PET/CT 영상 재구성시 감쇠보정맵을 사용하여 영상재구성에 적용한다. 감쇠보정 맵의 CT parameter을 변경하여 PET/CT 영상 재구성 할 때 적용하여 SUVmax에 어떤 영향을 미치는지 비교 평가해보고자 한다. 장비는 Biograph mCT 64를 사용하였고 Phantom은 NEMA IEC Body Phantom을 사용하였다. 실험을 위해 환자는 2017년 2월에서 3월까지 본원 PET/CT 검사를 시행한 환자 20명을 대상으로 Lung, Liver, Bone에 관심영역을 선택하여 기존 08f AC, 45f medium, 80f ultra sharp 방식의 CT kernel을 적용한 감쇠보정맵을 사용하여 PET/CT 영상 재구성에 도입 후 방사능 농도(kBq/mL), SUVmax, SD(standard deviation) 변화 유무를 평가하였다. Phantom 방사능 농도 측정 결과 B08f AC 대비 B45f 0.96%, B80f 6.58% 증가하였고 B08f AC 대비 B45f 0.86%, B80f 6.54%각각 증가하였고, SD의 경우 B08f AC 대비 B45f 1.27%, B80f 6.96% 증가하였다. 환자에서 부위별 SUV는 Lung에서 B08f AC 대비 B45f 1.6%, B80f 6.6%, Liver에서 B08f AC 대비 B45f 0.7%, B80f 4.7%, Bone에서 B08f AC 대비 B45f 1.3%, B80f 6.2% 증가를 보였다. 부위별 SD는 Lung에서 B08f AC 대비 B45f 6.2%, B80f 15.4%, Liver에서 B08f AC 대비 B45f 2.1%, B80f 11%, Bone에서 B08f AC 대비 B45f 를 사용할 때 2.3%, B80f 14.7% 증가를 보였다. CT Kernel변화에 따라 sharpness noise와 영상의 질은 변화를 보였으나 SUVmax와 SD는 통계적으로 유의한 차이가 없었다.(P>.05). 핵의학 영상은 정량적인 평가가 중요하다 따라서 부위에 따라 CT kernel이 적절하게 조절되고 noise level이 낮은 감쇠보정 맵을 사용하여 PET/CT 재구성시에 적용하여 정량적 평가에 오류를 줄이는 것이 중요하다고 사료되므로 따라서 같은 부위라 할지라도 sharpness noise가 인위적으로 증가된 kernel을 사용하는 것보다 noise가 낮은 kernel을 사용하는 것이 SD편차를 줄이고 정량적인 평가에 오류를 적게 하여 정확한 진단과 SUV 측정에 유용할 것으로 사료된다.
Siemens사의 CT 선량 감소 소프트웨어인 IRIS의 적용을 통하여 CT 선량 감소 시 노이즈 감소 효과와 해상력의 보존 그리고 ACCT에 IRIS 각 kernel의 적용 시 SUV 변화를 확인하는데 목적을 두었다. Biograph mCT 40 slice 스캐너를 이용하여 AAPM CT performance phantom, Anthropomorphic chest phantom을 관전압 120 kVp로 고정하고 100-10 mAs까지 15%감소하여 스캔 후 FBP, IRIS 각 kernel을 적용하여 재구성 하여 영상의 노이즈, 해상력, 영상 평가를 시행하였다. NEMA IEC body phantom을 이용하여 55.5 MBq를 background에 주입하고 열소와 배후 방사의 비를 8:1이 되도록 모형을 제작하였다. 120 kVp, 50 mAs 조건으로 1분, 2분, 3분, 4분 스캔하여 영상을 획득한 후 ACCT에 IRIS 각 kernel을 적용하여 기존 FBP 방식을 적용한 SUV와의 평가를 시행하였다. IRIS의 적용 시 기존 FBP 방식에 비하여 45% 선량을 감소하였음에도 불구하고 해상력 저하 없는 노이즈 감소 효과가 확인 되었으며 SUV 평가 실험에서 IRIS의 I70f kernel을 제외하고는 기존 FBP 방식을 통하여 획득된 SUV와 유의한 차이가 나타나지 않았다. 본 연구를 통하여 IRIS 적용 시 기존 FBP 방식에 비하여 CT 피폭선량 감소와 해상력 저하 없는 노이즈 감소 효과를 입증하였으며 IRIS kernel의 적절한 적용을 통하여 PET/CT 검사 시 환자 피폭선량 감소는 물론 FBP 방식에 비하여 우수한 영상의 획득이 가능할 것이라 사료된다.
