The relationship between the transverse discrepancy of the jaws and asymmetric growth of the condyles in children (임상가를 위한 특집 1 - 성장기 아동에서 상하악 폭경 부조화와 하악과두의 비대칭 성장과의 관계)

  • It has been established that disk displacement of the temporomandibular joint(TMJ) can cause mandibular asymmetry in growing subjects. One of the causes of internal derangement of TMJ seems to be the result of poor positioning of the joint structure in unilateral cross bite, and the subsequent occlusal trauma transferred to the functioning unit of the mandible, the joint and disc. Transverse discrepancy of the maxillary and the mandibular posterior dentoalveolar units was often found in mandibular asymmetric subjects. Most of the asymmetry in growing subjects becomes worse with further growth if left untreated. However once sufficient posterior overjet is gained through orthodontic treatment, many cases have shown improvement in facial asymmetry. Furthermore, the position of condyles in computed tomogram(con-beam CT) changed from anatomically unfavorable position to more concentric position.

Numerical and experimental flow visualization on nasal air flow (비강 내 공기유동에 대한 실험 및 전산유동가시화)

  • Knowledge of airflow characteristics in nasal cavities is essential to understand the physiological and pathological aspects of nasal breathing. Several studies have utilized physical models of the healthy nasal cavity to investigate the relationship between nasal anatomy and airflow. In our laboratory, there have been a series of experimental investigations on the nasal airflow in normal, abnormal, and deformed nasal cavity models cavity models by PIV under both constant and periodic flow conditions. In this time normal and several deformed nasal cavity models, which simulate surgical operation, Turbinectomy, are investigated numerically by the FVM general purpose code and PIV analysis. The comparisons of these results are appreciated. Dense CT data and careful treatment of model surface under the ENT doctor's advice provide more sophisticated cavity models. The Davis (LaVision Co.) code is used for PIV flow analysis. Average and RMS distributions have been obtained for inspirational and expirational nasal airflows in the normal and deformed nasal cavities.

  • Knowledge of airflow characteristics in nasal cavity is essential to understand the physiological and pathological aspects of nasal breathing. Several studies have utilized physical models of the healthy nasal cavity to investigate the relationship between nasal anatomy and airflow. Since the final goal of these works is their contribution to the diagnosisand treatment of nasal diseases, the next step on this topic is naturally studies for disordered nasal cavities. In this paper, as the first application, airflows in the normal and abnormal nasal cavities with adenoid vegetation are investigated experimentally by PIV, and comparisons of both cases are appreciated. Dense CT data and careful treatment of model surface under the ENT doctor's advice provide more sophisticatedcavity model. The CBC PIV algorithm with window offset is used for PIV flow analysis. Average and RMS distributions are obtained for inspirational and expirational nasal airflows. Airflow characteristics that are related with the abnormalities in nasal cavity are presented.

Adrenal Hemorrhage in a Neonate (신생아의 부신 출혈)

  • Neonatal adrenal hemorrhage is frequently associated with birth trauma or perinatal hypoxia. Hemorrhagic necrosis of the adrenal glands is often found at autopsy and many small lesions are usually asymptomatic. A palpable abdominal mass and jaundice are the usual presenting signs. Ultrasound is very useful in the diagnosis of this lesion; however, if the mass has mixed echoic pattern, magnetic resonance imaging (MRl) is helpful for the differential diagnosis from neuroblastoma. We present the case of a female newborn who was found to have a abdominal mass on physical examination. The patient showed anemia and hyperbilirubinemia. An ultrasonogram disclosed a $3.8{\times}3.0$ cm suprarenal mass with mixed echoic pattern. The mass was initially suspected to be neuroblastoma. An abdominal computed tomogram was not able to differentiate the mass. Magnetic resonance imaging revealed markedly increased signal intensity on T1 and T2-weighted sequences. This finding was consistent with adrenal hemorrhage. Serial sonogram demonstrated the mass that resolved completely by 12 weeks of age.

Particle Image Velocimetry Measurements in Nasal Airflow (코 내부 유로(비강) 내부 유동의 PIV해석)

  • For the first time, airflow in the nasal cavity of a normal Korean adult is investigated experimentally by PIV measurement. Nasal airflow can be subdivided into two interrelated categories; nasal airflow resistance and heat and mass transfer between the air stream and the walls of the nasal cavity. In this study, thanks to a new method for the model casting by a combination of the rapid prototyping and curing of clear silicone. a transparent rectangular box containing the complex nasal cavity can be made fur PIV experiments. The CBC PIV algorithm is used for analysis. Average and RMS distributions are obtained for inspirational and expiration nasal airflows. Data fer the airflow at the end of meatuses are obtained for the first time. Comparisons between western and Korean nasal airflows are appreciated. Due to the difference in geometry of the frontal part of nasal cavity, the flow near nares shows the difference.

  • Airflow in the nasal cavity of Korean adults is investigated experimentally by PIV measurement. Quantitative data for normal and abnormal nasal cavities with adenoid vegetation are obtained. The CBC PIV algorithm with window offset is used for PIV flow analysis. Average and RMS distributions are obtained for inspirational and expirational nasal airflows. Comparisons between western and Korean nasal airflows are appreciated. Due to the difference in geometry of the frontal part of nasal cavity, the flow near nares shows the difference. For the joint research on nasal deceases, PIV measurements of nasal airflow for nasal cavities with 50% and 70% adenoid vegetation are conducted for the first time. Comparisons in nasal airflows for both normal and abnormal cases are also appreciated.

An Organized Chronic Subdural Hematoma with Partial Calcification in a Child

  • The authors present a case in which an organized chronic subdural hematoma(CSDH) was incidentally found in a 9-year-old boy with no significant medical history after a pedestrian traffic accident. Preoperative magnetic resonance(MR) imaging showed calcification on the inner membrane and an irregular heterogeneous structure in the hematoma cavity. The findings from the preoperative brain computed tomogram(CT) and MR image were very useful for making the preoperative diagnosis and surgical decision. In choosing the proper surgical strategy for removing the organized CSDH, it was thought that burr hole trephination would present unnecessary difficulties. Thus, craniotomy was selected and the organized CSDH was successfully removed with no complications.

  • Intracranial giant aneurysms have been known to cause clinical signs and symptoms, either by rupture, compression of surrounding structures, repeated minor leakage, or cerebral ischemia due to thromboembolism. A giant aneurysm which manifests only a seizure disorder comprises relatively few contributions. The authors present a case of a giant, unruptured aneurysm solely presenting with generalized tonic-clonic type seizure in a 43-year-old man. Brain computed tomogram(CT) and 3-D CT angiogram demonstrated a huge calcified aneurysm at the bifurcation of right middle cerebral artery. Complete neck clipping and aneurysmectomy followed by uneventful neurologic recovery.

Knowledge of airflow characteristics in nasal cavities is essential to understand the physiological and pathological aspects of nasal breathing. Several studies have utilized physical models of the healthy nasal cavity to investigate the relationship between nasal anatomy and airflow. In our laboratory, there have been a series of experimental investigations on the nasal airflow in normal and deformed nasal cavity models by PIV under both constant and periodic flow conditions. In this time, airflow inside normal nasal cavity is investigated numerically by the FVM general purpose code. The comparisons with PIV measurement are appreciated. Heat and humidity transfer is dealt numerically. Dense CT data and careful treatment of model surface under the ENT doctor’s advice provide more sophisticated cavity models for both PIV experiment and numerical grid system. Average and RMS velocity distributions have been obtained for inspirational and expirational nasal. Temperature distribution, heat and humidity transfer through the mucosa are obtained.
