Mucoepidermoid tumor arising from the bronchial tree as one of the bronchial adenoma is a extremely rare in incidence. And its clinical and histopathologic behavior has been reported as varying degree of benign to extremely malignant. Because symptoms are usually related to the bronchial obstruction or obstructive pneumonitis followed by endobronchial growth of tumor, frequently error is made in diagnosis of this tumor as entity of obstructive lung disease. We present a case of mucoepidermoid tumor with review of relevant literatures arising from the right middle lobe bronchus extending to intermediate bronchus in 47 years old housewife and was surgically removed by middle and lower Iobectomy.
Occult bronchial foreign body is that long-standing foreign body lodge in bronchial tree. Occult bronchial foreign bodies arc rare in adults, whereas tracheobronchial aspiration of foreign bodies occurs commonly in children. A 65-year-old man with chronic cough, sputum production, and fever was transferred for treatment of right middle and lower lobc collapse and obstructive pneumonitis as evidenced by imaging studies. The patient was treated with right middle-lower bilobectomy because fiberoptic bronchoscopic removal of the foreign body failed. We report this case with review of literatures.
Bronchopleural fistula is an unnatural communication between the bronchial tree and pleural space. Closure of the bronchial stump using various muscular flaps has been previously reported. There have been few reports on treatment of large defects with bronchopleural fistula accompanied by surrounding muscle injury. We report on our experience with two patients suffering from large defect with bronchopleural fistula, who were treated with free flaps. No recurrence of bronchopleural fistula was observed during follow-up.
Bronchial adenoma,especially adenocystic carcinoma [cylindroma] of the tracheobronchial tree is relatlvely rare in incidence. This is the report of one case of cylindroma in a 28 year old female,surgically treated at the Deparment of Thoracic Surgery,Seoul National University Hospital. The tumor was located at the left main bronchus and completely obstructing the bronchial lumen.Left pneumonecromy was performed because of the irreversible bronchiectatic and fibrotic changes of the entire left lung.
Background : The development of bronchoscopic equipment along with the precision of radiographic techniques had reduced the mortality rate of patients with tracheobronchial foreign bodies but has been no change in the incidence of tracheobronchial foreign bodies since their introduction. The aim of this study was to assess the clinical characteristics of a tracheobronchial foreign body aspiration and to evaluate the efficacy of the treatment modality in children and adults. Methods : This is a retrospective review of 64 patients who underwent bronchoscopic procedures for the treatment of aspirated foreign bodies from December 1994 through March 2004 at the Chonnam national university hospital. Results : There were 47 males and 17 females, aged from 1 month to 78 years. Most of the patients had no underlying illness except for one patient with a cerebrovascular accident that contributed to the foreign body aspiration. The most common symptom was cough, which was noted in 54 patients (84.3%). The other presenting symptoms were dyspnea (48.8%), fever (20.3%), sputum (14%), vomiting (7.8%), and chest pain (4.6%). Those whose tracheobronchial foreign bodies were diagnosed more than 2 days after the aspiration (21 patients) were more likely to have pneumonia than those whose foreign bodies were diagnosed within 2 days (p = 0.009). Foreign bodies were visualized in the plain chest radiographs in 12 cases (18.8%), while others showed air trapping (21, 32.8%), pneumonia (15, 23.4%), atelectasis (7, 10.9%), and normal findings (9, 14.1%). The foreign bodies were more frequently found in the right bronchial tree (36) compared with the left bronchial tree (22, p = 0.04). In order to remove the foreign bodies, twenty (31.2%) cases were removed using flexible bronchoscopy, while 42 (65.6%) and 2 (3.2%) cases required rigid bronchoscopy and surgery, respectively. Conclusions : Tracheobronchial Foreign body aspiration had a bimodal age distribution in the infancy and old age around 60 years. They were found more frequently in the right bronchial tree. In addition, patients whose foreign bodies were diagnosed more than 2 days after the aspiration were more likely have a infection. Rigid bronchoscopy is the procedure of choice for uncooperative children and for those with foreign bodies lodged deeply in the small bronchial tree.
Objective : The aim of this study was to investigate risk factor, precaution and treatment of aspirated foreign body during dental procedure. Material and Methods : Twenty cases of accidental aspiration of the foreign body, which removed by bronchoscopy at the Asan Medical Center between 2008 and 2012, were analyzed retrospectively. Results : Ten cases of accidental aspiration were occurred during dental procedure. Symtoms include cough(65%), dyspnea(50%), sputum(25%) and wheezing(25%). The most common location of foreign body was right bronchial tree(50%), left bronchial tree(45%) and carina(5%). Patients risk factors were chronic obstructive pulmonary disease, lung cancer, pulmonary tuberculosis, esophageal cancer and vegetative state. Conclusion : Accidental aspiration or swallowing of dental instrument or material is not uncommon accidents in dental practice. Most foreign bodies enter into gastrointestinal tract spontaneously. But aspiration into broncho-trachea can be more serious events and must be treated as an emergency situation. Prompt emergency treatment and removal of the foreign body is necessary to avoid complication. Dentists must have knowledge about the precaution and be ready to deal with foreign body aspiration during dental procedures.
