몬테칼로 모의모사를 이용한 유방성형술 환자의 유방선량평가 (Evaluation of Breast Dose in Mammography for Breast Implant Patient using a Monte Carlo Simulation)

  • 유방촬영은 경제적이고 간편하며 미세석회화 검출에 효과적이라는 장점이 있으나, 유방은 감수성이 높은 장기이므로 확률적 영향에 의한 암 발생 위험을 동반한다. 이를 최소화하기 위해 정확한 유방 선량 평가가 필요하나, 현재 유방선량 평가는 일반여성들에게 한정되어 유방성형술 환자에게는 제한적이다. 본 연구는 유방성형환자의 정확한 선량 평가를 위하여 유방 보형물이 삽입된 모의피폭체를 구성하여 관전압에 따른 선량변화를 알아보고자 하였다. 그 결과, 유방 보형물이 있는 경우, 보형물이 없을 때의 선량보다 낮은 결과를 나타내었다. 검사 시 관전압 조건에 따른 선량 분포는 보형물의 유·무와 관계없이 관전압이 높아질수록 선량이 높아지는 양상을 보였다. 추후 본 연구를 통해 유방성형환자의 선량 관리 측면의 진단참고 수준 확립을 위한 기초적인 자료로서 활용 가능할 것으로 사료된다.

Digital Breast Tomosynthesis in Addition to Conventional 2D-Mammography Reduces Recall Rates and is Cost-Effective

  • Digital breast tomosynthesis (DBT) as a breast cancer screening modality, through generation of three-dimensional images during standard mammographic compression, can reduce interference from breast tissue overlap, increasing conspicuity of invasive cancers while concomitantly reducing false-positive results. We here conducted a systematic review on previous studies to synthesize the evidence of DBT efficacy, eventually 18 articles being included in the analysis. The most commonly emerging topics were advantages of DBT screening tool in terms of recall rates, cancer detection rates and cost-effectiveness, preventing unnecessary burdens on women and the healthcare system. Further research is needed to evaluate the potential impact of DBT on longer-term outcomes, such as interval cancer rates and mortality, to better understand the broader clinical and economic implications of its adoption.

X선 유방 탄성 영상을 위한 컴퓨터 모의 실험 (Computer Simulation for X-ray Breast Elastography)

  • Breast cancer is the most frequently appearing cancer in women, these days. To reduce mortality of breast cancer, periodic check-up is strongly recommended. X-ray mammography is one of powerful diagnostic imaging systems to detect 50~100 um micro-calcification which is the early sign of breast cancer. Although x-ray mammography has very high spatial resolution, it is not easy yet to distinguish cancerous tissue from normal tissues in mammograms and new tissue characterizing methods are required. Recently ultrasound elastography technique has been developed, which uses the phenomenon that cancerous tissue is harder than normal tissues. However its spatial resolution is not enough to detect breast cancer. In order to develop a new elastography system with high resolution we are developing x-ray elasticity imaging technique. It uses the small differences of tissue positions with and without external breast compression and requires an algorithm to detect tissue displacement. In this paper, computer simulation is done for preliminary study of x-ray elasticity imaging. First, 3D x-ray breast phantom for modeling woman's breast is created and its elastic model for FEM (finite element method) is generated. After then, FEM experiment is performed under the compression of the breast phantom. Using the obtained displacement data, 3D x-ray phantom is deformed and the final mammogram under the compression is generated. The simulation result shows the feasibility of x-ray elasticity imaging. We think that this preliminary study is helpful for developing and verifying a new algorithm of x-ray elasticity imaging.

일반여성과 외래내원 여성에서 유방촬영술 시행에 영향을 미치는 요인 비교 분석 (Comparative Analysis of Influencing Factors on Implementation of Mammography for Ordinary Women and Outpatients)

  • Purpose: This study is to conduct a comparative analysis of influencing factors on the experience of mammography targeting ordinary women and outpatients. Methods: The target subjects were 116 ordinary women and 105 outpatients, and the study period was around 8 months from May to December. Results: When mammography experience was examined, it was found that mammography experience was conducted in 44.8% of ordinary women and 59.0% of outpatients, but this study showed that there was a statistically significant difference. When the relation between the characteristics of the targeted objects and the experience of mammography was examined, it was found that ordinary women have a lot of experience when they have regular exercises and a high health belief. On the other hand, in case of outpatients. mammography experience was more frequent in the older group. In addition, outpatients had experiences in breast-related diseases or high self-efficacy. It was also found that the influencing factors on the experience of mammography were a high health belief in case of ordinary women, and old ages and high self-efficacy in case of outpatients. Conclusions: In order to increase the rate of conducting early detection behavior for mammography, it is needed to conduct an intervention that increases health belief for ordinary women, while it is effective to conduct an intervention that increases self-efficacy for outpatients.

Mammographic Screening of Women Attending a Reference Service Center in Southern Brazil

  • Background: To investigate the prevalence of and factors associated with performance of annual mammography by women above 40 years of age. Materials and Methods: This cross-sectional retrospective study was conducted at an oncology reference service in Southern Brazil from October 2013 to October 2014 with 525 women aged 40 years or older. Results: The prevalence of annual mammography was 54.1%; annual mammographic screening was performed for women without private medical insurance, who were under hormone replacement therapy and who had used contraception in the past. An association was found between non-performance of breast clinical and self-examination and non-performance of mammographic screening. Conclusions: Use of mammography for breast cancer screening in the public health care setting proved to be accessible; nevertheless, the proportion of screened women was low, and they exhibited poor adherence to the basic measures of care recommended for breast assessment. Thus, control of breast cancer requires implementing actions targeting the population most vulnerable to non-adherence to screening in addition to continuously monitoring and assessing that population to reduce the prevalence of this disease.

