뇌 CT 영상의 대칭성을 고려한 관심영역 중심의 효율적인 의료영상 압축 (An Efficient Medical Image Compression Considering Brain CT Images with Bilateral Symmetry)

  • 오늘날 의료정보화 수준향상과 디지털 병원화의 흐름에 따라 PACS는 의료기관의 핵심 인프라 중 하나로 자리매김하였다. 이와 함께 생산되는 디지털 의료영상의 종류 및 의료영상 데이터가 양적으로 급증하고 있으며, 이는 의료영상 데이터의 효과적인 보관을 위한 의료영상 압축을 중요한 요소로 부각시킨다. 현재 의료영상에 관한 사실상의 표준인 DICOM 규격에서는 의료영상 압축을 위하여 무손실 압축기법인 RLE를 명시하고 있으나, 무손실 범용 압축기법인 RLE는 인체의 대칭성을 가지는 많은 의료영상에 적용하면 높은 압축율 기대하기 힘들다. 이 논문에서는 다양한 의료영상 중 대칭 특성을 크게 내포하는 뇌 CT 영상을 대상으로 하여 영상 내 관심영역을 검출하고 대칭특성에 따라 영상의 픽셀 값을 재코딩하는 전처리 하고 영상을 압축하는 기법을 제안한다. 실험에 의하면, 제안한 기법은 RLE 압축과 영상 내 관심영역을 검출하지 않고 압축할 때와 비교하여 높은 압축률을 보인다.

두부 전산화단층촬영에서 노출 파라미터의 최적화 (Optimization of Exposure Parameters in Brain Computed Tomography)

  • 본 연구는 영상의 질을 저해하지 않는 범위 내에서 환자의 흡수선량을 최소화 할 수 있는 두부 전산화 단층촬영의 노출매개변수 값의 범위를 알아보고 선량감소가 필요한 환자에 대해서 적용 가능한 최적의 노출조건을 찾고자 하였다. 이를 위해 두부촬영 시 사용되는 기존의 전산화단층촬영 변수로부터 얻어진 영상의 선량과 잡음성분을 측정하였다. 또한 두부용 팬텀을 사용하여 관전압과 회전시간을 변화시켜서 영상을 얻었으며, 선량과 잡음(Noise), 최대량 신호대 잡음비(PSNR) 측정을 통해 실험 영상을 평가하였다. 실험결과는 첫째, 관전압과 회전시간을 변화시켜 선량을 측정한 결과 기존 두부촬영조건에서 얻어지는 선량의 재현성 실험에서 선량 측정값의 유효성이 입증되었다. 둘째, 저 선량과 고화질의 노출 조건을 찾기 위한 방법으로 선량과 잡음값의 관계는 의미가 없었으며, 선량과 PSNR값의 관계는 의미가 있었다. 이를 바탕으로 제조사에서 제시하는 촬영조건으로부터 선량의 감소가 필요한 경우 본 연구에서 제시한 새로운 노출조건을 사용한다면 환자의 흡수선량은 감소시키면서 기존에 제시된 노출조건으로부터 얻어진 영상과 유사한 화질의 영상을 얻을 수 있을 것이다.

Effects of Environmental Reinforcement Through Motivation on Motor and Cognitive Function in Rats With Focal Ischemic Brain Injury

  • It is known that individual factors as cognitive, perception, emotion, and motivation may greatly influence on recovery from neurologic region. This study was to investigate the effects of environmental reinforcement through motivation to perform the tasks voluntarily on motor and cognition function in rats with focal ischemic brain injury. Focal ischemic brain injury was induced in Sprague-Dawley rats (15 rats, $250{\pm}50$ g) through middle cerebral artery occlusion (MCAo). And then, experiment groups were randomly divided into three groups; The control group: MCAo induction ($n_1$=5), the environmental reinforcement (ER) group: the application for ER after MCAo induction ($n_2$=5), the environmental reinforcement through motivation (ERM) group: the application for ERM after MCAo induction ($n_3$=5). The climbing test (CT) and the modified limb placing tests (MLPTs) to measure the motor function and the Morris water maze acquisition test (MWMAT) and the Morris water maze retention test (MWMRT) to measure the cognitive function were performed. For the CT, the ERM group was significantly larger than the ER group. For the MLPTs, the ERM group was significantly decreased compared to other groups. For the MWMAT, the time to find the circular platform in the ERM group significantly decreased compared to other groups. For the MWMRT, the time to dwell on the quadrant circular platform in the ERM group was significantly increased compared to other groups. These results suggested that the ERM could improve the motor and cognitive functions in the rats with focal ischemic brain injury.