Bronchogenic cysts are thought to be uncommon developmental anomalies. They develop from small buds or diverticuli that separate from the foregut in the formation of the tracheo-bronchial tree. They are nearly always located near the tracheal bifurcation; However, these lesions can occur anywhere along the tracheo-bronchial tree. We experienced a case of cervical bronchogenic cyst presenting sorely as an anterior neck mass in a 26-year-old woman. The neck ultrasonography showed as $1.4{\times}1.1cm$ sized hypoechoic lesion with a well-defined margin on the isthmic portion of the thyroid gland. Excision of the mass was carried out. The mass was superfical to the strap muscle and was contained within the subcutaneous tissue in the midline without any connection to the trachea. Grossly, the mass was an oval-shaped cystic lesion which measured 1.5 cm in the greatest diameter. The cyst was filled with thick, yellow, jelly-like material and the inner surface was smooth and glistening. Microscopically, the cyst showed a lining of ciliated columnar epithelium, beneath which was a loose areolar stroma containing plaques of mucous glands and mature cartilage. We thought this cervical bronchogenic cyst appeared to represent an expression of complete aberrent accessory lung bud detachment from the primitive foregut.
The pulmonary sequestration is an uncommon congenital anomaly characterized by the presence of a part of lung tissue which is supplied by an aberrant artery from the aorta or its branch and usually has no communication with the normal bronchial tree. It was first presented by Hubber in 1777 and presented in details by Pryce in 1946. We present a case of extralobar pulmonary sequestration experienced recently with a case of intralobar type experienced in 1962. The patient was 11 year old male with the complaint of chronic productive cough. Serial chest films showed a large cyst with or without the air-fluid level on the posterobasal segment area of the left lower lobe. Bronchography showed no definite communication between the cyst and bronchial tree. On operation, the cystic lesion was supplied by an aberrant artery from the descending thoracic aorta 5 cm above the aortic hiatus and was sited at the posterobasal segment area of the left lower lobe. We performed the sequestrectomy and the sequestration was surrounded by its own pleura, 6.8x3.9x3.2 cm in size, contained the pale brown mucoid secretion in a large cyst and showed the primitive alveolar structure of the wall. The aberrant artery was 1 -5 cm long, 0.3 mm in internal diameter and arterio-sclerotic. We also compared 6 cases of collection, 5 intralobar and 1 extralobar type, presented in Korea.
Two hundred and seventeen patients underwent diagnostic rigid bronchoscopy or bionchofiberscopy to evaluate the cytologic diagnosis in the lung cancer patient at the department of chest surgery of Yon-Sei university, college of the medicine from 1971 to 1977 year. One hundred and twenty cases of these patients were taken rigid bronchoscopy and ninety four cases of these patient were taken bronchofiberscopy. Cytologic examination of the sputum was done in 214 cases and sputum cytology was positive in 50 cases [23.4%]. Rigid bronchoscopy was made in 120 cases and this bronchoscopic cytology including bronchial washing and bronchial biopsy was positive in 34 cases [28.5%]. Bronchofiberscopy was performed in 94 cases and was positive in 45 cases [47.5%]. Histopathologically, 41 cases [43.6%] were epidermoid cell carcinoma, 8 cases [8.5%]of undifferentiated cell type, 12 cases [12.8%]of adenocarcinoma, 8 cases [8.5%]of alveolar cell type, and the 25 cases were undetermined. Cytologic examination of the sputum lacks the accuracy of the bronchoscopies in terms of both localization and accurate histologic indentification of the type of neoplasm. Rigid bronchoscope has the advantage of permitting identification of a tumor in a central location and of providing a sufficient amount of biopsy material for accurate diagnosis of carcinoma. However, it has the disadvantage of limiting examination to the larger, more central portions of the tracheobronchial tree. Bronchofiberscope had the advantage of examine upper lobe as well as other portions of the tracheobronchial tree which could not be visualized with the rigid bronchoscopy. A positive diagnosis in bronchofiberscopy was obtained in the highest rate, 47. 8% [45 cases]. A1 last, if a bronchogenic carcinoma is suspected on the basis of either symptoms of an abnormality on the chest film the diagnostic work-up-sputum cytology, bronchial washing, bronchoscopic biopsy, scalene node biopsy, thoracentesis and mediastinoscopy explothoracotomy etc-should precede in an attempt not only to obtain the higher positive diagnosis but also to obtain a tissue diagnosis and to evaluate the stage of the disease and to ascertain the appropriate mode of therapy.
The anomalous systemic arterial supply to the lung without sequestration is an uncommon congenital anomaly characterized by the presence of a part of lung tissue which supplied by an aberrant artery from the aorta or its branch and normal communication with the normal bronchial tree, and it was reported by Huber in 1777 first. We experienced a case of anomalous systemic arterial supply to the lung without sequestration, which was confirmed preoperatively. The patient was 16 year old male and came to us with complaints of mild fever and profuse sputum with coughing. Chest film showed a ring-like soft tissue shadow in Rt. middle lung field. Aortogram revealed an aberrant artery originated from abdominal aorta at 12th thoracic vertebral level and supplying a part of Rt. lower lobe of lung. At. the time of operation, an aberrant systemic artery which originated from the abdominal aorta supplying the Rt. lower lobe was noted, and the bronchial communications were normal. After division and ligation of the aberrant artery, Rt. lower lobectomy was performed The postoperative courses were uneventful and the patient was discharged with good condition.
