The Spectrum of Genetic Mutations in Breast Cancer

  • Breast cancer is the most common malignancy in women around the world. About one in 12 women in the West develop breast cancer at some point in life. It is estimated that 5%-10% of all breast cancer cases in women are linked to hereditary susceptibility due to mutations in autosomal dominant genes. The two key players associated with high breast cancer risk are mutations in BRCA 1 and BRCA 2. Another highly important mutation can occur in TP53 resulting in a triple negative breast cancer. However, the great majority of breast cancer cases are not related to a mutated gene of high penetrance, but to genes of low penetrance such as CHEK2, CDH1, NBS1, RAD50, BRIP1 and PALB2, which are frequently mutated in the general population. In this review, we discuss the entire spectrum of mutations which are associated with breast cancer.

Risk of Breast Cancer in Relation to Reproductive Factors in North-west of Iran, 2013-2014

  • More than one million new patients suffer from breast cancer annually in the world. In developed countries, breast cancer is the most common malignancy diagnosed among women, and in developing regions, it often ranks second to cervical cancer. This study aimed to investigate the relationship between incidence of breast cancer and reproductive factors in North-West of Iran. This retrospective analytical control-case study was conducted with 235 breast cancer patients and 235 women in the control group. Data collection tools included a set of questions with interviews and patient medical records. Data were analyzed using statistical tests: t-test, Chi-square, Fisher, and Pearson correlation coefficient. Significantly increased risks were associated between breast cancer and older age at first pregnancy, age at menopause and history of contraceptive use. A trend for decreasing risk were observed with increasing parity. Findings of this study showed no association between breast cancer and age at menarche. The study results suggested that physiological and reproductive factors may play important roles in the development breast cancer among Iranian women.

  • Breast cancer is one of among all cancers with increased incidence, high mortality rate, and high economic and social costs. The the most common type of cancer among females worldwide, breast cancer is actually the uncontrolled proliferation of cells which attain malignancy. Recently it has shown that breast cancer contributes 11% among all types of cancer diagnosed globally on an annual basis and it is one of the leading causes of death among women. The human epidermal growth factor receptor 2 (HER-2) is a receptor tyrosine-protein kinase erbB-2 normally involved in the proliferation and division of breast cells. In some abnormal cases the HER2 gene does not work correctly and makes too many copies of itself. HER2-positive (HER2+) breast cancers constitute an aggressive type of breast cancer and tend to grow faster and are more likely to spread. However, therapies that specifically target HER2, such as Herceptin$^{(R)}$ (traztuzumab), are very effective. HER2 targeted therapies, has significantly improved the therapeutic outcome for patients with HER2 positive breast cancer.

Zinc finger protein 143 expression is closely related to tumor malignancy via regulating cell motility in breast cancer

  • We previously reported the involvement of zinc-finger protein 143 (ZNF143) on cancer cell motility in colon cancer cells. Here, ZNF143 was further characterized in breast cancer. Immunohistochemistry was used to determine the expression of ZNF143 in normal tissues and in tissues from metastatic breast cancer at various stages. Notably, ZNF143 was selectively expressed in duct and gland epithelium of normal breast tissues, which decreased when the tissue became malignant. To determine the molecular mechanism how ZNF143 affects breast cancer progression, it was knocked down by infecting benign breast cancer cells with short-hairpin (sh) RNA-lentiviral particles against ZNF143 (MCF7 sh-ZNF143). MCF7 sh-ZNF143 cells showed different cell-cell contacts and actin filament (F-actin) structures when compared with MCF7 sh-Control cells. In migration and invasion assays, ZNF143 knockdown induced increased cellular motility in breast carcinoma cells. This was reduced by the recovery of ZNF143 expression. Taken together, these results suggest that ZNF143 expression contributes to breast cancer progression.

Clinical Outcome of Breast Cancer BI-RADS 4 Lesions During 2003-2008 in the National Cancer Institute Thailand

  • To determine the clinical outcome of breast cancer BI-RADS 4 lesions and seek a more effective management guideline, we conducted a retrospective study of all BI-RADS4 patients diagnosed between 2003-2008 with follow up time not less than 2 years. A total of 392 cases of BI-RADS 4 were identified and 320 could be sub-categorised as 4a, 4b and 4c. Overall malignant positive results were 7.65, 38.7 and 58.percent, respectively. In all cases assigned to the close follow up group, no malignancy was detectable (P<0.02). The results of the study suggested that BI-RADS sub-categories have benefit for cancer diagnosis and treatment decisions of clinicians and it might be possible to set up a safe follow-up guideline in selected groups of patients to minimize un-necessary tissue biopsy for breast cancer detection.

  • Breast cancer is the most common malignancy in women around the world. Information on the incidence and mortality of breast cancer is essential for planning health measures. This study aimed to investigate the incidence and mortality of breast cancer in the world using age-specific incidence and mortality rates for the year 2012 acquired from the global cancer project (GLOBOCAN 2012) as well as data about incidence and mortality of the cancer based on national reports. It was estimated that 1,671,149 new cases of breast cancer were identified and 521,907 cases of deaths due to breast cancer occurred in the world in 2012. According to GLOBOCAN, it is the most common cancer in women, accounting for 25.1% of all cancers. Breast cancer incidence in developed countries is higher, while relative mortality is greatest in less developed countries. Education of women is suggested in all countries for early detection and treatment. Plans for the control and prevention of this cancer must be a high priority for health policy makers; also, it is necessary to increase awareness of risk factors and early detection in less developed countries.