Objective : Our retrospective study aimed to determine whether 16-slice computerized tomography (CT) angiography optimized sharp kernel is suitable for the evaluation of visibility, luminal patency and re-stenosis of intracranial stents in comparison with conventional angiography. Methods : Fifteen patients with symptomatic intracranial stenotic lesions underwent balloon expandable stent deployment of these lesions (10 middle cerebral arteries, 2 intracranial vertebral arteries, and 3 intracranial internal carotid arteries). CT angiography follow-up ranged from 6 to 15 months (mean follow-up, 8 months) after implantation of intracranial stents and conventional angiography was confirmed within 2 days. Curved multiplanar reformations with maximal intensity projection (MIP) with optimal window settings for assessment of lumen of intracranial stents were evaluated for visible lumen diameter, stent patency (contrast distal to the stent as an indirect sign), and re-stenosis by two experienced radiologists who blinded to the reports from the conventional angiography. Results : All of stents deployed into symptomatic stenotic lesions. All stents were classified as patent and no re-stenosis, which was correlated with results of conventional angiography. Parts of the stent lumen could be visualized in all cases. On average, 57% of the stent lumen diameter was visible using optimized sharp kernel. Significant improvement of lumen visualization (22%, p<0.01) was observed using the optimized sharp kernel compared with the standard sharp kernel. Inter-observer agreements on the measurement of lumen diameter and density were judged as good, respectively (p<0.05). Conclusion : Sixteen-slice CT using the optimized sharp kernel may provide a useful information for evaluation of lumen diameter patency, and re-stenosis of intracranial stents.
경부 CT검사 시 선속경화인공물(Beam Hardening Artifact)에 의해 제 6번 7번 경추 및 추간판 등의 질환 및 그밖에 해부학적 구조를 정확히 구분하기에 어려움이 있다. 경부 CT검사를 시행할 경우 자세의 변화방법을 적용한 견관절의 방향과 위치에 따른 영상평가 및 커널값의 변화에 따른 영상평가를 통하여 선속경화 인공물의 원인을 알아보고 가장 적절한 검사자세 및 Kernel값을 실험을 통하여 알아보고자 하였다. 경부 CT검사를 위해 내원한 환자 30명(2010년 7월1일 ~ 2010년 12월31일까지 내원한 환자)을 대상으로 Somatom Sensation 16(Siemens, Enlarge, Germany)장비를 이용하였고, workstation은 AW 4.4 version(GE, USA)을 이용하였다. 환자 자세는 견관절의 방향과 위치에 따라 세가지 자세로 변화를 주었으며 양쪽 팔을 편안하게 위치시킨 바로 누운 자세(group N), 왼쪽 팔을 거상 시킨 자세(group S) 그리고 양손을 외 선위(eversion)시켜 최대한 아래로 내리는 자세(group P)로 견관절의 방향을 변화를 주어 스캔을 시행하였고, 두 번째로 영상 재구성 방법을 이용하여 스캔 데이터에 커널값을 B10(very smooth), B20(smooth), B30(medium smooth), B40(medium), B50(medium sharp), B60(sharp), B70(very sharp)로 변화를 주어 재구성 하였다. 검사자세의 변화와 Kernel값의 변화를 주어 얻어진 영상 데이터를 이용하여 각 각의 노이즈 값 측정과 영상평가를 통하여 분석해보았다. 경부 CT검사 시 검사자세는 양손을 외선위(eversion)시켜 최대한 아래로 내리는 자세(group P)로 하며, 커널 값은 B40(medium)또는 B50(medium sharp)으로 재구성할 경우 가장 적절한 자세와 커널값으로 분석되어 임상에 적용 시 매우 유용할 것으로 사료된다.
