토모테라피에서 반복적 금속 인공물 감소 알고리즘의 유용성 평가: 팬톰 실험 (Effect of Iterative-metal Artifact Reduction (iMAR) at Tomotherapy: a Phantom Study)

  • 본 연구의 목적은 토모테라피 방사선치료에서 고밀도 알루미늄, 티타늄, 강철 금속 삽입물에 대한 단층촬영(CT)을 평가하고 자 하였다. 다양한 밀도의 원통형 막대를 포함한 금속 삽입물과 함께 원통형 토모팬텀을 이용하여 영상을 얻었다. 총 세 가지의 CT 영상에 대해 평균 CT 값(number)와 표준 편차를 구하고, 치료계획 선량평가도 수행하였다. 고밀도 금속 삽입물이 CT값과 변화가 가장 컸다. 타겟에 대한 선량평가(적합성 지수, CI)에서 반복적 금속 인공물 감소 알고리즘(iMAR)이 적용된 영상이 그렇지 않은 영상에 비해 약 20% 좋았으나 유의한 차이는 없었다. iMAR은 표적 및 장기의 묘사에 도움을 주고 토모테라피를 이용한 3차원 입체조형 방사선치료기술(3D-CRT)에서 불확실성을 줄이는 데 도움이 될 것으로 사료된다.

나선형영상획득에서 Pitch에 따른 CT 감약계수와 잡음의 변화 (Changes in CT Number and Noise Level according to Pitch in Spiral Image Acquisition)

  • 본 연구는 Pitch의 변화에 따른 CT 감약계수(CT Number)와 잡음(Noise)의 변화를 정량적으로 측정하고자 자체 제작한 맞춤형 팬텀(Customized Phantom)을 사용하였다. 팬텀을 이용한 영상의 획득을 위해 팬텀 내부는 멸균증류수로 가득 채웠다. 유리관 내부에는 생리식염수와 조영제의 비율을 각각 생리식염수 100%, 400:1, 200:1, 100:1, 50:1로 희석한 용액을 담은 후 영상화하였고, 이때 용액의 희석비율별로 pitch를 0, 0.35, 0.7, 1.05, 1.4의 단계로 나누어 각각 영상화하였다. 희석비율별로 모든 ROI에서 측정한 CT number와 noise 값의 평균이 pitch의 변화에 따라 유의한 차이를 보이는지 검증하고자 일원 배치 분산분석(One-way ANOVA Analysis)과 사후검정을 시행하였다. 실험 결과 각 희석비율별 pitch의 변화에 대한 CT number의 변화는 통계적으로 유의한 차이가 없었지만, noise 값은 pitch의 증가에 따라 증가하는 경향을 보였으며, 통계적으로도 유의한 차이를 보이는 것으로 나타났다. 나선형 영상획득 방식은 pitch에 따라 noise가 유의한 수준으로 달라질 수 있다. 따라서 나선형 영상획득 방식을 적용한 CT 영상의 화질평가 항목과 기준을 설정할 필요가 있을 것이다.

필터보정역투영 CT 영상재구성방법에서 잡음 특성 (Noise Properties for Filtered Back Projection in CT Reconstruction)

  • 전산화단층촬영장치의 영상재구성방법으로 필터보정역투영법이 광범위하게 사용되고 있다. 평행빔과 부채살빔의 재구성에 사용되는 투영에 잡음이 포함되었을 때 재구성 된 영상의 잡음을 살펴보았다. 평행빔과 부채살 구조에서 각각 360개, 720개의 투영으로 $512{\times}512$ 크기로 Visual C++을 이용하여 영상재구성하였고, 원본 Shepp-Logan 두부 모형을 매우 잘 복원한다는 것을 확인하였다. 필터보정역투영법의 현실적인 접근(유한한 투영 개수)으로 인해 입력 잡음이 없어도 영상재구성 과정에서 잡음이 발생하였다. 입력 잡음비 0.5% 이하에서 잡음이 빠르게 증가하기 때문에 CT 장치의 잡음 제거 기술 및 영상처리 기법의 개발이 필요할 것이다.

