Is the Mixed Use of Magnetic Resonance Enterography and Computed Tomography Enterography Adequate for Routine Periodic Follow-Up of Bowel Inflammation in Patients with Crohn's Disease?

  • Objective: Computed tomography enterography (CTE) and magnetic resonance enterography (MRE) are considered substitutes for each other for evaluating Crohn's disease (CD). However, the adequacy of mixing them for routine periodic follow-up for CD has not been established. This study aimed to compare MRE alone with the mixed use of CTE and MRE for the periodic follow-up of small bowel inflammation in patients with CD. Materials and Methods: We retrospectively compared two non-randomized groups, each comprising 96 patients with CD. One group underwent CTE and MRE (MRE followed by CTE or vice versa) for the follow-up of CD (interval, 13-27 months [median, 22 months]), and the other group underwent MRE alone (interval, 15-26 months [median, 21 months]). However, these two groups were similar in clinical characteristics. Three independent readers from three different institutions determined whether inflammation had decreased, remained unchanged, or increased within the entire small bowel and the terminal ileum based on sequential enterography of the patients after appropriate blinding. We compared the two groups for inter-reader agreement and accuracy (terminal ileum only) using endoscopy as the reference standard for enterographic interpretation. Results: The inter-reader agreement was greater in the MRE alone group for the entire small bowel (intraclass correlation coefficient [ICC]: 0.683 vs. 0.473; p = 0.005) and the terminal ileum (ICC: 0.656 vs. 0.490; p = 0.030). The interpretation accuracy was higher in the MRE alone group without statistical significance (70.9%-74.5% vs. 57.9%-64.9% in individual readers; adjusted odds ratio = 3.21; p = 0.077). Conclusion: The mixed use of CTE and MRE was inferior to MRE alone in terms of inter-reader reliability and could probably be less accurate than MRE alone for routine monitoring of small bowel inflammation in patients with CD. Therefore, the consistent use of MRE is favored for this purpose.

DMIDR 장치의 재구성 알고리즘 별 성능 평가 (Performance Evaluation of Reconstruction Algorithms for DMIDR)

  • DMIDR (General Electric Healthcare, USA)은 GE 사(社)의 최신 장비로써 PSF (Point Spread Function reconstruction), TOF(Time of Flight)와 Q.Clear의 적용이 가능하다. 특히, Q.Clear는 보정 알고리즘으로써 복셀(voxel)단위 신호 잡음 제거로 기존 OSEM (Ordered Subset Expectation Maximization)의 한계를 넘어설 수 있다. 따라서 이러한 재구성 및 보정 알고리즘의 성능 평가를 통해 정확한 SUV를 구현하며, 병변 검출 능력에 도움이 되는 알고리즘의 조합을 확인하고자 하였다. H/B(Hot & Background) Ratio 2:1, 4:1, 8:1의 비율로 NEMA/IEC 2008 PET phantom을 제작하였다. DMIDR의 NEMA test protocol을 이용하여 영상 획득을 하였다. 재구성 조합은 (1) VPFX(VUE point FX(TOF)), (2) VPHD-S(VUE point HD+PSF), (3) VPFX-S(TOF+PSF), (4) QCHD-S-400(VUE point HD+Q.Clear(${\beta}-strength$ 400)+PSF), (5) QCFX-S-400(TOF+Q.Clear(${\beta}-strength$ 400)+PSF), (6) QCHD-S-50(VUE point HD+Q.Clear(${\beta}-strength$ 50)+PSF), (7) QCFX-S-50(TOF+Q.Clear(${\beta}-strength$ 50) + PSF)의 7 가지로 구성하였다. H/B Ratio 및 재구성 알고리즘 별로 측정된 결과를 이용하여 CR (Contrast Recovery)와 BV (Background Variability)을 구하였다. 또한, 각 조합의 count를 측정하여 SNR (Signal to Noise Ratio)과 RC(Recovery Coefficient)를 구하고 SUV (Standardized Uptake Value)를 측정하였다. 구의 크기가 가장 작은 10 mm와 13 mm에서는 VPFX-S, 17 mm 이상에서는 QCFX-S-50에서 가장 높은 CR 결과를 보였다. BV와 SNR의 비교에서는 QCFX-S-400과 QCHD-S-400에서 좋은 값을 보였다. SUV 측정 결과는 H/B ratio와 비례하여 증감하는 양상을 보였다. SUV에 대한 RC의 경우 H/B ratio와 반비례하는 양상을 보였으며, 재구성 알고리즘 중에서는 QCFX-S-50이 가장 높은 값을 보였다. 또한, Q.Clear에 ${\beta}-strength$ 400이 적용된 재구성 알고리즘들이 낮은 값 분포를 보였다. Q.Clear가 적용된 재구성 조합은 ${\beta}-strength$를 높이면 신호잡음이 억제되어 영상 품질면에서 우수한 결과를 보였고 ${\beta}-strength$를 낮추면 선예도가 증가하며, partial volume effect가 감소하여 기존의 재구성 조건에 비하여 높은 RC에 근거한 SUV 측정이 가능하였다. 이러한 진보된 알고리즘의 사용으로 보다 정확한 정량화와 미세병변 검출능력을 향상 시킬 수 있으나 상관 관계를 고려하여 목적에 맞는 최적화 과정이 필요할 것으로 사료된다.

