The Japanese Encephalitis Vaccine: Worldwide and Korean Status (일본뇌염백신: 국제적 현황과 우리나라 현황)

  • Japanese encephalitis is the leading cause of viral encephalitis in Asia, where it accounts for up to 50,000 cases. Approximately 20% of affected patients die, and 30-50% of survivors have significant neurological sequelae. Inactivated mouse-brain derived Japanese encephalitis vaccines has been effectively implemented to control the disease effectively in Korea and several other Asian countries. However, the vaccine is expensive and difficult to produce, requires multiple doses, and has been associated with hypersensitivity reactions and rare adverse neurologicale events. The live-attenuated SA14-14-2 vaccine derived from primary hamster kidney (PHK) cells was developed in China and has been used there since 1988. Outside China, it has been licensed and used in Korea and several other Asian countries. This vaccine is effective and inexpensive. However, the lack of precedence for using a PHK cell substrate in a live-attenuated vaccine is a special issue of concern. The WHO working group has recommended additional safety studies in selected high-risk groups, as well as ongoing post-marketing studies to ensure long-term safety. Recently, a new inactivated vaccine and live-attenuated chimeric vaccine have been developed from vero cells. With this background, this article summarized the current status of Japanese encephalitis vaccination worldwide and in Korea.

Localization of Amygdaloid Nucleus Innervating the Stomach Using Genetically Engineered PRV-BaBlu in Rat Brain (유전자 조작된 PRV-BaBlu를 이용한 흰쥐 위 신경지배 편도핵의 동정)

  • Purpose: This study was carried out to investigate the spatiotemporal localization of the amygdaloid nucleus innervating the rat stomach using PRV-BaBlu, which has been known to be an excellent type of neurotracer with the ability to transpass the neuronalsynaptic cleft. Methods: Ninety Sprague-Dawley rats (250~300 g) that were injected with PRV-BaBlu into the stomach were randomly divided into 3, 4 and 5 day groups (each group n=30). $2{\mu}l$ of PRV-BaBlu, a genetically modified strain of PRV-Bartha with the lac-Z gene,was injected into the rat stomach and immunostained with a mouse anti-${\beta}$-galactosidase at 3, 4 and 5 days after the virus injection. Results: The PRV-BaBlu infected the neurons in the amygdaloid nucleus, and the degree of viral infection in experimental animals showed a tendency to increase significantly with time (p<0.05). The neurons between the left and right amygdaloid nucleus significantly differ (p<0.05). Conclusion: This showed that PRV-BaBlu was an excellent neurotracer for localizing the amygdaloid nucleus, and the amygdaloid nucleus has a sensory input and motor output on stomach movement, influencing emotional behavior.

Clinical Observation of 127 Cases of Wounds of Chest in Viet-Nam War (월남전에서 치험한 흉부손상 120례에 대한 임상적 고찰)

  • During the 35 month period from November 1966 to November 1967 and from June 1971 to March 1973 I had experienced 127 cases of non fatal wounds of chest in Viet-Nam. .Among these 127 cases, 62[45.4%] were gun shot wounds, 49[35.8%] were shrapnel wounds and the other were traffic accident. stab wounds and miscellanous. Approximately 21% of gun shot wound were perforating and 79% were penetrating but all cases of shrapnel wounds were penetrating. Of these 127 cases. 90% evacuated to hospital within 6 hours and average time 2.5 hours. The tranfusion requirement of these cases ranged from zero to 36 pints of whole blood with an average of 2.600cc. Initial intrathoracic findings were hemopneumothorax and hemothorax mostly. and the incidence of open thoracotomy was 9.5%[12cases] and closed thoracotomy was 82.8%[104cases], which were contrast to the reports from Korean conflict. I had experienced 24 cases with complication, such as large hematoma in lung parenchyme[8 cases], atelectasis[4 cases], pyothorax [3 cases], pneumonia [3 cases], fibrothorax [3 cases], pleural effusion [2 cases] and wound infection [2 cases]. Mortality rate for entire group was 4.7% but the cases associated with brain injury was 100%, with spinal cord injury was 50%, with large vessel 50%, and abdominal injury was 33.3%, and nobody died solely of thoracic injury.

