유방 양성 종괴 추적 관찰 환자에게서 발견된 관상피내암 증례 보고 (A Case Report of Intraductal Carcinoma Detected in a Patient Undergoing Surveillance for Benign Breast Mass)

  • 유방의 관상피내암(DCIS; ductal carcinoma in situ)은 유방의 상피세포가 악성화되었지만, 아직 정상적인 유관에 한정되어 있는 경우를 지칭하며, 최근 빠르게 증가하고 있다. 본 증례에서 추적관찰 2년 동안은 유방 촬영술 및 초음파 검사상 병변은 BI-RADS Category 3으로 분류되는 작은 mass가 관찰되었지만 낭성 mass 소견을 보여, 전형적인 악성 종양의 소견과는 거리가 있었으나 추적관찰 2년째에 유방초음파상 우측 유방 6시 방향에 불분명한 경계(Lobulated margin)를 보이는 저에코(hypoechoic) 종괴가 약 2.1 × 1.3 cm 크기로 보였으며 조직 검사상 관상피내암으로 진단되었다. 관상피내암은 특징적인 임상소견이 없고 초기에 양성의 임상, 영상 소견을 보일 수 있으므로 정기적인 추적관찰을 시행하는 것이 조기 진단에 중요할 것으로 사료되며, 유방촬영술과 초음파 검사로 관상피내암을 발견하는 것은 유방암 환자 전체의 예후를 좋게 하는 데에 중요하다고 할 수 있겠다. 4년 전 처음 검사 시 증상이 없으며 유방촬영상 정상이고 유방초음파상 악성 징후 없는 낭성병변이 추적관찰 중에 관상피내암의 소견인 비대칭 음영과 미세석회화, 분엽상의 mass가 관찰된 관상피내암의 증례를 보고한다.

유방암 환자의 MRI에서 발견된 병변의 악성 예측을 위한 점수체계: 진단적 능력과 이차 초음파 결정에 미치는 영향 (Scoring System to Predict Malignancy for MRI-Detected Lesions in Breast Cancer Patients: Diagnostic Performance and Effect on Second-Look Ultrasonography)

  • 목적 유방암 환자의 MRI에서 발견된 추가적 병변의 악성 예측을 위한 점수체계를 설계하고자 하였다. 대상과 방법 68명 유방암 환자의 86개 MRI 발견 병변(64 양성, 22 악성)이 후향적으로 연구되었다. 스튜던트 t 검정, Fisher 정확검정, 로짓 회귀분석을 이용해 영상적 소견과 조직학적 결과의 상관관계를 분석했다. 의미 있는 악성 시사 소견을 기반으로 한 점수체계를 설계하고 그 것의 진단적 능력을 Breast Imaging-Reporting and Data System (이하 BI-RADS) category와 비교하였다. 결과 병변 크기 ≥ 8 mm (p < 0.001), 주 병소와 동일한 사분면에 위치(p = 0.005), 지연기의 고원형 조영 증강(p = 0.010), T2 등신호(p = 0.034) 혹은 저신호 강도(p = 0.024), 불규칙한 종괴 모양(p = 0.028)이 악성과 관련 있었다. 이 소견들과 의심스러운 비종괴 내부 조영 양상을 바탕으로 한 점수체계는 BI-RADS의 진단적 능력을 향상시켰고(area under the curve, 0.918 vs. 0.727), 3개의 위음성 케이스를 방지할 수 있었다. 또한, 22개의 불필요한 2차 초음파 검사(22/66, 33.3%)를 줄일 수 있었다. 결론 병변 크기, 주 병소와의 상대적 위치, 지연기의 조영 증강 양상, T2 신호강도, 종괴의 모양 및 비종괴 내부 조영 양상을 기반으로 한 점수체계는 유방암 환자의 MRI 발견 병소를 평가하는데 있어 정확도를 높여 줄 수 있다.

