인면역결핍바이러스 감염자에서 ICD-p24 항원 탐지가 CD4+T 세포수 및 예후에 미치는 영향 (The Detection of ICD p24 Antigen Predicts Bad Prognosis in HIV-1 Infected Patients)

  • In order to evaluate the effect of viral load on the prognosis of human immunodeficiency virus-1 (HIV-1)-infected individuals, immune complex dissociated (ICD) serum p24 antigen (p24) by acid treatment was retrospectively measured for 50 HIV-infected patients for 60 months. Among them, 27 patients were p24 positive (p24+) above 25pg/ml for $40.4{\pm}12$ months and 23 patients were negative (p24-). Follow-up periods from HIV diagnosis were $63.0{\pm}19$ months (range; 40-112) for the p24+ and $68.4{\pm}19$ months (range; 38-106) for the p24-, respectively (P>0.05)Mean CD4+T cell counts in the p24+ group decreased from $473{\pm}$277/ul (median;373) to $157{\pm}150/ul$ (median; 111) for $60{\pm}16$ months (5.3/month P280/ul (median; 476) to $432{\pm}285/ul$ (median;382) for $63{\pm}19$ months (2.5/month, P<0.01). From CD4+T cell count >200/ul, the patient who progressed to AIDS of <200/ul were 13 of 23 (56%) in the p24+ and 4 of 22 (18%) in p24-, respectively (p<0.01). And the number of death in two groups were 6 (22%) and 1 (4%), respectively (p<0.01). Presumed survival in two groups were about 12 and 24.5 years. These data suggest that viral load itself be very important for the prognosis of HIV-infected patients.

A Case of Canine Colorectal Carcinoma In Situ with Regulatory T Cell Infiltration

  • An adult castrated male dog was presented with persistent hematochezia. Digital rectal examination and endoscopy found multiple colorectal masses. Complete blood count and serum biochemical results were within the reference interval. Fine needle aspirate of the masses indicated a diagnosis of inflamed polyps with a primary differential of malignancy. Histopathologic examination using endoscopy-guided incisional biopsy of the masses revealed an inflamed neoplasm with ossification. A colectomy was performed to remove the tumor. Subsequent histopathologic examination of the surgically resected masses resulted in a diagnosis of colorectal carcinoma in situ (CiS) with immune infiltrates, which were subject to immunohistochemical and flow cytometric immunophenotyping. The immunohistochemistry confirmed intraepithelial CD3+ T cells within CiS. The flow cytometric analysis indicated tumor-infiltrating CD4+ T, CD8+ T, and CD11b+ myeloid subsets. The flow cytometric analysis of circulating and tumor-infiltrating leukocytes demonstrated a preferential expansion of CD25+FOXP3+ regulatory T cells within CiS. To the author's knowledge, this is the first report to show clinical evidence emphasizing the immunogenicity and immune-suppressive environment of canine colorectal CiS. Our case will be valuable in providing a rationale for basic research that dissects the immune environment for canine colorectal cancers for the future development of cancer immunotherapy.

CD4+, IL17 and Foxp3 Expression in Different pTNM Stages of Operable Non-small Cell Lung Cancer and Effects on Disease Prognosis

  • Objective: To investigate the effects of $CD4^+$, IL17 and Foxp3 expression on prognosis of operable non-small cell lung cancer (NSCLC) with different pTNM stages. Methods: Expression of $CD4^+$, IL17 and Foxp3 in 102 cases of NSCLC tissues and adjacent cancer tissues was detected by immunohistochemistry and associations with prognosis with different pTNM stages were analyzed. The Chi-square test was used to compare count data. Survival differences were evaluated by Kaplan-Meier single factor analysis and the COX regression model was used to analyze the relationship between influential factors and the disease prognosis. The significance level was ${\alpha}$=0.05. Results: Expression of CD4, IL-17 and Foxp3 significantly varied in different pTNM stages of NSCLC tissues (P < 0.05). The same was true for CD4 expression (P < 0.05). The median survival time (MST) in the positive CD4 expression group was evidently higher than that in the negative group (25.8/23.9 months). Compared with stage III, the MST difference of stages I and II in the positive CD4 expression group were statistically significant (P < 0.05). The MST in positive IL-17 and Foxp3 expression groups was obviously lower than that in the corresponding negative group (P < 0.05) (25.6/35.1 months and 24/35.3 months, respectively). There was a significant difference of MST between any two of three stages of positive IL-17 expression group (P < 0.05), and it was the same with positive Foxp3 expression group. TNM stage, negative CD4 expression, and positive IL-17 and Foxp3 expression were the main risk factors for the prognosis of NSCLC. Conclusion: Surgical prognosis of NSCLC can be better assessed by the combination of clinical staging and expression of IL17 and Foxp3.

