  • This paper aims at estimating instantaneous prestress losses by measuring the actual prestress forces in prestressed concrete (PSC) box girder bridges. Measurement were taken to study initial prestress losses such as friction losses and slip losses. A new strain gauge system was developed to measure strains in internal tendons. The system was installed on a total of 20 tendons in a PSC box girder bridges. The variation of prestress forces were monitored during prestressing tendon and after prestress transfer. The prestress losses are also calculated including friction losses and slip losses. The measured data were compared with the theoretical values. The result shows that the measured prestress forces agree well with the theoretical values. It is shown that prestress force of each strand in the same tendon is a bit different. This study also shows that prestress losses of continuity tendons during prestress transfer are significantly different each other, which results from the variety of buttress location and tendon profile. The present study provides realistic information on the estimation of actual prestress forces and losses in PSC box girder bridges.

  • It is a common practice to design basement walls acting as a one-way slab or plate with idealized boundary conditions, resulting in potentially inefficient design. The walls are often supported by buttress columns and side walls in the vertical direction, thereby acting as a two-way slab. In this study, structural behavior of single-story, three-span basement wall subjected to lateral soil pressure was investigated. Three dimensional finite element analyses were conducted to determine the force distribution on the wall. Based on the numerical studies, a regression analysis was carried out to determine the design values of moments in vertical and horizontal directions as well as shear forces on the wall and design charts are developed. The proposed design method with accompanying design charts would enable practicing engineers to estimate member forces on the wall for preliminary design purpose without resorting to finite element analysis. Numerical examples demonstrated the applicability of the proposed method.

  • Background: Anterior open bite is a challenging malocclusion to correct orthodontic treatment. Anterior open bite associated with over-erupted posterior teeth and long lower facial height should be treated by reduction of posterior dimension for esthetic results. Although the possibility of orthodontic treatment of an anterior open bite has increased with the introduction of skeletal anchorage, there are still cases requiring surgery for various reasons. Case presentation: This case report covers an anterior open bite of a 25-year-old man successfully treated with the posterior maxillary segmental osteotomy (PMSO) and miniplates. After the pre-surgical orthodontic treatment, the PMSO between canines and first premolars was performed under local anesthesia and miniplates were placed on the zygomatic buttress. As a result of 28 months of treatment, an impaction amount of 3.5 mm was obtained in the maxillary posterior teeth, and the facial esthetics improved at rest and smile. Conclusion: The impaction of the posterior dentoalveolar segment using the PMSO can be a good treatment option in patients with anterior open bite showing long lower facial height.

  • Masonry structures are the most common structural systems that have been used almost all over the world from the earliest ages of history to the present day. These structural systems are often unfavorably affected by natural disasters such as earthquakes. The main reason for this is that they are built without sufficient engineering knowledge. On January 24, 2020, a severe earthquake occurred near the Sivrice District of Elazığ in eastern Turkey. According to the Turkish Directorate of Disaster and Emergency Management (AFAD), the magnitude of the earthquake was 6.8 and the focal depth 8 km. This earthquake caused damage and destruction to the masonry structures used extensively in the region. The Hacı Yusuf Taş (new) mosque in the Malatya city center, located about 64 km from the epicenter of the earthquake, was among the buildings affected by the earthquake. The mosque has smooth-cut stone walls and domes made of brick units. The main dome of the structure was severely damaged during the earthquake. In this study, information about the earthquake is first provided, and the damage to the mosque is then interpreted via photographs. In addition, two separate finite element models were produced, where the current state of mosque and solution suggestions are presented, and response spectrum analyses were carried out. According to these analyses and field observations, a buttress system to the main walls of the structure should be constructed in the direction which has little lateral rigidity.

  • A 57-year-old man presented with a pigmented papule, 0.4 cm in diameter, on the left lower eyelid. Skin biopsy revealed a basal cell carcinoma, which was excised through a wide excision followed by a full-thickness skin graft (FTSG). Two weeks after the surgery, an erythematous nodule developed in the lower margin of the graft recipient site. The nodule size increased rapidly over 2 weeks, becoming dome-shaped with a central hyperkeratotic plug. A diagnosis of keratoacanthoma (KA) was made, and surgical excision was performed. Histological findings revealed a large, well-differentiated squamous tumor with a central keratin-filled crater and buttress. The human papilloma virus (HPV) genotyping results were negative. Risk factors for KA include trauma, old age, exposure to ultraviolet (UV) radiation, immunosuppression, and HPV infection. KA has most often been reported to develop at the donor site. Although the pathogenesis of KA is unclear, trauma is believed to act as a second insult to a preceding oncogenic insult, such as exposure to UV radiation, resulting in a koebnerization. Herein, we report a case of solitary KA at a FTSG recipient site. This report presents information that may provide guidance during dermatologic surgeries.

