Vocal Fold Paralysis Following General Anesthesia with Endotracheal Intubation (기관내 삽관에 의한 전신 마취 후 발생된 성대 마비)

  • Background and Objectives : Vocal fold paralysis is an unusual complication following endotracheal intubation. We experienced five cases and analyzed their causes and preventions. Materials and Methods : We reviewed 5 cases of vocal fold paralysis following general anesthesia with endotracheal intubation at Ewha Womans University Hospital from September 1997 to May 1999 retrospectively. Results : Four cases were unilateral vocal fold paralysis(3 cases were left side, 1 case was right side) and a case was bilateral vocal fold paralysis. Conclusion : Vocal fold paralysis following endotracheal intubation is the result of recurrent laryngeal nerve damage. This damage can occur as the result of compressing the anterior branch of recurrent laryngeal nerve between an inflated endotracheal tube cuff and thyroid cartilage. Prevention of this complication lies in eliminating the use of endotracheal tubes with cuff inflated unevenly, desisting from the practice of deliberately placing the cuff within the larynx, and filling the cuff with a sample of the inspired mixture of gases.

  • In this article, the multi-objective optimization of cylindrical aluminum tubes under axial impact load is presented. The specific absorbed energy and the maximum crushing force are considered as objective functions. The geometric dimensions of tubes including diameter, length and thickness are chosen as design variables. D/t and L/D ratios are constricted in the range of which collapsing of tubes occurs in concertina or diamond mode. The Non-dominated Sorting Genetic Algorithm-II is applied to obtain the Pareto optimal solutions. A back-propagation neural network is constructed as the surrogate model to formulate the mapping between the design variables and the objective functions. The finite element software ABAQUS/Explicit is used to generate the training and test sets for the artificial neural networks. To validate the results of finite element model, several impact tests are carried out using drop hammer testing machine.

  • In vivo canine model was made in two mongrel dogs under the general Ⅰ-Ⅴ anesthesia. A vertical skin incision was made on the neck, the larynx and the trachea were dissected. Two tracheal openings were made : lower one for the insertion of the anesthesia tube and upper one for the delivery of air to the larynx to induce phonation. External branch of the superior laryngeal nerves and recurrent laryngeal nerves bilaterally were identified and stimulated electrically constantly. Subglottic pressure. fundamental frequency, intensity, and open quotient were measured when the air flow rate was varying low, medium and high. Glottic resistence was calculated. As the air flow rate was increased, the subglottic pressure and the sound intensity were increased. However, glottic resistance was decreased as the air flow was increased. In falsetto register, fundamenatal frequency was increased with the increment of air flow, but in modal register fundamental frequency was not increased statistically significant Open quotient by the electroglottography was increased according to the increment of airflow.

Assessment of cyclic behavior of chevron bracing frame system equipped with multi-pipe dampers

  • Spacious experimental and numerical investigation has been conducted by researchers to increase the ductility and energy dissipation of concentrically braced frames. One of the most widely used strategies for increasing ductility and energy dissiption, is the use of energy-absorbing systems. In this regard, the cyclic behavior of a chevron bracing frame system equipped with multi-pipe dampers (CBF-MPD) was investigated through finite element method. The purpose of this study was to evaluate and improve the behavior of the CBF using MPDs. Three-dimensional models of the chevron brace frame were developed via nonlinear finite element method using ABAQUS software. Finite element models included the chevron brace frame and the chevron brace frame equipped with multi-pipe dampers. The chevron brace frame model was selected as the base model for comparing and evaluating the effects of multi-tube dampers. Finite element models were then analyzed under cyclic loading and nonlinear static methods. Validation of the results of the finite element method was performed against the test results. In parametric studies, the influence of the diameter parameter to the thickness (D/t) ratio of the pipe dampers was investigated. The results indicated that the shear capacity of the pipe damper has a significant influence on determining the bracing behavior. Also, the results show that the corresponding displacement with the maximum force in the CBF-MPD compared to the CBF, increased by an average of 2.72 equal. Also, the proper choice for the dimensions of the pipe dampers increased the ductility and energy absorption of the chevron brace frame.

  • This paper deals with free vibration of FG sandwich annular sector plates on Pasternak elastic foundation with different boundary conditions, based on the three-dimensional theory of elasticity. The plates with simply supported radial edges and arbitrary boundary conditions on their circular edges are considered. The influence of carbon nanotubes (CNTs) waviness, aspect ratio, internal pores and graphene platelets (GPLs) on the vibrational behavior of functionally graded nanocomposite sandwich plates is investigated in this research work. The distributions of CNTs are considered functionally graded (FG) or uniform along the thickness of upper and bottom layers of the sandwich sectorial plates and their mechanical properties are estimated by an extended rule of mixture. In this study, the classical theory concerning the mechanical efficiency of a matrix embedding finite length fibers has been modified by introducing the tube-to-tube random contact, which explicitly accounts for the progressive reduction of the tubes' effective aspect ratio as the filler content increases. The core of structure is porous and the internal pores and graphene platelets (GPLs) are distributed in the matrix of core either uniformly or non-uniformly according to three different patterns. The elastic properties of the nanocomposite are obtained by employing Halpin-Tsai micromechanics model. A semi-analytic approach composed of 2D-Generalized Differential Quadrature Method (2D-GDQM) and series solution is adopted to solve the equations of motion. The fast rate of convergence and accuracy of the method are investigated through the different solved examples. Some new results for the natural frequencies of the plate are prepared, which include the effects of elastic coefficients of foundation, boundary conditions, material and geometrical parameters. The new results can be used as benchmark solutions for future researches.