  • As we enter the new millennium, there have been dramatic improvements in the care of patients with HIV infection. These have prolonged life and decreased morbidity and mortality. There are fourteen currently available antiretrovirals approved in the United States for the treatment of this infection. The medications, including their pharmacokinetic properties, side effects, and dosing are reviewed. In addition, the current approach to the use of these medicines is discussed.

인삼사포닌이 카드뮴의 면역독성에 미치는 영향 (The Effect of Ginseng-Saponin on Cd-Induced)

  • The purpose of this study is to investigate the effects of Ginseng saponin on the cadmium which is widely distributed in the environment, results in immune system alteration. For the experiments, 125 mice of ICR strain were used. The experimental groups were divided into 5 groups a control, a cadmium alone treatment group, three Cd and saponin (10, 50, 100 mg/kg) combined treatment groups. The mice were allocated 25 to each group and observed for 8 weeks. The results of experiment are as follows: 1. Body weight growth rates during 8 weeks were as this control group 36.47%, Cd alone group 32.48%, saponin combined treatment group (10, 50, 100 mg/kg) 32.49%, 39.17%, 24.27% respectively. 2. In all groups, the relative weights of liver and kidney were increased, compared with control group. In the case of spleen, saponin combined treatment group (50, 100 mg/kg) was high to the significant level compared with a control group (p<0.05). Thymus was not. 3. On blood lymphocyte count observation, Cd alone treament group has 25.6% less than control group, and saponin combined treatment group have increasing trends. but in thymus and spleen, there was no trends like blood. 4. On antibody titer, there was no difference among groups. 5. On total serum protein, saponin (100 mg/kg) combined treatment group was high to significant level compared with control group (p<0.05), and other treatment groups have increasing trends. 6. Cd accumulation in kidney was higher than in liver, and all treatment groups were high to the very significant level compared with the control group (p<0.05), but there was no difference among groups. From the results of this study, it can be concluded that the oral administration of Cd results in alteration of immune system and Ginseng saponin prevents this effect. But, Cd accumulation was not affected by saponin.

Effect of fractional ablative carbon dioxide laser with lidocaine spray on skin flap survival in rats

  • Background: Lidocaine spray is a local anesthetic that improves random-pattern skin flap survival. The fractional ablative carbon dioxide laser (FxCL) produces vertical microchannels that delivers topically applied drugs to the skin. In this study, we hypothesized that FxCL therapy would enhance the lidocaine effect to improve random-pattern skin flap survival in rats. Methods: McFarlane random-pattern skin flaps were elevated in 48 rats, which were divided into four groups according to treatment: FxCL+lidocaine, FxCL, lidocaine, and nontreatment (control). On postoperative day 7, necrotic flap areas, the number of capillary vessels, and neutrophil count were evaluated. Anti-rat vascular endothelial growth factor (VEGF) and CD31 antibody activity were also evaluated by immunohistochemical staining. Results: Flap survival rate was $53.41%{\pm}5.43%$, $58.16%{\pm}4.80%$, $57.08%{\pm}5.91%$, and $69.08%{\pm}3.20%$ in the control, lidocaine, FxCL, and FxCL+lidocaine groups, respectively. Mean neutrophil count in the intermediate zone excluding the necrotic tissue was $41.70{\pm}8.40$, $35.43{\pm}6.41$, $37.23{\pm}7.15$, and $27.20{\pm}4.24cells/field$ in the control, lidocaine, FxCL, and FxCL+lidocaine groups, respectively. Anti-rat VEGF and CD31 antibody activity were the highest in the FxCL+lidocaine group. Conclusion: FxCL with lidocaine had a positive effect on random-pattern skin flap survival in rats. Thus, FxCL with lidocaine spray should be considered as a new treatment option to improve flap viability.