  • Breast cancer is the most common malignancy in women in Asia and its incidence is rapidly increasing. Information on the incidence and mortality of breast cancer is essential for planning health and more studies. This study aimed to investigate the age-specific incidence and mortality of breast cancer in Asia in 2012. A total of 639,824 cases of breast cancer were recorded in Asian countries, those with the five highest standardized incidence rates being Israel, Lebanon, Armenia, Singapore, and Kazakhstan. The highest number of deaths was observed in India, China, Indonesia, Pakistan, and Japan, respectively. Tith increasing income and improving living standards in developing countries, the incidence of breast cancer increases. This may be due to longer life, higher exposure to risk factors, eating more fatty foods and obesity, and lower pregnancy rates. The variation in incidence rates of breast cancer is very pronounced in Asia (from 80.5 in Israel to 4.6 in Bhuta). A similar situation exists for mortality rates (from 1.8 in Bhutan to 25.2 in Pakistan).

Clinicopathological Profile of Breast Cancer Patients at a Tertiary Care Hospital in Marathwada Region of Westen India

  • Background: Breast cancer is the most prevalent malignancy among women with wide differences in clinical profile from region to region. The present study aimed to describe the profile of breast cancer patients attending a tertiary care hospital in Marathwada region of Western India. Materials and Methods: In this descriptive retrospective study, we reviewed records of pathologically diagnosed patients of breast cancer managed at our center from years 2009 to 2015. Data with respect to demographic status, detailed past, medical, familial and personal history, findings of clinical examination and histological features were obtained. Patients were staged according to the Tumor Node Metastasis (TNM) system. Results: Among 260 cases, mean age of presentation was 52.6, with average age of menarche of 11.3 and menopause of 52.6 years. The majority of patients were from urban regions and were postmenopausal (64.3%). Main clinical features presentation were breast lumps. Most patients were in stage II and had infiltrating duct carcinomas. Conclusions: Most common risk factors for breast cancer observed are increasing age, low parity and obesity. Breast cancer was more prevalent among postmenopausal women presenting in stage II with infiltating duct carcinoma in our region of India.

유방 침윤성 관상피암의 초음파 소견 중 후방 에코 양상과 조직소견의 상호 비교 (Correlation of Posterior Echo Patterns and Histopathologic Features in Invasive Ductal Carcinoma of Breast)

  • 통상 유방 초음파 검사에서 후방 에코 감소는 악성종괴를 나타내는 기준으로 여겨져왔다. 그러나 저자들의 입상경험에서 후방 에코 감소를 보이지 않던 종괴가 술후 조직검사에서 악성 종양으로 판명되는 경우가 적지 않았다. 하여 저자들은 유방의 조직학적 구성 요소중 어떤 것이 후방 에코 감소에 영향을 미치는가에 대해 침윤성 관상피암(유방암중의 한 종류에 국한되지만 유방암의 대부분을 차지한다.)으로 판정된 26례를 대상으로 후향적으로 분석해보았다. 후방 에코 양상을 다양한 조직학적 소견, 예를 들면, 결체조직의 양, 탄력섬유증 (elastosis)의 정도, 조직괴사의 정도, 육안적 경계, 종괴내 염증반응, 조직학적 분화도, 유분열지수 (mitotic index)등과 비교 분석하였다. 아홉예 (35%)가 후방 에코 감소를 보였고, 17례 (65%)가 후방 에코 증가를 보였다. 후방 에코 감소를 보인 종괴들은 많은 결체조직과 고탄력섬유증, 그리고 불분명한 경계를 보였다(p<0.05). 하지만 나머지 조직학적 요소들은 후방 에코 감소에 영향을 미치지 않았다. 저자들의 연구에 의하면 유방 초음파 검사에서 종괴내 결체조직의 양과 탄력섬유증의 정도에 의해 후방 에코 증감을 보여 단순히 후방 에코 감소 그 자체가 악성을 시사하는 소견으로 받아들이기 어려울 것으로 생각한다.

진단이 애매했던 유방 세침흡인 세포검사에서 등급 점수표의 역할 (Role of Cytologic Scoring System in Minimizing "Gray Zone" in Breast Aspiration Cytology)

  • 세침흡인 검사는 촉지되거나 혹은 촉지되지 않는 양성과 악성 유방 질환을 인지하는 매우 유용한 방법이나 진단이 애매한 회색 지역이 존재한다. 1년동안 시행한 697예의 유방 세침흡인 검사 중 111예가 비정형적 혹은 악성이 의심된 다고 보고되었으며 이 중 조직학적으로 확진된 74예를 재검색하여 Masood 등 (1990)이 제안한 등급 점수표가 회색 지역을 줄이는데 유용한지 살펴보았다. 기술적 문제가 19예에서 나타났으며 나머지 55예 중 18예는 양성으로, 37예는 악성으로 조직학적으로 진단되었다. 양성 질환 중 섬유선종(5예)및 섬유선종의 성상을 지닌 섬유낭성 질환(3예)이 가장 많았고 악성은 대부분 침윤성 관암종이였으며 (29예), 관상암종(3예), 사상암종(2예) 및 점액 암종(2예) 등 분화가 좋은 암종들이 상대적으로 많았다. 등급 점수표는 회색 지역을 줄이는데 상당히 유용하여 29예의 관암종중 27예가 세포학적 악성 범주에 속했다. 그러나 분화가 좋은 암종은 7예중 2예만이 세포학적 악성 범주에 속했고 섬유선종 및 섬유선종의 성상을 지닌 섬유낭성 질환은 비교적 높은 점수를 보였다. 따라서 등급 점수표의 응용은 유방 세침흡인 검사에서 양성과 악성 병변의 감별에 매우 유용하게 이용될 수 있으나 분화가 좋은 암종들과 섬유선종의 진단에는 주의가 필요할 것으로 사료되며 유방 세침흡인 세포학적 검사에는 세포 병리 의사의 경험 및 유방질환의 세포학적 변조에 대한 친숙성이 강조되어야 할 것이다.
