Recurrent Encapsulated Papillary Carcinoma in the Ipsilateral Internal Mammary Lymph Node: a Case Report

  • Encapsulated papillary carcinoma (EPC) is an uncommon breast malignancy that is known to be indolent and associated with an excellent prognosis. However, there is a rare possibility of locoregional relapse or metastasis. Here, we present a case of recurrent EPC in the ipsilateral internal mammary lymph node (IMLN) that was detected in the postoperative magnetic resonance imaging with abbreviated protocol (AB-MRI). AB-MRI could facilitate the early detection of recurrent disease in the IMLN and may provide prognostic gain for such patients.

  • We report the case of a patient who presented with rupture of a silicone breast implant showing acute and chronic inflammation. Magnetic resonance imaging (MRI) showed silicone foci outside the implant shell and inside the pectoralis muscles that represented intra- and extracapsular ruptures of the implant and silicone granuloma. There were distinct fluid-fluid levels of various signal intensities and no signs of implant collapse such as 'linguine sign.' Rather, we detected enlargement of both the implant shell and the breast.

Use of magnetic resonance imaging for evaluation of therapeutic response in breast cancer-related lymphedema: A systematic review

  • Breast cancer treatment-related lymphedema (BCRL) is a common comorbidity in breast cancer survivors. Although magnetic resonance imaging (MRI) is widely used to evaluate therapeutic response of patients with various medical conditions, it is not routinely used to evaluate lymphedema patients. We conducted a systematic review of the literature to identify studies on the use of MRI to evaluate therapy for BCRL. We hypothesized that MRI could provide information otherwise not possible through other examinations. On October 21, 2019, we conducted a systematic review on the PubMed/MEDLINE and Scopus databases, without time frame or language limitations, to identify studies on the use of MRI to evaluate therapy for BCRL. We excluded studies that investigated other applications of MRI, such as lymphedema diagnosis and surgical planning. Of 63 potential articles identified with the search, three case series fulfilled the eligibility criteria. In total, 53 patients with BCRL were included and quantitatively evaluated with MRI before and after manual lymphatic drainage. Authors used MRI or MR lymphagiography to investigate factors such as lymphatic vessel cross-sectional area, tissue water relaxation time (T2), and chemical exchange saturation transfer. The only study that compared MRI measurement with standard examinations reported that MRI added information to the therapy evaluation. MRI seems to be a promising tool for quantitative measurement of therapeutic response in patients with BCRL. However, the identified studies focused on only manual lymphatic drainage and were limited by the small numbers of patients. More studies are necessary to shed light on the topic.

  • 유방 MRI는 유방 질환의 진단과 치료 분야에 최첨단 기법으로서 지난 10년 동안 유방 MRI는 연구 분야에서 임상 분야로 진화해 왔다. 따라서, 유방 MRI의 적응증과 적절한 영상을 얻는 방법, 소견을 해석하고 보고하는 법에 대한 이해가 필요하다. 유방 MRI는 양성과 악성 종괴의 감별, 유방암 환자의 수술 전 병기 결정, 수술전 항암요법에 대한 종양의 반응 평가, 유방 성형 여성의 평가 등에 사용되고 있다. 또한 유방암 고위험군 여성에서 선별 보조 검사로서 사용될 수 있다. 이러한 목적을 성공적으로 이루기 위해서는 적절한 MRI기법과 다른 유방 영상 소견과 연관 지어 해석할 수 있는 영상의학과 의사가 중요하다. 본 소고에서는 유방 MRI의 적응증, 표준화된 용어와 카테고리의 사용에 중점을 두어 기술하고자 한다.

Primary Angiosarcoma of the Breast: MRI Findings

  • We present image findings, especially rare MRI of a primary breast angiosarcoma with its histopathology, and also analyze the relevant medical literature reports in terms of the MRI findings. As our patient had unique features of a primary breast angiosarcoma, this case could be very helpful for future diagnosis of this rare breast malignancy by MRI.