목적 필터보정역투영(filtered back projection; 이하 FBP)법과 적절한 커널로 재구성된 소아 저선량 안면 컴퓨터단층촬영(이하 CT)의 시행 가능성을 평가하고자 한다. 대상과 방법 응급실에서 안면 CT를 촬영한 10세 이하 환자의 임상 및 영상 데이터를 후향적으로 검토하였다. 환자들을 두 그룹으로 나누었다: 고정된 80 kVp와 자동관전류변조기법을 사용하는 저선량 CT (low-dose CT, 그룹 A, n = 73), 고정된 120 kVp와 자동관전류변조기법을 사용하는 표준 선량 CT (standard-dose CT, 그룹 B, n = 40). 모든 영상은 FBP로 재구성되었다: 그룹 A는 뼈와 연조직 커널을, 그룹 B는 뼈 커널을 이용하였다. 두 그룹의 영상 잡음, 신호대잡음비(signal-to-noise ratio; 이하 SNR), 그리고 대조대잡음비(contrast-to-noise ratio; 이하 CNR)를 비교하였다. 두 명의 영상의학과 의사가 뼈와 연조직의 영상 품질에 대해 주관적으로 점수화하였다. 용적 CT 선량지수(CT dose index volume)와 선량길이곱(dose length product)을 기록하였다. 결과 영상 잡음은 그룹 A가 그룹 B보다 높았다(p < 0.001). 연조직 커널을 사용한 그룹 A 영상에서 가장 높은 SNR과 CNR을 보였다(p < 0.001). 뼈의 정성적 평가에서 뼈 커널 영상들을 비교하면 그룹 A가 그룹 B보다 비슷하거나 높은 점수를 보였다. 연조직의 정성적 평가에서 연조직 커널을 이용한 그룹 A와 뼈 커널에 연조직 창 설정을 이용한 그룹 B 사이에는 통계적으로 유의한 차이가 없었다(p > 0.05). 그룹 A는 그룹 B에 비해 방사선 선량이 76.9% 감소했다(3.2 ± 0.2 mGy vs. 13.9 ± 1.5 mGy, p < 0.001). 결론 연조직 커널 영상을 FBP로 재구성된 전통적인 CT에 추가함으로써 영상 품질을 유지하면서 소아 저선량 안면 CT 프로토콜을 사용할 수 있다.
Advanced modeled iterative reconstruction (ADMIRE) represents a repetitive reconstruction method that can adjust strength and kernel, each of which are known to affect computed tomography (CT) image quality. The aim of this study was to quantitatively analyze the noise and spatial resolution of CT images according to ADMIRE control factors. Patient images were obtained by applying ADMIRE strength 2 and 3, and kernel B40 and B59. For quantitative evaluations, the noise level, spatial resolution, and overall image quality were measured using coefficient of variation (COV), edge rise distance (ERD), and natural image quality evaluation (NIQE). The superior values for the average COV, ERD, and NIQE results were obtained for the ADMIRE reconstruction conditions of ADMIRE 2 + B40, ADMIRE 3 + B59, and ADMIRE3 + B59. NIQE, which represents the overall image quality based on no-reference, was about 6.04 when using ADMIRE 3 + B59, showing the best result among the reconstructed image acquisition conditions. The results of this study indicate that the ADMIRE strength and kernel chosen for use in ADMIRE reconstruction have a significant impact on CT image quality. This highlights the importance of adjusting to the control factors in consideration of the clinical environment.
