Comparison of Motor Function and Skill between Stroke Patients with Cerebellar and Non-cerebellar Lesion in Sub-acute Stage

  • Purpose: The cerebellum is a region of brain structure that plays an important role in calibrating two different information of neural signal from descending motor commands and from ascending sensory inputs. Damage of the cerebellum shows a variety of classic motor symptoms such as postural and locomotor dysfunctions. Therefore, we tried to investigate motor function and skill in stroke patients with cerebellar lesions in sub-acute stage, and compare with these functions of patients with non-cerebellar lesions. Methods: Total twelve stroke patients with cerebellar lesion and 130 stroke patients with non-cerebellar lesions were retrospectively recruited in this study. For evaluation of motor strength, Motricity index (MI) for upper and lower limbs was tested. For measurement of motor skill function, the modified Brunnstrom classification (MBC), Manual function test (MFT), functional ambulatory category (FAC), and Barthel index were adopted. Results: In comparison of motor strength and motor skill function between two groups, statistical differences between the two groups were significantly observed only in upper MI and FAC. Although no significant differences were found in other variables, stroke patients with cerebellar lesion had higher scores in lower and total MI, MBC, and MFT, whereas they had lower scores in FAC and Barthel index. Conclusion: Our results showed that stroke patients with cerebellar lesion had greater impact on movement functions related to hand motor and walking ability in activities of daily life, compared with patients with non-cerebellar lesion, in spite of similar degree of motor function and skill between the two different lesioned-groups.

  • Thallium-201 brain SPECT is utilized in the diagnosis of brain tumor especially in cases where CT or MRI findings alone cannot differentiate malignant lesion from benign. Recently we came across two cases of positive T1-201 brain SPECT in clinically suspected brain tumor patients that turned out to be hemorrhagic cerebral infarction instead on biopsy. The findings in these cases demonstrate that thallium-201 accumulation may occur by the breakdown of the blood-brain barrier and phagocytic cell infiltration in the liquefaction stage of infarction.

뇌종양에서 PET의 임상이용 (Application of PET in Brain Tumor)

  • The annual incidence of primary brain tumors is 7-19 cases per 100,000 people. The unique capacity of visualizing biochemical processes allows PET to determine functional metabolic activities of the brain tumors. Like other malignant tumors, F-18 FDG has been used commonly in the imaging of brain tumors. FDG PET is valuable in grading malignancy, predicting prognosis, monitoring treatment, differentiating tumor recurrence from radiation necrosis, and detecting primary lesion in metastatric brain tumors. Among amino acids labeled with positron emitters, C-11 methionine is used clinically. Tumor delineation is much better with methionine PET than with FDG PET. Low grade gliomas, in particular, are better evaluated with methionine than with FDG. PET opens another dimension in brain tumor imaging. PET imaging has clearly entered the clinical area with a profound impact on patient care in many indications.

조영증강을 보이는 뇌종양의 평가에 있어 T1강조 FLAIR 영상과 스핀에코 MR 영상의 비교 (T1-weighted FLAIR MR Imaging for the Evaluation of Enhancing Brain Tumors: Comparison with Spin Echo Imaging)

