Clinical Application of Serum Tumor Associated Material (TAM) from Non-small Cell Lung Cancer Patients

  • Objective: To explore the associations of serum tumor associated material (TAM) with other common tumor markers like carcinoembryonic antigen (CEA), carbohydrate antigen 125 (CA125), carbohydrate antigen19-9 (CA19-9) and its clinical application in non-small cell lung cancer (NSCLC) patients. Methods: A total of 87 patients were enrolled into this study, all with histologically or cytologically confirmed NSCLC. With the method of chemical colorimetry, the level of TAM was determined and compared, while chemiluminescence was used to measure the levels of common tumor markers. Results: The level of TAM decreased after chemotherapy compared with before chemotherapy when CT or MRI scans showed disease control. Furthermore, it increased when disease progessed and there was no statistically significant difference in monitoring of TAM and common tumor markers (P>0.05). Conclusions: Detecting TAM in NSCLC patients has a higher sensitivity and specificity, so it can be used as an indicator for clinical monitoring of lung cancer chemotherapy.

한방치료와 XELOX 요법을 병행하여 호전된 간, 폐 전이 대장암 환자 1례 (A Case of Hepatic and Pulmonary Metastatic Colorectal Cancer Patient Treated by Traditional Korean Therapy and XELOX Chemotherapy)

  • Objective : This report is aimed to investigate the effectiveness of traditional korean therapy including intravenous Cultivated Wild Ginseng Pharmacopuncture (CWGP) and Soram immunopharmacopuncture with XELOX chemotherapy in treating metastatic colorectal cancer patient. Methods : A 47-year-old woman who was diagnosed as metastatic colorectal cancer on Oct 2011 was concurrently treated with traditional Korean therapy (TKT) and XELOX (capecitabine plus oxaliplatin) for 7 months. TKT includes intravenous CWGP, Soram immuno-pharmacopuncture, acupuncture, moxibustion, and herbal medicine. The effectiveness of therapies was evaluated with computed tomography and tumor marker levels such as carcinoembryonic antigen (CEA) and carbohydrate antigen 19-9 (CA19-9). And pain on the lateral abdomen was recorded with Visual Analogue Scale (VAS). Results : The tumor mass size of metastatic liver was decreased from 10 cm to 4.3 cm. The tumor marker levels such as CEA and CA19-9 are also decreased. From these results, this case report suggests that the TKT with palliative chemotherapy may be a useful method to treat unresectable metastatic colorectal cancer.

Comparison of Serum Tumor Associated Material (TAM) with Conventional Biomarkers in Cancer Patients

  • Objective: To compare expression level of serum tumor associated materials (TAM) with several conventional serum tumor biomarkers, eg., carcinoembryonic antigen (CEA), carbohydrate antigen19-9 (CA19-9), carbohydrate antigen 15-3 (CA15-3), alpha-fetoprotein(AFP), in selected solid tumors. Methods: Patients diagnosed histologically or cytologically with liver, breast, esophageal, gastric, colorectal or pancreatic cancers were enrolled into this study. After diagnosis, the level of TAM was determined by chemical colorimetry, and levels of conventional tumor markers was measured by chemiluminescence methods. Results: A total of 560 patients were enrolled into this study. No statistically significant difference was detected in TAM and the above mentioned tumor biomarkers in terms of their positivity and negativity ( P>0. 05). Conclusions: Detection of TAM in liver, breast, esophageal, gastric, colorectal, and pancreatic cancer patients demonstrates a good accordance with CEA, CA199, CA153, and AFP, thus suggesting that further study is warranted to verify whether TAM could be a surrogate for these conventional biomarkers.

Survival benefit of neoadjuvant FOLFIRINOX for patients with borderline resectable pancreatic cancer