유방암에서 PET의 응용 (Application of PET in Breast Cancer)

  • Positron emission tomography(PET) is an imaging method that employs radionuclide and tomography techniques. Since 1995, we applied PET not only to the diagnosis of breast cancer but also to the detection of abnormalities in the augmented breast and to the detection of metastasis. Until 2001, we evaluated 242 breast cases by PET at PET center of Seoul National University Hospital. Our group has reported serially at the international journals. In the first report, PET showed high sensitivity for detecting breast cancer, both the primary and axillary node metastasis. A total of 27 patients underwent breast operations based on PET results at Seoul National University Hospital from 1995 to 1996. The diagnostic accuracy of PET were 97% for the primary tumor mass and 96% for axillary lymph node metastasis. In case of the breast augmented, PET also showed excellent diagnostic results for primary breast cancer and axillary lymph node metastasis where mammography and ultrasound could not diagnose properly. PET also had outstanding results in the detection of recurrent or metastatic breast cancer(sensitivity 94%, specificity 80%, accuracy 89%). In addition, our study gave some evidence that PET could be applied further to evaluate the growth rate of tumors by measuring SUV, and finally to prognosticated the disease. PET could also be applied to evaluate the response after chemotherapy to measure its metabolic rate and size. In conclsion, PET is a highly sensitive, accurate diagnostic tool for breast cancer of primary lesion in various conditions including metastasis.

Incidence Data for Breast Cancer among Yemeni Female Patients with Palpable Breast Lumps

  • Purpose: To estimate the incidence of breast cancer in Yemeni female patients presenting with a breast mass. Materials and Methods: This retrospective study was carried out with 595 female patients with palpable breast lumps, attending to Alkuwait university hospital, Sana'a, Yemen. Triple assessment, including breast examination, mammography and biopsy (FNAC, core needle, or excision), for all patients were performed. Results: The incidences of benign and malignant lesions was calculated. Some 160 (26.9%) of 595 patients had malignancies; 213 (35.8%) were fibroadenomas; 12 (2.0%) were fibrocystic change; 143 (24.03%) were inflammatory lesions (including mastitis and ductectasia); 62 (10.4%) were simple cysts, while 5 (0.8%) were phyllodes tumors. The mean age of patients with malignant lumps was 44.3 years. Conclusions: Among Yemeni female patients with palpable breast lumps, the rate of breast cancer is high, with occurrence at an earlier age than in Western countries. Improving breast cancer awareness programs and increasing breast cancer screening centers inb different areas of Yemen are needed to establish early diagnosis and offer early and optimal treatment.

디지털 마모그램 반자동 종괴검출 방법 (Semi-automatic System for Mass Detection in Digital Mammogram)

  • Mammogram is one of the important techniques for mass detection, which is the early diagnosis stage of a breast cancer. Especially, the CAD(Computer Aided Diagnosis) using mammogram improves the working performance of radiologists as it offers an effective mass detection. There are two types of CAD systems using mammogram; automatic and semi-automatic CAD systems. However, the automatic segmentation is limited in performance due to the difficulty of obtaining an accurate segmentation since mass occurs in the dense areas of the breast tissue and has smoother boundaries. Semi-automatic CAD systems overcome these limitations, however, they also have problems including high FP (False Positive) rate and a large amount of training data required for training a classifier. The proposed system which overcomes the aforementioned problems to detect mass is composed of the suspected area selection, the level set segmentation and SVM (Support Vector Machine) classification. To assess the efficacy of the system, 60 test images from the FFDM (Full-Field Digital Mammography) are analyzed and compared with the previous semi-automatic system, which uses the ANN classifier. The experimental results of the proposed system indicate higher accuracy of detecting mass in comparison to the previous systems.

유방자기공명영상의 임상 적용 (Clinical Applications of Breast MRI)

  • 유방 MRI는 유방 질환의 진단과 치료 분야에 최첨단 기법으로서 지난 10년 동안 유방 MRI는 연구 분야에서 임상 분야로 진화해 왔다. 따라서, 유방 MRI의 적응증과 적절한 영상을 얻는 방법, 소견을 해석하고 보고하는 법에 대한 이해가 필요하다. 유방 MRI는 양성과 악성 종괴의 감별, 유방암 환자의 수술 전 병기 결정, 수술전 항암요법에 대한 종양의 반응 평가, 유방 성형 여성의 평가 등에 사용되고 있다. 또한 유방암 고위험군 여성에서 선별 보조 검사로서 사용될 수 있다. 이러한 목적을 성공적으로 이루기 위해서는 적절한 MRI기법과 다른 유방 영상 소견과 연관 지어 해석할 수 있는 영상의학과 의사가 중요하다. 본 소고에서는 유방 MRI의 적응증, 표준화된 용어와 카테고리의 사용에 중점을 두어 기술하고자 한다.

