On The Development of The Stern Form with Low Resistance and High Propulsive Efficiency for Full Ships (저저항(低抵抗) 고추진(高推進) 효율(效率)의 비대선(肥大船) 선미선형(船尾船型)의 개발(開發)에 관하여)

  • It is required to develop a hull form with low resistance and high propulsive efficiency for the improvement of the ship-board operational economy. Since the hull forms with low resistance frequently have lower propulsive efficiency and on the other hand the hull forms with higher propulsive efficiency don't show good resistance characteristics, it is always very difficult to obtain economical hull forms which require less propulsive power accordingly. Efforts have been made to pursue a stern form with excellent resistance and propulsion characteristics together by shaping the run of the so-called buttock-flow type stern, which is known to have good viscous resistance performance, like that of conventional aftbody(U-type or Hogner type) featured by high propulsive efficiency. First model tests confirmed that the above concept can be one of the alternative approaches to the design of the good stern form and by the continuing efforts thereafter for the refining of the concept, propelled by the first promising results, stern form of good resistance performance together with good propulsive efficiency has been realized to some extent. In addition, it is confirmed that the new new stern can have better cavitation and vibration characteristics due to uniform wake-fields and the compact engine room arrangement can be possible due to it's larger floor area in way of engine room double bottom as compared with usual barge stern.

  • Objectives : This report was performed to evaluate the treatment of acupuncture Therapy. Bee venom acupuncture Therapy, Herbal medication and Moxibustion In Chronic lschial bursitis. Methods : We treated 1 case of chronic Ischial Bursitis with acupuncture therapy, Bee Venom Acupuncture therapy, herbal medication and Moxibustion Results : After 8 weeks of acupuncture Therapy. Bee venom acupuncture Therapy, Herbal medication and Moxibustion. a remarkable improvenment was made for Chronic lschial bursitis. The patient couldn't feel painful swelling over the center of the buttock and down the back of the leg. Conclusion : Acupuncture Therapy, Bee venom acupuncture Therapy, Herbal medication and Moxibustion were effective In reducing the symptoms. We think that it need the further study and clinical trial for Chronic lschial bursitis.

  • The phosphaturic mesenchymal tumor mixed connective tissue variant (PMTMCT) is an extremely rare disease, and is frequently associated with oncogenic osteomalacia showing an paraneoplastic syndrome, which is characterized by phosphaturia, hypophosphatemia, normocalcemia and decreased levels of 1,25-dihydroxyvitamin D3 associated with a tumor. We experienced a 45-year-old female who had a soft tissue tumor on her right buttock causing oncogenic osteomalacia, which was satisfactorily treated by surgical excision of the mass.

  • We are reporting about a case of a rheumatoid nodule, beneath the ischial tuberosity, mimicking ischiogluteal bursitis. Rheumatoid nodules are frequently seen, at the subcutaneous soft tissues of repetitive mechanical irritation points, and prominent bones. There have been no reported cases of rheumatoid nodules, extending just beneath the ischial tuberosity bone. A 68-year-old woman with a seven-year history of rheumatoid arthritis, suffered for six months, from right buttock swelling and discomfort in seating. A cystic lesion postero-inferior to the ischial tuberosity, was noted in the MRI scan, which was thought to be ischiogluteal bursitis, because of its characteristic location and appearance. Histopathologic analysis and gross findings on the operation, revealed no evidence of bursitis.

  • Extraskeletal osteosarcoma is an uncommon tumor originated from soft tissue without evidence of skeletal involvement. It usually affects adults and its common locations are extremity, buttock, and retroperitoneum. Although the histologic feature of this tumor is well known, there have been few reports on the fine needle aspiration cytologic findings. We report the imprint cytologic feature of extraskeletal osteosarcoma. The patient was a 49-year-old man with a mass of the left anterior chest for 2 years. On the Imprint preparation, the smears showed malignant round, polygonal or spindle cells with coarsely clumped chromatin and occasionally prominent nucleoli. The malignant cells occur singly, in clusters, or associated with amorphous eosinophilic osteoid. Mitotic figures are also seen.