Fatal Traumatic Subarachnoid Hemorrhage due to Acute Rebleeding of a Pseudoaneurysm Arising from the Distal Basilar Artery

  • Isolated traumatic pseudoaneurysms of the basilar artery are extremely rare but often fatal resulting in a mortality rate as high as 50%. A 51-year-old man presented with craniofacial injury after blunt trauma. A brain computed tomography (CT) scan showed thick basal subarachnoid hemorrhage associated with multiple craniofacial fractures, while CT angiography revealed contrast extravasation at the distal basilar artery with pseudoaneurysm formation. After this primary survey, the condition of the patient suddenly deteriorated. Conventional angiography confirmed the contrast extravasation resulted from pseudoaneurysm formation, which was successfully treated with endovascular coil embolization. Decompressive craniectomy and coma therapy with propofol were also performed. However, the patient died on the 7th hospital day because of the poor initial clinical condition. The current case is the first report of acute pseudoaneurysm rupture arising from the basilar artery within the first day after trauma. Our findings suggest the possibility that pseudoaneurysm rupture should be considered if brain CT shows thick traumatic subarachnoid hemorrhage on the basal cistern with a basal skull fracture.

Patients on Anticoagulants after a Head Trauma : Is a Negative Initial CT Scan Enough? Report of a Case of Delayed Subdural Haematoma and Review of the Literature

  • Mild traumatic brain injury is common in elderly patients, many of whom are on anticoagulant. The common practice is to discharge these patients from the emergency room if the computed tomography (CT) of the brain is normal. However, a very small proportion of these patients may develop a life threatening intracranial haematoma in the following days. We present here a case of a 66-year-old male on anticoagulant therapy that developed a subdural haematoma 48 hours after a mild head injury, with a normal initial CT scan of the brain. The patient underwent a craniotomy with evacuation of a large subdural clot. Postoperatively he had progressively improved and six months later has a Glasgow Outcome Score of three. This case is characterized by the delayed onset of a subdural haematoma in a patient on anticoagulation and we discuss here the possible pathogenesis related to this phenomenon. We also briefly review the pertinent literature and the current guidelines for the management of this type of head injuries.

CT HEAD IMAGES SEGMENTATION USING UNSUPERVISED TECHNIQUES

  • In this paper, a new approach is proposed for the segmentation of Computed Tomography (CT) head images. The approach consists of two-stage segmentation with each stage contains two different segmentation techniques. The ultimate aim is to segment the CT head images into three classes which are abnormalities, cerebrospinal fluid (CSF) and brain matter. For the first stage segmentation, k-means and fuzzy c-means (FCM) segmentation are implemented in order to acquire the abnormalities. Whereas for the second stage segmentation, modified FCM with population-diameter independent (PDI) and expectation-maximization (EM) segmentation are adopted to obtain the CSF and brain matter. The experimental results have demonstrated that the proposed system is feasible and achieve satisfactory results.

산과적 합병증과 유전적 위험도가 정신분열증 환자의 뇌구조에 미치는 영향 : 전산화단층촬영 소견에 의한 연구 (Effects of Obstetric Complications and Genetic Risk on Brain Structures in Schizophrenic Patients)

  • The present study investigated the associations between CT measures and obstetric complications and family history of major mental illnesses. We had the hypothesis of diosthesis-stress model in the etiology of schizophrenia. We had the following findings. 1) Family history of major mental illnesses is inversely related to obstetric complications. 2) Prefrontal sulcal widening and family history of schizophrenia. are inversely related. 3) In female but not in male patients those with family history of schizophrenia tended to have less prefrontal sulcal widening and ventricular brain ratio. These results suggest that more genetic factors contribute to structural brain abnormalities in female than in male patients.

