Spatial Dose Distribution for C-arm Examination within Operation Room Using Monte Carlo Method (몬테카를로 방법을 이용한 이동형 X선 투시검사 시 수술실 내 공간선량평가)

  • The purpose of this study was to analyze the spatial dose according to the distance by location of medical workers when using a mobile X-ray fluoroscopy device in the operating room through a simulation experiment. The MCNPX program was used for the simulation, and the location of medical workers was set around the operating table, and the spatial dose distribution according to the distance and changes in imaging conditions was evaluated. As a result, The highest score was 2.74×10-4 mGy, 2.72×10-4 mGy, and 1.18×10-4 mGy based on the 10 cm distance from the operating table. Spatial dose depending on the distance 100cm, A point 5.15×10-5 mGy is decreased 19% of 10cm, D point 5.12×10-5 mGy, 19 % of 10cm, and G pint, 1.73×10-5 mGy is reduced by 15% of 10cm. Based on this study, medical-related workers directly or indirectly participating in surgery carry potential risks of radiation exposure during surgery, but there are difficulties in radiation protection due to the nature of their work. Therefore, efforts to reduce exposure suitable for the operating room environment will be required.

  • During an X-ray examination, the beam of radiation is dispersed in many directions. We believe that managing radiation dose is about providing transparency to users and patients in the accurate investigation and analysis of radiation dose. The purpose of measuring the radiation dose as a function of location is to ensure that medical personnel using the equipment or participating in the operating room are minimally harmed by the different radiation doses depending on their location. Four mobile diagnostic X-ray units were used to analyze the radiation dose depending on the spatial location. The image intensifier and the flat panel detector type that receives the image analyzed the dose by angle to measure the distribution of the exposure dose by location. The radiation equipment used was composed of four units, and measuring devices were installed according to the location. The X-ray (C-arm) was measured by varying the position from 0 to 360 degrees, and the highest dose was measured at the center position based on the abdominal position, and the highest dose was measured at the 90° position for the head position when using the image intensifier equipment. The operator or medical staff can see that the radiation dose varies depending on the position of the diagnostic radiation generator. In the image intensifier and flat panel detector type that accepts images, the dose by angle was analyzed for the distribution of exposed dose by position, and the measurement method should be changed according to the provision of dose information that is different from the dose output from the equipment according to the position.

Learning Curve of C-Arm Cone-beam Computed Tomography Virtual Navigation-Guided Percutaneous Transthoracic Needle Biopsy

  • Objective: To evaluate the learning curve for C-arm cone-beam computed tomography (CBCT) virtual navigation-guided percutaneous transthoracic needle biopsy (PTNB) and to determine the amount of experience needed to develop appropriate skills for this procedure using cumulative summation (CUSUM). Materials and Methods: We retrospectively reviewed 2042 CBCT virtual navigation-guided PTNBs performed by 7 novice operators between March 2011 and December 2014. Learning curves for CBCT virtual navigation-guided PTNB with respect to its diagnostic performance and the occurrence of biopsy-related pneumothorax were analyzed using standard and risk-adjusted CUSUM (RA-CUSUM). Acceptable failure rates were determined as 0.06 for diagnostic failure and 0.25 for PTNB-related pneumothorax. Results: Standard CUSUM indicated that 6 of the 7 operators achieved an acceptable diagnostic failure rate after a median of 105 PTNB procedures (95% confidence interval [CI], 14-240), and 6 of the operators achieved acceptable pneumothorax occurrence rate after a median of 79 PTNB procedures (95% CI, 27-155). RA-CUSUM showed that 93 (95% CI, 39-142) and 80 (95% CI, 38-127) PTNB procedures were required to achieve acceptable diagnostic performance and pneumothorax occurrence, respectively. Conclusion: The novice operators' skills in performing CBCT virtual navigation-guided PTNBs improved with increasing experience over a wide range of learning periods.

Nutrition Status of Elderly Female Patients in Long-term Care Hospital according to Meal Types and Eating Ability (식사형태와 식사능력에 따른 요양병원 여자 노인 환자의 영양섭취조사)

