Purpose : Through a survey undertaken among doctors practicing pediatric patients in primary medical facilities on nutrition during infancy, the present study was done to determine whether these doctors were giving appropriate advice on infant nutrition. Methods : We visited pediatric specialists(group one) and non-pediatric specialists or general practitioners(group two) who were practicing in Daegu to ask them to fill out a questionaire on infant nutrition. Results : When 10 points were given to each question for a total of 100 points, the results of the evaluation showed that the score in group one was $73.0{\pm}15.8$ points and $45.1{\pm}18.4$ points in group two, showing a significant difference. No statistical difference was seen in the scores between non-pediatric specialists and general practitioners. Each answer to each question on the feeding period of breast milk, on loose stools during breast milk feeding, methods to deal with jaundice during breast milk feeding, choice of commercial baby food, criteria on directing soy milk, and criteria of guiding infant cereal was different between group one and two. Conclusion : The results of the present investigation showed that specialists in other fields or general practitioners were giving inappropriate advice on nutrition during the infant stage compared with pediatric specialists; thus, we need to confirm methods to prevent inappropriate consultations by specialists of other fields, together with continued education of pediatric specialists.
We investigated 23 lactic acid bacteria isolated from Korean breast milk-fed infant in order to select strains which show superior anti-allergic effect. The candidates were cultivated and then we obtained dried powders of tyndallized cells and supernatant concentrate separately. Screening was carried out with down-regulation of interleukin (IL) 4 and up-regulation of IFN-${\gamma}$ in mouse splenocytes. As a result of the screening, we selected Lactobacillus rhamnosus IDCC 3201 (RH3201) for oral feeding to ovalbumin-sensitized BALB/c mice. Oral administration of RH3201 as dead cell bodies and supernatant concentrate suppressed hyper-production of serum immunoglobulin (Ig) E levels compared to vehicle group. Such anti-allergic effects were achieved by improvement of the balance between cytokines produced from type-1 helper T (Th1) and type-2 helper T (Th2) lymphocytes. Therefore, RH3201 has potential to improve atopic symptoms by immunomodulatory effect.
Nitrogen(N) concentrations of human milk in various fractions, such as total, protein, nonprotein, whey protein and casein were determined at 2-3 days, 1, 2, 4, 6 and 12 weeks of postpartum. significant decreases in total N, nonprotein N, protein N, whey protein N and casein N were found with time postpartum. Total nitrogen decreased from 401mg/dl at 2-3 days to 211mg/dl at 12 week. Whey protein nitrogen was found to contribute to the total nitrogen decrease with time. The percentage of nonprotein nitrogen was 13% in colostrum and 17-18% in mature milk. The proportions of whey protein and casein nitrogen were 55:45 at 2-3 days and 34:66 at 12 week postpartum. These determinations will provide the basic information on the variability of nitrogen components as lactation proceed.
To clarify the necessity of surgery for fistula-in-ano (FIA) in infant, a retrospective analysis of 82 cases FIA in infant were performed for 11 years period from 1987 to 1998. Group A included 44 cases in the period of surgery oriented treatment to 1992, and group B contained 38, period of feeding control oriented management (FC) after 1993. FC, that was indicated in case with loose stool and eczematoid perianal skin (LSES), composed of quit breast feeding, change of cow milk or complete weaning. Surgical decision was made after improvement of stool condition. Fistulectomy was performed in 29 cases (65.9 %) of group A. However, in group B, only 15 cases (39.5 %) required surgery (p=0.0036). Thirty-four cases in B had the history of LSES, and Fe was applied in 25, and was effective in 24. Among 24 cases, 21 showed complete healing with FC, 3 had surgery because of the persistent FIA, and 1 got operation due to no improvement of stool condition. In conclusion, Fe has to be applied to FIA in infant before surgery, particularly in cases with history of LSES. As one of the etiologic factors for FIA, LSES could be considered to this particular age group.
This paper covers algorithms for the management of regurgitation, constipation and infantile colic in infants. Anti-regurgitation formula may be considered in infants with troublesome regurgitation, while diagnostic investigations or drug therapy are not indicated in the absence of warning signs. Although probiotics have shown some positive evidence for the management of functional gastrointestinal disorders (FGIDs), the evidence is not strong enough to make a recommendation. A partially hydrolyzed infant formula with prebiotics and ${\beta}$-palmitate may be considered as a dietary intervention for functional constipation in formula fed infants. Lactulose has been shown to be effective and safe in infants younger than 6 months that are constipated. Macrogol (polyethylene glycol, PEG) is not approved for use in infants less than 6 months of age. However, PEG is preferred over lactulose in infants >6 months of age. Limited data suggests that infant formula with a partial hydrolysate, galacto-oligosaccharides/fructo-oligosaccharides, added ${\beta}$-palmitate may be of benefit in reducing infantile colic in formula fed infants in cases where cow's milk protein allergy (CMPA) is not suspected. Evidence suggests that the use of extensively hydrolyzed infant formula for a formula-fed baby and a cow's milk free diet for a breastfeeding mother may be beneficial to decrease infantile colic if CMPA is suspected. None of the FGIDs is a reason to stop breastfeeding.