  • 장수허리노린재(Anoplocnemis dallasi Kiritschenko)의 第1齡 幼蟲(50個體)부터 成蟲까지(各 70個體씩)에서 얻은 實測値를 定式化하여 成長 및 相對成長을 分析 觀察한 結果 다음과 같은 結論을 얻었다. $\circled1$ 成長量 : 口吻이 가장 緩慢한 直線狀 增加를 하고 그 外는 모두 急激한 抛物線狀 增加를 나타내며 體長, 後脚, 中脚, 前脚, 觸角, 前胸背板의 幅, 口吻의 順으로 體長이 가장 急激하게 增加한다. $\circled2$ 成長比 : 口吻의 增加率 3.054倍가 가장 적으며 直線狀 增加를 나타내고 그 外는 모두 抛物線狀 增加를 보이며 前胸背板의 幅의 增加率 8.186倍가 가장 크다. $\circled3$ 各 齡別成長比 : 가장 큰 成長比는 若齡 幼蟲쪽에 있으며 가장 적은 成長比는 老齡 幼蟲쪽에 있다. $\circled4$ 成長率 : 觸角의 그것이 減少되었으며 그 外는 모두 增加되었다. $\circled5$ 比成長率 : 觸角이 가장 急하게 減少되었으며 前脚, 口吻, 後脚, 中脚의 順으로 緩慢해졌고 前胸背板의 幅과 體長은 上昇되다가 減少되었다. $\circled6$ 相對成長係數($\alpha$)는 위의 모든 部位에 對해서 前胸背板의 幅의 그것이 항상 가장 强한 優成長이었고 口吻은 가장 弱한 劣成長이었다. $\circled7$ 對 體長, 對前胸背板의 幅, 對觸角, 對口吻, 對前脚, 對中脚, 對後脚 相對成長에서 各 部分의 相對成長係數($\alpha$)는 同一한 樣式(順序)을 나타낸다. $\circled8$ 相對成長係數 $\alpha$가 增加하면 相對成長 初量 b가 減少하는 傾向을 나타내며 그 逆도 成立한다. $\circled9$ 劣成長일 境遇는 成長中心이 若齡基에 있고 優成長일 境遇는 老齡基에 있다.

기관지확장증의 흉부고해상전산화단층촬영소견과 폐기량 및 폐확산능과의 관계 (Lung Volumes and Diffusing Capacity in Bronchiectasis: Correlation with the Findings of High Resolutional CT)