Supratentorial Gangliocytoma Mimicking Extra-axial Tumor: A Report of Two Cases

  • We report two cases of supratentorial gangliocytomas mimicking an extra-axial tumor. MR imaging indicated that the tumors were extra-axial, and meningiomas were thus initially diagnosed. Relative to gray matter, the tumors were hypointense on T1-weighted images and hyperintense on T2-weighted images. On contrast-enhanced T1-weighted images, homogeneous enhancement was observed, while CT scanning revealed calcification in one of the two cases.

구강암 환자에서 $^{18}F$ FDG-PET/CT의 경부 림프절 전이 평가 유용성 (USEFULNESS OF $^{18}F$-FDG PET/CT IN THE EVALUATION OF CERVICAL LYMPH NODE METASTASIS IN PATIENTS WITH ORAL CANCER)

  • Purpose: The present study was aimed to examine the usefulness of 18F-FDG PET/CT in the evaluation of cervical lymph node metastasis in patients with oral cancer. Materials and methods: Twenty-two patients who underwent neck dissection to treat oral cancer were subjected for examination. The cervical node metastasis was evaluated by means of clinical examination, CT scan, PET, and histologic examination. By comparing the results of each examination modality with those of histologic examination, it's sensitivity, specificity, positive predictive value, and negative predictive value were determined. Results: The oral cancer was more frequent in males with a ratio of 2.14:1. The sixth decade showed the highest incidence in age distribution with mean of $56{\pm}16$. Histologic findings showed that squamous cell carcinoma was the most common (15 patients), and mucoepidermoid carcinoma (3), malignant melanoma (2), and adenoid cystic carcinoma and ghost cell odontogenic carcinoma (1 each), in order. In most cases, wide surgical excision of the primary cancer and neck dissection was performed, followed by reconstruction with free flaps when necessary. When comparing the results of each examination modality with those of the histologic examination, clinical examination showed sensitivity, specificity, positive predictive value, and negative predictive value at 11%, 85%, 33%, and 58%, respectively. CT scans showed at 67%, 77%, 67%, and 77%, while $^{18}F$-FDG PET/CT at 78%, 77%, 70%, and 83%, respectively. Conclusions: These results suggest that PET is more useful, compared with clinical examination and CT scans, in the evaluation of cervical lymph node metastasis in patients with oral cancer.

지역적 가중치 거리맵을 이용한 3차원 영상 정합 (Three-Dimensional Image Registration using a Locally Weighted-3D Distance Map)

  • 본 논문에서는 동일 환자에 대해 시간차를 두고 촬영한 뇌 CT-CT 혈관조영영상간 움직임을 보정하기 위한 강인하고 고속의 정합방법을 제안한다. 먼저, 두 영상에서 3차원 경계검출 기법을 이용하여 특징점을 추출하고, 기준영상에서는 이를 지역적 가중치 3차원 거리맵으로 변환한다. 부유영상을 기준영상으로 강체변환하면서 두 경계간의 상관관계가 최대인 위치를 탐색한다. 이 때, 최대위치가 더 이상 변화하지 않고 일정 이상 반복되면 해당위치를 최적위치로 하여 부유영상을 최적위치로 변환시켜 두 영상을 정합한다. 실험을 위하여 인공영상을 사용하여 정화성과 강인성을 평가하였고, 육안평가를 위하여 뇌 CT-CT 혈관조영영상을 사용하였다. 본 제안방법은 지역적 가중치 3차원 거리맵을 이용함으로써 적은 샘플링 개수에도 국부최대인 위치에 수렴하지 않고 최적위치로 강인하면서 고속으로 영상이 정합되었다