The article demonstrates a method of studying human health and nutrition by applying a multi-disciplinary approach and examines how humans developed and survived by adjusting to their environment. This process involves physiological, cultural and genetic adaptation both independently and interactively. This study postulates that a sound human health may be the result of balance between nutrition and environmental conditions. It is noted that there is a positive correlation between malaria and fava bean intake, and sickle cell anemia and cassava intake. It is also suggested that the difference in disease structure in soybean and non-soybean consumption cultures can be explained by an ecological approach to studying nutrition. This study further suggest that the relationship between nutrition and socio-cultural system. epidmiological study of nutrition and cultural environment nutrition and conceptual characteristics, nutrition and food intake pattern, nutrition and health sociological functions and the physioloical, cultural and genetic adaptation can all be stimulating research subjects to be studied form and ecological point of view. This article also includes the results from a series of ecological studies conducted by the author investigating the relationship between nutritional status of Korean breast-feeding mothers and the composition of the human milk and also the Vitamin D status of Korean and their lifestyle. Invasive Candida infections (ICI) have become the third most common cause of late-onset infection among premature infants in the neonatal intensive care unit (NICU). Risk factors include birth weight less than 1,000 g, exposure to more than two antimicrobials, third generation cephalosporin exposure, parenteral nutrition including lipid emulsion, central venous catheter, and abdominal surgery. Candida colonization of the skin and gastrointestinal tract is an important first step in the pathogenesis of invasive disease. Strict infection control measures against the infection should be done in the NICU. The following practices are likely to contribute to reducing the rate of ICI: (1) restriction of broad-spectrum antibiotics, antacids and steroid; (2) introduction of early feeding and promoting breast milk. Fluconazole prophylaxis may be an effective control measure to prevent Candida colonization and infections in individual units with high incidence of fungal infection. In addition, there is a need of further data including the development of resistant strains and the effect on long-term neurodevelopmental outcomes of infants exposed to drugs before the initiation of routine application of antifungal prophylaxis in the NICU.
Preterm infants are frequently discharged from the hospital with growth retardation. Given the potentially lifelong effects of growth impairmnet during a critical time of development, considerable effort should be focused on improving growth after discharge. Growth monitoring must be based on regular measurements of weight, length, and head circumference to identify those preterm infants with poor growth that may need additional nutritional support. Although prior studies vary in design and the intervention used, the evidence supports the use of fortified formulas in formula-fed preterm infants after discharge. The situation for infants fed human milk is much less clear, it seems prudent to concentrate our efforts on the encouragement of breast-feeding in this population. Catch up growth may have many benefits, and may lead to improved development. However, its long-term metabolic consequences are currently unclear. Understanding the optimal means of providing nutrition after discharge is an ongoing process.
Objectives: The objective of this study was to provide an actual data for parents of infants about growth developmental status of brestfed infants. Methods: The subjects were 150 brestfed infants of 5, 6, 7 months as a primary nutrient source. Results: 1) The mean score of growth developmental status of subjects were $8.6({\pm}1.15)$ and $9.5(({\pm}.58)$ at the highest points of 10.0. 2) The growth status was signifant difference according to feeding type(t=-2.076, p=.040), development status was signifant difference according to infant age(F=8.272, p=.000). Conclusions: The main point infants breast milk is very beneficial to infants growth development status. This result can be used as an important guide for nurses to teach the infants parents.

The instruments used for this study were IQ test by Korea Behavioral Science institute and family fostering environment 25 items. Teachers linguistic evaluation. The data analysis was done using Pearson's Correlation Coefficient, t-test, ANOVA using SPSS/pc program. This study found following results. 1) The similarly agreement degree between children lexical abilities with teachers comprehending evaluation maler r= .745, female r=0.809 The similarly agreement degree between children lexical abilities with teachers expressing evaluation, male r= .657, female r= .797(P<.01) 2) In comparing of the gender difference of urban, the femail is a little high than male(t= .5065), but the especial point is more large difference in male than in female. In comparing of the gender difference of rural, the female is especial high than the male(t=-4.411). (P<.01) 3) The higher factor of influencing the linguistic development is the breast-feeding(r=-.3279) in all the gender(P<.01), So that the brast-feeding better than cow's milk, mother better than the other family members in feeding, 6~8months better than one or two years in feeding term.