  • Backgrounds/Aims: While patients with borderline resectable pancreatic cancer (BRPC) are a target population for neoadjuvant chemotherapy (NAC), formal guidelines for neoadjuvant therapy are lacking. We assessed the perioperative and oncological outcomes in patients with BRPC undergoing NAC with FOLFIRINOX for patients undergoing upfront surgery (US). Methods: The AHPBA criteria for borderline resectability and/or a CA19-9 level > 100 µ/mL defined borderline resectable tumors retrieved from a prospectively populated institutional registry from 2007 to 2020. The primary outcome was overall survival (OS) at 1 and 3 years. A Cox Proportional Hazard model based on intention to treat was used. A receiver-operator characteristics (ROC) curve was constructed to assess the discriminatory capability of the use of CA19-9 > 100 µ/mL to predict resectability and mortality. Results: Forty BRPC patients underwent NAC, while 46 underwent US. The median OS with NAC was 19.8 months (interquartile range [IQR], 10.3-44.24) vs. 10.6 months (IQR, 6.37-17.6) with US. At 1 year, 70% of the NAC group and 41.3% of the US group survived (p = 0.008). At 3 years, 42.5 % of the NAC group and 10.9% of the US group survived (p = 0.001). NAC significantly reduced the hazard of death (adjusted hazard ratio, 0.20; 95% confidence interval, 0.07-0.54; p = 0.001). CA19-9 > 100 µ/mL showed poor discrimination in predicting mortality, but was a moderate predictor of resectability. Conclusions: We found a survival benefit of NAC with FOLFIRINOX for BRPC. Greater pre-treatment of CA19-9 and multivessel involvement on initial imaging were associated with progression of the disease following NAC.

  • 목적: 대장암 재발의 조기진단은 생존율을 증가시키는 중요한 인자이다. 대장암의 재발이나 전이를 진단하는데 있어 혈청 CEA, CA 19-9 등의 종양표지 자와 CT 등의 가장 많이 이용되어 왔고 최근 새로운 진단기술로서 PET/CT의 이용이 증가되고 있다. 이 연구에서는 근치적 수술을 포함한 치료를 시행하고 재발이나 전이의 증거가 없었던 상태에서 추적관찰을 받았던 대장암 환자에서 FDG PET/CT의 정기 추적 검사로서의 진단 성능을 혈청 CEA, CA 19-9, CT와 비교하고자 시행하였다 대상 및 방법: 수술적 치료 후 추적관찰 중인 대장암 환자 189명을 대상으로 하였다. PET/CT상에서 관찰된 비정상적인 FDG 섭취 증가 병소는 생검이나 6개월 이상 방사선학적 검사의 추적관찰로서 전이여부 판단하였다. PET/CT시행시기에서 6주 이내에 모든 환자에서 혈청 CEA, 174명의 환자에서 CA 19-9를 측정하였다. 115명의 환자에서 PET/CT 시행 시기 2개월 이내에 CT를 시행하였다. PET/CT 상에서 관찰된 비정상적인 FDG 셥취 증가 병소는 생검이나 6개월 이내의 방사선학적 검사와 임상적 진단의 추적 관찰로 재발이나 전이 병소로 진단하였다. 결과: 국소적 재발 및 전이가 발견된 환자는 76명(40.2%)이었다. 재발 또는 전이 병소는 총 81개였고 간과 폐에서 29개와 16개로 가장 많이 나타났다. PET/CT의 민감도, 특이도, 양성예측도, 음성예측도는 94.7%, 91.1%, 87.8%, 96.2%이고 혈청 CEA 치수는 44.7%, 97.3%, 91.8%, 72.3%였다. 종양표지 자와 PET/CT와의 비교에서는 PET/CT가 종양표지 자보다 민감도(94.2%, 52.1%)와 특이도(90.4%, 88.5%)가 높았다. 종양표지자와 CT를 종합하여 PET/CT와 비교하였을 때 민감도는 92.9%로 동일하였지만 특이도는 PET/CT가 91.3%로 높았다. 결론: 이러한 결과로 보아 FDG PET/CT가 근치적 절제술 후 대장암의 정기적 추적 관찰에도 유용하게 이용될 수 있을 것이다.

IMACIS-1을 이용한 위장관 종양의 방사면역신티그램 (Radioimmunoscintigraphy Using IMACIS-1 in Gastrointestinal Cancer)