  • In every cancer early detection and early treatment is the best way to decrease mortality of patients. Moreover early detection of breast cancer increases the possibility of breast conservation treatment. Although mammography is the most powerful modality for early detection, it is hazardous to be used for young women due to X-ray exposure. Another modality of image diagnosis is ultrasound echo technique. But it is not so powerful to detect breast cancer compared to mammography. Palpation is another modality, but is largely dependent on the skill and experience of medical doctors. A new technique is tested its validity in phantom experiments with good results.

머신러닝을 이용한 에너지 선택적 유방촬영의 진단 정확도 향상에 관한 연구 (A Feasibility Study on the Improvement of Diagnostic Accuracy for Energy-selective Digital Mammography using Machine Learning)

  • Although digital mammography is a representative method for breast cancer detection. It has a limitation in detecting and classifying breast tumor due to superimposed structures. Machine learning, which is a part of artificial intelligence fields, is a method for analysing a large amount of data using complex algorithms, recognizing patterns and making prediction. In this study, we proposed a technique to improve the diagnostic accuracy of energy-selective mammography by training data using the machine learning algorithm and using dual-energy measurements. A dual-energy images obtained from a photon-counting detector were used for the input data of machine learning algorithms, and we analyzed the accuracy of predicted tumor thickness for verifying the machine learning algorithms. The results showed that the classification accuracy of tumor thickness was above 95% and was improved with an increase of imput data. Therefore, we expect that the diagnostic accuracy of energy-selective mammography can be improved by using machine learning.

  • The average glandular dose (AGD) is determined by the breast entrance skin exposure, x-ray tube target material, beam quality (half-value layer), breast thickness, and breast composition. Almost breast cancer always arises in glandular breast tissue. As a result, the average radiation absorbed dose to glandular tissue is the preferred measure of the radiation risk associated with mammography. If the normalized average glandular dose is known, the average glandular dose can be computed from the product of the normalized average glandular dose and breast entrance skin exposure. In this study, AGD was calculated by the breast thickness and various x-ray energy (HVL) in 50% glandular 50% adipose breast by Mo.-Rh. assembly. AGD is 84 mrad in compressed 5 cm breast. These results show that as increasing the breast thickness, dose also increases. But as increasing the x-ray tube voltage, dose decreases because of high penetrating ratio through the object. But high tube voltage is reducing the subject contrast. From this result, we have to consider the trade-off between subject contrast of image and dose to the patient and choose proper x-ray energy range.

Breast Cancer Knowledge and Screening Behavior among Female School Teachers in Gaza City

  • A cross-sectional survey of 370 female teachers working at Governmental schools in Gaza city was conducted. Twenty four schools were selected randomly of all female schools of the city that included primary, preparatory and secondary. In each school all-female teachers aged 35-45 year were invited to fill out a self-administered questionnaire to investigate knowledge and behavior toward breast cancer screening. The survey revealed that more than 75% of women had never undergone clinical breast examination and 60% had never undergone mammography, whereas 62% performed breast self-examination (BSE). Women who performed BSE had significantly higher knowledge about breast cancer screening (P=0.001). Women attending CBE and mammography screening also had significantly higher knowledge (P=0.001). There were significant associations between the practices and presence of positive breast cancer family history (P=0.002) and the level of education of husbands (P=0.024). The oldest women demonstrated higher performance rates of screening methods than the youngest (P=0.001). Lack of breast screening knowledge was identified among more than one third of the women, and 24.6% of women did not know any screening method. About a half of women harboured misconceptions about breast cancer screening, including the belief that breast cancer not treatable. Women residing in Gaza city (P=0.00) and with husbands less educated were more likely to have a high level of misconceptions (P=0.01).

Level of Knowledge and Behavior of Family Health Personnel Workers in Izmir about Early Diagnosis for Breast and Cervix Cancer

  • Purpose: The present study was carried out to measure knowledge level and behavior of family health personnel (FHP) in Izmir on early diagnosis of breast and cervical cancers. Materials and Methods: The study population of this cross-sectional study was not selected. A questionnaire was applied to all FHP to measure knowledge level and behavior about cancer. The participation rate was 88%. Breast examination, mammography analysis, Papanicolaou smear applications were determined as dependent variables, and knowledge level about breast and cervical cancer, age, professional time as FHP as independent variables. Data were evaluated using definitive statistics, chi-square and logistic regression tests in SPSS software package for Windows 15.0. Results: A total of 970 family health personnel participated in the research. The age range was 20-45 years (82.4%). Mean age was $37.9{\pm}7.4$. Response rate was 87.3%. Of the participants, 88.4% performed breast self-examination. Rate of performing mammography at least once was 24.1%. Rate of performing Pap-smear examination at least once was 61.0%. In logistic regression analyses, it was determined that people with knowledge on breast and cervical cancer were those performing breast self-examination, mammography and Pap-smear examinations (p<0.05. Conclusions: It is essential that the knowledge, behavior and manners of health providers on early diagnosis for cancer increases awareness in the general population and provides information on execution ofthe most effective methods for generating a healthy society.
