Microvascular Anastomosis with Non-penetrating Vascular Clips in Head and Neck Free Flap Surgery (두경부 유리피판 수술에 있어서의 비천공성 혈관 클립을 이용한 미세혈관 문합술)

  • Microvascular anastomosis with suture technique is a basic skill but there are several problems such as stenosis, thrombosis and long operating time. Recently plastic surgeons have developed non-suturing mechanical coupling devices for microvascular anastomosis. The authors applied non-penetrating vascular clips (VCS clips) in the field of free flap surgery of head and neck area. Between August of 2004 and January of 2005, we performed 9 free flaps (16 vessels) using small-sized VCS clips. Four stay sutures were applied first and then VCS clips were placed between sutures about 1 mm apart. Vascular pedicle of free flap included the descending branch of lateral circumflex femoral vessel, thoracodorsal vessel, deep inferior epigastric vessel and cephalic vein. The recipient vessels were the superior thyroid artery, superficial temporal artery, internal jugular vein, external jugular vein, and superficial temporal vein. We performed 13 end-to-end (4 arteries and 9 veins) and 3 venous end-to-side anastomoses. No flap related complication occurred but we applied additional clips or sutures in two cases due to blood leakage after completion of anastomosis. Primary patency rates seemed to be good and more rapid anastomosis could be done than conventional suture technique. Advantages of VCS technique are high patency rate, low thrombogenecity and rapidity. Although the high cost of VCS instrument may be a problem, this clip could be applied safely in microvascular free tissue transfer.

Comparison of Two Techniques on Titanium Clip Application for Ovariohysterectomy in Bitches (개의 난소자궁적출술에 있어서 티타늄클립의 두 가지 적용방법에 대한 비교)

  • The objective of this study was to assess the complications of titanium clip application after ovariohysterectomy (OHE) and find the proper surgical technique to minimize the complications by comparing the ligation method of ovarian pedicle. Six female beagle dogs were used for this study and they were divided randomly into two groups by ligation techniques: ligation and non-ligation of the suspensory ligament of ovary including ovarian pedicle. To evaluate the difference between two techniques, the location and embedded pattern of applied clips in the abdominal cavity were identified through radiographic tests and autopsy. Hematology tests were also performed to check the existence of inflammation by applied clips and all tests were carried out monthly for 1 year. There were few changes in location of clips of both groups on X-ray views without any migration. Almost all values in hematology of two groups were within the normal range and there were no inflammatory symptoms. All of applied clips in two groups were found near the kidney, but the embedded forms of them showed slight differences. The clips ligating suspensory ligament and ovarian pedicle were located in deep place and thickly surrounded with thin layers of peritoneum. But the clips ligating only ovarian pedicle were found with ease on surface of layer and located in lower place compared with the clips ligating suspensory ligament. These results indicate that the suspensory ligament holds applied clips and minimizes the movement of them. Therefore, ligation of suspensory ligament would be the preferred technique in titanium clip application for OHE.

Performance evaluation of suspended ceiling systems using shake table test

  • The national standard being used in Turkey for suspended ceiling systems (SCS) regulates material and dimensional properties but does not contain regulations regarding installation instructions which cause substandard applications of SCSs in practice. The lack of installation instructions would potentially affect the dynamic performance of these systems. Also, the vast majority of these systems are manufactured using substandard low-quality materials, and this will inevitably increase SCS related damages during earthquakes. The experimental work presented here focuses on the issue of dynamic performance of SCSs with different types of carrier systems (lay-on and clip-in systems), different weight conditions, and material-workmanship qualities. Moreover, the effects of auxiliary fastening elements, so called seismic perimeter clips, in improving the dynamic performance of SCSs were experimentally investigated. Results show that clip-in ceiling system performs better than lay-on system regardless of material and workmanship qualities. On the other hand, the quality aspect becomes the most important parameter in affecting the dynamic performance of lay-on type systems as opposed to tile weights and usage of perimeter clips. When high quality system is used, tile weight does not change the performance of lay-on system, however in poor quality system, tile weight becomes an important factor where heavier tiles considerably decrease the performance level. Perimeter clips marginally increase the dynamic performance of lay-on ceiling system, but it has no effect on the clip-in ceiling system under the shaking levels considered.