  • Most of the diagnostic methods currently used for the detection of neoplastic masses provide indirect evidence. To obtain greater specificity in the interpretation of neoplasias by in vivo methods, the immunological approach appears to be most promising. Two problems that interfered with progress in this field were the lack of tumor specific antigen and the lack of well-defined and reproducible antibodies. To improve the sensitivity and specificity of radioimmunoscintigraphy as a technique for tumor localization, the use of monoclonal antibodies, fragments of antibodies and single photon emission computerized tomography (SPECT) are reasonable. The obvious advantages of monoclonal antibodies are their homogeneity, their specificity for the immunizing antigen and the reaction with a single determinant-thus no large immunecomplexes with antigen are formed. Monoclonal antibody technique has recently provided an opportunity to reevaluate the role of nuclear medicine for the diagnosis of malignant diseases by using the immunological approach. Out first results by means of radioimmunoscintigraphy of CEA and CA 19-9 producing tumors using a cocktail of fragments F $(ab')_2$, of mocolonal antibodies to CA 19-9 and CEA labeled with $^{131}I$ (IMACIS-1) are reported. The aims of this investigation was to evaluate the role of immunoscintigraphy in patients with colorectal and other cancers for diagnosis of local recurrences and metastasis. This report contains results of the first 8 colorectal and pancreas cancer patients with the elevation of the level of serum CEA and/or CA 19-9. IMACIS-1 was injected intravenously during 30 minutes in 100 ml saline solution after skin test. Planar scintigrams were recorded 3, 5 and 7 days after the injection of the IMACIS-1. Anterior, lateral and posterior views of the liver as well as anterior and posterior views of the pelvis were obtained in each patients as an $^{131}I-antibody$ image. We were able to localize exactly the malignant process with the double-nuclide double-compound $^{99m}Tc\;^{131}I$ (Tc+l) scintigrams. In Tc & I double-nuclide scintigraphy, computer subtraction display provided more clear localization of the tumor. We compared the results of radioimmunoscintigraphy with CT, ultrasonograms, conventional scintigrams. The results were as follows: 1) The sensitivity and specificity of radioimmunoscintigraphy using the fragments $F(ab')_2$ of the cocktails of CEA and CA 19-9 monoclonal antibodies were 80% and 100% respectively. 2) Tumor detection rate was not proportionated to the level of serum tumor markets. 3) Second tracer technique was essential for tumor localization as an anatomic landmark using double-nuclide scintigraphy. 4) A slow infusion of the antibodies was necessary to prevent the formation of large immune complexes. 5) Tumor/non-tumor radioactivity was most elevated at 7 days delayed imaging. 6) Using planar scintigraphic technique of $^{131}I$ labeled monoclonal antibodies are possible for imaging most of the tumors.

Serum Carbohydrate Antigen 19-9 as an Indicator of Liver Metastasis in Colorectal Carcinoma Cases

  • Purpose: The liver is the organ to which colorectal carcinomas (CRCs) most commonly metastasize, and surgical resection has been established as the most effective and potentially curative treatment for CRC with liver metastasis (LM). Therefore, surveillance of LM is vital for improvement of prognosis of CRC patients. In this study, we aimed to explore the potential value of carbohydrate antigen 19-9 (CA 19-9), carcinoembryonic antigen (CEA), and marker enzymes in indicating LM with CRC. Methods: Three groups of eligible patients with metastatic cancers were retrospectively included: CRC patients with LM (CRC-LM) or without LM (CRC-NLM), and non-CRC patients with LM (NCRC-LM). All metastatic lesions were identified by CT or MRI. Data on characteristics of the patients, the primary site, the locations of metastasis, CA 19-9, CEA, and biochemical parameters were collected for analysis. Results: A total of 493 patients were retrospectively included. More alcohol consumption was found in CRC-LM than CRC-NLM. Some biochemical enzymes were found to be significantly higher in groups with LM than without (CRC-LM or NCRC-LM v.s CRC-NLM). Both CEA and CA 19-9 were much higher in CRC-LM than CRC-NLM or NCRC-LM. For CRC patients, CA 19-9, ${\gamma}$-glutamyl transpeptidase, CEA and alcohol consumption were identified as independent factors associated with LM. Conclusion: Our analysis suggested the CA 19-9 might be a potential valuable indicator for LM of CRC in the clinic.