Microsurgical Experience with Supraorbital Keyhole Operations on Anterior Circulation Aneurysms

  • Objective: Conventional pterional approach is a commonly used neurosurgical technique for the treatment of cerebral aneurysms. However, this technique requires more extensive brain exposure than other key hole approaches and is sometimes associated with surgical traumatization or cosmetic problems. The aim of this study was to compare the postoperative outcome between pterional and supraorbital keyhole approaches in the patients with anterior circulation aneurysms. Methods: The authors reviewed patients with anterior circulation aneurysms who underwent aneurysm clipping via pterional or supraorbital keyhole approach at a single institute over a period of 2 years. Ninety-eight patients harboring 108 aneurysms were included in this study. Various outcomes were recorded, which included clinical grade, cosmetic problems, patients' satisfaction and complications such as chewing discomfort, frontal muscle weakness, hyposmia, infection. Results: The supraorbital approach exhibited a shorter operation time compared with the pterional approach. Complications such as chewing discomfort occurred less frequently in the supraorbital approach group. Moreover, the cosmetic outcome was significantly better in the supraorbital group than in the pterional group. Conclusion: The supraorbital keyhole approach reduced intra- and postoperative complications, including chewing discomfort and cosmetic disturbances, compared with the conventional pterional approach.

  • Sparganosis is a parasitic infection caused by the plerocercoid tapeworm larva of the genus Spirometra. Although the destination of the larva is often a tissue or muscle in the chest, abdominal wall, extremities, eyes, brain, urinary tract, spinal canal, and scrotum, intramuscular sparganosis is uncommon and therefore is difficult to distinguish from a soft tissue tumor. We report a case of intramuscular sparganosis involving the gastrocnemius muscle in an elderly patient who was diagnosed using ultrasonography and MRI and treated by surgical excision. At approximately 1 cm near the schwannoma at the right distal sciatic nerve, several spargana worms were detected and removed.

Pathologic Studies in Piglets Naturally Infected with Porcine Reproductive and Respiratory Syndrome Virus (돼지 생식기 호흡기 증후군 바이러스 자연감염 예의 병리학적 연구)

  • Porcine Reproductive and Respiratory Syndrome Virus infection (PRRSV) was confirmed by serology histopathology immunohistochemistry and bacteriologic examination in young pigs. Four suckling and six weaned piglets submitted from three different farms showed coughing sneezing labored rapid abdominal respiration lethargy and anorexia. Grossly apical and cardiac lung lobes appeared mottled with pale to dark tan discoloration. Submandibular and bronchial lymph nodes were tan and enlarged. All piglets were seropositive for PRRSV antibodies by the indirect immunofluorescent antibody(IFA) test. Microscopically lung lesions were characterized by hyperplasia and hypertrophy of type 2 pneumocytes infiltration of mononuclear cells in alveolar intersitium accumulation of necrotic debris in alveolar spaces accompanied by proliferation of alveolar multinucleated syncytial cells. Using immunohistochemical technique PRRSV antigens were demonstrated in alveolar macrophages and type 2 pneumocytes in histologic lung tissue sections. Also PRRSV antigens were detected in brain lymph nodes spleen and heart. Additionally piglets showed nonsuppurative meningoencephalitis mandibular necrotic lymphadenopathy splenic atrophy and myocardial necrosis.

