영유아기 식이가 알레르기질환 발생에 미치는 영향 (The Effect of Nursing Patterns on the Development of Allergic Disorders)

  • Allergic disorders may be defined as the abnormal reactions to proteins. Cow milk is the first foreign protein which the human being would contact postnatally. Serveral studies suggested that this contact had certain effects on the development o allergic disorders with great controlversies. Seven hundred seventy eight students from middle class in Seoul, aged 6 to 17 wears, were surveyed to delineate the relationshpis between the cow's milk contacts in infancy and the subsequent development of allergic disorders in Koran population. Questionaire were included the nursing patterns in infancy, the past medical history of asthma or wheezy bronchitis and the presence of infantile eczema. Skin tests to three common resporatory allergens in Korea(house dust, house dust mite; D. farinae, D. ptoeronssinus) were performed. The positivity was determined by the wheal size larger than that to histamine. 70.2% of whole mateials was fed by breast milk, 13.1% by cow's milk and 16.7% by mixed nutsing. From the obsevation of nursing patterns by (table 2), we find that the number of breast milk group has been decreasing in 1970's. 10.1% of breast milk group(BMG) had wheezy bronchitis, 8.8% of cow's milk group(CMG) and 11.5% of mixed nursing group(MNG). Infantile eczema history was positive in 27.8% of BMG, in 26.5% of CMG and in 29% of MNG. Skin test positivity was 23.8% in BMG, 25.5% in CMG and 26.2% in MNG. There prevalences of allergic disorders in each nursing group did not reveal any statistically significant differences. In conclusion, nursing patterns in infancy do not seem to have certain effects on the development of allergic disorders such as infantile eczema. wheezv bronchitis or skin test positivity.

수유 기간별 모유 중 단백질 분비량과 영아의 단백질 섭취량 (The Amount of the Protein Secretion of Human Milk and the Protein Intake of Infant during Breast-feeding)

  • The longitudinal changes in protein secretion from 27 lactating women(primiparae = 10, multiparae=17) and protein intake of infants have been studied from 0.5 to 6 months postpartum in Chungju and Anseong area. The protein contents o breast milk in primiparae appeared significantly higher than in multipaae at 0.5 and 1 month postpartum. The protein secretion of primiparae and multiparae was not significantly different. In breast-feeding period, there was a tendency that protein secretion from 0.5 to 2 months postpartum was higher than thereafter. Average protein intake of boys from milk from 0.5 to 6 months postpartum was significantly higher than that of girls(p<0.05), because volume of milk intake of boys was much more than that of girls. In the amount of protein intake per infant weight, there was no significant difference between boys and girls. Protein intakes per infant weight decreased during lactation. Mean energy consumption of lactating women was 2,327㎉/day, which was 93.1% of recommended energy allowance for Koreans. Mean dietary protein consumption of lactating women was 81g/day, which was 101.3% of recemmended protein allowance for Koreans. Energy ratio of carbohydrate : protein : lipid was appeared to 68.8 : 13.9 : 17.3.

생후 6개월 동안 모유영양아의 단백질 섭취량과 성장과의 관계 (Protein Intakes and Growth of Breastfed and Breastfed Supplemented Infants During the First Six Months of Age)

  • Protein concentration in human from 39 well-norished American women and its adequacy for growth of exclusively breastfed infants(BF) and breastfed infants fed supplementary foods(BFS)from 1-6 months postpartum were studied. Mean protein concentration of breast milk measured by Lowry et al., using human serum albumin as a standard, over the first 6 months lactation was 1.31$\pm$0.13g/dl. Concentration of protein was singnificantly higher at the first month of lactation (1.55$\pm$0.23g/dl)(P<0.05) than any other month studied. Mean volume of breast milk ranged from 662-848ml/day in the BE group and from 415-661ml/day in the BFS group during the first 6 months of lactation. Mean protein intake of infants ranged from 1.3-2.2g/kg in the BF group and from 1.4-2.1g/kg in the BFS group. Mean protein intake (g/kg body weight) of both BF and BFS groups was less than Recmmended Dietary Allowance(1989, USA) of 2.2g/kg except at 1 month of age. However, mean growth of the infants was normal according to NCHS reference, suggesting that the RDA for protein was unrealistically high for infants during 2-6 months of age. Protein provided by breast milk alone appeared adequate for normal growth during this time.

서울 거주 산모의 초유 중 노닐페놀 분석을 통한 인체노출평가 (Exposure Monitoring of Nonylphenol in Preterm Breast milk in Seoulers)

  • Biomonitoring of nonylphenol (NP), an endocrine disrupting chemical, is required in Korea to perform its proper regulation. Thus, we analyzed exposure levels of nonlyphenol (NP) in breast milk from the mothers who delivered babies within 10 days (N=325). We analyzed free and total forms of NP in breast milk with LC/MS/MS (LOD, 0.5 ppb). In addition, we obtained questionnaires concerning lifestyle from the subject. As results, ranges of total NP were ${\mu}g/L$ (median, $3.51{\pm}4.98{\mu}g/L$ vs. normal, N=281, $2.07{\pm}3.76{\mu}g/L$; p<0.05). In conclusion, we suggest that exposure monitoring of NP should be continuously performed, even though the risks of NP are not clear, yet.