Synthesis of spherical SiO2 using scaled-up ultrasonic pyrolysis process (스케일-업 된 초음파 분무 열분해 공정을 이용한 구형 SiO2 분말 합성)

  • The spherical $SiO_2$ powders were synthesized by the scaled-up ultrasonic pyrolysis (USP). The aqueous $SiO_2$ sol, which contained 20~30 nm $SiO_2$ particles, was used as a precursor for the scaled-up USP. The effects of the USP operating conditions and precursor conditions were systematically investigated, including reaction temperature, gas flow rate, and the concentration of $SiO_2$ sol on the morphologies of synthesized $SiO_2$ particles. the synthesized $SiO_2$ particle showed a pseudo-crystal phase, spherical morphology, and a smooth surface. The size of the spherical $SiO_2$ particle decreased as both reaction temperature increased and precursor concentration decreased. In addition, the synthesized $SiO_2$ particle size was increased by increasing the gas flow rate. Lastly, the scaled-up USP was compared with the lab-scale USP based on the same process conditions. Due to a short retention time in the reaction tube during the USP process, the $SiO_2$ particle synthesized via the lab-scale USP showed a larger particle size.

  • An eight-day-old neonate was diagnosed with dextro-transposition of the great arteries, atrial septal defect, patent ductus arteriosus, and a single sinus origin of the coronary arteries. The single coronary artery originated from the left sinus (sinus 2), had a proximal left circumflex arterial branch, and passed anteriorly to the right side of the aorta, further branching into the right coronary and left anterior descending arteries. We successfully performed an arterial switch operation and coronary transfer by tube graft reconstruction with autologous aortic tissue to treat the dextro-transposition of the great arteries and atrial septal defect with a single-sinus origin of the coronary arteries.

  • In this paper, process of dynamic powder compaction is investigated experimentally using impact of drop hammer and die tube. A series of test is performed using aluminum powder with different grain size. The energy of compaction of powder is determined by measuring height of hammer and the results presented in term of compact density and rupture stress. This paper also presents a mathematical modeling using experimental data and neural network. The purpose of this modeling is to display how the variations of the significant parameters changes with the compact density and rupture stress. The closed-form obtained model shows very good agreement with experimental results and it provides a way of studying and understanding the mechanics of dynamic powder compaction process. In the considered energy level (from 733 to 3580 J), the relative density is varied from 63.89% to 87.41%, 63.93% to 91.52%, 64.15% to 95.11% for powder A, B and C respectively. Also, the maximum rupture stress are obtained for different types of powder and the results shown that the rupture stress increases with increasing energy level and grain size.

Protoplast Regeneration and Reversion in Pleurotus cornucopiae (노랑느타리버섯의 원형질체(原形質體) 재생(再生) 및 환원(還元)에 관한 연구(硏究))

  • Protoplasts of P. cornucopiae were reverted to normal hyphal growth and reversion frequency was $0.04{\sim}19%$. The complete medium stabilized with 0.6 M sucrose was most effective for regeneration of protoplasts. When hypertonic mushroom complete medium not containing agar was overlaid, regeneration frequency of protoplasts was the highest rate among the others of topagar. The protoplast reversion frequency and mycelial growth of P. cornucopiae were increased when various amino acids, nucleic acid components and vitamin compound were added to the hypertonic minimal medium. The relation between sources increasing reversion frequency and sources accelerating mycelial growth was similar in amino acids and nucleic acid components but it was different in vitamins. The protoplast reversion frequency showed the highest rate when all sources were added to the regeneration minimal medium. Microscopically, regeneration patterns of protoplasts showed formation of a bud-like structure, direct germination, yeast-like cell chain of the protoplast, and the production of both direct germ tube and yeast-like cell chain from a protoplast.

Evaluation on diagnostic efficiency of the standard tube agglutination test for bovine brucellosis (소 브루셀라병 표준시험관응집반응법 진단효율 평가)

  • A confirmatory serological test, the standard tube agglutination test (STAT) is evaluated for the diagnostic efficiency in brucellosis Korea. A total of 345 bovine samples were collected from regional veterinary branch under national brucellosis monitoring program from January 2010 to June 2012 in Korea. These samples were diagnosed as suspected serum and brucellosis positive by the Rose Bengal test (RBT) and the STAT, respectively. The STAT was compared and evaluated with three serological test such as the indirect-enzyme linked immunosorbent assay (I-ELISA), competitive-enzyme linked immunosorbent assay (C-ELISA) and fluorescence polarisation assay (FPA) prescribed for international trade by OIE. Among the 345 bovine serum samples, 302 (87.5%) were diagnosed as positive in the STAT, while 215 (62.3%), 223 (64.6%) and 194 (56.2%) serum samples were diagnosed as positive for brucellosis in the I-ELISA, C-ELISA and FPA, respectively. The STAT showed quite high positive results as compared with three prescribed tests of OIE. FPA, I-ELISA and C-ELISA have shown 60.6%, 64.9% and 67.2% correlation, respectively as compared to the STAT. However correlations of three prescribed tests ranged high 84.1~97.7%. Especially, correlation between I-ELISA and C-ELISA is quite high, 97.7%. These results suggest that the STAT has shown many false-positive reactions. Therefore, additional serological test, such as ELISAs and FPA, would be necessary to adopt as a confirmatory test in the national surveillance program of bovine brucellosis in Korea.