A Case of Type 1 Herpes Simplex Virus Encephalitis Detected by Polymerase Chain Reaction (중합효소연쇄반응으로 확진된 Herpes Simplex virus 뇌염 1례)

  • Herpes simplex virus(HSV) infections of the CNS are associated with significant morbidity and mortality even when appropriate antiviral therapy is administered. HSV infections of the brain can be subdivided into two categories : neonatal HSV infections, which usually are caused by HSV type 2, and herpes simplex encephalitis(HSE), which occur in patients over 3 months old and is nearly uniformly caused by HSV type 1. The clinical presentation of HSE is one of the focal encephalopathic process associated with altered levels of consciousness, fever, focal seizures and hemiparesis. But because of the lack of pathognomic clinical presentation and diagnostic procedure, the efforts to develop alternative diagnostic procedure have led to the use of new diagnostic technique such as polymerase chain reaction(PCR). We report a case of HSV type 1 encephalitis in 13 month old male infant who presented with altered level of consciousness, fever and focal seizures. With the use of the PCR, HSV-1 DNA was detected in cerebrospinal fluid from the patient. The symptoms and signs of encephalitis subsided by treatment with acyclovir in 14 days.

  • Shiga-like toxin-II(SLT-II)-producing Escherichia coli 0157 : H7 strain B2387이 분비하는 SLT-II가 gnotobiotic자돈에서의 뇌혈관 병변을 일으키는 pathogenesis에 관해서 실험을 했다. 제왕절개 수술로 태어난 자돈들을 두 그룹으로 나누어서, 한 그룹에는 SLT-II 중화항체를 포함한 혈청을 구강을 통해서 수동면역을 시키고, 또다른 한 그룹에는 SLT-II 중화항체가 포함되어 있지 않은 혈청을 구강을 통해서 수동면역시켰다. 24시간후 두 그룹 모두에게 SLT-II producing Escherichia coli O157 : H7 strain B2387를 구강으로 접종했다. SLT-II 중화항체가 포함되어 있지 않은 혈청으로 수동면역시킨 그룹의 자돈들은 설사와 맹결장염, 신경증상, 뇌혈관병변을 일으키고, plasma의 prostacyclin의 level이 증가했다. 하지만 SLT-II 중화항체가 포함되어 있는 혈청으로 수동면역시킨 그룹의 자돈들은 설사와 맹결장염은 유발했지만, 신경증상과 뇌혈관병변은 관찰되지 않았고, prostacyclin의 level도 증가하지 않았다. 이런 실험결과는 SLT-II 중화항체는 뇌혈관병변은 방어하지만 맹결장염은 방어하지 못한다는 의미를 나타내며, prostacylin의 증가는 뇌혈관의 endothelium의 병변을 의미한다.

Pathological description and immunohistochemical demonstration of ovine abortion associated with Toxoplasma gondii in Iran

  • The obligatory intracellular protozoan parasite Toxoplasma gondii is a major world wide cause of infectious ovine abortion. In some different diagnostic techniques that are being used to detect this pathogen in ovine fetuses, immunohistochemistry (IHC) is a very sensitive and expensive one. Histopathology is not truly a specific and sensitive test for Toxoplasma infection but it can be helpful to choose some suspected tissues for IHC. In this study 9.5% of 200 samples (aborted ovine fetuses internal organs such as brain, liver, heart, lung, kidney, spleen) (4.6~14.4% with 95% CI) were positive in IHC with a very good logical agreement among different diagnostic techniques (${\kappa}=0.73$, 0.8) and with no significant difference among different fetal age groups (p > 0.05).

Arterioesophageal Fistula Due to an Aberrant Right Subclavian Artery -A case report- (우측 쇄골하동맥 기시 이상에 의한 동맥-식도루)

  • A case of fatal hematemesis due to an aberrant right subclavian arterioesophageal fistula which is a rare complication of the vascular ring is presented. A 42-year-old man with multiple injury by traffic accident presented severe upper gastrointestinal bleeding and was taken emergent operation. He was keeping tracheostomy tube and nasogastric tube for 7 weeks. We could find an aberrant right subclavian arterioesophageal fistula through left thoracotomy which was made by irritation of the prolonged nasogastric ube. We carried division of the aberrant right subclavian artery and fistulectomy. He was doing well postoperatively. But massive bleeding occurred at the fifth postoperative day. We performed emergent reoperation at CCU and found the tear point on the suture site of the aorta, which might be developed due to irritation of the chest tube andfor infection of the surrounding tissues. He was expired at the 8th postoperative day due to ischemic brain damage.
