추간판 내부 파열 증후군(Internal disc disruption) 2례 (Two Cases of Disc Internal Disruption Syndrome)

  • 추간판 내부 파열 증후군은 심한 요통, 하족부 동통, 에너지 소실, 체중감소, 그리고 심한 우울증을 동반하며 척추 단순 촬영, 척수강 조영술, 전산화 단층 촬영, 혈액 검사, 그리고 신경학적 검사에서 이상소견을 발견할 수 없고 추간원판 조영술 상에서만 이상소견 및 동통의 증가로 진단이 가능한 증후군으로 알려져 있다. 저자들은 요통을 주소로 내원한 환자들 중 2례에서 추간판 내부 파열 증후군으로 진단되어 수술적 치료 후 만족할 만한 결과를 얻었기에 문헌고찰과 함께 보고하는 바이다.

  • The sacroiliac joint is difficult to appropriately examine and treat because it is extremely complex and also is difficult to distinguish dysfunctions from those of the spine and hip which are highly intergrated functionally. In addition generally traditional x-rays and CT scans also are not beneficial in detecting sacroiliac joint dysfunction. The manipulative physiotherapist should seek to establish a series of relevant finding that build into a case implicating the sacroiliac joint. When deciding to use these diagnostic tests, the examiner must determine if the test will give reliable and useful information that will help in the diagnosis and subsequent treatment. To be useful diagnostic tests must give reliable data and be valid and the most useful methods of determining whether a test is a good test for pathology under consideration are sensitivity and specificity. In the ideal world, one would want a test that has both high sensitivity and high specificity. The purpose of this review is to ascertain diagnostic value on the tests for sacroiliac joint dysfunction by literature.

The Versatility of Cervical Vertebral Segmentation in Detection of Positional Changes in Patient with Long Standing Congenital Torticollis

  • Background Congenital muscular torticollis (CMT) is a benign condition. With early diagnosis and appropriate management, it can be cured completely, leaving no residual deformity. However, long-standing, untreated CMT can lead to permanent craniofacial deformities and asymmetry.Methods Nineteen patients presented to the author with congenital muscular torticollis. Three dimensional computed tomography (3-D CT) scans was obtained upon patient’s admission. Adjustment of skull’s position to Frankfort horizontal plan was done. Cervical vertebral segmentation was done which allowed a 3D module to be separately created for each vertebra to detect any anatomical or positional changes.Results The segmented vertebrae showed an apparent anatomical changes, which were most noticeable at the level of the atlas and axis vertebrae. These changes decreased gradually till reaching the seventh cervical vertebra, which appeared to be normal in all patients. The changes in the atlas vertebra were mostly due to its intimate relation with the skull base, while the changes of the axis were the most significantConclusion Cervical vertebral segmentation is a reliable tool for isolation and studying cervical vertebral pathological changes of each vertebra separately. The accuracy of the procedures in addition to the availability of many software that can be used for segmentation will allow many surgeons to use segmentation of the vertebrae for diagnosis and even for preoperative simulation planning.

이하선에 발생한 상피근상피암이 치험례 (A Case of Epithelial-myoepithelial Carcinoma of the Parotid Gland)

  • Epithelial-myoepithelial carcinoma (EMC) of the salivary gland is a rare tumor that comprises approximately 1% of all salivary gland tumors. It has a distinctive histological appearance comprising ductal structures with an inner epithelial cell component and an outer layer of myoepithelial cells. We report a case of EMC of the parotid gland in a 41- year-old man. He presented left-sided subauricular swelling developed 3-month earlier. Neck CT scans revealed a well-defined mass in the left superficial parotid gland. He underwent superficial parotidectomy and was diagnosed as EMC. He was taken postoperative radiotherapy. There was no evidence of recurrence during a follow-up period of 12 months. A EMC is a low grade malignant tumor which can cause diagnostic confusion during workup and also frequently misdiagnosed as other benign or malignant tumor.

벤츠 패턴을 가진 두개골 조기 유합증: 증례보고 (Mercedes Benz Pattern Craniosynostosis: A Case Report)

  • Purpose: Craniosynostosis of three or more cranial sutures was not common. "Mercedes Benz pattern," named by Moore1 was a rare form of craniosynostosis and had an atypical pattern of premature closure of cranial suture. It was not reported in Republic of Korea. We report this case with literature review. Methods: A 13-months-old male patient visited our clinic due to exophthalmos. He showed normal developmental course. Other neurological tests were normal but he was Crouzon syndrome patient. CT scans showed bilateral lambdoid and posterior sagittal sutures were fused and the length of the skull was extended. Cranioplasty with pi craniotomy & Barrel-Stave osteotomy and recombination of the bone flap was performed. Results: The patient was discharged after post operative 10 days without any complications. In follow up visit after 2.7 years, he was in good state without recurrence and functional abnormality of skull. Conclusion: This was the first case of Mercedes Benz pattern craniosynostosis with Crouzon syndrome in Korea. This type of craniosynostosis has to be considered differently from single type of craniosynostosis or typical syndromic craniosynostosis clinically and surgically.