Association of CD4+CD25+FoxP3+ regulatory T cells with natural course of childhood chronic immune thrombocytopenic purpura

  • Purpose: The purpose of this study was to determine the frequency of $CD4^+CD25^+FoxP3^+$ regulatory T cells (Treg) in the peripheral blood of patients with childhood chronic immune thrombocytopenic purpura (ITP) exhibiting thrombocytopenia and spontaneous remission. The findings of this study indicate the possibility of predicting spontaneous recovery and pathogenesis of childhood chronic ITP. Methods: Eleven children with chronic ITP (seven thrombocytopenic and four spontaneous remission cases; mean age, 8.8 years; range, 1.7-14.9 years) were enrolled in this study. Five healthy children and eight healthy adults were included as controls. The frequency of Treg was evaluated by flow cytometry in the peripheral blood. Results: In this study, four patients (36%) achieved spontaneous remission within 2.8 years (mean year; range, 1.0-4.4 years). The frequency of Treg was significantly lower in patients with persisting thrombocytopenia ($0.13%{\pm}0.09%$, P<0.05), than that in the patients with spontaneous remission ($0.30%{\pm}0.02%$), healthy adults controls ($0.55%{\pm}0.44%$), and healthy children controls ($0.46%{\pm}0.26%$). A significantly positive correlation was found between the frequency of Treg and the platelet count in children. Conclusion: These data suggest that a lower frequency of Treg contributes to the breakdown of self-tolerance, and may form the basis for future development of specific immunomodulatory therapies. Furthermore, Treg frequency has prognostic implication toward the natural course and long-term outcomes of childhood chronic ITP.

항생제 대체제로서 생균제가 이유자돈의 성장능력 및 영양소 이용률, 설사 빈도, 면역 반응에 미치는 영향 (Evaluation of Probiotics as an Alternative to Antibiotic on Growth Performance, Nutrient Digestibility, Occurrence of Diarrhea and Immune Response in Weaning Pigs)

  • 본 실험은 이유자돈 120두를 이용하여 생균제의 첨가가 이유자돈의 성장 능력, 영양소 소화율, 혈중 요소태 질소, 설사 빈도, 면역 반응에 미치는 영향에 대해 조사하였다. 이유자돈 사료에 항생제 대체제인 생균제의 첨가는 전 사양 기간에 걸쳐 사양 성적이 항생제 첨가구(PC)에 비해 유의적으로 낮게 나타났다. 항생제 첨가구(PC)를 제외하고 비교하였을 때에는 비록 유의적인 차이는 발생하지 않았지만 생균제 첨가구가 항생제 무첨가구(NC)에 비해 일당증체량, 사료효율에서 높은 경향을 보였으며, 이는 항생제 첨가 효과에는 미치지 못하지만 항생제 대체제로서 그 가능성을 보인 것이라고 하겠다. 특히 생균제 A 첨가구는 0~5 주차 사료 효율(P<0.05)에서 항생제 첨가구(PC)와 비슷한 성적을 나타내어, 항생제 대체제로서 이유자돈의 성장을 개선 시킬 가능성이 가장 높다고 하겠다. 본 실험에서는 5주동안 이유자돈의 사료에 생균제를 첨가하여 사양성적을 보았는데, 생균제의 급여기간을 좀 더 늘리면 생균제의 처리효과가 더욱 뚜렷하게 나타날 수도 있으리라 생각된다. 이유자돈 사료에 생균제의 첨가가 건물, 조단백질, 조지방, 조회분 소화율에 어떠한 영향도 미치지 않았으며(P>0.10), 질소 축적율은 생균제 첨가구에서 항생제 첨가구(PC)에 비해 높게 나타났으나 일관된 효과를 보이지는 않았다. 혈중 요소태 질소 농도에서는 2주차와 4주차에서 유의적인 차이가 나타났다(P<0.05). 또한 생균제 B 첨가구가 다른 처리구에 비해 혈중 요소태 질소 농도가 가장 낮게 나타났다. 설사 발생 빈도 측정 결과, 항생제 첨가구(PC)에서 설사 발생 빈도가 유의적으로 낮게 나타났으며, 생균제 C 첨가구에서 다음으로 낮게 나타났다(P=0.18). 면역 반응 실험 결과, $CD^{4+}$ T-cell 수의 경우 생균제 C 첨가구에서 다른 처리구에 비해 가장 낮게 나타났으며(P<0.01), $CD^{8+}$ T-cell 수와 $CD^{4+}:CD^{8+}$ 비율의 경우에는 모든 처리구에서 유의적인 차이가 발생하지 않았다(P>0.10). 결론적으로 이유자돈사료 내 생균제의 첨가가 이유자돈의 성장 능력, 질소축적율, 혈중 요소태 질소, 설사 빈도, 및 면역 능력에 일정한 개선 효과를 갖고 있지만, 항생제 첨가 효과에는 미치지 못한다고 하겠다. 본 실험을 통해 이유자돈 사료에 생균제의 첨가는 항생제 대체제로서 일정 부분 개선효과가 있는 것으로 사료된다.