영상 획득과 재구성 방법에 따라 CT 감약계수는 다양성을 보이고 관심 영역의 노이즈는 정밀도에 영향을 준다. 인체에서 간 실질조직과 위장의 물의 CT 감약계수와 노이즈를 커널에 따라 측정하였다. 다중채널 CT 스캐너를 이용하여 복부를 스캔 하였고, 커널은 B10 (very smooth), B20 (smooth), B30 (medium smooth), B40 (medium), B50 (medium sharp), B60 (sharp), B70 (very sharp), B80 (ultra sharp)으로 재구성하여 간의 실질 조직과 물이 들어 있는 위장 부위를 ROI 기능을 이용하여 평균의 CT감약계수와 표준편차인 노이즈를 측정하여 영상을 비교하였다. 간의 실질 조직에서 CT감약계수는 커널에 따라 60.4에서 69.2 HU사이에서 분포하여 차이가 없었으나, 노이즈는 커널(7.6$\sim$63.8 HU)이 높아질수록 증가하였다. 물의 CT감약계수는 -2.2 HU에서 0.8 HU사이에서 측정되었고, 노이즈는 커널(10.1$\sim$82.4 HU)이 높아질수록 증가하였다. 영상의 질을 높이기 위해서는 검사 부위에 따라 노이즈를 감소하기 위해 적절한 커널을 선택하여 CT 검사를 하여야 한다.
Background: Persistent uroliths after a cystotomy in dogs are a common cause of surgical failure. Objectives: This study examined the following: the success rate of retrograde urohydropropulsion in male dogs using non-enhanced computed tomography (CT), whether the CT mean beam attenuation values in Hounsfield Units (mHU) measured in vivo could predict the urolithiasis composition and whether the selected reconstruction kernel may influence the measured mHU. Methods: All dogs and cats that presented with lower urinary tract uroliths and had a non-enhanced CT preceding surgery were included. In male dogs, CT was performed after retrograde urohydropropulsion to detect the remaining urethral calculi. The percentage and location of persistent calculi were recorded. The images were reconstructed using three kernels, from smooth to ultrasharp, and the calculi mHU were measured. Results: Sixty-five patients were included in the study. The success rate of retrograde urohydropropulsion in the 45 male dogs was 55.6% and 86.7% at the first and second attempts, respectively. The predominant components of the calculi were cystine (20), struvite (15), calcium oxalate (8), and urate (7). The convolution kernel influenced the mHU values (p < 0.05). The difference in mHU regarding the calculus composition was better assessed using the smoother kernel. A mHU greater than 1,000 HU was predictive of calcium oxalate calculi. Conclusions: Non-enhanced CT is useful for controlling the success of retrograde urohydropropulsion. The mHU could allow a prediction of the calculus composition, particularly for calcium oxalate, which may help determine the therapeutic strategy.

Objective: The purpose of this meta-analysis was to investigate the pooled agreements of the coronary artery calcium (CAC) severities assessed by electrocardiogram (ECG)-gated and non-ECG-gated CT and evaluate the impact of the scan parameters. Materials and Methods: PubMed, EMBASE, and the Cochrane library were systematically searched. A modified Quality Assessment of Diagnostic Accuracy Studies-2 tool was used to evaluate the quality of the studies. Meta-analytic methods were utilized to determine the pooled weighted bias, limits of agreement (LOA), and the correlation coefficient of the CAC scores or the weighted kappa for the categorization of the CAC severities detected by the two modalities. The heterogeneity among the studies was also assessed. Subgroup analyses were performed based on factors that could affect the measurement of the CAC score and severity: slice thickness, reconstruction kernel, and radiation dose for non-ECG-gated CT. Results: A total of 4000 patients from 16 studies were included. The pooled bias was 62.60, 95% LOA were -36.19 to 161.40, and the pooled correlation coefficient was 0.94 (95% confidence interval [CI] = 0.89-0.97) for the CAC score. The pooled weighted kappa of the CAC severity was 0.85 (95% CI = 0.79-0.91). Heterogeneity was observed in the studies (I2 > 50%, p < 0.1). In the subgroup analysis, the agreement between the CAC categorizations was better when the two CT examinations had reconstructions based on the same slice thickness and kernel. Conclusion: The pooled agreement of the CAC severities assessed by the ECG-gated and non-ECG-gated CT was excellent; however, it was significantly affected by scan parameters, such as slice thickness and the reconstruction kernel.