모야모야병으로 인한 출혈성 뇌졸중 환자 1례에 대한 증례보고 (Case Report on Traditional Korean Medicine of an Acute Intracerebral Hemorrhage Patient Diagnosed with Moyamoya Disease)

  • Objective: This case report examined a patient to determine if traditional Korean medicine treatment was effective for treating acute intracerebral hemorrhage (ICH) diagnosed with moyamoya disease.Method: The patient was treated with herbal medicine, acupuncture, and Western medicine and was followed up with for symptoms, vital signs, and brain computed tomography (CT) scans.Results: During the treatment, the patient’s mental state was mostly alert. The patient’s headaches were improved after the use of painkillers; the patient’s fever and blood pressure were well controlled. The cerebral hematoma was removed smoothly.Conclusion: Traditional Korean medical therapy (herbal medicine and acupuncture) along with Western methods appeared to be effective in managing the acute phase of ICH diagnosed with moyamoya disease.

Aberrant Carotid Artery Associated with Throat Discomfort and Retropharyngeal Mass

  • We present four cases of aberrant carotid arteries manifesting as pharyngeal masses with throat discomfort. The symptoms of this anomaly are very similar to those of laryngopharyngeal reflux or globus pharyngeus. The identification of this anomalous artery is essential for the head and neck surgeon because the anomaly frequently manifests as a symptomatic pulsatile mass in the pharynx. All patients complained of foreign body sensation in the throat and showed protrusion of the posterolateral pharyngeal wall anteromedially. CT scans demonstrated that there were two cases involving the right common carotid artery (CCA) and two cases involving both CCAs in the retropharyngeal space. As conclusion, the aberrant course of the artery can occur on both sides of CCAs, and it can manifest as a pharyngeal mass. When the patients complain of foreign body sensation in the throat without any other abnormal findings in the pharynx, we should consider the possibility that it may be due to the aberrant course of the CCA and its bifurcations in the retropharyngeal space.

Validation of a low-cost portable 3-dimensional face scanner

  • Purpose: The goal of this study was to assess the accuracy and reliability of a low-cost portable scanner (Scanify) for imaging facial casts compared to a previously validated portable digital stereophotogrammetry device (Vectra H1). This in vitro study was performed using 2 facial casts obtained by recording impressions of the authors, at King's College London Academic Centre of Reconstructive Science. Materials and Methods: The casts were marked with anthropometric landmarks, then digitised using Scanify and Vectra H1. Computed tomography (CT) scans of the same casts were performed to verify the validation of Vectra H1. The 3-dimensional (3D) images acquired with each device were compared using linear measurements and 3D surface analysis software. Results: Overall, 91% of the linear Scanify measurements were within 1 mm of the corresponding reference values. The mean overall surface difference between the Scanify and Vectra images was <0.3mm. Significant differences were detected in depth measurements. Merging multiple Scanify images produced significantly greater registration error. Conclusion: Scanify is a very low-cost device that could have clinical applications for facial imaging if imaging errors could be corrected by a future software update or hardware revision.

Erythrocyte Sedimentation Rate: Its Determinants and Relationship with Risk Factors Involved in Ischemic Stroke

  • Erythrocyte sedimentation rate (ESR) evaluation is a useful tool for monitoring disease activity in various inflammatory and non-inflammatory conditions. ESR is known to be influenced by a multitude of confounding factors. The present study aimed to assess the possible determinants of the ESR and its relationship with various risk factors involved in ischemic stroke. ESR and other hematological and biochemical parameters were investigated in 163 ischemic stroke patients (107 males and 56 females) selected based on imaging techniques including magnetic resonance imaging (MRI) or computed tomography (CT) scans. Statistical analysis was performed using the SPSS 16.0 software. Linear regression analysis showed a significant inverse relationship of hemoglobin (Hb) and hematocrit or packed cell volume (PCV) (P<0.001 for females; P<0.01 for males) with the ESR. It was observed that the red blood cell (RBC) count was not strongly correlated with the ESR (P<0.05 for both males and females). It was also observed that sex significantly affected the variables determining the ESR levels, whereas age had no effect. Gender differences were also observed with respect to Hb, RBC, PCV, mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), and ESR. The possible determinants of higher ESR levels in ischemic stroke may be sex, Hb, hematocrit, and RBC count, but the role of other clinical and laboratory parameters cannot be underestimated.

Anomalies of the clivus of interest in dental practice: A systematic review

  • Purpose: The clivus is a region in the anterior section of the occipital bone that is commonly imaged on large-volume cone-beam computed tomography (CBCT). There have been several reports of incidental clivus variations and certain pathological entities that have been attributed to the variations. This study aimed to evaluate the effects of these variations within the scope of dentistry. Materials and Methods: Medical databases (PubMed, Scopus, and Web of Science) were searched using a controlled vocabulary (clival anomalies, cone-beam CT, canalis basilaris medianus, fossa navicularis magna, clival variation). The search was limited to English language, humans, and studies published in the last 25 years. The articles were exported into RefWorks® and duplicates were removed. The remaining articles were screened and reviewed for supporting information on variations of the clivus on CBCT imaging. Results: Canalis basilaris medianus and fossa navicularis magna were the most common anomalies noted. Many of these variations were asymptomatic, with most patients unaware of the anomaly. In certain cases, associated pathologies ranged from developmental (Tornwaldt cyst), to acquired (recurrent meningitis). While no distinct pathognomonic aspects were noted, there were unique patterns of radiographic diagnosis and treatment modalities. Most patients had a normal course of follow-up. Conclusion: Interpretation of CBCT volumes is a skill every dentist must possess. When reviewing large-volume CBCT scans, the clinician should be able to distinguish pathology from normal anatomic variations within the skull base. The majority of clivus variations are asymptomatic and will remain undetected unless incidentally noted on radiographic examinations.
