Surgical Resection of Intralobar Pulmonary Sequestration Misconceived of the Lung Abscess -1 Case Report- (폐농양으로 오인한 폐엽내형 폐격리증 수술 치험 1례)

  • Pulmonary sequestration is an uncommon congenital pulmonary malformation characterized by presence of nonfunctioning lung tissue which receives its blood supply mostly from the anomalous systemic arteries - descending thoracic aorta or abdominal aorta, subclavian artery, innominated artery and internal mammary artery, etc. In our country, 39 cases were reported previously. The patient was a 40 years old woman and admitted due to productive cough for 1 year. The chest X-ray and chest C-T showed a dense mass containing a large cavity with air-fluid level and multiple radiolucent cysts in the right lower lung field. On the operative field, we could identify an aberrant large artery [ $\phi$7mm which arose directly from the descending thoracic aorta at eighth thoracic spinal level and fed the sequestrated portion of the right lower lobe. The aberrant artery was double ligation after division. Only sequestrated lobe on the superolateral lesion of the right lower lobe was resected because of nonseparated lobes in all the right lobes. An abnormal vein and bronchiole were ligated with black silk. The patient`s postoperative course was unevenful.

  • Forests potentially contribute to global climate change through their influence on the global carbon (C) cycle. The Kyoto Protocol provides for the involvement of developing countries in an atmospheric greenhouse gas reduction regime under its Clean Development Mechanism (CDM). Carbon credits are gained from reforestation and afforestation activities in developing countries. Bangladesh, a densely populated tropical country in South Asia, has a huge degraded forestland, which can be reforested by CDM projects. To realize the potential of the forestry sector in developing countries like Bangladesh for full-scale emission mitigation, the carbon sequestration potential should be integrated with the carbon trading system under the CDM of the Kyoto Protocol. This paper discusses the prospects of carbon trading in Bangladesh, in relation to the CDM, in the context of global warming.

Estimating Carbon Sequestration by Planting a Leisure-Recreation Place in Gangwon Province, Korea (레저휴양공간의 수목 추가식재가 탄소흡수기능 향상에 미치는 효과)

  • This study estimated how much carbon can be sequestered if we plant trees in non-forested areas in the 36 hole Oak Valley Golf Courses in Gangwon Province, Korea. We identified plantable areas where planting trees will not affect golf game using high resolution aerial photography and ground survey and estimated the annual carbon sequestration rate of the planted trees using biomass equations. Of the golf courses, 30.3% were covered by forest. Other major land cover types include lawn, waterbody, baresoil, buildings, and roads. The plantable areas consist of $106,101m^2$ of lawn (6.0% of the study site) and $177,531m^2$ of low density forest (10.1% of the study site). We assumed to plant Mongolian oaks with 10 cm in diameter at brest height with the density of $0.3/m^2$ in the lawn and $0.2/m^2$ in the low density forest. The planting simulation shows that the total number of the newly planted trees were 67,336, and the total carbon sequestered during the subsequent year was 392.9 tC/yr, which offset 12.5% of the total carbon emitted from the golf courses. The annual carbon sequestration rate gradually increases and reaches its maximum level at 440.5 tC/yr in 15 years since the initial reforestation (14.0% of the carbon emission from the golf courses).

Effect of the Physicochemical Properties of Soil on the Arsenic Bioaccessibility (비소용출에 대한 토양의 물리화학적 특성 영향)

  • Four well-characterized soils collected from A- and B-horizon in the Department of Energy Oak Ridge Reservation in USA, mainly distributed with Inceptisol(Inc) and Ultisol(Ult) soils, were used in this work. The bioaccessibility of arsenic as well as oxidation phenomena of As(III) was investigated with soils spiked with As(III) and As(V) using a physiologically based extraction test(PBET) at pH 1.5 and 1:100 soil to solution ratio. Also effect of aging time on the bioaccessibility of arsenic was investigated over the 6 months. After 48 hours(fresh) contacting As(V) solution with soils, all soils rapidly and strongly sequestrated As(V), especially Ult-B. However, little sequestration was observed after 3-months. When As(III) was spiked on the same soils, a great portion of As(III) was oxidized to As(V) after 48 hrs, especially Inc-A and Ult-A soils, which is strongly related with Mn content in soils. By using As(III)-spiked soils, much reduced bioaccessibility as total arsenic was observed from Inc-B and Ult-B soils over the 6 months aging time compared to that from Inc-A and Ult-A soils. This result can be explained by the continuous sequestration of As(V), produced from oxidation of As(III), onto Inc-B and Ult-B soils having much amount of iron. The trend of As(III) sequestration over six months aging time was quite similar with that of As(V) sequestration.