모유수유 실태와 관련요인에 관한 연구 (The Study on the Incidence of Breast feeding and Related Factors)

  • This descriptive study was done to investigate the proportion of mothers breast feeding and identify factors which influence breast feeding. The subjects were 320 mother who child under five years of age, living in WonJu and their children who numbered 530. Data collection was conducted at various places in the WonJu area using survey questionnaire. The results were as follows : 1. The proportion of children receiving breast feeding was 23.3%, mixed feeding 31.7%, milk feeding 45.1%. The most frequent duration for breast feeding was one month, the mean duration was six months. 2. The major reasons for breast feeding were for the health of the child 52.5%, for emotional attachment with the child 34.2%, and as the expected way of feeding 29.5%. 3. The major reasons for interrupting breast feeding were jobs 26.1%, illness of the child or mother 16.1% abnormality of the breast of nipple 12.7% others 31%. 4. The major reasons for quitting breast feeding were insufficient breast milk 34.5%, job 19.6%, diarrhea in the child 9.9%. 5. The factors influencing breast feeding were educational status and the presence of a job. 6. The husband's attitude to breast feeding was very positive 82.3% and the subjects were supported by their mothers and husbands. But the major information sources about breast feeding were the mass media 30.4% and relatives 22.0%. 7. The breast feeding mother reported experiencing positive and satisfactory feelings, but the milk feeding mother reported experiencing negative and guilt feelings. Also the breast feeding mothers perceived their children as very healthy with a stable emotional status. The results showed the breast feeding ratio in WonJu to be very low and problematic and the subject mothers strongly need information about breast feeding and support while breast feeding. Therefore it is recommended that an education program for community women and other people, like husbands, and grandmothers, be developed along with an education program for university students and other young people to provide them with preparatory information as they begin to think about marriage and families.

영아의 섭식패턴에 따른 성장발육의 종단적인 비교 연구 (The Longitudianl Study of the Growth by Feeding Practice in Early Infancy)

  • The objectives of this study were to examine the growth pattern of infants by anthropometric measurement according to the 5 feeding practices of infants with the subject of two hundred healthy newborn babies from their birth till sixty month of age at intervals of two months. Breast group(BF, n=38), formula group(FF, n=102) and mixed group(ME, n=14) were fed breast milk, formula milk, breast and formula milk, from birth till 6 mo. of age, respectively. Convert 1 group (C1F, n=14) and covert 2 group(CF, n=32) were fed breast milk and mixed milk at 2 mo. of age afterthat switched to formula milk, respectively. From these, the following results were made. All the infants of this study showed superiority to Korean standard growth rate in regards to each growth item for each month age. In the case of males, at their birth, the subscapular skinfold thickness and the total skin fold thickness in the BF group was significantly larger than in MF group and FF group(p<0.05). At 6th month age, the chese circumference of MF group was 45.9cm, and significantly larger than those of BF, FF and C2F groups(p<0.05). In the case of females, at theri birth and 2nd month age, there was no difference among all the feeding groups in regards to each growth rate. At 4th month age, the Kauf index of C1F group was 16.21 and significantly lower than those of four groups(p<0.05). And total skinfold thickness in BF group was larger than in C1F group. The increase rate per month age of all growth items were larger at 2nd month age than at the later months both in males and in females. And until 2nd month age males showed more increase than females in regards to each growth item but after 2nd month age, this sapect did not show up. Multiple linear regression was used to determine predictive factors for infant growth. It was expected that at 6th month age, in the measurement of head circumference and chest circumference and cross-sectional fat area, BF-males were bigger by 22-39% of the explanation index than the infants of other groups. As a result, in spite of the significant lower intakes of energy and nutrients in breast-fed infants than in formula-fed infants, breast-fed infants showed more growth than the average of Korean infant standard growth rate at every month age, and showed no significant growth difference among feeding groups.

대도시에 거주하는 여성의 모유 중 Indicator PCBs와 식이습성 및 인구통계학적 인자간의 관계 (The Relationship among the Indicator PCBs in Breast Milk and Dietary Habits and Demographic Factors in Women Living in Urban Areas)