  • 목적: T1강조 MR영상은 뇌의 해부학적 구조와 병리학적 이상을 보여 주는 기본적인 영상기법의 하나로, 전통적으로 스핀에코(SE) 기법을 이용하여 획득하고 있다. 최근 FLAIR 기법을 이용하여 T1강조영상을 얻을 수 있게 되었으며, SE보다 높은 대조도의 영상을 제공한다고 알려져 있다. 그러나 조영증강을 보이는 뇌종양의 평가에 있어 T1 FLAIR 영상의 유용성에 대해서는 논란이 있다. 본 연구의 목적은 조영증강을 보이는 두개 내 종양의 평가에 있어 SE T1강조영상과 비교하여 T1 FLAIR영상의 유용성을 평가하고자 하였다. 대상과 방법: 총 52명 환자의 79개 병변을 대상으로 하였다. 각 환자에서 조영증강 후 SE T1강조영상과 T1 FLAIR 영상을 획득하였다. 정량적 분석으로 각각의 영상에서 병변, 뇌회색질(GM), 뇌백질(WM), 뇌척수액(CSF), 배경(background)의 신호강도를 측정하였고, 이를 바탕으로 병변과 WM, 병변과 GM, 병변과 CSF, WM와 GM의 contrast ratio(CR), contrast-to-noise(CNR)를 계산하였다. 정성적 분석으로 두 명의 영상의학과 의사가 각 영상에서 병변의 명확도(lesion conspicuity)를 비교 하였다. 결과: 정량적 분석 결과에서 T1 FLAIR영상의 병변과 GM, 병변과 CSF, WM와 GM의 CR, CNR 모두 SE T1강조영상보다 우월하였으며 이는 통계적으로 유의하였다. 그러나 병변과 WM의 CR, CNR은 비슷하였으며 통계적으로 유의한 차이를 보이지 않았다. 정성적 분석에서 두 영상의학과 의사 모두 병변의 명확도에 있어 T1 FLAIR영상이 SE영상보다 우월하다고 평가하였다. 결론: 조영증강을 보이는 뇌종양의 평가에 있어 T1 FLAIR영상은 SE T1강조영상보다 우수하거나 필적한 결과를 보였다.

Treatment of Scalp Arteriovenous Malformation

  • Objective : The object of this study is to present the treatment experience of the 6cases of scalp arteriovenous malformations[AVMs] focus on treatment strategy. Methods : Six patients with scalp AVM were treated during past 12years. We analysis the clinical characteristics of the lesions, treatment methods and management outcomes. Results : The lesions were located on temporal in 2patients, parietal in 2patients, frontal and occipital area in each one. Four of six patients had a trauma history on scalp. The presenting symptoms were progressive enlarged pulsating mass with or without bruit. Four of the six lesions had the large fistula in the lesion. Two patients were treated with surgical resection alone, three patients with proximal feeding artery balloon[s] occlusion followed by surgical resection, and one patient with coil embolization through trans-venous route alone. We obtained good results in all patients. Conclusion : Most of scalp AVM can be completely cured by Judicious selection and a combination of treatment modalities, i.e., surgery only, or embolization only, or embolization plus surgical therapy. Although embolization became a primary therapy for this sort of scalp AVM recently, the selection of treatment modality should be chose based on the size, angioarchitecture, and clinical presentations of the lesion.

Rapid Development of Brain Abscess Caused by Streptococcus Pyogenes Following Penetrating Skull Injury via the Ethomoidal Sinus and Lamina Cribrosa

  • Objective : Streptococcus pyogenes is a beta-hemolytic bacterium that belongs to Lancefield serogroup A, also known as group A streptococci (GAS). There have been five reported case in terms of PubMed-based search but no reported case of brain abscess caused by Streptococcus pyogenes as a result of penetrating skull injury. We present a patient who suffered from penetrating skull injury that resulted in a brain abscess caused by Streptococcus pyogenes. Methods : The patient was a 12-year-old boy who fell down from his bicycle while cycling and ran into a tree. A wooden stick penetrated his skin below the right lower eyelid and advanced to the cranium. He lost consciousness on the fifth day of the incident and his body temperature was measured as $40^{\circ}C$. While being admitted to our hospital, a cranial computed tomography revealed a frontal cystic mass with a perilesional hypodense zone of edema. There was no capsule formation around the lesion after intravenous contrast injection. Paranasal CT showed a bone defect located between the ethmoidal sinus and lamina cribrosa. Results : Bifrontal craniotomy was performed. The abscess located at the left frontal lobe was drained and the bone defect was repaired. Conclusion : Any penetrating lesion showing a connection between the lamina cribrosa and ethmoidal sinus may result in brain abscess caused by Streptococcus pyogenes. These patients should be treated urgently to repair the defect and drain the abscess with appropriate antibiotic therapy started due to the fulminant course of the brain abscess caused by this microorganism.