Comparison of Outcomes in Immediate Implant-Based Breast Reconstruction: Acellular Dermal Matrix versus Inferior Dermal Flap

  • Background Implant-based breast reconstruction has evolved tremendously in the last decades, mainly due to the development of new products and techniques that make the procedure safer and more reliable. The purpose of this study was to compare the outcomes in immediate one-stage breast reconstruction between acellular dermal matrix (ADM) and inferior dermal flap (IDF). Methods We conducted a retrospective comparative study of patients submitted to immediate breast reconstructions with an anatomical implant and ADM or IDF in a single center between 2016 and 2018. Outcomes evaluated included major complications, early complications, reinterventions, readmissions, and reconstruction failure. Simple descriptive statistics and univariate analysis were performed. Results A total of 118 breast reconstructions (85 patients) were included in the analysis. Patients in the IDF group had a higher body mass index (median = 27.0) than patients in the ADM group (median = 24). There were no statistically significant differences among both groups regarding immediate major complication, early complications, readmissions, and reinterventions. Conclusion There are no significant differences in complications between the ADM and IDF approach to immediate implant breast reconstruction. In patients with higher body mass index and large, ptotic breasts, we recommend an immediate implant reconstruction with IDF.

Role of Breast Tomosynthesis in Diagnosis of Breast Cancer for Japanese Women

  • Introduction: Mammography is the most basic modality in breast cancer imaging. However, the overlap of breast tissue depicted on conventional two-dimensional mammography (2DMMG) may create significant obstacles to detecting abnormalities, especially in dense or heterogeneously dense breasts. In three-dimensional digital breast tomosynthesis (3DBT), tomographic images of the breast are reconstructed from multiple projections acquired at different angles. It has reported that this technology allows the generation of 3D data, therefore overcoming the limitations of conventional 2DMMG for Western women. We assessed the detectability of lesions by conventional 2DMMG and 3DBT in diagnosis of breast cancer for Japanese women. Methods: The subjects were 195 breasts of 99 patients (median age of 48 years, range 34~82 years) that had been pathologically diagnosed with breast cancer from December 20, 2010 through March 31, 2011. Both conventional 2DMMG and 3DBT imaging were performed for all patients. Detectability of lesions was assessed based on differences in category class. Results: Of the affected breasts, 77 (75.5%) had lesions assigned to the same categories by 2DMMG and 3DBT. For 24 (23.5%) lesions, the category increased in 3DBT indicating improvement in diagnostic performance compared to 2DMMG. 3DBT improved diagnostic sensitivity for patients with mass, focal asymmetric density (FAD), and architectural distortion. However, 3DBT was not statistically superior in diagnosis of the presence or absence of calcification. Conclusions: In this study, 3DBT was superior in diagnosing lesions in form of mass, FAD, and/or architectural distortion. 3DBT is a novel technique that may provide a breakthrough in solving the difficulties of diagnosis caused by parenchyma overlap for Japanese women.

  • Lipomas are benign mesenchymal tumors that develop in areas of abundant adipose tissue. Due to the fatty composition of the breast, difficulties in diagnosis, treatment, and reconstruction are often encountered. We report a case of a 55-year-old female with a giant tumor of the right breast that comprised most of its mass, causing breast asymmetry. A thorough preoperative evaluation, followed by an uneventful difficult surgical resection and reconstruction, resulted in diagnosis of a benign lipoma. The case prompted this report because of its challenging size, location, diagnosis, and reconstructive solution.

  • Fine needle aspiration cytology (FNAC) has become a highly preferred, minimally invasive diagnostic tool of choice in the diagnosis of a palpable breast mass owing to its sensitivity, specificity, cost-effectiveness, and expediency. Although breast needle biopsies have been widely employed recently due to the increased detection rate of non-palpable early lesions, the importance of the use of FNAC cannot be underestimated. It comprises part of the diagnostic triad for the breast along with a physical examination and mammography, which together contribute to an increasing diagnostic accuracy. The differential diagnosis of a benign and malignant lesion is of the utmost importance in the diagnosis of breast lesions, and therefore the understanding of the possible diagnostic pitfalls is of great importance.