  • At OPD follow-up in December 1998, chest x-ray of a 42-year-old female showed a solit ary pulmonary nodule at a superior basal segment in the right lower lobe. After percut aneous transthoracic needle aspiration failure, wedge resection of the superior basal segment of lower lobe in right lung was performed for diagnosis and therapy. Three years ago, she had received surgery to remove a mass in the left buttock. The mass was pathologically diagnosed as malignant fibrous histiocytoma. She subsequently received 4500 rad radiotherapy for 35 days. Pathology confirmed metastatic malignant fibrous histiocytoma of the lung.

A Case of Cutaneous Myiasis Caused by Cordylobia anthropophaga Larvae in a Korean Traveler Returning from Central Africa

  • The cutaneous myiasis has been rarely reported in the Republic of Korea. We intended to describe here a case of furuncular cutaneous myiasis caused by Cordylobia anthropophaga larvae in a Korean traveler returned from Central Africa. A patient, 55-year-old man, had traveled to Equatorial Guinea, in Central Africa for a month and just returned to Korea. Physical examinations showed 2 tender erythematous nodules with small central ulceration on the left buttock and thigh. During skin biopsy, 2 larvae came out from the lesion. C. anthropophaga was identified by paired mouth hooks (toothed, spade-like, oral hooklets) and 2 posterior spiracles, which lack a distinct chitinous rim. Although rarely described in Korea until now, cutaneous myiasis may be encountered more frequently with increasing international travel and exchange workers to tropical areas.

Skin Temperature Responses of Hanbok When It Worn (한복 착용에 따른 피보온의 변화)

  • The objective of the study was to investigate skin temperature responses of Hanbok when it was worn. Two healthy females(average 21 years, 155cm and 60kg were exposed to a climatic chamber(Room Temp. $21{\pm}1^{\circ}C,\;52{\pm}2%R.H.$, 0.15m/s). During the experiment, rectal temperature, skin temperature of 9 areas, clothing microclimate, subjective sensation were measured. Chima and Jogory to be made of silk nobang(SN) or Ramie were worn for summer. Polyester(P) Chima and Jogori(R) could be wort for spring and autumn. For winter, silk Chima, Jogori(S) and Durumagi(D) were commonly worn. Rectal temperature was high in order of naked(N), R, SN, P, S, D. However Mean skin temperature was reversely high in order of D, S, SN, R, P, naked. In naked, skin temperature was high in order of head, trunk upper extremity and lower extremity. But on wearing of Hanbok, it was the highest at the chest except head regardless of kinds of clothing ensembles. Skin temperature of upper arm was secondly highest on wearing the silk ensemble and the Durumagi ensemble, but skin temperature of buttock was secondly highest on wearing the silk nobang ensemble and the ramie ensemble. Skin temperature on wearing the silk ensemble was generally higher than those on other clothing ensembles. Local and mean skin temperatures on wearing the silk ensemble and the Durumagj ensemble were generally higher than on other clothing ensembles. Heat resistance of the fabric might have affected on the local skin temperature.

Pulsed Radiofrequency Application for the Treatment of Pain Secondary to Sacroiliac Joint Metastases

  • Sacroiliac (SI) joint pain can result from degeneration, infection, malignancy, and trauma. Patients with metastatic bone pain who do not respond to conventional treatment may need more aggressive neuroinvasive approaches. Recently, pulsed radiofrequency (PRF) neuromodulation has emerged as a promising treatment alternative for refractory cases of SI joint pain. Nevertheless, there is no report on the treatment of pain arising from SI joint metastases with PRF. We are reporting about a 63-year-old woman suffering from buttock pain due to breast cancer metastases in the SI joint. We treated this patient with PRF neuromodulation of the L4-S3 primary dorsal rami and lateral branches using a rotating curved needle technique. The patient tolerated the procedures well, without any complications. She experienced about 70% reduction in pain, and pain relief was sustained for 10 months. This result suggests that PRF neuromodulation is a safe, effective treatment for pain from SI joint metastases.

Intracardiac Metastatic Rhabdomyosarcoma

  • A 70-year-old man who visited Samsung Medical Center reported experiencing palpitation for 2 weeks. He had undergone excision of a mass in the right buttock due to rhabdomyosarcoma 7 years prior to this visit. Transesophageal echocardiography showed a pedunculated mass in the left ventricle, which was thought to be a vegetation of infective endocarditis, metastasis of the primary tumor, or thrombus. He underwent removal of the cardiac tumor, and the pathologic report was metastatic rhabdomyosarcoma. Thus, here, we report a rare case of metastatic rhabdomyosarcoma in the left ventricle.