3D CT Image Processing for 3D Printed Auricular Reconstruction of Unilateral Microtia Patient

  • Purpose Microtia is congenital anomaly of external ear and the reconstruction method for the external ear of microtia patient was based on autogenous costal cartilage framework. The application of 3D printing technique in medical science has made more possibility of human tissue restoration, and we tried to apply this technique in auricular reconstruction field. Materials and Methods As for unilateral microtia patient, the contralateral side ear is normal and reconstructive surgeon tried to mimic it for reconstruction of affected ear. So, we obtained facial CT scan of microtia patient and made mirror image of normal side ear. Moreover, to make the 3D scaffold based on the mirror image of normal ear and to apply this scaffold for the auricular reconstruction surgery, we included auriculocephalic sulcus and anterior fixation part. Results We could successfully obtain mirror image of normal ear, auriculocephalic sulcus and anterior fixation part for 3D scaffold printing. Conclusions Using this CT image processing and 3D printing technique, we will be able to make the scaffold for auricular reconstruction of unilateral microtia patient, and perform auricular reconstruction in near future.

3차원 다중 치과 CT 영상의 고화질 스티칭 기법 (High-quality Stitching Method of 3D Multiple Dental CT Images)

  • In this paper, we propose a high-quality stitching method of 3D multiple dental CT images. First, a weighted function is generated using the difference of two distance functions that calculate a distance from the nearest edge of an overlapped region to each position. And a blending ratio propagation function for two gradient vectors is parameterized by the difference and magnitude of gradient vectors that is also applied by the weighted function. When the blending ratio is propagated, an improved region growing scheme is proposed to decide the next position and calculate the blending intensity. The proposed method produces a high-quality stitching image. Our method removes the seam artifact caused by the mean intensity difference between images and vignetting effect. And it removes double edges caused by local misalignment. Experimental results showed that the proposed method produced high-quality stitching images for ten patients. Our stitching method could be usefully applied into the stitching of 3D or 2D multiple images.

  • 본 연구에서는 다중영상 획득용 두뇌팬텀을 사용하여 CT, MR, 그리고 PET영상을 획득하였으며, 이를 정합대상으로 하여 해부학적 영상과 핵의학 기능영상과의 정합을 실시하였다. 정합 알고리즘으로는 현재 널리 사용되고 있는 'Chamfer Matching' 알고리즘과 Mutual Information Maximization' 알고리즘을 이용하였고, 기존 연구에서 타당성이 검증된 정합 프로그램을 사용하였다. 정합 결과, CT-MR, CT-PET, MR-PET세 가지 정합경우에 대하여 두 알고리즘 모두 효과적인 방법임을 알 수 있었으나, 저해상도의 핵의학 영상인 PET과의 정합에선 Mutual Information Maximization'알고리즘이 더 효과적인 방법임을 확인하였다.

안면 비대칭 환자의 하악골 수술 후 하악골 변화에 대한 3차원 CT 영상 비교 (Comparision of Mandible Changes on Three-Dimensional Computed Tomography image After Mandibular Surgery in Facial Asymmetry Patients)

  • Background : When surgeons plan mandible ortho surgery for patients with skeletal class III facial asymmetry, they must be consider the exact method of surgery for correction of the facial asymmetry. Three-dimensional (3D) CT imaging is efficient in depicting specific structures in the craniofacial area. It reproduces actual measurements by minimizing errors from patient movement and allows for image magnification. Due to the rapid development of digital image technology and the expansion of treatment range, rapid progress has been made in the study of three-dimensional facial skeleton analysis. The purpose of this study was to conduct 3D CT image comparisons of mandible changes after mandibular surgery in facial asymmetry patients. Materials & methods : This study included 7 patients who underwent 3D CT before and after correction of facial asymmetry in the oral and maxillofacial surgery department of Yeungnam University Hospital between August 2002 and November 2005. Patients included 2 males and 5 females, with ages ranging from 16 years to 30 years (average 21.4 years). Frontal CT images were obtained before and after surgery, and changes in mandible angle and length were measured. Results : When we compared the measurements obtained before and after mandibular surgery in facial asymmetry patients, correction of facial asymmetry was identified on the "after" images. The mean difference between the right and left mandibular angles before mandibular surgery was $7^{\circ}$, whereas after mandibular surgery it was $1.5^{\circ}$. The right and left mandibular length ratios subtracted from 1 was 0.114 before mandibular surgery, while it was 0.036 after mandibular surgery. The differences were analyzed using the nonparametric test and the Wilcoxon signed ranks test (p<0.05). Conclusion: The system that has been developed produces an accurate three-dimensional representation of the skull, upon which individualized surgery of the skull and jaws is easily performed. The system also permits accurate measurement and monitoring of postsurgical changes to the face and jaws through reproducible and noninvasive means.