전이 췌장암 환자의 Gemcitabine과 방암탕 가감방 및 건칠단 병용투여 치험 1례 (A Case Report of Patient with Metastatic Pancreatic Cancer Treated with modified Bangam-tang and Gunchil-dan in conjunction with Gemcitabine)

  • Objectives: This study was aimed to report a patient with metastatic pancreatic cancer treated with modified Bangam-tang and Gunchil-dan in conjunction with gemcitabine. There were better survival-related outcomes compared to gemcitabine alone. Methods: The patient with metastatic pancreatic cancer received gemcitabine as palliative chemotherapy since June 2016 concurrent with modified Bangam-tang and Gunchil-dan since October 2016 to October 2017. To evaluate the effect of treatment, tumor markers (carbohydrate antigen 19-9 (CA19-9) and carcinoembryonic antigen (CEA)), Response Evaluation Criteria in Solid Tumors (RECIST) version 1.1, and overall survival were checked. Adverse events were evaluated according to the Common Terminology Criteria for Adverse Events (CTCAE) version 5.0. Results: After 12 months with the combination treatment, levels of CA19-9 were decreased from 8747 to 265.7 ng/ml and CEA from 42.2 to 6.5 U/ml. Clinical partial response state was shown until May 2, 2017 and stable disease state was maintained from August 4, 2017. In March 2018, the patient got an operation including pancreatectomy and diagnosed with no evidence of disease state in September, 2018. In conclusion, it showed the overall survival of 29 months from June, 2016 to November, 2018. Serious adverse events were not identified. Conclusions: This study suggested that combined treatment with modified Bangam-tang and Gunchil-dan may show better outcome in patient with metastatic pancreatic cancer than gemcitabine alone.

구강 편평세포암종 환자의 혈청 종양표지자의 유용성 (EFFICIENCY OF SERUM TUMOR MARKERS ORAL SQUAMOUS CELL CARCINOMA PATIENTS)

  • Purpose: Recently, the role of serum tumor marker has been studied for an important issue on diagnosing and treating tumors in the head and neck region because tests using tumor markers need relatively simple procedures and are acceptable to patients, compared with other test methods. Tumor marker tests were performed on patients with squamous cell carcinoma, which were known to have the highest prevalence among tumors in the head and neck region. Association between each tumor marker, and diagnosis and prognosis of tumors was assessed. Materials and methods: Tumor marker tests were carried out on 31 patients who visited Oral and Maxillofacial Surgery Department in Dankook University Dental Hospital between January 2003 and August 2008 and who were diagnosed as primary oral squamous cell carcinoma through out histopathologic diagnosis. Blood sample from these patients was performed to measure tumor markers using nuclear medicine diagnostic equipment. Measured entries were as follows: PSA(prostate-specific antibody), SCCAg( Squamous Cell Carcinoma Related Antigen), CA 19-9(Cancer Antigen 19-9), Ferritin, $\alpha$- FP(Alpha-Fetoprotein), Cyfra 21-1, CA125 (Cancer Antigen 125) and p53. Results: Analyses on each tumor marker indicated that squamous cell carcinoma in the head and neck region had statistically significant correlation with p53, SCC-Ag(TA-4), Cyfra 21-1 and Ferritin. p53 demonstrated the highest sensitivity. Especially, 4 cases among 18 cases which Ferritin was measured exhibited metastasis. In all those 4 cases, Ferritin values were higher than the standards (15 - 332ng/ml). Therefore, Ferritin is considered to have a close relation with metastasis of squamous cell carcinoma. Conclusion: This study shows that tumor marker tests are more useful in evaluating progression and prognosis of tumors rather than in diagnosing them. Particularly, serum Ferritin is considered to be beneficial in assessing metastasis of squamous cell carcinoma in the head and neck region and in developing treatment plans based on the assessment.

수술과 항암치료 후 종양표지자 검사의 상승으로 복막 가성점액종의 재발을 확인한 1례 (A Case of Recurred Pseudomyxoma Peritonei Identified by Serially Elevated Serum CEA Levels after Cytoreductive Surgery and Intraperitoneal Chemotherapy)

  • 복막 가성점액종은 표준치료인 종양감축수술과 함께 복강 내 고온열 항암요법과의 병합치료로 생존율의 향상을 기대할 수 있다. 재발성 병변에 대해서는 추가적인 종양감축 수술이 권고되고 있지만, 치료시기에 따른 이환율과 사망률의 차이가 있어 조기발견이 중요하다. 수술 전 혈청 종양표지자 검사의 수치 이상 여부에 따른 재발과 생존율의 상관관계를 알아보고자 하는 연구들은 있지만, 추적관찰 중 수치 이상에 따른 재발여부에 관한 연구는 거의 없다. 저자들은 CSR과 HIPEC, 그리고 전신 항암화학요법을 시행받은 환자에서 CEA의 수치 상승이 질병의 재발과 상관관계가 있었음을 경험하였기에 보고하는 바이다.