  • Deep learning has been incorporating various optimization techniques motivated by new pragmatic optimizing algorithm advancements and their usage has a central role in Machine learning. In recent past, new avatars of various optimizers are being put into practice and their suitability and applicability has been reported on various domains. The resurgence of novelty starts from Stochastic Gradient Descent to convex and non-convex and derivative-free approaches. In the contemporary of these horizons of optimizers, choosing a best-fit or appropriate optimizer is an important consideration in deep learning theme as these working-horse engines determines the final performance predicted by the model. Moreover with increasing number of deep layers tantamount higher complexity with hyper-parameter tuning and consequently need to delve for a befitting optimizer. We empirically examine most popular and widely used optimizers on various data sets and networks-like MNIST and GAN plus others. The pragmatic comparison focuses on their similarities, differences and possibilities of their suitability for a given application. Additionally, the recent optimizer variants are highlighted with their subtlety. The article emphasizes on their critical role and pinpoints buttress options while choosing among them.

  • Background As the indications for postmastectomy radiotherapy expand, innovative solutions are required to reduce operative complications and reconstructive failure after prosthetic breast reconstruction. In this study, we investigated the effectiveness of acellular dermal matrix (ADM) inlay grafts in preventing postoperative wound dehiscence of irradiated breasts in the context of prosthetic breast reconstruction. Methods A retrospective analysis was conducted of 45 patients who received two-stage prosthetic reconstruction and radiotherapy following mastectomy. An ADM graft was placed beneath the incisional site during the second-stage operation in 19 patients using marionette sutures, whereas the control group did not receive the ADM reinforcement. Patient demographics and complications such as wound dehiscence, capsular contracture, peri-prosthetic infection, cellulitis, and seroma were compared between the two groups. Results During an average follow-up period of 37.1 months, wound dehiscence occurred significantly less often in the ADM-reinforced closure group (0%) than in the non-ADM group (23.1%) (P=0.032). There was no significant difference between the two groups in relation to other complications, such as capsular contracture, postoperative infection, or seroma. Conclusions The ADM inlay graft is a simple and easily reproducible technique for preventing incisional dehiscence in the setting of radiotherapy after prosthetic breast reconstruction. The ADM graft serves as a buttress to offload tension during healing and provides a mechanical barrier against pathogens. Application of this technique may serve to reduce complications in prosthetic breast reconstruction after radiotherapy.

  • Drawing a sample can be costly or time consuming in some studies. However, it may be possible to rank the sampling units according to some baseline auxiliary covariates, which are easily obtainable, and/or cost efficient. Ranked set sampling (RSS) is a method to achieve this goal. In this paper, we propose a modified approach of the RSS method to allocate units into an experimental study that compares L groups. Computer simulation estimates the empirical nominal values and the empirical power values for the test procedure of comparing L different groups using modified RSS based on the regression approach in analysis of covariance (ANCOVA) models. A comparison to simple random sampling (SRS) is made to demonstrate efficiency. The results indicate that the required sample sizes for a given precision are smaller under RSS than under SRS. The modified RSS protocol was applied to an experimental study. The experimental study was designed to obtain a better understanding of the pathways by which positive experiences (i.e., goal completion) contribute to higher levels of happiness, well-being, and life satisfaction. The use of the RSS method resulted in a cost reduction associated with smaller sample size without losing the precision of the analysis.

  • Objective : Complete sellar floor reconstruction is critical to avoid postoperative cerebrospinal fluid (CSF) leakage during transsphenoidal surgery. Recently, the pedicled nasoseptal flap has undergone many modifications and eventually proved to be valuable and efficient. However, using these nasoseptal flaps in all patients who undergo transsphenoidal surgery, including those who had none or only minor CSF leakage, appears to be overly invasive and time-consuming. Methods : Patients undergoing endoscopic endonasal transsphenoidal tumor surgery within a 5 year-period were reviewed. Since 2009, we classified the intraoperative CSF leakage into grades from 0 to 3. Sellar floor reconstruction was tailored to each leak grade. We did not use any tissue grafts such as abdominal fat and did not include any procedures of CSF diversions such as lumbar drainage. Results : Among 200 cases in 188 patients (147 pituitary adenoma and 41 other pathologies), intraoperative CSF leakage was observed in 27.4% of 197 cases : 14.7% Grade 1, 4.6% Grade 2a, 3.0% Grade 2b, and 5.1% Grade 3. Postoperative CSF leakage was observed in none of the cases. Septal bone buttress was used for Grade 1 to 3 leakages instead of any other foreign materials. Pedicled nasoseptal flap was used for Grades 2b and 3 leakages. Unused septal bones and nasoseptal flaps were repositioned. Conclusion : Modified classification of intraoperative CSF leaks and tailored repair technique in a multilayered fashion using an en-bloc harvested septal bone and vascularized nasoseptal flaps is an effective and reliable method for the prevention of postoperative CSF leaks.
