 0.932 for bicep sites. The mean percentage error between the two measurements was 13.12%. We conclude that fat thickness can be efficiently measured using the simple sensor module.

  • We have performed long time REMD simulation on 15-19 residues of human calcitonin hormone (DFNKF) which is known to form highly ordered amyloid fibril. The simulation started from randomly oriented multiple DFNKF strand. Using all-atom level simulations with the generalized Born solvation (GB) model (param99MOD3), we observed spontaneous formation of ${\beta}$-sheet for tetramer. Interestingly, the current simulation gives anti-parallel sheet as a major conformation, consistent with experiments. The major interaction stabilizing the anti-parallel sheet seems to be the inter-strand hydrogen bond.

Reconstruction of the Inferior Orbital Wall with Simplified Simulation Technique in Case of the Fracture Extending to the Posterior Orbital Floor

  • A 37-year-old male was assaulted and complained of severe periorbital swelling. Physical examination revealed that there were limitation of eyeball movement on upper gaze, diplopia, and hypoesthesia on the infraorbital nerve innervating region. Three-dimensional (3D) computed tomography (CT) of facial bone exhibited the fracture of orbital floor accompanying the significant amount of orbital contents' herniation extending to the far posterior part. To recover the orbital volume and restore orbital floor without threatening the optic nerve, preoperative simplified simulation was applied. The posterior margin of the fractured orbit was delineated with simulation technique using cross-linkage between the coronal and sagittal sections based on the referential axial view of the CT scans. Dissection, reduction of orbital contents, and insertion of the absorbable mesh plate molded after the prefabricated template by the simulation technique was performed. Extensive orbital floor defect was successfully reconstructed and there were no serious complications. The purpose of this report is to emphasize the necessity of preoperative simulation in case of restoring the extensive orbital floor defect.

Developing Lessons and Rubrics to Promote Computational Thinking (Computational Thinking역량 계발을 위한 수업 설계 및 평가 루브릭 개발)

  • This study aims to suggest lesson plans and evaluation methods for primary pre-service teachers by reviewing the concept of computational thinking(CT) skills and its sub components. To pursue this goal, a literature review has been conducted in regards to CT and the effectiveness of programming courses. In addition, the Scratch educational programming functions were analyzed yielding six CT elements(data representation, problem decomposition, abstraction, algorithm & procedures, parallelization, simulation). With these six elements, one semester lesson plans for 15 weeks that represent the connections with six CT elements were designed. Based on the PECT(Progression of Early Computational Thinking) model and the CT framework a rubric to evaluate learners' proficiency levels(basic, developing, proficient) revealed in their final projects was developed as well. Upon a follow-up empirical study, the lesson plans and the rubric suggested in the current study are expected to be utilized in teachers' colleges.
