식도결핵은 매우 드문 질환으로 원발성으로 발생하는 경우는 거의 없고 발생한다 하더라도 대부분 속발성으로 발생한다. 가장 흔한 원인으로는 종격동 림프절의 침범에 의해 인접해 있는 식도로 전파되는 것으로 이러한 경우 식도-종격동 누공 등의 합병증이 발생할 수가 있다. 저자들은 식도결핵 및 이의 합병증으로 식도-종격동 누공이 형성된 환자를 항결핵제 투여만으로 식도결핵의 호전과 누공의 폐쇄를 경험하였기에 보고하는 바이다.

폐농양의 치료 중에 발생하는 기질화 폐렴은 흔하게 관찰 되는 소견은 아니다. 기질화 폐렴은 특별한 원인 없이 발생하기도 하지만, 감염의 치료 과정에서 이차적 기질화 폐렴이 발생 할 수도 있다. 본 증례는 62세 남자 환자에서 폐농양을 치료하는 과정에서 발생한 기질화 폐렴을 경기관지 폐생검을 통해 확인하고 부신피질 호르몬을 투여로 효과적으로 치료한 예를 경험하였기에 보고하는 바이다.

Leukotriene $B_4\;(LTB_4)$ is a pro-inflammatory mediator synthesized in myeloid cells from arachidonic acid. Elevated levels of $LTB_4$ have been found in a number of inflammatory diseases and levels are related to disease activity in some of these. Because $LTB_4$ interacts with cells through specific cell surface receptors, $LTB_4$ receptor blockade is the most specific approach to reduce the pathogenic role of $LTB_4$. In order to find $LTB_4$ receptor antagonist from plants, we screened the $LTB_4$ receptor antagonistic activity of the methanol extract and solvent fractions of herbal drugs. The ability of samples to inhibit specific binding of $[^3H]-LTB_4$ to human peripheral neutrophils was used as assay to evaluate the antagonistic activity of plant materials. Among the tested methanol extracts of herbal drugs, Mori Radicis Cortex, Perillae Semen, Armeniacae Semen and Sophorae subprostratae Radix showed potent inhibitory activity above 70% at the concentration of $100\;{mu}g/ml$. The inhibitory activities of $LTB_4$ binding to human neutrophils were evaluated for several solvent fractions at three different concentrations. Especially, hexane soluble fractions of Anemarrhenae Rhizoma and Embeliae Radix, and ethyl acetate soluble fractions of Aristolochiae Fructus, Magnoliae Cortex and Zingiberis Rhizoma crudus showed moderate activity at $25\;{mu}g/ml$. These fractions were promising candidates for the study of the activity-guided chromatographic purification of active compounds. Silica gel column chromatography of hexane soluble fractions of Anemarrhenae Rhizoma and Embeliae Radix gave very active sub-fractions, AA-4 and ES-4, and their inhibition activities of $LTB_4$ binding to human neutrophil at $30\;{mu}g/ml$ were 78% and 62%, respectively. From these results we could anticipate new $LTB_4$ receptor antagonist from herbal drugs, and the block of $LTB_4$ effects may provide beneficial in neutrophil mediated diseases such as inflammation and bronchial asthma.

여러 국내외 논문에서 hormone 치료는 우수한 효과를 나타내었으나 치료 종료 후 객혈이 다시 발생하여 추가적인 수술을 받는 경우가 많았으며, GnRH analogue의 장점에도 불구하고 hormone 치료로 완치된 예는 대부분 danazol에 의한 경우가 많았다. 저자들이 경험한 환자는 월경과 동반된 객혈, 월경주기에 따라 변하는 방사선학전 변화와 GnRH analogue 치료에 대한 극적인 증상의 호전 등의 전형적인 소견으로 폐실질 자궁내막증에 의한 catamenial hemoptysis로 진단되었다. 6회의 hormone 치료 기간동안 임상양상의 호전을 보였고, 치료를 중단한 후에도 재발의 증거가 없는 상태로 외래를 다니고 있다.

59세의 여자 환자가 반복되는 폐렴 및 흉수를 주소로 내원하였다. 흉부 전산화 단층촬영상 우폐하엽 기저분절에 기관지 확장을 동반한 폐허탈 및 염증성 침윤소견이 관찰되었으며, 기관지내의 종괴소견은 관찰되지 않았다. 굴곡성 기관지경 검사상 우하엽 기저분절 기관지 입구에 점액성의 백색 표면을 보이는 돌출성 종괴가 관찰되었고 관강은 좁아져 있었으며 감자 생김상 세기관지 폐포암으로 확진되었다. 환자는 우하엽 절제술을 시행받았으며 수술후 증상없는 상태로 12개월째 추적관찰중이다. 세기관지 유두종은 극히 드문 양성 폐종양으로 알려져 있고 국내에는 아직 보고된 바가 없어 문헌 고찰과 함께 보고하는 바이다.

Background: Recent studies show that mitophagy, the autophagy-dependent turnover of mitochondria, mediates pulmonary epithelial cell death in response to cigarette smoke extract (CSE) exposure and contributes to the development of emphysema in vivo during chronic cigarette smoke (CS) exposure, although the underlying mechanisms remain unclear. Methods: In this study, we investigated the role of mitophagy in the regulation of CSE-exposed lung bronchial epithelial cell (Beas-2B) death. We also investigated the role of a phosphodiesterase 4 inhibitor, roflumilast, in CSE-induced mitophagy-dependent cell death. Results: Our results demonstrated that CSE induces mitophagy in Beas-2B cells through mitochondrial dysfunction and increased the expression levels of the mitophagy regulator protein, PTEN-induced putative kinase-1 (PINK1), and the mitochondrial fission protein, dynamin-1-like protein (DRP1). CSE-induced epithelial cell death was significantly increased in Beas-2B cells exposed to CSE but was decreased by small interfering RNA-dependent knockdown of DRP1. Treatment with roflumilast in Beas-2B cells inhibited CSE-induced mitochondrial dysfunction and mitophagy by inhibiting the expression of phospho-DRP1 and -PINK1. Roflumilast protected against cell death and increased cell viability, as determined by the lactate dehydrogenase release test and the MTT assay, respectively, in Beas-2B cells exposed to CSE. Conclusion: These findings suggest that roflumilast plays a protective role in CS-induced mitophagy-dependent cell death.

본 증례는 평소 건강히 지내던 38세 여자에서 아토피나 천식과의 연관성이 없이 호흡기계 증상 및 폐침윤을 동반한 예로, 비특이적인 증상 및 방사선학적 소견으로 일반적인 폐침윤에 대한 치료에 반응하지 않고 병변이 좌폐상엽의 설상엽에 국한되어 있어 수술을 통해 확진 하였기에 문헌고찰과 함께 보고하는 바이다.