악골 병소의 감별 진단시 골스캔의 임상적 유용성 (CLINICAL EFFECTIVENESS OF BONE SCAN FOR DIFFERENTIAL DIAGNOSIS OF JAW LESION)

  • Bone scan using radioactive isotope can be more effective than conventional X-ray radiograph for finding jaw lesion because it takes an image of the physiologic change of bone. This study is designed to show how available bone scan is able to diagnose jaw lesion better than simple X-ray and CT, as well as to determine a basis of diagnosis for jaw lesion using bone scan. The 77 patients, visiting the Oral & Maxillofacial Surgery, Department of Dankook University Hospital from January 2002. to August 2005. who were diagnosed histopathologically with postoperative malignant tumor, osteomyelitis, and bone infiltrative benign disease. Preoperative X-ray, CT, bone scan were taken and were compared with histopathologic finding. Also to compare specificty of each lesion in bone scan, bone density was measured to compare. The results were as follows. 1. Among the 25 cases of oral malignant tumor of bony invasion, a positive diagnosis associated with histopathologic evaluation, 22 cases(88%) in bone scan, 14 cases(56%) in CT image, and 10 cases40%) in simple X-ray. 2. Among the 31 cases of osteomyelitis, a positive diagnosis associated with histopathologic evaluation, 30 cases(97%) in bone scan, 23 cases(74%) in CT image, and 19 cases(61%) in simple X-ray. 3. Among the 11cases of bone infiltrative benign disease, a positive diagnosis associated with histopathologic evaluation, 11 cases(100%) in bone scan, 10 cases(91%) in CT image, and 6 cases(55%) in simple X-ray. 4. Measurement of bone density in each group showed no statistical significant difference between malignant tumor and osteomyelitis as well as benign bone disease. But, a statistical significance was seen between osteomyelitis and benign bone disease. From this results, bone scan are more sensitive than simple X-ray and CT image in jaw lesion diagnosis, but specificity shows no significant difference. Therefore, it should be suggested that evaluation of bone scan must be carrying out in reference to final histopathologic diagnosis.

  • Now a days, there's many change over for PACS among the most of hospital and it standard for DICOM 3.0. These kind of using of DICOM 3.0 improves increasing of medical imaging exchange and service for patient. However, there's some problems of compatibility caused during carry out CD and DVD from hospital. For this reason, this thesis analyzed patients image targeting those storages requested to hospitals in Seoul by using Validation Toolkit which is recommended from KFDA. The analyze type is like this. Make 100 data, total 500, each of MRI CT Plain x-ray Ultrasound PET-CT images and analyzed type of error occurred and loyalty of information. If express percentage of error occurred statistically, we can get a result as follows MRI 5%, Plain x-ray 11%, CT 18%, US 25%, PET-CT 30%. The reson why percentage of error occurred in PET-CT is because of imperfective support and we could notice that we weren't devoted to information. Even though, PET-CT showed highest percentage of error occurred, currently DICOM data improved a lot compare to past. Moreover, it should be devoted to rule of IHE TOOL or DICOM. In conclusion, we can help radiographer to analyze information of image by providing clues for solving primary problem and further more, each of PACS company or equipment company can enhance fidelity for following standard of image information through realizing the actual problem during transfer of image information.

노이즈 레벨 및 유사도 평가 기반 저선량 조건의 전산화 단층 검사 영상에서의 비지역적 평균 알고리즘의 최적화 (Optimization of Non-Local Means Algorithm in Low-Dose Computed Tomographic Image Based on Noise Level and Similarity Evaluations)

  • In this study, we optimized the FNLM algorithm through a simulation study and applied it to a phantom scanned by low-dose CT to evaluate whether the FNLM algorithm can be used to obtain improved image quality images. We optimized the FNLM algorithm with MASH phantom and FASH phantom, which the algorithm was applied with MATLAB, increasing the smoothing factor from 0.01 to 0.05 with increments of 0.001 and measuring COV, RMSE, and PSNR values of the phantoms. For both phantom, COV and RMSE decreased, and PSNR increased as the smoothing factor increased. Based on the above results, we optimized a smoothing factor value of 0.043 for the FNLM algorithm. Then we applied the optimized FNLM algorithm to low dose lung CT and lung CT under normal conditions. In both images, the COV decreased by 55.33 times and 5.08 times respectively, and we confirmed that the quality of the image of low dose CT applying the optimized FNLM algorithm was 5.08 times better than the image of lung CT under normal conditions. In conclusion, we found that the smoothing factor of 0.043 among the factors of the FNLM algorithm showed the best results and validated the performance by reducing the noise in the low-quality CT images due to low dose with the optimized FNLM algorithm.