Common 단자를 이용한 3상 Spindle Motor 안정제어 (A 3-Phase Spindle Motor Stable Control by Using Common Terminal)

  • 80년대 초에 개발된 CD(Compact Disc) Disc는 지난 27여 년간 급속한 발전을 하였다. 95년에 DVD가 발표되면서 영화 Title제작이 되어 우리는 안방에서 고화질의 DVD를 감상할 수 있었다. CD-R(Recordable) 로부터 시작된 기록 기술은 CD-RW를 거쳐 DVD-Recordable, DVD-ReWritable 및 DVD-RAM, Light Scribe등 거쳐 BD(Blu-Ray) 기술에 까지 이르게 되었다. 광 Disc 기술은 3가지 분야로 나눌 수 있다. 안정된 제어를 위한 Servo 기술, Mechanical 기술, PC와communication 할 수 있는 Interface 기술 등으로 나눌 수 있다. Servo 제어 기술은 다음과 같이 4가지 분야로 구성되어 있다. Focus Servo, Tracking Servo, Sled Servo, Spindle Servo. Disc 회전을 제어를 담당하고 있는 Spindle Servo Block에서는 Hall Sensor를 이용하여 Disc의 회전을 Count하여 Feedback 하였다. 그러나 갈수록 심화 되고 있는 가격 경쟁 상황에서 Hall Sensor를 대체하여 회전수를 Conut하는 방법이 필요 하였다. BEMF(역기전력)을 이용한 Zero Crossing 방법이 이를 대체 하였지만, Motor의 전기적인 특성 편차 등에 안정된 control이 안되어서, Motor를 관리하는 방법으로 진행되어 왔다. 이것은 생산 Loss로 이어져서 Hall Sensorless의 장점을 훼손하게 되었다..본 논문에서는 이러한 Sensorless Type Spindle Motor의 문제점에 대해 Common 단자를 이용하여 안정된 제어 방법을 제안 한다. less Type의 문제점인 초기 기동 문제를 Common 단자를 이용하여 안정되게 Spindle motor를 Control 하는 방법에 대해 연구 하고자 한다.

Ten years of experience in the prevention of mother-to-child human immunodeficiency virus transmission in a university teaching hospital

  • Purpose: Administration of antiretroviral drugs to mothers and infants significantly decreases mother-to-child human immunodeficiency virus (HIV) transmission; cesarean sections and discouraging breastfeeding further decreases this risk. The present study confirmed the HIV status of babies born to mothers infected with HIV and describes the characteristics of babies and mothers who received preventive treatment. Methods: This study retrospectively analyzed medical records of nine infants and their mothers positive for HIV who gave birth at Korea University Ansan Hospital, between June 1, 2003, and May 31, 2013. Maternal parameters, including HIV diagnosis date, CD4+ count, and HIV ribonucleic acid (RNA) copy number, were analyzed. Infant growth and development, HIV RNA copy number, and HIV antigen/antibody test results were analyzed. Results: Eight HIV-positive mothers delivered nine babies; all the infants received antiretroviral therapy. Three (37.5%) and five mothers (62.5%) were administered single- and multidrug therapy, respectively. Intravenous zidovudine was administered to four infants (50%) at birth. Breastfeeding was discouraged for all the infants. All the infants were negative for HIV, although two were lost to follow-up. Third trimester maternal viral copy numbers were less than 1,000 copies/mL with a median CD4+ count of $325{\mu}L$ ($92-729{\mu}L$). Among the nine infants, two were preterm (22.2%) and three had low birth weights (33.3%). Conclusion: This study concludes that prophylactic antiretroviral therapy, scheduled cesarean section, and prohibition of breastfeeding considerably decrease mother-to-child HIV transmission. Because the number of infants infected via mother-to-child transmission may be increasing, studies in additional regions using more variables are necessary.