Numerical parametric analysis on the ultimate bearing capacity of the purlin-sheet roofs connected by standing seam clips

  • This paper presents the parametric numerical analysis on the ultimate bearing capacity of the purlin-sheet roofs connected by standing seam clips. The effects of several factors on failure modes and ultimate bearing capacity of the purlins are studied, including setup of anti-sag bar, purlin type, sheet thickness and connection type et al. A simplified design formula is proposed for predicting the ultimate bearing capacity of purlins. Results show that setting the anti-sag bars can improve the ultimate bearing capacity and change the failure modes of C purlins significantly. The failure modes and ultimate bearing capacity of C purlins are significantly different from those of Z purlins, in the purlin-sheet roof connected by standing seam clips. Setting the anti-sag bars near the lower flange is more favorable for increasing the ultimate bearing capacity of purlins. The ultimate bearing capacity of C purlins increases slightly with sheet thickness increasing from 0.6 mm to 0.8 mm. The ultimate bearing capacity of the purlin-sheet roofs connected by standing seam clips is always higher than those by self-drilling screws. The predictions of the proposed design formulas are relatively in good agreement with those of EN 1993-1-3: 2006, compared with GB 50018-2002.

A Study on the Structural Performance of the Building Exterior Panel Using the Moving Clips (이동 클립을 이용한 건축물 외장재의 구조적 성능에 관한 연구)

  • A recent global trend in the increase of earthquake-related disasters has become so frequent as to cause various damages to a wide range of mid- to high-rise buildings. Particularly, more attention is being paid to the effect of horizontal load in high-rise buildings not only on the key structural elements of the structures, but also on the possibility of the secondary damages to them due to the failure of exterior panels, which are non-structural elements, but such damages are difficult to cope with as they may be caused by unexpected changes. The present study examined exterior panels using moving clips to prevent such secondary damages on the non-structural elements and analyzed the structural performance of these exterior panels through the finite element analysis and the shaking table test. The analysis results showed that the exterior panels using moving clips satisfied the structural performance against the allowable story drift of KBC2009 and the safety of the exterior panels was verified by the shake table test.

Perceived Substitutability between Video Clips of Naver TV Cast and Regular TV program (네이버TV캐스트 클립영상의 TV 방송프로그램에 대한 인지된 대체가능성)

  • This study aims to figure out perceived substitutability between video clips distributed on Naver TV cast and regular TV program. An online survey was conducted for a week on October 2017, and we collected several responses, such as viewing time, viewing motive, the extent of viewing for video clip categories, and perceived substitutability between video clips and regular TV program. The dependent variable is Naver TV cast users' perceived substitutability between video clips and regular TV program, and the independent variables are viewing time, viewing motive and viewing degree by video clip categories. With these variables, hierarchical regression was conducted. The result shows that viewing time, use motives, including personal relations, booming issue and selective use, and the extent of web-based video content viewing positively affect on Naver TV cast users' perceived substitutability.

  • Structural design, generally engineering design, is a complex process combinding design knowledges and analysis techniques. While analysis techniques can be automated in an algorithmic fashion, relatively little work has been done in the area of the design automation. An effect approach method for the automation of the engineering design may be a hybrid system, in which design knowledges, specification requirements and interpretations are represented using an expert system methodology and numerically intensive operations of the design process are implemented using an algorithmic language such as FORTRAN. The purpose of this paper is concentrated on the knowledge of K-CLIPS(KAIST-C Language Integrated Production System) used to design and implement this hybrid system. In K-CLIPS, some representation methods : frame, production rule, fact and user defined function are used to construct the knowledge base. The hierarchical knowledges could be expressed more naturally with a little number of rules than other plain production systems.

Virtual Reality to Help Relieve Travel Anxiety

  • This study presents empirical evidence of the benefit of viewing narrative video clips on embedded virtual reality (VR) websites of hotels to relieve travel anxiety. As the effectiveness of using VR functions to relieve travel anxiety has been shown, we proposed that a VR website enhanced with narrative video clips could relieve travelers' anxiety about accommodations by showing the important aspects of a hotel. Thus, we created a website with a narrative video showing the escape route from a hotel room and another narrative video showing the surrounding neighborhood. We then conducted experiments by having human subjects explore the enhanced VR website and fill out a questionnaire. The results confirmed our hypothesis that there is a statistically significant relationship between relief from travel anxiety and the use of narrative videos on embedded VR websites of hotels.