Dynamics of Carbon Sequestered in Concentric Layers of Soil Macroaggregates

  • Methods used to study carbon sequestration by soil aggregates have often excluded the concentric spatial variability and other dynamic processes that contribute to resource accessibility and solute transport within aggregates. We investigated the spatial gradients of carbon (C) and nitrogen (N) from the exterior to interior layers within macroaggregates, $6.3\sim9.5$ mm, sampled from conventional tillage (CT) and no tillage (NT) sites of a Hoytville silt clay loam. Spatial gradients in C accumulation within macroaggregates were related to the differences in C dynamics by determining the sizes and the turnover rates of fast C and slow C pools in the concentric layers of aggregates. Aggregate exteriors contained more labile C and were characterized by greater C mineralization rates than their interiors in both management systems. In contrast, C in the interior layers of aggregates was more resistant in both systems. These results indicated the spatial differentiation of C dynamics within macroaggregates, i.e., exterior layers as a reactive site and interior layers as a protective site. Greater total C distribution in the exterior layers of NT aggregates indicated more influx of C from the macropores in interaggregate space than C. mineralization (net gain of C), whereas lower C distribution within the exterior layers of CT aggregates indicated net loss of C by greater C mineralization than C influx. We found total C increased approximately 1.6-fold by the conversion of CT soils to NT management systems for a period of 36 years. Differences in total accumulation and the spatial distribution of C within aggregates affected by management were attributed to the differences in aggregate stability and pore networks controlling the spatial heterogeneities of resource availability and microbial activity within aggregates.

  • The concentration of atmosphere carbon dioxide ($CO_2$) which is one of the major greenhouse gas, continues to rise by the increase in fossil fuel consumption, forest destruction and decrease of biological diversity, etc. In order to weaken the global warming, a reduction of $CO_2$ discharge to the atmosphere is required. The $CO_2$ ocean sequestration technology utilizes the intrinsic oceanic capacity of $CO_2$ absorption, diluting and/or dispersing the liquefied $CO_2$ in the deep ocean (>2,000 m). This geo-engineering approach is regarded as one of the occasions to mitigate the $CO_2$ concentration in the atmosphere. Some developed centuries such as Japan, USA, Norway, etc. have intensively carried out the projects on the research and development of $CO_2$ ocean sequestration since 1990s. There have been several approaches to develop the relative technological system to mitigate the increasing $CO_2$, however, there was no systematic and practical R&D programme in the $CO_2$ ocean sequestration. This paper has described the state of the art on the three optional methods of $CO_2$ sequestration, and compared with them in the aspect of the applicable possibility.

Leukocyte Sequestration and Free Radical-Mediated Lung Injury in Ovine Cardiopulmonary bypass Using Membrane Oxygenator (양에서 막형 산화기를 사용하여 심폐바이패스할 경우 백혈구격리 및 자유라디칼로 중재되는 폐손상)

  • Background: Complement activation with transpulmonary leukocyte sequestration is considered a main mediator leading to ischemia-reperfusion lung(I-R) injury. We studied the role of leukocytes in the formation of I-R injury in ovine cardiopulmonary bypass(CPB) model with a membrane oxygenator. Material and Method: Five sheep were used. CPB circuitry consisted of a roller pump(American Optical Corp., Greenwich, CT, USA) and a membrane oxygenator(UNIVOX-IC, Bentley, Baxter Health Corp, Irvine, CA, USA). The CPB time was fixed at 120 min. Ten minutes after the start of CPB, total CPB was established. Thereafter a total CPB of 100 min was performed, followed by another 10 min of partial CPB. The CPB was discontinued and the animals were fully recovered. For measuring left and right atrial leukocyte counts, blood samples were taken before thoracotomy, 5 min and 109 in after the start of CPB, and 30 min and 120 min after weaning. C3a was measured before thoracotomy, 109 min after the start of CPB, and 30 min and 120 min after weaning. Plasma malondialdehyde(MDA) was checked before thoracotomy, 109 min after the start of CPB, and 30 min after weaning. One to two grams of lung tissue were taken for water content measurement before thoracotomy, 109 min after the start of CPB, and 30 min after weaning. Lung biopsy specimens were examined by light and electron microscopy. Result: Of 5 animals, 4 survived the experimental procedures. Of these, 3 animals survived on a long-term basis. No significant differences in transpulmonary gradients of leukocyte were found and no significant complement activation was expressed by C3a levels. MDA level did not show significant changes related to lung reperfusion despite an increase after the start of CPB. On both light and electron microscopic examinations, mild to moderate acute lung change was observed. Interstitial edema, leakage of erythrocytes into the alveolar space and endothelial cell swelling were the main findings. Water content of the lung showed a slight increase after the start of CPB, but there was no statistical significance. Conclusion: These findings indicate that ischemia-repersusion lung injury may not be from complement activation-leukocyte sequestration but from another source of oxygen free radicals related to CPB.