  • 연구배경: 기관지확장증은 폐쇄성환기장애와 더불어 반복적인 폐감염으로 인한 주위 폐실질의 섬유화 및 흉막유착 등으로 인하여 경한 제한상 환기장애가 동반될 수 있다. 그리고 주위 폐실질의 과팽창이나 폐기종성변화도 관찰되어 이로 인한 폐용량 및 폐확산능의 변화가 예상된다. 방 법: 저자들은 1994년 1월부터 1996년 12월까지 경북대학교병원 호흡기내과에서 고해상전산화단층촬영으로 기관지확장증으로 진단된 40예를 대상으로 기관지확장증의 형태, 중증도, 범위 및 동반된 폐기종유무와 폐기량, 폐확산능의 관계를 후향적으로 조사하여 다음과 같은 결과를 얻었다. 결 과: 남자가 18예, 여자가 22예였으며 연령은 평균 53세였다. 고해상전산화단층촬영소견상 원통형이 24예, 낭종형이 16예였으며, 기관지확장증이 있는 평균 폐 구역수는 원통형과 낭종형에서 각각 4.7개 및 7.0개로 낭종형에서 유의하게 많았다 (p<0.05). 확장증의 형태 및 중증도에 따른 VC는 낭종형에서 그리고 기관지확장증의 중증도가 심할수록 감소하는 경향이 있었고 RV, TLC, RV/TLC는 유의한 차이가 없었다. LCI는 원통형에 비해 낭종형에서 유의하게 증가하였으며(p<0.05), 기관지확장증의 중증도에 따를 차이는 없었다. $\triangle$N2/L은 확장증의 형태 및 중증도에 따른 차이가 없었으며 개인차가 심하였다. $D_{LCO}$$D_{LCO}/V_A$는 원통형에 비해 낭종형에서 유의하게 감소하였으며(각각 p<0.1, p<0.05), 확장증의 중증도에 따라서 뚜렷히 감소하였다 (p<0.05). 기관지확장증의 범위와 폐기능성적들과의 상관계수는 VC, $D_{LCO}$ 및 LCI가 각각 -0.322, -0.339, 0.487로 유의한 관계가 있었으나 RV, TLC, RV/TLC, $\triangle$N2/L와 $D_{LCO}/V_A$는 유의한 관계가 없었다. 고해상전산화단층촬영상의 폐기종의 범위와 폐기능성적들과의 상관계수는 VC 및 $D_{LCO}$는 각각 -0.574, -0.381로 유의한 관계가 있었으며 RV, RV/TLC, LCI와 $\triangle$N2/L도 각각 0.435, 0.552, 0.333과 0.427로 유의한 관계가 있었다. 결 론: 이상의 결과로 낭종형과 기관지확장증의 중증도에 따른 폐활량 및 폐확산능의 감소와 흡기의 폐내분포는 기관지확장증의 범위 및 동반된 폐기종의 범위와 관계가 있고, 잔기량은 동반된 폐기종과 관련이 있음을 알 수 있었다.

디지털 유방단층영상합성법의 FBP 알고리즘 적용을 위한 다양한 필터 조합에 대한 연구 (A Study of Various Filter Setups with FBP Reconstruction for Digital Breast Tomosynthesis)

  • 최근에 디지털 유방촬영술(digital mammography, DM)의 해부학적 구조의 겹침 현상과 컴퓨터단층촬영영상(computed tomography, CT)의 높은 환자 선량을 해결하기 위해 디지털 유방단층영상합성장치(digital breast tomosynthesis, DBT)에 대한 연구 개발이 활발하게 수행되고 있다. 하지만 DBT 시스템은 제한된 각도로 영상을 획득하면서 급격한 데이터 결핍으로 인해 다른 층의 간섭으로 인한 인공물(artifacts)이 발생한다. 이를 완화시키기 위해 적절한 필터가 필요하다. 본 논문에서는 DBT 시스템에서 FBP 알고리즘을 이용하여 영상재구성 시 발생되는 인공물을 줄이기 위해 적절한 필터조합을 찾는 것이 궁극적인 목적이다. 시뮬레이션과 실제 실험을 통해 동일한 영상 획득조건에서 FBP 알고리즘을 이용해 재구성된 영상을 분석하여 다양한 필터 조합들의 특성을 조사했다. 필터 특성에 대한 평가를 하기 위해 영상 및 프로파일의 분석과 COV (coefficient of variation)를 이용하여 인공물과 잡음에 대한 평가를 하였다. 본 연구 결과를 통해 분광필터(spectral filter)의 파라미터 인자 값들을 조절하여 cut-off frequency를 설정함으로써 고주파 영역에 있는 영상의 잡음을 줄일 수 있었다. DBT 시스템에서 유방팬텀을 재구성한 영상들을 비교했을 때 분광필터의 파라미터 인자를 0.25로 적용한 영상의 결과는 분광필터를 적용하지 않았을 때보다 영상의 잡음을 10%로 감소시킬 수 있었다. 절편두께필터(slice thickness filter)의 파라미터 인자의 값들을 조절하여 정보들의 불균형을 줄임으로써 다른 층의 간섭으로 인한 인공물을 감소시킬 수 있었다. 결론적으로, 본 연구를 통해 FBP 알고리즘으로 재구성했을 때 필터들의 기본 특성을 확인 할 수 있었으며, 적절한 필터 조합이 실제 화질 개선에 기여한 것으로 확인할 수 있었다. 이 연구 결과는 다양한 필터 조합에 따른 잡음과 데이터 결핍에 의한 인공물에 대한 정보를 제공하여 DBT 시스템의 개발 및 임상화적용 연구에 기반이 될 것으로 기대된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.