  • Objectives: This study was carried out to determine nutritional status of elderly patients in a long-term care hospital according to meal type and eating ability. Methods: Subjects were 47 female patients aged over 65 ($79.3{\pm}7.1$ years) who resided in a long-term care hospital in Seoul. Thirty seven patients who ate diet orally were grouped according to meal type (27 general diet and 10 soft diet) and eating ability (26 eating by oneself and 11 eaten with help) and 10 were on tube feeding. Nutritional status was determined by food consumption and mid-arm circumference. Results: The mean adequacy ratios (MARs) of 12 nutrients (protein, calcium, phosphorus, zinc, vitamin A, vitamin $B_1$, vitamin $B_2$, vitamin $B_6$, niacin, folic acid, vitamin C) were 0.687 for general diet, 0.565 for soft diet, 0.680 for eating by oneself and 0.677 for eaten with help, which were significantly lower than 0.982 for tube feeding (p < 0.05 or p < 0.01). The patients on tube feeding had significantly lower % arm circumference compared to those who ate general diet (84.0% vs. 95.4%, respectively, p < 0.05). Nutrients intakes, nutrient adequacy ratio (NAR) and index of nutritional quality (INQ) were not different between meal types as well as eating ability. The most insufficiently consumed nutrients by the patients on diet were folic acid, vitamin $B_2$, and calcium (NAR 0.334~0.453, 0.515~ 0.539, and 0.516~0.533, respectively). Conclusions: The results suggested that regardless of meal type or eating ability, the subjects who were on diets in this study might have inadequate intake of folic acid as well as vitamin $B_2$, and calcium, which need to be reflected on menu planning. The measurement of mid-arm circumference presented more risk of malnutrition of patients on tube feeding than those on diets, despite apparently better nutrient consumption.

A Multipurpose Design Framework for Hardware-Software Cosimulation of System-on-Chip (시스템-온-칩의 하드웨어-소프트웨어 통합 시뮬레이션을 위한 다목적 설계 프레임워크)

  • As the complexity of SoC (System-on-Chip) design increases dramatically. traditional system performance analysis and verification methods based on RTL (Register Transfer Level) are no more valid for increasing time-to-market pressure. Therefore a new design methodology is desperately required for system verification in early design stages. and hardware software (HW-SW) cosimulation at TLM (Transaction Level Modeling) level has been researched widely for solving this problem. However, most of HW-SW cosimulators support few restricted ion levels only, which makes it difficult to integrate HW-SW cosimulators with different ion levels. To overcome this difficulty, this paper proposes a multipurpose framework for HW SW cosimulation to provide systematic SoC design flow starting from software application design. It supports various design techniques flexibly for each design step, and various HW-SW cosimulators. Since a platform design is possible independently of ion levels and description languages, it allows us to generate simulation models with various ion levels. We verified the proposed framework to model a commercial SoC platform based on an ARM9 processor. It was also proved that this framework could be used for the performance optimization of an MJPEG example up to 44% successfully.

  • The purpose of the survey was to clarify the clinical features and management of the anorectal malformations (ARM) in Korea. Twenty-seven members of the Korean Association of Pediatric Surgeons were surveyed. The members completed a case registration form for each patient during the two years period of 1996-1997, and a questionnaire that contained their management preferences for primary and complicated patients. Twenty-four members of the 22 institutions registered 295 cases of the ARM, and 27 out of 34 members (77.1 %) responded to the questionnaire. The patients were classified by the Wingspread classification of ARM(1984). The case registration form was a modification of Wingspread workshop for the postoperative assessment and case registration. The male to female ratio was 1.7 : 1. The average number of patients per surgeon was 6.1 cases per year. Prematurity(< 36 weeks) was present in 1.9 % of cases and low birth weight (< 2.6 Kg) in 12.1 %. Among 187 male patients, 62 were high(H) type, 29 intermediate(l) type, 88 low(L), and 8 unclassified. In female, there were 2 cases of cloacal anomalies(C), 4 H type, 30 I type, 66 L type and 6 unclass ified. For male, 87.9 % of H and I type were operated by the Pena procedure, but only 7.9 % in L types. In female, all of C, H and I types, and 40.9 % of L type were done by the Pena procedure. One case expired (MR; 0.003%) as a result of surgical complication. Over-all complication rate was 12.5 %. Among 6 cases of reoperation, 4 were failed Pena procedures. Among 140 colostomies sigmoid colon was utilized in 75.7%, and loop colostomy was commonly used. Ten surgeons prefered primary maturation of the stoma. In conclusion, posterior sagittal anorectoplasty is popular for high types of ARM in this country, but considerable number of patients are still suffering from failed operations. For better understanding and analysis, necessity of prospective study by new classification was discussed.