Characterization of Crop Residue-Derived Biochars Produced by Field Scale Biomass Pyrolyzer

  • Application of biochar to soils is proposed as a significant, long-term, sink for atmospheric carbon dioxide in terrestrial ecosystems. In addition to reducing emissions and increasing the sequestration of carbon, production of biochar and its application to soils will contribute improve soil quality and crop productivity. Objectives were i) to evaluate biochar productivity from crop residues using a low-cost field scale mobile pyrolyzer and ii) to evaluate characteristics of feedstocks and biochars from locally collected crop residues. Pyrolysis experiments were performed in a reactor operated at $400-500^{\circ}C$ for 3-4 hours using biomass samples of post-harvest residues of corn (Zea mays L.), cotton (Gossypium spp.), rice (Oryza sativa L.), sorghum (Sorghum bicolor L.) and wheat (Triticum aestivum L.). Feedstocks differed, but average conversion to biochar was 23%. Carbon content of biomass feedstock and biochar samples were 445 g $kg^{-1}$ and 597 g $kg^{-1}$, respectively. Total carbon content of biochar samples was 34% higher than its feedstock samples. Significant increases were found in P, K, Ca, Mg, and micro-nutrients contents between feedstock and biochar samples. Biochar from corn stems and rice hulls can sequester by 60% and 49% of the initial carbon input into biochar respectively when biochar is incorporated into the soils. Pyrolysis conversion of corn and rice residues sequestered significant amounts of carbon as biochar which has further environmental and production benefits when applied to soils. Field experiment with crop residue biochar will be investigated the stability of biochars to show long-term carbon sequestration and environmental influences to the cropping systems.

Low Fetal Weight is Directly Caused by Sequestration of Parasites and Indirectly by IL-17 and IL-10 Imbalance in the Placenta of Pregnant Mice with Malaria

  • The sequestration of infected erythrocytes in the placenta can activate the syncytiotrophoblast to release cytokines that affect the micro-environment and influence the delivery of nutrients and oxygen to fetus. The high level of IL-10 has been reported in the intervillous space and could prevent the pathological effects. There is still no data of Th17 involvement in the pathogenesis of placental malaria. This study was conducted to reveal the influence of placental IL-17 and IL-10 levels on fetal weights in malaria placenta. Seventeen pregnant BALB/C mice were divided into control (8 pregnant mice) and treatment group (9 pregnant mice infected by Plasmodium berghei). Placental specimens stained with hematoxylin and eosin were examined to determine the level of cytoadherence by counting the infected erythrocytes in the intervillous space of placenta. Levels of IL-17 and IL-10 in the placenta were measured using ELISA. All fetuses were weighed by analytical balance. Statistical analysis using Structural Equation Modeling showed that cytoadherence caused an increased level of placental IL-17 and a decreased level of placental IL-10. Cytoadherence also caused low fetal weight. The increased level of placental IL-17 caused low fetal weight, and interestingly low fetal weight was caused by a decrease of placental IL-10. It can be concluded that low fetal weight in placental malaria is directly caused by sequestration of the parasites and indirectly by the local imbalance of IL-17 and IL-10 levels.

Surgical Treatment of Congenital Lung Cysts (선천성 폐낭성 질환의 외과적 치료)

  • A total of 69 patients were treated for congenital lung cysts at Seoul National University Hospital during the period between 1960 and Aug, 1989. They were 37 patients of bronchogenic cyst, 18 patients of pulmonary sequestration, 7 patients of congenital cystic adenomatoid malformation [C.C.A.M.], and 7 patients of congenital lobar emphysema. In cases of bronchogenic cyst, involved age was variable from 7 months to 53 years and most cases were asymptomatic. They were located 14 cases in the right lung, 6 cases in the left lung and 17 cases in the mediastinum. Their sizes were also variable from 2.5 to 12 cm and most of them except 3 cases were solitary cysts. There were 6 cases that had bronchial communication. In cases of pulmonary sequestration, aged from 3 months to 29years, all were intrapulmonary type. 15 cases of them were found in the left lower field and 3 cases in the right lower field. Anomalous vessels were confirmed in 16 cases by aortography and/or surgical exploration. In cases of congenital cystic adenomatoid malformation, aged from 7 days to 6 years, 3 cases of infants were admitted due to severe dyspnea and cyanosis but the remaining 4 cases were admitted due to large cyst or abscess on chest roentgenogram. 3 cases were located in the right lung and 4 cases in the left lung. In cases of congenital lobar emphysema, aged from 14 days to 11 years, 6 cases were located in the right lung and one case was located in the left lung. All of them were male and had shown some degree of dyspnea from birth There were 2 cases of mortality, one in C.C.A.M and one in congenital lobar emphysema. But the remaining cases were well treated and recovered with the surgical resections.