Impact of Radiotherapy on Background Parenchymal Enhancement in Breast Magnetic Resonance Imaging

  • Background: While many studies have shown that hormones can influence background parenchymal enhancement (BPE) in breast magnetic resonance imaging (MRI), only few have directly address the effect of radiotherapy. The purpose of this study was to evaluate the impact of radiotherapy on BPE in breast MRI. Materials and Methods: A retrospective search identified 62 women with unilateral breast cancer who had a breast MRI both before and after radiotherapy following breast-conserving surgery. In our study, we assumed that systemic therapy affected both breasts equivalently. We rated the level of BPE of both breasts using a four-point categorical scale. A change in the level of BPE prior to and after treatment was compared in the diseased and contralateral breasts. Results: All patients received a 4256 to 6480 cGy dose of whole breast radiotherapy over 3-7 weeks. The mean timing of the follow-up study was 6.6 months after completion of radiotherapy. Although the BPE showed a decrease in both breasts after treatment, there was a significant reduction of BPE in the irradiated breast compared with the contralateral breast (1.18 versus 0.98 average reduction in BPE level, p=0.042). Conclusions: Radiotherapy is associated with decrease in BPE with MRI.

  • 유방 MRI는 유방암의 유방촬영 스크리닝법에 대한 가장 정확한 보완 검사법이다. 아직까지 MRI 스크리닝의 효과에 대한 무작위 대조군연구가 없음에도 불구하고, 현재 미국암학회와 국립 종합암네트워크 (NCCN)에서는 MRI 스크리닝을 유방암 고위험여성에서 권고하고 있다. 국내에서는, 한 후향적 연구에서 유방암 환자의 반대쪽 유방암에 대한 MRI 스크리닝법이 유방촬영술과 초음파에서 발견하지 못한 암을 발견하고 이는 이후 유방암 발병을 감소시킴을 보고하고 있다. 국내에 유방촬영 스크리닝법의 보완법으로 MRI를 도입하기 위해서는 판독의 표준화와 교육, MR 유도하 조직검사법과 적절한 적응증의 확립, 비용-효용성에 대한 평가가 선결되어야 한다.

Three-Dimensional Surface Imaging is an Effective Tool for Measuring Breast Volume: A Validation Study

  • Background Accurate breast volume assessment is a prerequisite to preoperative planning, as well as intraoperative decision making in breast reconstruction surgery. The use of three-dimensional surface imaging (3D scanning) to assess breast volume has many advantages. However, before employing 3D scanning in the field, the tool's validity should be demonstrated. The purpose of this study was to confirm the validity of 3D-scanning technology for evaluating breast volume. Methods We reviewed the charts of 25 patients who underwent breast reconstruction surgery immediately after total mastectomy. Breast volumes using the Axis Three 3D scanner, water-displacement technique, and magnetic resonance imaging (MRI) were obtained bilaterally in the preoperative period. During the operation, the tissue removed during total mastectomy was weighed and the specimen volume was calculated from the weight. Then, we compared the volume obtained from 3D scanning with those obtained using the water-displacement technique, MRI, and the calculated volume of the tissue removed. Results The intraclass correlation coefficient (ICC) of breast volumes obtained from 3D scanning, as compared to the volumes obtained using the water-displacement technique and specimen weight, demonstrated excellent reliability. The ICC of breast volumes obtained using 3D scanning, as compared to those obtained by MRI, demonstrated substantial reliability. Passing-Bablok regression showed agreement between 3D scanning and the water-displacement technique, and showed a linear association of 3D scanning with MRI and specimen volume, respectively. Conclusions When compared with the classical water-displacement technique and MRI-based volumetry, 3D scanning showed significant reliability and a linear association with the other two methods.

A Case Report of Juvenile Hypertrophy of the Breast in a 15-Year-Old Girl: Presented with Asymmetric Breast Enlargement and a Focal Mass-like Lesion

  • Juvenile hypertrophy of the breast is a rare condition, leading to hyperplastic breast anomalies in adolescents. Here, we report a case involving a 15-year-old girl, presented with asymmetric enlargement of the left breast. Pronounced parenchymal thickening was found on initial ultrasonography (US). MRI and second-look US revealed a focal mass-like lesion on the left mid-lateral breast, confirmed as juvenile hypertrophy of the breast on pathology.

  • 유방의 악성 섬유성 조직구종은 극히 드문 질환으로 이전에 보고된 대부분의 증례들은 임상병리학적 특성에 초점을 맞추어 이 질환의 영상소견에 대한 분석은 제한적이었다. 우리는 좌측 전완부로 부터 전이된 좌측 유방과 좌측 액와부의 악성 섬유성 조직구종 1예를 유방촬영, 유방 초음파 및 유방 자기공명영상 등의 영상의학적 소견에 초점을 맞추어 보고 하고자 한다.