A Hierarchical Group-Based CAVLC Decoder (계층적 그룹 기반의 CAVLC 복호기)

  • Video compression schemes have been developed and used for many years. Currently, H.264/AVC is the most efficient video coding standard. The H.264/AVC baseline profile adopts CAVLC(Context-Adaptive Variable Length Coding) method as an entropy coding method. CAVLC gives better performance in compression ratios than conventional VLC(Variable Length Coding). However, because CAVLC decoder uses a lot of VLC tables, the CAVLC decoder requires a lot of area in terms of hardware. Conversely, since it must look up the VLC tables, it gives a worse performance in terms of software. In this paper, we propose a new hierarchical grouping method for the VLC tables. We can obtain an index of codes in the reconstructed VLC tables by simple arithmetic operations. In this method, the VLC tables are accessed just once in decoding a symbol. We modeled the proposed algorithm in C language, compiled under ARM ADS1.2 and simulated it with Armulator. Experimental results show that the proposed algorithm reduces execution time by about 80% and 15% compared with the H.264/AVC reference program JM(Joint Model) 10.2 and the arithmetic operation algorithm which is recently proposed, respectively.

Incubation Time Required for Hatching, and Ecological Characteristics of the Mode of Life Related with Total Numbers of the Suckers on Each Short Arm of the Hatched Juvenile Larvae of Octopus ocellatus (Cephallopoda: Octopodidae), in Western Korea

  • The incubation time required for hatching of O. ocellatus were investigated through the processes of egg and embryonic developments by the dissecting microscopic and visual observations. And differences in ecological characteristics of the plankton mode of life or the benthic mode of life according to total numbers of the suckers on each short arm of the hatched juvenile larvae of O. ocellatus were studied by comparisons with other octopodidae species. Compared with the recent a few results reported by other researchers associated with the incubation time required for hatching by female adult mother of O. minor (73-90 days after spawning at $20.9-21.5^{\circ}C$ ranges), in this study, the incubation time required for hatching by female adult mother of O. ocellatus was 56-57 days after spawning at $11.0-20.4^{\circ}C$. Therefore, the incubation time required for hatching by female adult mother varied with Octopodidae species. In this studies, each ovarian egg laid by a female was connected to an egg string attaching to the surface of the wall or bottom of vacunt shell of Rapana venosa. Egg and embryonic developments of this species were studied in the indoor aquaria, in the specific gravity ranging 1.024-1.025. the hatched juvenile of O. ocellatus is 10.3 mm in the mean total length and 4.5 mm in mantle length, and each of its short arms has 18-20 suckers. The just hatched juvenile larvae of O. ocellatus enter the benthic mode of life (benthic larval stage) after hatching. In particular, regarding differences in ecological characteristics of the mode of life according to total numbers of the suckers, O. vulgaris may not need to have many suckers because they enter the planktonic mode of life after hatching, however O. ocellatus may need to have many suckers, because they should adapt to the benthic mode of life. And also the just hatched juvenile larvae of O. minor (bearing many suckers more than O. ocellatus) enter the benthic mode of life (benthic larval stage) after hatching. Therefore, the total number of the suckers on each short arm of the hatched juvenile larvae can be used for determining whether an octopus species has planktonic larval stages or benthic larval stage (benthic mode of life). In particular, The intracohort cannibalism phenomena appeared at the hatched juvenile larval stage because the larval stage of O. ocellatus and O. minor enter into the benthic larval stage in the early stage, unlike entering into the plaktonic larval stage in other Octopus species such as O. vulgaris: at this time, the early hatched larvae fed the late hatched larvae (they are the same species and almost same ages). Therefore, the intracohort cannibalism pheneomena occur in the just hatched juvenile stage of only O. ocellatus and O. minor.

Development of Embedded System Based on Windows CE 5.0 (S3C2410A와 Windows CE 5.0 기반의 임베디드시스템 개발에 관한 연구)

  • In this paper, development of embedded system based on Windows CE 5.0 which released recently is studied. Embedded softwares for the target board using S3C2410A SOC based on ARM920T core are composed of (1) BSP(Board Support Package) contains an OAL(OEM Adaptation Layer) which includes a boot loader for initializing and customizing target hardware, device drivers, and a corresponding set of configuration files (2) Windows CE 5.0 kernel (3) SDK and MP3 test application. Particularly, PB(Platform Builder) provides the efficient functions to build, test and debug the BSP and CE kernel. It is looked forward to being widely spread that Windows CE 5.0 will be utilized at smart devices such as PMP, CNS and DMB phone which inevitably require a display device.

  • In the present study, front lower arm and engine cradle which are automotive chassis parts were developed using the hydroforming technology. For systematic establishment of parts development process, material properties of tube were reflected at the start of design and problems of initial design drawing were solved by forming analysis. Design and manufacturing technology of hydroforming die were established and the relationship between internal pressure and feeding stroke was studied during try-out. Durability and buckling strength of hydrofromed parts were estimated.