  • In this study, breast milk levels of indicator polychlorinated biphenyls (PCBs) were estimated, and statistics drawn, for 22 Koreans in the general population, aged 26-38, who had resided in metropolitan areas for more than 5 years without occupational exposure to organochlorine pollutants. Concentrations of indicator PCBs were measured using the isotope dilution method with a high resolution gas chromatograph/high resolution mass detector, which provided accurate and precise data for investigation of trends, and international comparisons. The geometric mean of total indicator PCB levels was 22.3 ng/g lipid, which is significantly lower than the level in individuals from European countries. Overall, the geometric mean and 95th percentile of the most abundant congener PCB 153 were 8.04 ng/g lipid and 16.4 ng/g lipid, respectively. PCB congeners 138, 153 and 180 together accounted for about 75% of the indicator congeners analyzed. Breast milk total indicator PCB concentrations were significantly associated with age and parity, but not with body mass index (BMI), rate of body weight increase, or smoking habits. The geometric mean level of PCB 153 in breast milk from mothers with a pre-pregnant BMI < $21\;kg/m^2$ was significantly higher than in pre-pregnant women with a BMI of ${\geqslant}\;21\;kg/m^2$ (p<0.05). Women who ate more fish, meat, and milk products did not have significantly higher total indicator PCB levels than those who ate less of these products, but a significant difference in PCB 153 levels was demonstrated between the subjects consuming higher (11.45 ng/g lipid) and lower (6.79 ng/g lipid) amounts of fish (p<0.05), after adjusting for confounders. These results suggest that age, parity and fish intake are the important factors affecting the concentrations of indicator PCBs in these subjects.

채식 수유부의 수유기간별 영야의 에너지, 단백질, 지질, 락토오스 섭취량에 관한 연구 (A Longitudinal Study on Energy, Protein, Lipid and Lactose Intakes of Breast-fed Infants of Lacto-ovo-vegetarian)

  • To investigate the energy, protein, lipid, and lactose intakes of breast-fed infants of lacto-ove-vegetarians, 25 infants(13 boys, 10 girls) were examined on the 0.5th, 1st, 2nd, and 3nd months of lactation. The amount of breast milk intake was determined by a test-weighing method, and the energy content of protein, lipid, and lactose was calculated using the Atwater factor. Thedaily energy intake for boys averaged 316, 436, 447, and 431kcal and that of girls averaged 284, 399, 401, and 390kcal during lactation, respectively. Average energy intake of boys from milk from 2nd week to the 3rd month postpartum was significantly higher than that of girls(p<0.05), because milk intake volume of boys was much higher than that of girls. The protein intake of boys on 2nd week-3rd month averaged 7.33g/day and that of girls averaged 6.29g/day. The lipid intake of 2nd week, months averaged 21.8g/day and that of girls averaged 20.1g/day. The lactose intake of boys averaged 45.48g/day, while that of girls averaged 41.09g/day. The mean intake of protein and lactose in boys was significantly higher than that of girls.

서울 지역 여성의 영아 영양법에 관한 실태조사 연구 (A Survey on the infant feeding in Seoul area)

  • This research was conducted to survey the feeding methods of Korean women. The survey was executed for 355 mothers bringing up a child below 24 months in some part of Seoul in Korea from April to May, 1990. Incidence of breast feeding was 29.1% of the interviewed mothers in Seoul, which showed lower percentages than those of previous surveys. This research proved the major influencing factor generally depended on its mother rather than babies or labor condition. For example, the higher income and the higher educational degree of its mothers, they tended to prefer the bottle feeding to the others. Generally mothers knew the informations of infant nutrition very well, 84.5% of the interviewed mothers in Seoul had the opinion of 'breast feeding is better', but their knowledge actually didn't work. The source of mothers‘ informations on the infant nutrition showed the books and magazines, friend and relatives, grandmother were many by the order. But there were some discrepancies between the actual and needed source of information; i.e. mothers in Seoul mostly(44.8%) wanted to acquire their informations from mass communications. The reason of breast feeding showed 'for baby's health' was the greatest, and then 'immunity of breast milk' and 'baby's emotion' were great by the order, and the reason of bottle feeding showed 'not enough breast milk' was the greatest. There were no problems in feeding attitude and position of breast feeding, but there were problems in feeding volume, consistency of formula milk and feeding position of bottle feeding.

조산모와 정상모의 초유성분에 관한 비교연구 -제1보 : 조산모와 정상모 초유의 당질, 단백질 및 무기질 성분에 관한 비교 (A Comparative Study on the Composition of Preterm and Fullterm Human Milk in colostrum -I. Comparison of the Lactpse, Protein and Mineral Contents in Contents in Closterum from Mothers of Premature and Fullterm Infants)

  • In this study, the nutritional composition of colostrum milk obtained from 22 mothers delivering perterm infants and 17 mothers delivering term infants was analyzed. The energy content of colostrum was yielded based on nitrogen, lactose, and lipid data. Preterm milk is more close the nutritional needs of the preterm infant than dose other breast milk. The results are as follows : 1) Protein concentration was significantly higher, but carbohydrate and lipid concentration were lower in preterm milk than fullterm milk. Energy content yielded from total nitrogen, lactose and lipid concentration was higher in the fullterm milk than preterm milk. 2) Calcium was higher in the preterm milk than fullterm colostrum. Phosphate was lower in fullterm milk than pretem milk. Calcium/Phosphate ratio were 2.61/1 in the preterm and 2.06/1 in the fullterm milk. Magnesium was higher in the preterm milk than fullterm milk. Zinc, Copper and Manganese concentrations were tended to lower in the preterm milk than fullterm milk.