뇌 정위 생검술을 위한 영상지원 3차원 국재 프로그램 개발 (Development of Image Guided 3D Localization Program for Stereotactic Brain Biopsy)

  • 뇌 신경계에 발생한 비정상조직 등 병소들을 수술적 방법으로 생체 조직검사 시 방사선 영상 공간과 실제 물리적 공간을 3 차원적으로 일치시키고 국재(localization)하는 영상지원 뇌 정위 수술은 혹시 발생할지도 모르는 후유증을 최소화할 수 있다. 본 연구는 대부분 고가의 대형 정위 수술 프로그램에 포함되어 있는 뇌수술을 영상 지원하는 정위적 국재 프로그램들 중 임상에서 수술 빈도가 높은 생체조직 검사 시술 시에 최소한의 기능으로 수술을 지원할 수 있는 프로그램을 개발하고 팬텀을 이용해 국재 정확도를 측정한 후 임상에서 사용 가능성을 평가하였다. 프로그램은 영상자료 입력, fiducial marker 등록, 목표점 좌표 지정 및 좌표 값 표시 등 3 부분으로 구성하였고 팬텀을 이용한 프로그램의 정확도를 측정결과 임상에서 요구되는 일반적인 국재 정확도 한계인 2 mm 이내였다. 이 프로그램은 고난도의 뇌 정위 생검 수술시 정확한 시술이 가능하도록 지원하여 수술의 위험도를 줄이고 수술 성공률 향상에 기여할 수 있으리라 생각된다.

Brain Abscess Showing a Lack of Restricted Diffusion and Successfully Treated with Linezolid

  • A 59-year-old patient with a history of hepatocellular carcinoma presented with decreased consciousness and left hemiparesis. A rim-enhanced mass lesion without diffusion restriction was observed in contrast-enhanced MRI including diffusion-weighted imaging. Based on these findings, metastatic brain tumor was suspected. However, brain abscess (BA) was diagnosed after multiple bacterial colonies were observed in aspiration biopsy. Initial conventional antibiotic treatment including vancomycin had failed, so linezolid was used as second-line therapy. As a result, infection signs and clinical symptoms were resolved. We report a case with atypical imaging features and antibiotic susceptibility of a BA in an immunocompromised patient undergoing chemotherapy.

말초성 안면신경 마비로 발현된 교뇌-연수 인접 부위 뇌경색 1예 (A Case of Peripheral Facial Palsy in Ponto-medullary Junction Area Infarction)

  • A 69 year-old woman was admitted with sudden left facial weakness. She had no other neurologic deficit, except for left peripheral type facial palsy. She had a presumptive diagnosis of Bell's palsy. The blink test was indicative of left facial neuropathy due to left medullary lesion. Diffusion weighted (DWI) brain MRI demonstrated high signal signal lesion in left dorsolateral ponto-medullary junction. Apperant diffusion coefficient (ADC) brain MRI showed low signal lesion in the same area. We present an unusual case of ipsilateral peripheral facial palsy in dosolateral ponto-medullary infarction without other neurologic deficits.

A Case of Lumbar Metastasis of Choriocarcinoma Masquerading as an Extraosseous Extension of Vertebral Hemangioma

  • We report here on an uncommon case of metastatic choriocarcinoma to the lung, brain and lumbar spine. A 33-year-old woman was admitted to the pulmonary department with headache, dyspnea and hemoptysis. There was a history of cesarean section due to intrauterine fetal death at 37-weeks gestation and this occurred 2 weeks before admission to the pulmonary department. The radiological studies revealed a nodular lung mass with hypervascularity in the left upper lobe and also a brain parenchymal lesion in the parietal lobe with marginal bleeding and surrounding edema. She underwent embolization for the lung lesion, which was suspected to be an arteriovenous malformation according to the pulmonary arteriogram. Approximately 10 days after discharge from the pulmonary department, she was readmitted due to back pain and progressive paraparesis. The neuroradiological studies revealed a hypervascular tumor occupying the entire L3 vertebral body and pedicle, and the tumor extended to the epidural area. She underwent embolization of the hypervascular lesion of the lumbar spine, and after which injection of polymethylmethacrylate in the L3 vertebral body, total laminectomy of L3, subtotal removal of the epidural mass and screw fixation of L2 and L4 were performed. The result of biopsy was a choriocarcinoma.
